Intraoral ageing of aligners and attachments: Adverse effects on clinical efficiency and release of biologically-active compounds

  • The clinical application of aligners is accompanied by the ageing of the polymer appliances and the attachments used, which may result in inefficiency in reaching the predicted range of tooth movement, and release of compounds and microplastics in the oral cavity as a result of the friction, wear and attrition of the aligner and composite attachment. The purpose of this review is to present the mechanism and effects of in vivo ageing; describe the hydrolytic degradation of aligners and enzymatic degradation of composite attachments; examine the ageing pattern of aligners in vivo, under actual clinical scenarios; and identify a link to the discrepancy between predicted and actual clinical outcome. Lastly, strategies to deal with three potentially critical issues associated with the use of aligners, namely the necessity of weekly renewal, the dissimilar mechanical properties of aligner and attachment resulting in wear and plastic deformation of the aligner, and the development of integuments and biofilms with microbial colonization of the appliance, are discussed.

The Actual Conditions of Patients Health at 'S' college Dental Clinic (S대학에 내원한 환자의 구강건강관리실태)

  • For study on the mouth health care of patients for practical training of dental hygiene department, this survey was conducted among patients for practical training of the junior class of dental hygiene department of S College from September 2, 2004 to May 29, 2005. Before scaling, the purposes of the study and contents of the questionnaire were explained and the questionnaires were distributed and the patients were requested to complete them personally and then they were collected. 249 copies except 11 copies of insincere answers were used for the analysis. The subject of study was selected by convenience sampling, nonprobability sampling. 1. As for the characteristics of the subject of study, for sex, male occupied 138 (55.4%) and female occupied 111(44.6%). For the habitation site, Chungcheongdo residents were 181 people (72.7%), Incheon Gyeonggi 55(22.1%) and Seoul 13(5.2%). For ages, 19-29 age people were 122(49.0%), 30- 39 age 25(10.0%), 40-49 age 45(18.1%), 50-59 age 42(16.9%) and 60-70 age 15(6.0%). For scaling experience, 144 people (57.8%) had it and 105 people (42.2%) did not have it. 2. As for the mouth health care, for brushing method, crossways brushing was 164 people (65.9%), rotation brushing was 63 people (25.3%) and longways brushing was 22 people (8.8%). For brushing times, 2 times was 134 people (53.8%), over 3 times was 99 people(39.8%), and 1 time was 16 people (6.4%). For brushing time, after a meal was 182 people (73.1%) and before a meal was 67 people (26.9%). As for the mouth aids, 40 people (16.1%) used them and 209 people (83.9%) answered not to use them. 3. As for brushing method according to sex, it was found that both male and female use crossways brushing most, and male uses crossways brushing and longways brushing more than female and female uses rotation brushing more. For brushing times, it showed that 2 times was the most as 59.4% for male and over 3 times was the most as 50.5% for female. The survey of brushing time found that 33.3% of male brush their teeth before a meal and 81.1% of female brush their teeth after a meal. 4. For brushing method according to the habitation site, it was found that Chungcheongdo, Incheon Gyeonggi and Seoul use crossways brushing most and longways brushing least. The mouth aids were proved to be used in Incheon Gyeonggi most and in Chungcheongdo least. For brushing times, 2 times was the most in all three locations. 5. As to brushing times according to ages, only 30-39 age did brushing over 3 times, and the rest ages did brushing 2 times most. The survey of the use of the mouth aids according to the experience of scaling found that as they have the experience of scaling, they use them.

  • Cardiopulmonary resuscitation (CPR) is an emergency treatment that stimulates blood circulation and breathing when the function of the heart stops or stops breathing. CPR can be divided by basic life support (BLS) and advanced life support (ALS). BLS involves the use of chest compression to force the blood flow to the main organs, rescue breathing to improve the breathing to the respiratory failure patient and the use automated external defibrillator (AED). The categories of advanced life support include advanced cardiovascular life support (ACLS) for adult and pediatric advanced life support (PALS) for children. In the treatment of dental care for children, which is extremely difficult to deal with, and for a variety of reasons, the use of sedation is considered to treat the children who are unlikely to cooperate with dentistry. This is why there is an increasing possibility of an emergency situation involving cardiac arrest. PALS includes the BLS, and it presents a systematic algorithm to treat respiratory failure, shock and cardiogenic cardiac arrest. In order to manage emergency situations in the pediatric dental clinic, respiratory support is most important. Therefore, mastering professional PALS, which includes respiratory care and core cases, particularly upper airway obstruction and respiratory depression caused by a respiratory control problem, would be highly desirable for a physician who treats pediatric dental patients. Regular training and renewal training every two years is necessary to be able to immediately implement professional skills in emergency situations.

