Treatment Pattern of Patients with Neuropathic Pain in Korea (한국인 신경병성 동통 환자의 치료 양태 연구)

  • The purpose of this study was to investigate the treatment pattern of patients with neuropathic pain (NeP) in Korea through computerized database of Health Insurance Review and Assessment Service (HIRAS) over three years' period from 2003 to 2005. The results showed that the numbers of treatment visits were the highest for diabetic neuropathy (DN), followed by postherpetic neuralgia (PHN) and trigeminal neuralgia (TN) in order. Top 3 specialties for treatment visits due to NeP conditions were neurology, neurosurgery and anesthesiology. While cost of a treatment visit was higher in anesthesiology and emergency clinics compared to other clinics, there was a tendency to increase costs for visits to clinics of rehabilitation medicine and family medicine over the three years. Cost of dental visits was relatively high for TN, atypical facial pain (AFP) and atypical odontalgia (AO). Surgeries frequently applied to patients with NeP were sympathetic plexus or ganglion block, block of peripheral branch of spinal nerve and cranial nerve or its peripheral branch block. Most common prescribed medication were anticonvulsants, anti-inflammatory analgesics and anti-psychotic drugs while anti-inflammatory analgesics were overwhelmingly frequently prescribed for AO and glossodynia. Based on the results of this study, NeP disorders more relevant to dentists were AO, TN and AFP, TN of which seems to be the most important in terms of patients' number and cost for treatment visits. This indicates that dentists, especially oral medicine specialists should actively participate in management of TN, AO and AFP and share relevant information with patients and community.

The Effect of Pilocarpine-containing Chewing Gum on Anti-microbial Components in Whole Saliva of Xerostomic Patients (구강건조증 환자에서 필로카핀 함유 껌 사용에 따른 전타액내 항균성분의 변화)

  • The purpose of this study was to investigate the effect of pilocarpine containing chewing gum on anti-microbial components in whole saliva of xerostomic patients, The objective xerostomic patients were instructed to use 5mg-pilocarpine containing chewing gum for 20minutes three times per day, and the author measured the flow rates of unstimulated whole saliva and stimulated whole saliva at the beginning the treatment, 1,2,3, and 4 weeks after. The concentration and flow rate of anti-microbial components in whole saliva were quantitated by enzyme-linked immunosorbent assay(ELISA). The obtained results were as follows: 1. There were significant increase in the unstimulated and stimulated whole salivary flow rate after using pilocarpine-containing chewing gum in xerostomic patients. 2. The concentrations of IgA in the unstimulated and stimulated whole saliva showed increasing pattern but, no significant changes, arid the flow rates of IgA in the unstimulated and stimulated whole saliva showed significant increase after using pilocarpine-containing chewing gum in xerostomic patients. 3. The concentrations of IgM in the unstimulated and stimulated whole saliva showed increasing pattern but, no significant changes, and the flow rates of IgM in the unstimulated and stimulated whole saliva showed significant increase after using pilocarpine-containing chewing gum in xerostomic patients. 4. The concentrations of lactoferrin in the unstimulated and stimulated whole saliva showed no significant changes, and the flow rates of lactoferrin in the unstimulated and stimulated whole saliva showed significant increase after using pilocarpine-containing chewing gum in xerostomic patients. 5. The concentrations of lysozyme in the unstimulated and stimulated whole saliva showed no significant changes, and the flow rates of lysozyme in the unstimulated whole saliva showed significant increase, but in stimulated whole saliva showed no significant changes after using pilocarpine-containing chewing gum in xerostomic patients.

Study on the Activity Patterns of Masticatory Muscles according to the Levels of Occlusal Force (교합력 수준에 따른 저작근 근활성도에 관한 연구)

