Application of radiographic images in diagnosis and treatment of deep neck infections with necrotizing fasciitis: a case report

  • The advent and wide use of antibiotics have decreased the incidence of deep neck infection. When a deep neck infection does occur, however, it can be the cause of significant morbidity and death, resulting in airway obstruction, mediastinitis, pericarditis, epidural abscesses, and major vessel erosion. In our clinic, a patient with diffuse chronic osteomyelitis of mandible and fascial space abscess and necrotic fasciitis due to odontogenic infection at the time of first visit came. We successfully treated the patient by early diagnosis using contrast-enhanced CT and follow up dressing through the appropriate use of radiographic images.

  • We investigated 974 male and female students who are in 4th, 5th and 6th year of 8 elementary schools are located win Puan-kun, the farm area of Chollabuk-do, and the results are as follows. 1. Visiting dental clinic in order to be treated for dental caries hold the highest rate as 26.1% of male students and 30.3% of female students. 2. On fear when children visit dental clinic, first, 25.6% of male students and 35.1% of female students mark the highest rate as some what fear. Second, 23.1% of male students represent to be usual and 29.4% of female students represent to be very fear. There fore it speaks for dissimilarity in character between male and female students. 3. On reason of developing dental caries, 62.5% of male students and 55.8% of female students replied that it was developed because they frequently ate the sweet things. 4. Children who pull out decidous teeth alone mark the highest rate as 44.7% of male students and 38.6% of female students. 5. On important thing for ordinary dental health, 56.2% of male students and 58.5% of female students as the highest rate replied that they often brushed their teeth.

  • Recently computer-aided technology has been widely used in dentistry. DENTCA$^{TM}$ CAD/CAM denture system (DENTCA Inc.), one of CAD/CAM systems for fabricating complete denture, tries to collect and store all of a patient's information at the first visit. This system aims to deliver denture at the second visit through utilizing the CAD/CAM software to access the stored data for designing the 3D denture model. The 3 dimensional (3D) denture will then be fabricated with 3D printer. Many case reports have evaluated clinical application of CAD/CAM system for fabricating complete dentures. This case report is about fabricating of complete dentures using DENTCA system and conventional method in same patient. With two cases, usefulness and limitation of DENTCA system could be evaluated.

  • In dental trauma, reattachment of the original tooth fragment improves the reproduction of original tooth shape, texture, color, and radiolucency; thus, it provides good aesthetics. A 9-year-old boy was referred due to complicated crown-root fracture of the maxillary right central incisor. Although it had poor prognosis due to severe coronal damage and subcrestal fracture, reattachment of the tooth fragment was chosen due to the patient's age. One-visit apexification with mineral trioxide aggregate (MTA) was performed, followed by osteotomy and reattachment of the tooth fragment with post placement. Regular observation revealed no clinical signs or symptoms and no radiologic complications.

Community periodontal index treatment needs in relation to dental health care of migrant worker (이주 노동자의 구강건강관리에 따른 치주치료요구도)

  • Migrant Worker are rapidly increasing in Korea since 1990. They are nowadays main sources of laborer groups engaging in medium-sized factories. The purpose of this study is to provide the basic information to establish proper oral health policy. Dental caries and periodontal disese are the most common disease that occur in the mouth. Periodontal disease is the most common disease in humans and the biggest reason for the loss of the teeth in the adult population. The CPITIN has been developed jontly by the international Dental Federation and the World Health Organization. CPITN is now an established index of level, of periodontal condition in populations for which specific intervention might be considered. This study was conducted to obtain the information regarding to CPITN of migrant workers located in Daegu, Questionaire survey was carried out for 289 workers from July 9 to August 8, 2006. Total survey was 289, 224 males and 65 females. The result was as follows: First, The number of nationality was 14. The first majority was China as 31.8% Among 14 nationalities were Cambodia 18.0%, Vietnam 12.8%, Sri Lanka 12.1%, Indonesia 7.3%, Nepal 4.2%, the other 13.8%, those who are aged from 20 to 29 were 43.9%, and salary from 1,000,000 won to 1,490,000 won 51.2%.(as for their residence, those who resided) over 3 years were 42.6% and not insured reached 68.5%. Second, more than 93.4% of the subjects need periodontal treatment, only 4.9% of non-smoker was health periodontal states, four times frequency of tooth brushing per a day was 16.7%. Third, 28.7% of migrant workers had experienced visit of dental clinic, and 22.9% had received treatment of decayed tooth. Fourth, It is difficult for them 65.1% of them to visit dental clinic in korea, the First was a communication problem and the second was time. Fifth, Most of them didn't have a oral health education but 85.1% of them said that they wish they attend oral health education. We conclude that the situation of migrant workers was very bad considering their working conditions, circumstances, and health condition. According to this study, more than 93.4% of the people need periodontal treatment. Besides they didn't have accurate knowledge about prevention of periodontal disease. Therefor our considering these facts, the policy of dental health by government should be established for migrant workers.

