Dental treatments under sedation-analgesia in patients who are unable to collaborate: a prospective observational study

  • Background: Excessive fear of dental procedures leads to disruptive behavior during dental examinations and treatments. Dental examinations and treatments of these patients usually require additional techniques, such as sedation. The most commonly used techniques are inhalation of nitrous oxide, infusion of propofol with fentanyl, and premedication and infusion of midazolam. Methods: A prospective observational epidemiological study was conducted on patients who required sedoanalgesia techniques for dental exploration and procedures. The reasons for the inability of patients to cooperate (excessive fear or intellectual disability), age, sex, weight, systemic pathology, oral pathology, treatment performed, time of intervention, anesthetic technique performed, and occurrence of complications were recorded. Results: In total, 218 patients were studied. Sixty-five patients came for fear of dental treatment and 153 for presenting with a diagnosis of intellectual disability and not collaborating in the treatment with local anesthesia. The average age of all patients was 30.54±17.30 years. The most frequent oral pathologies found in patients with excessive fear were tartar (6.8%) and wisdom teeth (6.4%), followed by missing teeth (5%). In patients with disabilities, a combination of tartar and cavities appeared most frequently (41.3%), followed by cavities (15.6%). The most frequently used sedoanalgesia technique was the infusion of propofol with fentanyl in both groups of patients, followed by nitrous oxide. Conclusion: The combination of propofol and fentanyl was the most frequently used alternative in patients who were unable to collaborate because of intellectual disability or carry out longer or more complex treatments. Inhaled nitrous oxide and midazolam were the sedative techniques of choice for simpler oral treatments, such as tartrectomies, shallow obturations, and shorter interventions, or in younger patients.

치과위생사에 대한 환자만족도와 치과 치료지시 이행도의 관계 (Relationship between dental care compliance and patients' satisfaction about dental hygienist)

  • Objectives: This study aimed to examine the correlation between dental care compliance and patients' satisfaction about dental hygienists who are oral health care experts likely to lead patients' motivation. Methods: From June 11 to September 30, 2019, a questionnaire survey was conducted and a total of 189 people completed a structured questionnaire evaluating patient's satisfaction and dental compliance. Data were analyzed using non-parametric tests such as Mann-Whitney test, Kruskal-Wallis rank test, and Spearman's correlation. Results: There was a statistically significant correlation between dental care compliance and the area of good impression (r=0.187, p<0.001), support for interpersonal support (r=0.346, p<0.001), and dissatisfaction (r=0.304, p<0.001). Conclusions: It is necessary to continuously study and identify ways to enhance dental care compliance and patients' satisfaction.

Minimal sedation using oral sedatives for multi-visit dental treatment in an adult patient with dental phobia

  • In adult patients with dental phobia, dental treatment may be difficult, or may not be possible. Depending on the level of fear or anxiety, non-pharmacological or pharmacological behavior management techniques are used in the dental treatment of such patients. Among the pharmacological behavior management techniques, minimal sedation, that is, the lowest depth of sedation, can be easily obtained in adult patients using oral sedatives, does not require special equipment or tools, and does not affect ventilatory and cardiovascular function. Diazepam is an anxiolytic drug belonging to the benzodiazepine family that, in addition to relieving anxiety, produces muscle relaxation, and is a representative drug used in adult patients with fear of dental treatment. Herein, we report the case of a 50-year-old woman with severe dental fear who successfully underwent long-term dental treatment in approximately 20 visits with minimal sedation using oral diazepam. In addition, we reviewed the considerations for the use of benzodiazepines for minimal sedation.