  • In order to evaluate muscular activity patterns of masticatory muscles and asymmetry patterns of muscular activity according to the levels of occlusal force, twenty-one individuals of age ranged from 23 to 27 years were examined. They were selected according to the following criteria : 1) no symptoms of temporomandibular disorder, 2) complete dentition except third molars, 3) normal or Angle's class I molar relationship and 4) no experience of dental treatment. The electromyographic amplitudes was measured for evaluation of muscular activity and asymmetric patterns of masseter and anterior temporal muscle during unilateral clenching at the levels of 10%, 20%, 40% and 50% of the maximum occlusal force by use of electromyogram and bite force meter. The obtained results were as follows : 1. The muscle activity indices of masticatory muscles of clenching side at the clenching levels of 10%, 20% and 30% of the maximum occlusal force were -19.12, -9.87, -0.49%, so that activity of anterior temporal muscle was dominant than that of masseter muscle. At the levels of 40%, 50% of the maximum occlusal force, muscle activity indices were 4.68%, 6.70%, so that activity of masseter was dominant at all level and as the levels of occlusal force was increased, muscular activity index was tend to decrease. 2. In masseter, asymmetry indices of muscular activity at the levels of 10%, 20% of maximum occlusal force were -10.34 and -1.24%, so that muscular activity of non-clenching side were dominant and at the levels of 30%, 40% and 50% each of maximum occlusal force, muscular activity was dominant on clenching side as 4.68, 7.18 and 10.9%. In anterior temporal muscle, asymmetry indices were 33.38%, 25.46, 2095, 10.23 and 15.45% at the levels of 10%, 20%, 30%, 40% and 50% each of maximum occlusal force, so that activity of clenching 15.45% at the levels of 10%, 20%, 30%, 40% and 50% each of maximum occlusal force, so that activity of clenching side was dominant than that of non-clenching side at all levels, but as the levels of occlusal force was increased, asymmetry indices of muscular activity was tend to decrease. 3. Between both sides, average electromyographic amplitudes of masseter and anterior temporal muscle were correlated, so that as the levels of occlusal force was increased, average electromyographic amplitudes of both side in same muscle were increased proportionally. But asymmetry indices between muscular activities of masseter and anterior temporal muscle were not correlated.

A Evaluation Method for the Effectiveness of Anti-snore Pillow (코골이 방지 베개의 효율성 검증을 위한 방법)

  • In this study, the parameters of Polysomnography (PSG) test, such as total sleep time, snoring time, had been analyzed to evaluate the effectiveness of a developed anti-snore pillow. The developed anti-snore pillow is made up of two polyvinylidene fluoride (PVDF) vibration sensors, pumps, valves, and air bladders. The two PVDF sensors inside the pillow can acquire the sound signals and the algorithm was perfectly designed to extract snoring by removing unwanted noise accurately and automatically. Once the pillow recognizes snore, a pump inside the hardware activates, and a bladder under the neck area inside the pillow will be inflated. The PSG test was used and two volunteers were participated for the study. The parameters of the PSG results were analyzed to evaluate the effectiveness of the anti-snore pillow. The total sleep time of each volunteer was similar on each phase of test, but the snoring time and the longest snoring episode were significantly decreased with the use of anti-snore pillow. The overall results showed excellent possibilities for reducing snoring for the person who snores during sleep by using the anti-snore pillow. The effectiveness of the anti-snore pillow can be evaluated by the PSG test. Moreover, the relationship between each parameter of PSG test and the quality of sleep will be used for further researches.

Effect of Lidocaine-HCl on Microviscosity of Phosphatidylcholine Model Membrane

  • In order to provide a basis for studying the molecular mechanism of pharmacological action of local anesthetics and to develop a fluorescence spectroscopic method which can detect the microviscosity of native and model membranes using intramolecular excimerization of 1,3-di(l-pyrenyl)propane (Py-3-Py), we examined the effect of lidocaine HCl on the microviscosity of model membranes of phosphatidylcholine fraction extracted from synaptosomal plasma membrane vesicles (SPMVPC). The excimer to monomer fluorescence intensity ratio (I'/I) of Py-3-Py in liquid paraffin was a simple linear function of $T/{\eta}.$ Based on this calibration curve, the microviscosity values of the direct probe environment in SPMVPC model membranes ranged from $234.97{\pm}48.85$ cP at $4^{\circ}C$ to %19.21{\pm}1.11$ cP at $45^{\circ}C.$ At $37^{\circ}C,$ a value of $27.25{\pm}0.44$ cP was obtained. The lidocaine HCl decreased the microviscosity of SPMVPC model membranes in a concentration-dependent manner, with a significant decrease in microviscosity value by injecting the local anesthetic even at the concentration of 0.5 mM. These results indicate that the direct environment of Py-3-Py in the SPMVPC model membranes is significantly fluidized by the lidocaine HCl. Also, the present study explicitly shows that an interaction between local anesthetics and membrane lipids is of importance in the molecular mechanism of pharmacological action of lidocaine HCl.

