Recent advances in the reconstruction of cranio-maxillofacial defects using computer-aided design/computer-aided manufacturing

  • With the development of computer-aided design/computer-aided manufacturing (CAD/CAM) technology, it has been possible to reconstruct the cranio-maxillofacial defect with more accurate preoperative planning, precise patient-specific implants (PSIs), and shorter operation times. The manufacturing processes include subtractive manufacturing and additive manufacturing and should be selected in consideration of the material type, available technology, post-processing, accuracy, lead time, properties, and surface quality. Materials such as titanium, polyethylene, polyetheretherketone (PEEK), hydroxyapatite (HA), poly-DL-lactic acid (PDLLA), polylactide-co-glycolide acid (PLGA), and calcium phosphate are used. Design methods for the reconstruction of cranio-maxillofacial defects include the use of a pre-operative model printed with pre-operative data, printing a cutting guide or template after virtual surgery, a model after virtual surgery printed with reconstructed data using a mirror image, and manufacturing PSIs by directly obtaining PSI data after reconstruction using a mirror image. By selecting the appropriate design method, manufacturing process, and implant material according to the case, it is possible to obtain a more accurate surgical procedure, reduced operation time, the prevention of various complications that can occur using the traditional method, and predictive results compared to the traditional method.

  • Purpose: This study evaluated the prognosis and survival rate of SLA (Sandblasted, Large-grit and Acid-etched) implants and it also evaluated the prosthodontic complications and the associated factors. Methods: Twenty seven patients (14 men and 13 women, mean age: 54.9) who visited Chosun University Hospital Implant Center with the chief desire for placement of an implant in an edentulous area from March, 2008 to December 2008 and who received placement of a SLA implant ($Implantium^{(R)}$, Dentium Co., Korea) were selected for this study. Results: The average follow-up period was 15 months and the study was based on the treatment records, radiographs and clinical examinations. A total of 69 implant cases were retrospectively assessed for the width and length of the implant, the primary and secondary stability, the combined surgery, the employed bone graft material and barrier membrane, the status of the opposing tooth, implant failure and the prosthetic complications. During the follow-up period (average: 15 months), the accumulative survival rate of the 69 implants in 27 patients was 100%. Complications such as infection, sinusitis and fixture exposure after surgery were seen for 5 implants in 4 patients. Complications such as screw loosening, contact loosening and peri-implant gingivitis after prosthodontic treatment occurred in 7 cases (10.14%). Conclusion: This study reports placement of SLA implants may cause various complications, yet the final accumulative survival rate was 100%. The SLA implant ($Implantium^{(R)}$) has an excellent clinical survival rate and outcome.

THE EFFECT OF INTERNAL IMPLANT-ABUTMENT CONNECTION AND DIAMETER ON SCREW LOOSENING

  • Statement of problem. One of the common problems of dental implant prosthesis is the loosening of the screw that connects each component, and this problem is more common in single implant-supported prostheses with external connection, and in molars. Purpose. The purposes of this study were: (1) to compare the initial abutment screw detorque values of the six different implant-abutment interface designs, (2) to compare the detorque values of the six different implant-abutment interface designs after cyclic loading, (3) to compare the detorque values of regular and wide diameter implants and (4) to compare the initial detorque values with the detorque values after cyclic loading. Material and methods. Six different implant-abutment connection systems were used. The cement retained abutment and titanium screw of each system were assembled and tightened to 32Ncm with digital torque gauge. After 10 minutes, initial detorque values were measured. The custom titanium crown were cemented temporarily and a cyclic sine curve load(20 to 320N, 14Hz) was applied. The detorque values were measured after cyclic loading of one million times by loading machine. One-way ANOVA test, scheffe’s test and Mann-Whitney U test were used. Results. The results were as follows : 1. The initial detorque values of six different implant-abutment connections were not significantly different(p>0.05). 2. The detorque values after one million dynamic cyclic loading were significantly different (p<0.05). 3. The SS-II regular and wide implant both recorded the higher detorque values than other groups after cyclic loading(p<0.05). 4. Of the wide implants, the initial detorque values of Avana Self Tapping Implant, MIS and Tapered Screw Vent, and the detorque values of MIS implant after cyclic loading were higher than their regular counterparts(p<0.05). 5. After cyclic loading, SS-II regular and wide implants showed higher detorque values than before(p<0.05).

A STUDY ON THE CORRELATION BETWEEN IMPLANT STABILITY VALUES AND INITIAL INSERTION TORQUE

  • Statement of problem. Osseointegration is important mechanism of dental implant but it is not easy to evaluate. Indirect measurement is non-invasive and clinically applicable but they need more study about correlation between indirect values and degree of osseointegration. Purpose. The aims of this study were to evaluate the coefficient of correlation between indirect measurement and direct measurement under different healing time, and assessment of effect of initial insertion torque to the implant stability. Material and Methods. 20 rabbits received 3 implants on each side of tibia. Three kinds of implants (machined surface implant, Sandblasted with Large grit and Acid etched implant, Resorbable Blast Media treated implant) were used. During the surgery implant insertion torque were measured with $Osseocare^{TM}$. After the 1, 4, 8, 12 weeks of healing time, animals were sacrificed and stability values (Implant Stability Quotient with $Osstell^{TM}$, removal torque with torque gauge) were measured. Results. The Bone quality of rabbit tibia was classified into 2 groups according to the insertion torque. Resonance frequency analysis (ISQ) and removal torque showed positive correlation until $4^{th}$ week (r=0.555, p=0.040). After $8^{th}$ week (r=0.011, p=0.970) the correlation became weak and it turned negative at $12^{th}$ week (r=-0.074, p=0.801). Insertion torque and ISQ showed changing correlation upon the healing time ($1^{st}$ week: r=0.301, p=0.033, $4^{th}$ week: r=-0.429, p=0.018, $8^{th}$ week: r=0.032, p=0.865, $12^{th}$ week: r=-0.398, p=0.029). Insertion torque and removal torque has positive correlation but it was not statistically significant ($1^{st}$ week: r=0.410, p=0.129, $4^{th}$ week: r=0.156, p=0.578, $8^{th}$ week: r=0.236, p=0.398, $12^{th}$ week: r=0.260, p=0.350). Conclusion. In this study, bone quality may affect the degree of osseointegration positively during healing time and correlation between ISQ and degree of osseointegration can be different according to the healing time and bone quality.

