임플란트와 상부구조물 사이의 micromotion에 관한 연구 (A study on the micromotion between the dental implant and superstructure)

  • Treatment with implants of single tooth missing cases is both functional and esthetic. Although the success rate of single-tooth implant treatments is increasing, sometimes it makes some problems. Problems with single-tooth implant treatments include soft tissue complications, abutment screw fracture, and most commonly, abutment screw loosening, and these involve the instability of the dental implant-superstructure interface. This study investigated and compared dental implant screw joint micromotion of various implant system with external connection or internal connection when tested under simulated clinical loading, Six groups (N=5) were assessed: (1) Branemark AurAdapt (Nobel Biocare, Goteborg, Sweden), (2) Branemark EsthetiCone (Nobel Biocare, Goteborg, Sweden), (3) Neoplant Conical (Neobiotec, Korea), (4) Neoplant UCLA (Neobiotec, Korea), (5) Neoplant 5.5mm Solid (Neobiotec, Korea), and (6) ITI SynOcta (Institute Straumann, Waldenburg, Switzerland). Six identical frameworks were fabricated. Abutment screws were tightened to 32-35 Ncm and occlusal screw were tightened to 15-20 Ncm with an electronic torque controller. A mechanical testing machine applied a compressive cyclic load of 20kg at 10Hz to a contact point on each implant crown. Strain gauge recorded the micromotion of the screw joint interface once a second. Data were selected at 1, 500, 5,000, 10,000, 20,000, 30,000, 40,000 and 50,000 cycle and 2-way ANOVA test was performed to assess the statistical significance. The results of this study were as follows; The micromotion of the implant-superstructure in the interface increased gradually through 50,000 cycles for all implant systems. In the case of the micromotion according to cycle increase, Neoplant Conical and Neoplant UCLA system exhibited significantly increasing micromotion at the implant-superstructure interface (p<0.05), but others not significant. In the case of the micromotion of the implant-superstructure interface at 50,000 cycle, the largest micromotion were recorded in the Branemark EsthetiCone, sequently followed by Neoplant Conical, Neoplant UCLA, Branemark AurAdapt, ITI SynOcta and Neplant Solid. Internal connection system showed smaller micromotion than external connection system. Specially, Neoplant Solid with internal connection system exhibited significantly smaller micromotion than other implant systems except ITI SynOcta with same internal connection system (p<0.05). In the case of external connection, Branemark EsthetiCone and Neoplant Conical system with abutment showed significantly larger micromotion than Branemark AurAdapt without abutment (p<0.05).

  • CAD/CAM systems (computer-aided design / computer-aided manufacturing) used for decades in restorative dentistry have its application to implant dentistry. This study aimed to overview CAD/CAM systems used implant dentistry, especially emphasizing custom implant abutments manufacturing. CAD/CAM custom abutments present the advantages of being specific to each patient and providing a better fit than the stock and cast custom abutments. This cutting edge technology of virtual-designed and computer-milled implant abutments will likely replace traditional implant restorative protocols and become the standard for implant dentistry in the foreseeable future.

  • Statement of problem : Dental implant which has been developed gradually through many experiments and clinical applications is presently used to various dental prosthetic treatments. To conduct a successful function of implant prosthesis in oral cavity for a long time, it is important that not only structure materials must have the biocompatibility, but also the prosthesis must be designed for the stress, which is occurred in occlusion, to scatter adequately of load support. Therefore, it is essential to give the consideration about the stress analysis of supporting tissues for higher successful rates. Purpose : Recently, many implant manufacturing company produce various taper design of root form implant, the fixture is often selected. However, the stress analysis of taper form fixture still requires more studies. Material and method : This study we make the element models that five implant fixture; Branemark system(Nobel Biocare, Gothenberg, Sweden), Camlog system(Altatec, Germany), Astra system(Astra Tech, Sweden), 3i system(Implant Innovations Inc, USA), Avana system(Osstem, Korea) were placed in the area of mandibular first premolar and prosthesis fabricated, which we compared with stress distribution using the three-dimension finite element analysis under two loading condition. Results : This study compares the aspect of stress distribution of each system with the standard of Von mises stress, among many resulted from finite element analysis so that this research gets the following results. 1. In all implant system, oblique loading of maximum Von mises stress of implant, alveolar bone and crown is higher than vertical loading of those. 2. Regardless of loading conditions and type of system, cortical bone which contacts with implant fixture top area has high stress, and cancellous bone has a little stress. under the vertical loading, maximum Von mises stress is more higher in order of Branemark, Camlog, Astra, 3i, Avana. under the horizontal loading, maximum Von mises is more higher in order of Camlog, Branemark, Astra, 3i, Avana.

