PET/CT 영상에서의 치과재료에 의한 인공물에 관한 연구 (Evaluation of Artifacts by Dental Metal Prostheses and Implants on PET/CT Images: Phantom and Clinical Studies)

  • PET/CT 검사는 기술의 발전에 따라 감쇄보정 방법이 $^{68}Ge$이나 $^{137}Cs$등의 동위원소를 사용하지 않고, CT 기반의 감쇄보정 영상을 구현하여 검사 시간의 단축과 해부학적인 영상을 제공한다. 그러나 CT 기반의 금속에 의한 CT 영상의 인공물이 발생하여 감쇄보정된 PET 영상에 영향을 준다. 그러므로 본 연구는 임상 실험과 phantom 실험을 통해 금속 치과 임상 실험은 구강 내질환이 없는 40명의 환자(평균나이: $56{\pm}17$세)를 대상으로 하였으며, 치아에 치과보철을 매식한 환자 20명과 치과 임플란트를 매식한 환자 20명을 대상으로 PET/CT 검사를 시행했다. phantom 실험은 원형 phantom내에 치과보철과 치과 임플란트를 매식한 치아모형을 이용하여 PET/CT 검사를 시행했다. 분석방법은 같은 단층상의 PET/CT 영상에서 CT 영상의 CT 값과 PET 영상의 표준섭취계수 변화를 인공물의 영향이 없는 부분, 어둡게 인공물이 나타난 부분, 밝게 인공물이 나타난 부분, 세 부분의 관심영역을 설정하여 측정했고, 통계분석은 대응표본 t-test를 이용했다. 치아 보철을 매식한 실험에서 환자의 경우 표준섭취계수가 인공물의 영향이 없는 부분에 비해 어둡게 인공물이 나타난 부분은 약 19.6% (p<0.05) 감소됐고, 밝게 인공물이 나타난 부분은 90.1% (p>0.05) 증가했다. phantom의 경우 표준섭취계수가 인공물의 영향이 없는 부분에 비해 어둡게 인공물이 나타난 부분은약 18.1% 감소됐고, 밝게 인공물이 나타난 부분은 18.0% 증가했다. 치아 임플란트를 매식한 실험에서 환자의 경우 표준섭취계수가 인공물의 영향이 없는 부분에 비해 어둡게 인공물이 나타난 부분은 약 19.1% (p<0.05) 증가됐고, 밝게 인공물이 나타난 부분은 96.6% (p>0.05) 증가했다. phantom의 경우 표준섭취계수가 인공물의 영향이 없는 부분에 비해 어둡게 인공물이 나타난 부분은 약 14.4% 감소됐고, 밝게 인공물이 나타난 부분은 7.0% 증가했다. PET/CT 검사의 CT 기반 보정 영상 구현 시 금속 치과 재료로 인해 CT 값에 영향을 주어 PET 영상에서도 표준섭취계수에 영향이 미치는 것을 볼 수 있다. 그리고 치아 임플란트보다 치아 보철에서 과 보정이 된 것을 볼 수 있다. 특히, 임상 실험에서 치아 보철의 표준섭취계수가 어둡게 인공물이 나타난 부분이 19.6% 감소되어 나타난 것을 확인 할 수 있어 위 음성으로 오인할 가능성이 있다. 그러므로 금속 치아 보철이나 임플란트를 시행한 환자들의 PET/CT 검사 시 구강내 병변이 의심 된다면, CT 영상으로 보정이 되지 않은 무보정(non-attenuation correction) PET 영상을 확인하는 것이 정확한 진단에 도움을 줄 것으로 사료된다.

하악 구치부에 식립된 Br${\aa}$nemark 임프란트와 ITI 임프란트에서 임프란트간 치조정간골의 높이변화에 대한 방사선학적 비교 (RADIOGRAPHIC EVALUATION OF THE PROXIMAL BONE LEVEL BETWEEN TWO IMPLANTS : A 3-YEAR COMPARATIVE STUDY BETWEEN BR$BR{\AA}$NEMARK AND ITI IMPLANTS IN THE MANDIBULAR POSTERIOR REGION)