The determinants of purchasing private health insurance among middle-aged and elderly Korean adults (중.고령자의 민간의료보험 가입 여부의 결정 요인)

  • Objectives : The coverage of Korean National Health Insurance is limited to basic level. Korean government encourages private health insurance for covering medical securities. So, many studies examined the determinants of purchasing private health insurance. However, 11% of Korean population is older than 65 in 2011. Considering the elderly is important to establish a health policy. The aim of this study is to examine factors determining the purchase of private health insurance among middle-aged and elderly Korean adults. Methods : We used the second Korean Longitudinal Study of Ageing (KLoSA), selected 8,688 sample of the aged 47 or older for the analysis. KLoSA collected information on demographic characteristics, income, health- related factors. KLoSA data include in the number of outpatient, inpatient, oriental hospital visit, dental clinic visit for two years. Logistic regression was used to examine the relationship between the determinants of purchasing private health insurance and the factors which include age, gender, education, residential district, marital status, smoking, drinking, physical exercise, economic activity status, national health insurance type, income, the number of chronic disease, and the number of outpatient, inpatient, oriental hospital visit, dental clinic visit for two years. Results : People who were older, did not live in a city, had higher IADL, currently drunk alcohol, did exercise regularly and had chronic diseases more than three were inclined not to purchase private health insurance. Females, the married, well-educated, past & currently smokers, the employed, high income earners, national health insurers, metropolitan citizens and someone who got high MMSE were more likely to purchase private health insurance. The more people experienced outpatients, inpatients, dental clinics and Chinese medicine clinics, the more private health insurance was purchased. The elderly people over 75 had more private health insurance than the aged 65-74. The strongest factors for private health insurance is gender, and economic status such as income. Conclusion : In this study, we found healthy-high income people were more likely to purchase private health insurance. In contrast, unhealthy-low income and older people did not. The economic factors were strongly related with private health insurance in aged over 75. These mean inequality exists in the using private health insurance. Therefore, the government should consider vulnerable social group before expanding private health insurance.

A SURVEY OF REPEATED DENTAL TREATMENT UNDER GENERAL ANESTHESIA FOR DISABILITIES (장애인 환자의 전신마취 하 반복된 치과치료에 대한 조사)

  • The purpose of this study was to analyze the dental treatment of patients with disabilities, especially according to the frequency of general anesthesia, and to propose the improvements in oral care. The subjects of the present study were 85 patients including age, sex, medical condition, dental treatment and the number of general anesthesia. The patients were divided into regular and irregular check groups according to their follow-up patterns. These two groups were compared for the frequency of general anesthesia and the type of repeated treatment. The results showed that restorative treatment was superior in numbers under first visit of general anesthesia. And more general anesthesia was performed in the irregular recall check group compared with the regular recall check group. This survey suggest that easy access to a dental clinic and the convenience of treatment is needed. On the other hand, there is a time limit on the dental care for disabilities by the dentist. Therefore oral care training program should be simultaneously provided for parents to improve the efficiency of dental care at home. In conclusion, efforts should be made for more comprehensive and effective dental care including regular recall check and preventive home care for disabilities.