인공치아의 임플란트 탈착을 위한 유도가열장치 연구 (Induction Heating Device for Dental Implant Removal)

  • 유도가열은 유도전류를 이용하여 도체에 열을 가하는 방식이다. 유도가열은 열을 가하고자 하는 물체 내부에서 열이 발생하고, 비접촉이며 안전하기 때문에 산업이나 의료분야에서 광범위하게 사용되고 있다. 최근에 형상기억합금을 사용하여 열에 의해 인공치아가 임플란트에서 쉽게 빠질 수 있게 하는 임플란트 시스템이 개발되었다. 본 논문에서는 새로운 임플란트 시스템에서 인공치아를 쉽게 제거할 수 있는 유도가열 장치를 개발하였다. 먼저 전자장-열 구조 유한요소 연성 해석을 통해 다양한 입력 전류와 코일 형상에 대해 온도를 시뮬레이션 상으로 확인해보았다. 해석 결과를 토대로 유도가열장치 시작품을 제작하여 실험을 통해 86초에 인공 치아가 분리됨을 확인하였다.

Immediate loading on mandibular edentulous patient with SFI Bar$^{(R)}$ overdenture

  • Despite the greater retention and low maintenance of bar attachment system, the longer clinical time and increased number of visits were the assignments to overcome in bar attachment system. This case report describes SFI-Bar$^{(R)}$ (Cendres et Me$\'{e}$taux, Biel/Bienne, Switzerland) to be solve those problems. A 65-year-old female, who had severely absorbed mandible, hoped to wear a stable mandibular denture without pain. As soon as two implants were placed on mandible, a tube bar was connected to two adaptors connected to each implant. The length of the tube bar was adjusted considering inter implant distance, and reconnected to the adaptors. Finally a female part was seated beneath the denture. This case report showed that a satisfactory clinical result was achieved by delivering bar overdenture immediately after implant placement without laboratory procedure.

A COMPARISON OF THE MASTICATORY FUNCTION BETWEEN TWO DIFFERENT TYPES OF IMPLANT SUPPORTED PROSTHESES AND COMPLETE DENTURE FOR FULLY EDENTULOUS PATIENTS

  • STATEMENT OF PROBLEM: The improvement in oral function and comfort from the dental implant appears to depend on the particular type of implant support used with the denture. The number and positioning of implants have an influence on the force transfer and subsequent stress distribution around implants. Nevertheless, a quantitative comparison has not been made between the types of implant prosthesis used with different materials compared to conventional complete denture. PURPOSE: The objective of this study is to assess the masticatory performance, bite force and impact of two different type of implant supported prostheses on oral health-related quality of life compared to conventional complete denture with GOHAI, validated oral-specific health status measures, the sieving method, and the Prescale Dental System. MATERIAL AND METHODS: From the years 1999 to 2006, a total of 30 completely edentulous patients in a single arch were selected from the Yonsei University Dental Hospital, Department of Prosthodontics and Implant Clinic in Seoul, S. Korea. Patients were divided into 3 groups of 10 each. Group HR was restored with fixed-detachable hybrid prostheses with resin teeth. Group FP had fixed dentures with porcelain teeth while Group CD had a complete denture. The masticatory performance was compared between 3 groups. RESULTS: The results showed a significant improvement in oral health-related quality of life with dental implants compared to a conventional denture in GOHAI comparison. Overall, implant prostheses showed a higher masticatory performance ($S_{50}$) and maximum bite force compared with conventional dentures (P < .05) but no differences between different implant supported prostheses (P > .05). CONCLUSION: Within the limitation of this study, the numbers of implant and material of implant prostheses does not appear to impact patient satisfaction, masticatory performance or bite force.