  • Statement of problem: Br${\aa}$nemark or ITI are two currently most widely used implant systems but with contrasting design, surgical and restoration methods. Purpose: The purpose of this study was to compare changes and its statistical significance in bone height and shape which may rise due to the differences between two implant systems. Also to analyse the effect of inter-implant distance on annual bone height changes. Material & Method: Those patients who were treated with two or more of either Br${\aa}$nemark or ITI implants at posterior mandibular area at Yonsei University Dental Hospital, Implant Clinic were selected. At annual examination appointments, standardised radiographs using parallel technique were taken. Marginal bone and inter-implant crestal bone changes were measured and following results were obtained. Results: 1) When ITI and Br${\aa}$nemark system were compared, both annual marginal and inter-implant crestal bone height changes in ITI system in the first two years were smaller than Br${\aa}$nemark and they were statistically significant. On the third year, however, there was no statistical difference between two implant systems on their annual bone level changes (p>0.05). 2) The Marginal and inter-implant crestal bone changes were compared when inter-implant distance was less than 4mm. Statistically significant bone level changes were noted on the first year only for ITI implants but in the first and second year for Br${\aa}$nemark implants (p>0.05). 3) When comparing angulation changes between marginal bone and implant fixture, ITI system had smaller angulation changes but the annual changes were not statistically significant (p>0.05). Conclusion: Within the limitation of this study, it could be concluded that Br${\aa}$nemark implant systems had more changes in marginal and inter-implant crestal bone level in the first and second year after loading with statistical significance. Further studies are recommended to see the effects of these bone loss during the first and second year after loading on the long term prognosis of Br${\aa}$nemark Implants.

Implant and root supported overdentures - a literature review and some data on bone loss in edentulous jaws

  • PURPOSE. To present a literature review on implant overdentures after a brief survey of bone loss after extraction of all teeth. MATERIALS AND METHODS. Papers on alveolar bone loss and implant overdentures have been studied for a narrative review. RESULTS. Bone loss of the alveolar process after tooth extraction occurs with great individual variation, impossible to predict at the time of extraction. The simplest way to prevent bone loss is to avoid extraction of all teeth. To keep a few teeth and use them or their roots for a tooth or root-supported overdenture substantially reduces bone loss. Jaws with implant-supported prostheses show less bone loss than jaws with conventional dentures. Mandibular 2-implant overdentures provide patients with better outcomes than do conventional dentures, regarding satisfaction, chewing ability and oral-health-related quality of life. There is no strong evidence for the superiority of one overdenture retention-system over the others regarding patient satisfaction, survival, peri-implant bone loss and relevant clinical factors. Mandibular single midline implant overdentures have shown promising results but long-term results are not yet available. For a maxillary overdenture 4 to 6 implants splinted with a bar provide high survival both for implants and overdenture. CONCLUSION. In edentulous mandibles, 2-implant overdentures provide excellent long-term success and survival, including patient satisfaction and improved oral functions. To further reduce the costs a single midline implant overdenture can be a promising option. In the maxilla, overdentures supported on 4 to 6 implants splinted with a bar have demonstrated good functional results.

THE EFFECT OF AUTOLOGOUS PLATELET-RICH PLASMA (PRP) ON BONE FORMATION AROUND DENTAL IMPLANT IN THE RABBIT: A HISTOMOR-PHOMETRIC AND REMOVAL TORQUE STUDY

  • Platelet-rich plasma(PRP) has been known to increase the rate and degree of bone formation by virtue of growth factors in concentrated platelets. Although its great healing effect on bone defect or pre-implantation site preparation in conjunction with bone substitute has been reported, the effect associated with implant is unknown. The purpose of this study was to investigate the effect of PRP on rapid osseointegration of endosseous dental implants in the rabbit tibiae. Twenty two adult female New Zealand white rabbits, weighing approximately 2.7-3.3kg, were used for this study. Twelve of the 22 animals were used for histomorphometric analysis and ten of the 22 were for removal torque test. Each animal received two implants in each tibia (two treated with PRP and two as control) and was given fluorochrome intramuscularly. For histomorphometric analysis, rabbits were divided into four groups according to the healing period. At 1 week, 2 weeks, 4 weeks and 8 weeks postoperatively, each three animals were sacrificed serially and the amount and rate of bone formation around dental implant were examined on the undecalcified sections under fluorescent microscope, polarized microscope and light microscope connected to a personal computer equipped with image analysis system. For removal torque test, rabbits were divided into two groups and removal torque tests were performed at 4 weeks, 10 weeks after implant placement. In total, 88 screw shaped, commercially pure titanium implants (Neoplant, Neobiotech, Seoul, Korea) were used in this study. Labeling pattern reflected differences of two groups in bone formation rate at each period. Histomorphometrically, PRP group showed significantly higher bone volume within threads compared to control group at 2 weeks ($70.30{\pm}4.96%$ vs. $50.68{\pm}6.33%$; P < .01) and 4 weeks ($82.59{\pm}5.94%$ vs. $72.94{\pm}4.57%$; P < .05 ). PRP group at 1, 2 and 4 weeks revealed similar degree of bone volume formation comparable to control group at 2, 4 and 8 weeks, respectively. On the other hand, while PRP group showed higher bone-implant contact ($47.37{\pm}8.09%$) than control group ($33.16{\pm}13.47%$) at 2 weeks, there were no significant differences between PRP group and control group for any experimental period. Removal torque values also showed no significant differences between PRP group and control group at any experimental period (P > .05). These findings imply that PRP could induce rapid, more bone formation around implant during early healing period and get faster secondary stability for reducing healing period, though it has not induced bone maturation enough to resist functional loading.