치과적 중증 장애인 환자를 위한 치료 계획 수립 (How to establish dental treatment plans for the patients with severe disabilities)

  • This article discusses how to conduct treatment planning and decision making in special needs dentistry. Special needs patients often lack cooperative ability during dental treatment and have a deteriorated oral health status. To overcome the limitation in communication with special needs patients and solve their complicated dental problems, dentists need to have extensive preoperative information about the patients and their caregivers. Treatment procedures should be organized in a patient-centered and cost-effective manner. Additionally, clinical outcomes need to be predicted taking into consideration of the patients' condition. The clinical experience of committed dentists is another factor that enhances the benefits of extensive treatment in special needs patients with many limitations. The insightful treatment decision-making of dentists will contribute to improving the oral health of special needs patients despite the various obstacles.

임플란트 보유자의 진료기관 내 구강보건교육경험에 따른 구강건강관리 수준 (Oral hygiene maintenance in patients with dental implants following oral health education)

  • Objectives: The aim of this study was to investigate the impact of oral hygiene education provided by dental hygienists in patients with dental implants. Methods: Data were obtained from a survey that involved 300 patients who had one or more dental implants. Based on the calculated sample size (n = 274), 300 questionnaires were used for further analyses. Results: The patients who received oral hygiene education had higher levels of satisfaction after dental implant surgery (p<0.05). Patients who had repeated educational sessions, in the form of interactive or hands-on sessions or lectures, with continuous feedback had higher scores for oral hygiene maintenance (p<0.05). Conclusions: This study indicated that patients who received oral hygiene education, in the form of interactive or hands-on sessions or lectures, with continuous post-educational feedback were better at maintaining oral hygiene.

치과의료기관을 선택하는 요인에 관한 조사연구 (A Study on the Primary Factors in the Selection of Dental Medical Organizations)

  • This study was conducted to find out basic information on better customer satisfaction and service by examining and analyzing in what routes patients mostly have selected their dental medical organizations and how they have reacted to their dissatisfaction. The subject of study included the patients who have come to dental clinics and dental hospitals to be treated in Cheonan City, Chungnam and the researcher of this study used a survey and analyzed its gathered data using an SPSS(Statistical Package for the Social Science) WIN 11.5 Program. As analysis methods, frequency and percentage were calculated to figure out the general characteristics of its subject. In addition, a ${\chi}^2$(Chi-square) test was performed to figure out the factors that patients have selected their dental medical organizations and the following results were obtained. 1. The largest number of patients selected their dental medical organizations by hearsay and more female and married patients selected them by hearsay than other patients, and there were significant differences(p<.05). 2. The dental patients put the priority on treatment techniques in their visiting hospitals and the patients who were more than 40 years old and married patients put more emphasis on treatment techniques in their visiting hospitals than other patients and there were significant differences(p<.05). 3. The said dental patients were not satisfied with the kindness of medical staffs in their visiting medical organizations; meanwhile, the patients who were aged below 30 and had an academic background of more than university degree and were unmarried were less satisfied with the kindness of medical staffs in their visiting medical organizations than other patients and there were significant differences(p<.05). 4. In terms of improvement methods or complaints, the largest number of them said that they just ignored and endured; meanwhile, the patients who were aged below 30 and had an education of more than university degree and were unmarried were more patient of improvement methods or complaints than other patients and there were significant differences(p<.05). 5. There were more patients who have introduced their visiting medical organizations to their neighboring people than the patients who have not done it; in the meantime, the patients who were aged over 40 and were married have introduced their visiting medical organizations to their neighboring people more than other patients(p<.05).

Management of patients with allergy to local anesthetics: two case reports

  • Lidocaine is the most commonly used local anesthetic (LA) agent in various dental as well as oral and maxillofacial procedures. Although rare, adverse effects and allergic reactions to lidocaine have been reported. In patients with suspected allergy to LA or a history of such reaction, careful history-taking and allergy testing should be performed to choose an alternative LA agent to avoid any adverse effects. Here, we present two cases of delayed hypersensitivity reaction to lidocaine, wherein the patients presented with erythema, edema, and itching. Intradermal testing confirmed allergic reaction to lidocaine, and the patients underwent successful dental treatment using an alternative LA agent. This report highlights the importance of allergy testing prior to LA use considering the serious consequences of allergy to these agents and describes the management of such patients using an alternative LA agent.