임시 의치와 이중 디지털 스캐닝 기법을 활용한 전악 고정성 임플란트 수복 증례 (Full mouth rehabilitation with fixed implant-supported prosthesis using temporary denture and double digital scanning technique: a case report)

  • 임플란트 고정성 보철물 제작 시 디지털 시스템을 이용하여 수복할 때, 3차원적인 임플란트 위치의 구내 스캔과 임시 의치 혹은 임시 보철물 장착 후 구내 스캔을 이용한 이중 디지털 스캐닝(double digital scanning) 기법이 필요하다. 구내 스캔 시, 스캔 바디를 안정적인 랜드마크로 이용하면 다지털 인상의 정확도 향상과 기공과정이 효율적이고 단순화될 수 있다. 본 증례는 완전 디지털 시스템을 활용하여 맞춤형 치과용 임플란트 지대주 및 임시 보철물, 최종 보철물을 계획하고 제작하였다. 임플란트 식립 후, 임플란트에 체결한 구내 스캔 바디와 임시 의치 조직면에 인기 된 구내 스캔 바디의 스캔 영역을 중첩하였다. 중첩된 파일에서 임시 의치의 수직 고경에 맞는 맞춤형 치과용 임플란트 지대주와 임시 보철물을 제작하여 환자에게 시적 하였고, 임시 보철물을 토대로 최종 보철물을 제작하였다. 임시 의치 제작 시 설정한 수직 고경, 중심위 등을 최종 보철물까지 활용하여 기능 및 심미적으로 만족할 만한 결과를 얻어 이를 보고하고자 한다.

Implant system의 매몰조건과 소환온도에 대한 조사 (Research for Implant system according to investment condition and Bum out temperature)

  • This study was investigated on effect of implant system about investment condition and burn out temperature. Posterior bridges were fabricated by using plastic sleeve and gold sylinder. The results were as follows; In plastic sleeve test and fitting cast copings. the phosphate bonded investment showed better gypsum bonded investment. The pure special liquid showed better margin fitness compared with mixed liquid (special liquid + distilled water). In gold cylinder test. the good burn out temperature of Ceramic PFG and Crown Gold was evaluated $0^{\circ}C$ to $820^{\circ}C$ and was decreased on $780^{\circ}C$ for 30 minutes. $0^{\circ}C$ to $750^{\circ}C$ and was decreased on $650^{\circ}C$ for 30 minutes.

A novel retentive type of dental implant prosthesis: marginal fitness of the cementless double crown type implant prosthesis evaluated by bacterial penetration and viability

  • PURPOSE. This study aims to compare the marginal fitness of two types of implant-supported fixed dental prosthesis, i.e., cementless fixation (CL.F) system and cement-retained type. MATERIALS AND METHODS. In each group, ten specimens were assessed. Each specimen comprised implant lab analog, titanium abutment fabricated with a 2-degree tapered axial wall, and zirconia crown. The crown of the CL.F system was retained by frictional force between abutment and relined composite resin. In the cement-retained type, zinc oxide eugenol cement was used to set crown and abutment. All specimens were sterilized with ethylene oxide, immersed in Prevotella intermedia culture in a 50 mL tube, and incubated with rotation. After 48 h, the specimens were washed thoroughly before separating the crown and abutment. The bacteria that penetrated into the crown-abutment interface were collected by washing with 500 µL of sterile saline. The bacterial cell number was quantified using the agar plate count technique. The BacTiter-Glo Microbial Cell Viability Assay Kit was used to measure bacterial adenosine triphosphate (ATP)-bioluminescence, which reflects the bacterial viability. The t-test was performed, and the significance level was set at 5%. RESULTS. The number of penetrating bacterial cells assessed by colony-forming units was approximately 33% lower in the CL.F system than in the cement-retained type (P<.05). ATP-bioluminescence was approximately 41% lower in the CL.F system than in the cement-retained type (P<.05). CONCLUSION. The CL.F system is more resistant to bacterial penetration into the abutment-crown interface than the cement-retained type, thereby indicating a precise marginal fit.