Influence of abutment height and convergence angle on the retrievability of cement-retained implant prostheses with a lingual slot

  • PURPOSE. Cement-retained implant prostheses can lack proper retrievability during repair, and residual cement can cause peri-implantitis. The purpose of this in vitro study was to evaluate the influence of abutment height and convergence angle on the retrievability of cement-retained implant prostheses with lingual slots, known as retrievable cement-type slots (RCS). MATERIALS AND METHODS. We fabricated six types of titanium abutments (10 of each type) with two different heights (4 mm and 6 mm), three different convergence angles ($8^{\circ}$, $10^{\circ}$, and $12^{\circ}$), a sloped shoulder margin (0.6 mm depth), a rectangular shape ($6mm{\times}6.5mm$) with rounded edges, and a rectangular ledge ($2mm{\times}1mm$) for the RCS. One monolithic zirconia crown was fabricated for each abutment using a dental computer-aided design/computer-aided manufacturing system. The abutments and crowns were permanently cemented together with dual-curing resin cement, followed by 24 hours in demineralized water at room temperature. Using a custom-made device with a slot driver and torque gauge, we recorded the torque ($N{\cdot}cm$) required to remove the crowns. Statistical analysis was conducted using multiple regression analysis and Mann-Whitney U tests (${\alpha}=.05$). RESULTS. Removal torques significantly decreased as convergence angles increased. Multiple regression analysis showed no significant interaction between the abutment height and the convergence angle (Durbin-Watson ratio: 2.186). CONCLUSION. Within the limitations of this in vitro study, we suggest that the retrievability of cement-retained implant prostheses with RCS can be maintained by adjusting the abutment height and convergence angle, even when they are permanently cemented together.

상악 전부 무치악 환자에서 CAD/CAM 시스템으로 제작되는 Pd-Ag 합금을 이용한 고정성 임플란트 전악 수복 증례 (Maxillary cement retained implant supported fixed prosthesis using the millingable Pd-Ag alloy generated by CAD/CAM system: clinical report after two years in service)

  • 골흡수가 심한 환자에서 임플란트를 이용한 전악 수복의 경우 치은과 치아의 외형을 심미적이면서도 기능적으로 회복하기 위해서는 보철물 제작에 있어 신중을 기해야 한다. 이러한 전악 수복 증례에서 CAD/CAM 시스템이 높은 정확성을 가지는 보철물 제작을 가능하게 함으로써 사용이 증가하고 있다. 최근에 임상에서 티타늄이나 지르코니아의 단점을 보완하면서 높은 생체적합성을 가지고 CAD/CAM 시스템으로 제작할 수 있는 팔라듐-은(Pd-Ag) 합금이 개발되어 사용되고 있다. 본 증례에서는 상악 전부 무치악 환자에서 임플란트를 식립하고 도재의 chipping이나 파절의 위험을 감소시키기 위해 Pd-Ag 합금을 이용해 제작된 임플란트 고정성 보철물로 수복한 환자에 대해 보고하고자 한다.