Comparison of Oral Care Patterns Before and During the Pregnancy (임산부의 임신 전과 임신 중의 구강건강관리 행태 비교)

  • This study was performed to compare the status of oral care knowledge, oral care practice, and the patterns of the oral health care utilization before and during the pregnancy. It was conducted on the basis of a survey of 291 pregnant women who were in 9 post-natal care centers for post-natal care after delivery located in Daegu Metropolitan city and Changwon city of Gyeongsangnam-do from April 1st till April 29th on 2010. Forty five percent of them were experienced with the gum bleeding before pregnancy and 55.7% were experienced during pregnancy. The number of average tooth brushing per day was 3.05 times before pregnancy and 2.99 times during pregnancy in the survey. The patients who were experienced in dental health care during pregnancy out of the subjects for study were 51 for 17.5%. The score for the dental health knowledge was 7.82 for 10 grade scale and 5.38 before pregnancy and 5.14 during pregnancy for 10 grade scale in the actual performance scoring for dental care. Pre-dental care, experience in activity restriction due to dental disease, concern about oral health and regular visit to dental clinic were significant associated with use of dental care services during pregnancy.

Infection control realities and relevant factors in dental hygienists (치과위생사의 감염관리실태와 관련요인)

  • Objectives : The purpose of this study was to improve the practice level on dental hygienists'infection prevention by examining the actual condition of dental hygienists'infection control and analyzing factors of having influence upon this. Methods : A questionnaire survey was carried out from July 18, 2011 to August 26 targeting dental hygienists who work at medical institutions where are located in Daegu, Busan and Ulsan Metropolitan Cities and Gyeongsangnam buk-do. After then, the following conclusions were obtained. Results : 1. As a result of surveying practice by item in the practice level of infection prevention, the items with high practice level in the management of infectious diseases were surveyed to be 12 months(89.6%) for health-checkup cycle and to be having experience of vaccination(78.0%) for hepatitis type B. The items with high practice level in the management and practice of washing hands were indicated to be in order of regularly paper towel(87.7%) and hands cleaning after regular medical examination(80.5%). In the item of the practice on wearing and managing individual protection equipment, regularly wearing(93.1%) rubber globes given washing implements was indicated to be high. 2. As a result of analyzing working career, working institution, working region, and practice level of infectious-disease management, the appearance of fulfilling infection control guidelines at medical institution and the experience of education for infection control were indicated to have difference depending on working institution. Regularly health checkup was indicated to have difference depending on respondents' working career and working institution. 3. The whole average in the practice level of infection prevention according to working career, working institution, and working region was indicated to be 2.55 out of 3-point perfection. 4. Wearing latex gloves was indicated to have statistically significant difference depending on working institution(p<0.001) and working region(p<0.001). The exchange of latex gloves every patient and the use of paper apron had statistically significant difference depending on working region(p<0.001). 5. As a result of comparing the frequency of using protection equipment for preventing infection according to the management of infectious diseases, the statistically significant difference was shown depending on the appearance of infection control guidelines at medical institution(p<0.001), the appearance of having experience of health checkup(p<0.01), and the appearance of having experience of vaccination for hepatitis type B(p<0.05). Conclusions : The above-mentioned findings showed that the denture satisfaction of the denture-wearing senior citizens was linked to their subjective oral health awareness. Therefore it will be possible to improve denture-wearing elderly people's quality of life when oral health plans geared toward boosting their denture satisfaction are carried out.

The Effect of Carious Pit and Fissure Sealant on Secondary Caries and Using Status (우식성 열구에 적용한 치면열구전색의 이차 우식에 대한 영향과 사용 실태)

  • This study was designed to investigate the using status and awareness of pit and fissure sealant, and how it affects on secondary caries when performed on above of initial caries. After classifying 446 occlusal surface into four groups, performed pit and fissure sealant, induced artificial caries, used DIAGNOdent (Kavo) to measure degree of secondary caries. Also, distributed a questionnaire on dental clinic in metropolitan area to find out using status and awareness of pit and fissure sealant, the following results were obtained. 1) On inspection and percussion, Group 4 corresponding to the enamel caries showed the highest secondary caries after sealant and was statistically significant difference in the order of initial group, stained group, sound group (p<0.05). 2) Inspection showed the highest percentage on tooth fissure caries diagnostic methods before sealant. 3) 56.6% didn't know about DIAGNOdent, 91.6% didn't have it. 4) In clinically, the most cause of secondary caries after sealant was a broken sealant, making caries on the downward. Based on the results of above study, degree of caries under sealant could affect on secondary caries, needs publicity about the use and necessity of objective fluorescence device.