  • Objectives: The aim of this study is to investigate the relationship between diabetes mellitus and oral disease in the Korea adults by using the representative data of Korea National Health and Nutrition Examination Survey (KNHANES) V. Methods: Using data from a cross-sectional survey of Korea National Health and Nutrition Examination Survey V. The subjects were 5,602 adults over 19 years old. The survey data and the examination data are used for the independent variables. KNHANES included health status, nutrition survey, and oral examination. The health status was obtained by a self-reported questionnaire of the study subjects and direct interview was carried out on economic status, prevalence rate, and use of medical services. Results: The regular dental checkup of diabetes patients was significantly lower than the general population. Regular dental visits allow adults to receive early diagnosis and obtain restorative care at the first visit. The regular annual oral examination is very important to prevent and manage the chronic diseases including diabetes mellitus. Conclusions: Diabetes mellitus can be fully prevented and managed by the regular annual dental checkup. The establishment of the regular dental checkup system can prevent and manage the complication due to diabetes mellitus and other chronic diseases.

Dental fear cause and oral health care behavior in the dental hygiene clinic clients (치위생학과 구강위생교육실에 내원한 대상자의 치과공포 원인과 구강건강관리행태)

  • Objectives : This study is aimed to seek how to promote oral health care by leading the clients with dental fears to change their behaviors in oral hygiene care based on the analysis of the relationships between the dental fears and oral health care behaviors of the clients who visit the Dental Hygiene Education Room. Methods : This study analyzed the 361 dental hygiene records of the subjects with finished treatment and accurate records among a total of 370 clients who had received dental hygiene care at the Dental Hygiene clinic in Y University from March 2007 till June 2009. According to their general features, the study analyzed whether they had any dental fears and why such fears occurred and conducted Chi-square test to compare their oral hygiene care experiences and behaviors with dental fears. Data analysis was made using the SPSS 12.0K for Window, with level of significance set at 0.05. Results : 1. In terms of dental fear or non-fear, the comparison by gender showed that women had a higher level than men, with 76 women(47.2%) answering yes; the comparison by age showed that the age group of 30 years old or older had a higher level than other groups. 2. In terms of dental fear reasons, "the past pain experiences" recorded 34.2%, showing the highest rate. 3. In addition, dental fears had influences on regular dental examination, scaling and dental hygiene education and the subjects without dental fear showed higher levels in terms of tooth-brushing frequency and time. Conclusions : To control "the past pain experiences", future dental treatment should place priority on preventive treatment and get clients to cope with pains through the preliminary education with pains. Besides, to make an effective oral health care of the clients with dental fears, dental hygienists should control the fears of the clients and perform an appropriate oral health care for them using the communication techniques to make friendly and trustworthy impressions.

The effect of factors dental treatment fear of level middle-school student's (중학생들의 치과치료 공포수준에 미치는 요인)

  • Objectives : The purpose of this study were to measure the effect of factors analyse the associated by the dental treatment fear of level middle school student's, and then to provide basic material which can help to improve adolescent's oral health and dental service. Methods : This subject of study consists of 342 middle schools each 1,2,3 grade Daegu city. The data were collected from July through August 2009, by way of the self-reported questionnaire. The data materials are analyzed by general characteristics and dental service behavior frequency analysis, dental fear of levels general characteristics, dental service behavior, the burden of dental visits, the pain experience of the past ${\chi}^2$-test and dental treatment fear relationship with multiple regression analysis. Results : General characteristics, 56.7% for boys and girls, with 43.3 percent of boys and girls high. dental service behavior in the dental visited was treatment purpose 76.5%, preventive purpose 23.5%. high-level dental treatment fear of the female 60.8%, male 33.0% was higher, The higher age also increased dental fear of the levels, dental services dental visit in 1-2 year the dental fear level of the higher, was statistically significant (p<0.05). Medical purpose 'preventive' than 'treatment' is a higher dental fear. Factors affecting dental treatment fear experiences directly from your pain (${\beta}$=0.116), inadequate anesthesia, treatment experience (${\beta}$=0.126), indirect painful experience (${\beta}$=0.125) in both a statistically significant positive relationship was found (p<0.05). Conclusions : Finally, it is necessary carry out further studies on the improvement of dental service of oral health of adolescent based on the dental treatment fear.