부분 무치악 환자에서 텔레스코프 시스템을 적용한 전악 수복 증례 (A Telescopic System and Its Clinical Application for the Restoration of the Partially Edentulous Arch)

  • Prosthodontic treatment planning for various edentulous arches is not an easy issue for dentists. Especially, in case of removable partial denture, we must have a knowledge of overall treatment procedures, and a careful approach is needed. Recently, interest of dental implant and case reports are increasing, the decrease of the removable partial denture is true, but dental implantation takes longer treatment period and it is more expensive. Also, there are still some limitations like lack of available alveolar bone, patient's general condition, and chronic periodontitis. Therefore, sometimes implantation is impossible. Finally, implantation cannot be adapted to every single patient. Currently, the clasp type removable partial dentures are used routinely, giving patients many unesthetic and functional difficulties. With better laboratory technique, removable partial denture with attachment and removable partial denture in rigid type increase patient's happiness level much more than predicted. The case presented in this article, clinically demonstrate the efficiency of using a telescopic system to improve esthetic and functional recovery for patients who lose multiple teeth.

THE MANAGEMENT OF A COMPLEX IMPLANT CASE USING CAD-CAM TECHNOLOGY: A CLINICAL REPORT

  • PURPOSE: The application of computer-aided technology to implant dentistry has created new opportunities for treatment planning, surgery and prosthodontic treatment, but the correct selection and combination of available methods may be challenging in times. Hence, the purpose of this case report is to present a combination of several computer-aided tools as approaches to manage complicated implant case. MATERIAL AND METHODS: A 47 year-old female patient with severe dental anxiety, high expectations, financial restrictions and poor compliance presented for a fixed rehabilitation. A CT scan with a radiographic template obtained with software (SimPlant, Materialize, Leuven, Belgium) was used for treatment planning. The surgical plan was created and converted into a stereolithographic model of the maxilla with bone-supported surgical templates (SurgiGuide, Materialise, Leuven, Belgium), that allowed for the precise placement of 7 implants in a severely resorbed edentulous maxilla. After successful osseointegration, an accurate scan model served as the basis for the fabrication of a one-piece milled titanium framework using the Procera (Nobel Biocare, Gothenburg, Sweden) technology. The final rehabilitation of the edentulous maxilla was rendered in the form of a screw-retained maxillary metal-reinforced resin-based complete prosthesis. RESULTS: Despite challenging circumstances, 7 implants could be placed without bone augmentation in a severely resorbed maxilla using the SimPlant software for pre-implant analysis and the SurgiGuide-system as the surgical template. The patient was successfully restored with a fixed full arch restoration, utilizing the Procera system for the fabrication of a milled titanium framework.

심한 치과공포증 환자에서 임플란트 식립을 위한 Propofol과 Remifentanil 진정법 -증례 보고- (Sedation for Implant Surgery using Propofol and Remifentanil in Severe Dental Phobia Patient -A Case Report-)

  • Anxiety and fear is two main factors that keep patients from going to dental clinic. Especially, patients may feel implants operations are more traumatic. Intravenous conscious sedation for dental treatment can make patient comfortable and relaxable. Midazolam is more popular for sedation for dental treatment, but target-controlled infusion (TCI) of propofol and remifentanil is gaining wide popularity. A 54-year-old female patient who had severe dental phobia was referred to our dental hospital. She had past history of 2 times of hyperventilation and syncope during dental treatment. The patient showed a lot of dental anxiety and fear to dental treatments and stress reduction protocol was needed. We administered intravenous conscious sedation using target controlled infusion system with remifentanil and propofol. During sedation, we monitored the status of consciousness with bispectral index and vital signs. Dental treatment could be finished successfully without any problems.

코로나바이러스감염증-19 상황에서 임상실습 중 치위생(학)과 학생의 감염관리 인식과 치과의료기관의 감염관리 수행정도 (Dental infection control in clinical practice institutions experienced by dental hygiene students in the COVID-19 situation)

  • The aim of this study was to investigate the level of dental infection control experienced by dental hygiene students in clinical practice institutions to identify problems and improve infection control in dental institutions. This study conducted online surveys targeting 269 dental hygiene students from universities that conducted clinical practice to determine the students' level of awareness regarding dental infection control in dental institutions and the reality of infection control in dental institutions. The results showed that dental hygiene students recognized the need for infection control and education about infection control at a high level. However, only 47% of the students were accurately informed about COVID-19. Basic instruments, periodontal instruments, and implant surgical instruments were sterilized after use for each patient, mostly by the institution, but 3-way syringe tips, preservation instruments and prosthetic instruments were more frequently reused without sterilization immediately after use. For dental infection control to be practiced at dental institutions, it is necessary to establish a systematic and safe infection control system, including infection control education, designation of infection managers, and provision of infection control guidelines.