임플란트 환자의 치료 만족도 및 구강건강관련 삶의 질(OHIP-14)에 관한 연구 (A study on treatment satisfaction and oral health-related quality of life (OHIP-14) among implant patients)

  • Objectives: This study aimed to investigate factors affecting treatment satisfaction of patients with dental implants and quality of life. Methods: A questionnaire survey was conducted for adults who underwent dental implant treatment. at dental departments of general hospitals or dental clinics in Seoul and Gyeonggi-do. Data was analyzed to get descriptive statistics of variables, and was under independent t-test, one way ANOVA, and multiple regression analysis. Results: The mean of total satisfaction with implants was $3.73{\pm}0.41$ in the 5-point scale, and that of general satisfaction was $3.97{\pm}0.53$, followed by mean values of psychological satisfaction of $3.68{\pm}0.51$, aesthetic satisfaction of $3.67{\pm}0.49$, and masticatory function of $3.65{\pm}0.53$. Factors that affect implant treatment satisfaction among patients were monthly income (more than 4 million won), hesitation of the procedure (treatment period), regular checkup (yes) and concerns during treatment (work). Factors that affect the quality of life of patients who underwent implant treatments were satisfaction of aesthetic function and overall satisfaction. Conclusions: The above results showed that the higher the aesthetic satisfaction and overall satisfaction with the implant are, the higher the quality of life of patients get. Therefore, it is necessary for dental specialists to perform dental implants well to improve quality of life of patients.

  • Dental emergency varies from toothache to trauma of oromaxillofacial area. Recently, the number of emergency cases after dental treatment is increasing because surgical treatments such as implant installation rise. Dental emergency patients have been looking for emergency room of general hospital and they could be treated only when the dentist of night duty exists. Although there are many analyses on the dental emergency patients visiting general hospitals, research on the dental treatment only has not been provided enough. We anticipated different aspects between the dental emergency patients visiting independent dental hospital and general hospitals. We analyzed dental emergency patients' chief complaints and received procedures for almost 1-year period in Won-Kwang dental hospital in Dae-jeon where there had been no dental emergency settings. With this 1574 patients' result, we tried to find appropriate treatment plans regarding dental emergency patients occurring at night and holidays.

치과내원 환자의 치과공포 감소를 위한 중재법 적용 (Intervention for Reducing Dental Fear and Anxiety of Dental Patients)

  • 본 연구는 서울시 일개 치과의원에 내원한 환자를 대상으로 치과공포 수준을 측정하고, 치과공포 수준 감소를 위한 중재법에 대해 환자가 요구하는 방법을 적용한 후 치과공포 중재법의 효과를 측정하였다. 개입한 치과공포 중재법에 대해서는 전체적으로 '도움이 된다(3.57점)'는 의견이었고, 항목별로는 '어떤 진료인지 미리 설명해 준다(4.38점)', '진료 중에 환자가 손을 들면 언제든 치료를 잠시 멈추어 준다(4.35점)', '진료 중 통증이 발생할 것 같으면 미리 말해준다(4.24점)', '의료진이 부드럽고 웃는 표정을 한다(4.06점)' 순으로 도움이 된다고 하였다. 치과공포 중재법 적용 전과 후의 전체적인 만족도는 각각 4.37점과 4.35점으로 높은 수준이었으나, 적용 후 미미하게 감소하였으며 이는 치과치료 항목과 밀접한 연관이 있을 것으로 사료된다. 치과공포 수준을 감소하기 위해서는 환자의 치과공포 수준 뿐 아니라 치과공포 중재를 위한 요구도를 파악하여 이에 맞는 개입방법을 적용하여 주는 것이 효과적이며, 이에 대한 치과의료 인력에 대한 교육과 매뉴얼 개발이 필요하리라 생각된다.