  • Tidal volume by sevoflurane in small amounts is stable due to the increase in the breathing rate. But alveolus ventilation decreases due to sevoflurane as the degree of sedation increases; this ultimately causes $PaCO_2$ to rise. The occurrence of suppression of breath increases the risk of severe hypoxia and hypercapnia in deeply sedated patients with disabilities. Sevoflurane inhalation anesthesia has a number of risks and may have unexpected problems with hemodynamic changes depending on the underlying state of the body. This study was conducted to examine the stability of internal acid-base system caused by respiratory depression occurring when patients with disabilities are induced by sevoflurane. Anesthetic induction was carried out by placing a mask on top of the patient's face and through voluntary breathing with 4 vol% of sevoflurane, 4 L/min of nitrous oxide, and 4 L/min of oxygen. After the patient's loss of consciousness and muscle relaxation, IV line was inserted by an expert and intravenous blood gas was analyzed by extracting blood from vein. In a deeply sedated state, the average amount of pH of the entire patients was measured as $7.36{\pm}0.06$. The average amount of $PvCO_2$ of the entire patients was measured as $48.8{\pm}8.50mmHg$. The average amount of $HCO_3{^-}$ of the entire patients was measured as $27.2{\pm}3.0mmol/L$. In conclusion, in dental treatment of patients with disabilities, the internal acid base response to inhalation sedation using sevoflurane is relatively stable.

COMPARATIVE STUDY ON THE EARLY DETECTION OF ENAMEL LESIONS USING DIFOTI AND LASER FLUORESCENCE (Digital Imaging Fiber-Optic Trans-Illumination과 Laser Fluorescence를 이용한 법랑질 우식증의 조기 진단에 관한 비교 연구)

  • The newly developed equipments for the early detection of carious lesion are LFD (laser fluorescence device), Ultrasonic diagnostic system, CLSM(confocal laser scanning microscopy), QLF(quantitative light-induced fluorescence) and DIFOTI (digital imaging fiber-optic trans-illumination) system. In this study, DIFOTI system and LFD were used for the detection of early enamel caries. Twenty five primary teeth extracted from twenty one children at around the dentitional exchanging period were selected as samples. The results obtained from DIFOTI imaging and LFD measurement were compared with those of CLSM and comprehensive evaluations were made for the diagnostic capacity of each device. In vitro test, 40 sample teeth with their buccal & lingual surface formed by a window of $2{\times}3mm$ in diameter were immersed in artificial demineralizing solution for the period of 4, 8, 12 and 16 days. The results obtained from the experimental groups (DIFOTI, LFD) were compared to control group (CLSM) and we have reached to the following conclusions. 1. The sensitivity and specificity of DIFOTI system operated in oral environment was 88.2% and 76.9% respectively. 2. The sensitivity and specificity of LFD measured in oral environment was 76.5% and 69.2% respectively. 3, Regression analysis on the light transparent rate of DIFOTI showed its decrease according to the length of primary enamel decalcification performed in vitro(r=-0.96, p<0.05). 4. No statistically significant difference between LFT measurement and the length of in vitro decalcification was found in regression analysis (p>0.05). 5. The correlation coefficient of DIFOTI image transparent rate and the lesion depth of CLMS was -0.6988 (p<0.05), whereas no statistically significant difference was found for LFD measurement.

THE EFFECT OF STREPTOCOCCUS ORALIS ON THE FORMATION OF ARTIFICIAL PLAQUE (Streptococcus oralis의 인공치태 억제효과에 대한 연구)

  • This study was performed to evaluate the effect of Streptococcus oralis on the formation of artificial plaque and the replication of Streptococcus mutans. S. mutans was incubated alone and in the combination with S. oralis in the beaker with wires. The produced plaque weight and the viable cells of S. mutans were compared between those cultures. Various factors were studied about the effect on the formation of plaque and the replication of S. mutans. Followings are the results. 1. Lower amount of plaque was produced and fewer cells of S. mutans were replicated at the mixed culture of S. mmutans and S. oralis than S. mutans alone. 2. When 10 mM glucose was added, the plaque weight was increased in the culture of S. mutans alone. But in the mixed culture of S. mutans and S. oralis, the plaque weight was not increased when 10 mM of glucose was added. 3. When 10 mM fructose was added, the plaque weight was increased in the culture of S. mutans alone or combined S. mutans and S. oralis. 4. In the mixed culture of S. mutans and S. oralis with different concentration, the more S. oralis exist, the less plaque and the fewer viable cells of S. mutans were observed. 5. The plaque weight and the viable cells of S. mutans were more decreased in the mixed culture of S. mutans and S. oralis than S. mutans alone after 12 hours. 6. When Staphylococcus epidermidis consuming hydrogen peroxide was added to the mixed culture of S. mutans and S. oralis, the plaque weight and the viable cells of S. mutans were increased. These results indicate that S. oralis inhibited the formation of plaque and the replication of S. mutans, and this may result from the formation of hydrogen peroxide S. oralis.