Oral health literacy among foreign residents in South Korea (국내 거주 외국인의 한국형 구강건강정보 이해능력)

  • Objectives: This study was conducted to evaluate the oral health literacy of foreign students in Korea regarding their utilization of dental clinic services and oral care products. Methods: This study measured the oral health literacy through a self-administered questionnaire that were distributed among 145 foreign students in Seoul and 153 Korean students in Wonju, Gangwon province. The questionnaire is used to assess the oral health literacy with a total of 92 questions including 30 questions on linguistic oral health literacy, and 40 questions on functional oral health literacy (sentence translation ability 27 questions, document decoding ability 13 questions), and 22 questions on the general characteristics. The collected data were analyzed by frequency test, ${\chi}^2$, independent t-test, and ANOVA with p-value of <0.05 was considered statistically significant. Results: The linguistic oral health literacy awareness score was doubly lower in foreign students $20.5{\pm}22.4%$ than Korean students $53.9{\pm}18.4%$ (p<0.05), three words were not statistically significant with less than 10% of all the foreign and Korean students. Correct answer rate of sentence translation ability was statistically significant in all questions by foreign students $26.7{\pm}27.1%$ and Korean students $99.0{\pm}2.3%$ (p<0.05). Correct answer rate of document decoding ability showed a relatively small difference between foreign students and Korean students with $54.7{\pm}33.1%$ and $87.3{\pm}8.7%$, respectively, but it was statistically significant in all questions (p<0.05). Oral health literacy according to residence period and Korean language class level of foreign students were the most correlated among the other variables (p<0.05). Conclusions: Dental terminology was difficult for ordinary people to understand regardless of the Korean language proficiency levels, so it is recommended and needed to express dental clinical terms in simple layman's term or to use illustrations to dental patients. In case of foreign residents in Korea, interpretation services are needed. Additionally, labels and instructions of oral hygiene products retailed in Korea with the consideration for foreigners are required.

The Equality of Keywords of Journal of KAPD with Medical Subject Headings (대한소아치과학회지의 주요어와 의학주제표목의 일치도)

  • The purpose of this study is to analyze the equality between keywords used in the Journal of the Korean Academy of Pediatric Dentistry and medical subject headings (MeSH). A total of 4,353 English keywords in 1,165 papers from 1998 to 2014 were eligible for this study. We classified them according to equality to MeSH. We assayed patterns of errors in using MeSH, and reviewed frequently used non-MeSH terms. 24.9% of total keywords were completely coincident with MeSH terms, 75.1% were not MeSH terms. The results show that the accordance rate of keywords with MeSH terms in the Journal of the Korean Pediatric Dentistry is at a low level. Therefore, there is a need for authors to understand MeSH more specifically and accurately. Use of proper keywords aligned with the international standards such as MeSH is important to be properly cited. Authors should pay attention and be educated on the correct use of MeSH as keywords.

Use of the ID Migraine Questionnaire for Migraine in TMJ and Orofacial Pain Clinic (턱관절 및 안면통증 클리닉에서 편두통 진단을 위한 ID Migraine 설문지의 이용)

  • As migraine pain represents a substantial personal and social burden worldwide, there has been a great deal of effort in developing a screening instrument for migraine. Lipton et al(2003) developed and validated the ID Migraine questionnaire, which is a self-administered screener for migraine in primary care, and it is brief and easy to use for a primary care provider. The aim of this study was to determine if the ID Migraine questionnaire could be applied successfully to assess the headache patients with temporomandibular disorders(TMD) and orofacial pain. This study found that nausea, photophobia and headache-related disability had the highest individual sensitivities and specificities, and the performance of the three-item screener was equivalent to that reported in a previous study. Although the sensitivity of the three-item screener in this study (0.58) was lower than in a previous study (0.81), the specificity (0.98) was higher and the positive predictive value was 93.9%. This suggest that the ID Migraine questionnaire is very efficient in this setting. In conclusion, the ID Migraine questionnaire, which is a three-item screener consisting of nausea, photophobia and headache-related disability, is effective as a self-administered report for detecting migraine headaches in patients with temporomandibular disorders(TMD) and orofacial pain.

Effect of the combined use of bone morphogenetic protein and platelet-derived growth factor on bone formation in nude mouse (누드마우스에서 골 형성에 대한 BMP와 PDGF 복합사용의 효과)

  • Bone morphogenetic protein(BMP) and platelet-derived growth factor(PDGF) have been demonstrated tostimulate bone formation when applied locally in vivo. To explore whether or not the combined use of BMP and PDGF could have promotive effect and synergic interaction on bone formation in vivo, bone marrow mesenchymal stem cells were treated with BMP-2, PDGF-BB, or BMP-2 plus PDGF-BB, and then these cells were injected into the subcutaneous space on the dorsum of nude mice. The bone formation was evaluated after 12 weeks. Histomorphometric analysis demonstrated that the subcutaneous nodules formed in nude mice contained 25.3% newly formed bone in the BMP-2 treated cells, 14.4% newly formed bone in the PDGF-BB treated cells, and 8.9% newly formed bone in the RMP-2 plus PDGF-BB treated cells. The results showed that the combination of BMP-2 and PDGF-BB had neither a promotive effect nor synergic interact on bone formation in vivo.