임플랜트의 체결방식에 따른 초기조임력에 의한 응력분포 및 전하중에 관한 연구 (THREE-DIMENSIONAL FINITE ELEMENT ANALYSIS OF STRESS DISTRIBUTION AND PRELOAD OF DIFFERENT CONNECTION TYPES IMPLANT WITH INITIAL CLAMPING)

  • Statement of problem: One of common problems associated with single teeth dental implant prosthetic is the loosening of screws that retain the implants. Purpose: The maintenance of screw joint stability is considered a function of the preload achieved in the screw when the suggested initial tightening torque is applied. The purpose of this study was to investigate acquired preload after initial clamping torque for estimating screw joint stability. Material and methods: A comparative study on the effect of initial clamping of two types of implant systems with different connections was conducted Three dimensional non-linear finite element analysis is adopted to compare the characteristics of screw preloads and stress distributions between two different types of implant systems composed with abutment, screw, and fixture under the same loading and boundary conditions. Results: 1. When the initial clamping torque of 32Ncm was applied to the implant systems, all types of implants generated the maximum effective stress at the first helix region of screw. 2. Morse taper connection types of implants generate lower stress distributions compared to those by butt joint connection types or implants due to large contact surface between abutment and fixture. 3. The internal types of implant systems with friction grip type implant systems have higher resistance to screw loosening than that of the external types of implant systems since the internal types of implant systems generated larger preload than that generated by the external types for the same tightening moments.

Physicochemical characterization of porcine bone-derived grafting material and comparison with bovine xenografts for dental applications

  • Purpose: The physicochemical properties of a xenograft are very important because they strongly influence the bone regeneration capabilities of the graft material. Even though porcine xenografts have many advantages, only a few porcine xenografts are commercially available, and most of their physicochemical characteristics have yet to be reported. Thus, in this work we aimed to investigate the physicochemical characteristics of a porcine bone grafting material and compare them with those of 2 commercially available bovine xenografts to assess the potential of xenogenic porcine bone graft materials for dental applications. Methods: We used various characterization techniques, such as scanning electron microscopy, the Brunauer-Emmett-Teller adsorption method, atomic force microscopy, Fourier-transform infrared spectroscopy, X-ray diffraction, and others, to compare the physicochemical properties of xenografts of different origins. Results: The porcine bone grafting material had relatively high porosity (78.4%) and a large average specific surface area (SSA; $69.9m^2/g$), with high surface roughness (10-point average roughness, $4.47{\mu}m$) and sub-100-nm hydroxyapatite crystals on the surface. Moreover, this material presented a significant fraction of sub-100-nm pores, with negligible amounts of residual organic substances. Apart from some minor differences, the overall characteristics of the porcine bone grafting material were very similar to those of one of the bovine bone grafting material. However, many of these morphostructural properties were significantly different from the other bovine bone grafting material, which exhibited relatively smooth surface morphology with a porosity of 62.0% and an average SSA of $0.5m^2/g$. Conclusions: Considering that both bovine bone grafting materials have been successfully used in oral surgery applications in the last few decades, this work shows that the porcinederived grafting material possesses most of the key physiochemical characteristics required for its application as a highly efficient xenograft material for bone replacement.

임플란트-지대주-나사의 적합에 관한 연구 (A STUDY ON THE FIT OF THE IMPLANT-ABUTMENT-SCREW INTERFACE)

  • Statement of problem : There have been previous studies about considerable variations in machining accuracy and consistency in the implant-abutment-screw interfaces. Purpose : The purpose of this study was to evaluate the machining accuracy and consistency of implant/abutment/screw combinations on two randomly selected implants from each of four manufactures. Material and methods : In this study, screws were respectively used to secure a cemented abutment, to a hexlock implant fixture ; teflon coated titanium alloy screw(Torq-Tite) and titanium alloy screw in Steri--Oss system, gold-plated gold-palladium alloy screw(Gold-Tite) and titanium alloy screw in 3i system gild screw ana titanium screw in AVANA Dental Implant system, and titanium screws in Paragon System. The implants were perpendicularly mounted in polymethyl methacrylate autopolymerizing acrylic resin block(Orthodontic resin, Densply International Inc. USA) by use of dental surveyer. Each abutment screw was secured to the implant with recommended torque value using a digital torque controller. Each screw was again tightened after 10 minutes. All samples were cross sectioned with grinder-polisher unit(Omnilap 2000 SBT Inc) after embeded in liquid unsaturated polyester (Epovia, Cray Valley Inc) Results : There were the largest gaps in the neck areas of screws in hexagonal extension implants which were examined in this study. The leading edge of the abutment screw thread (superior surface) was in contact with the implant body thread, and the majority of the contacting surfaces were localized to the middle portion of the mating threads. Considerable variation in the contacting surfaces was noted in the samples evaluated. Amounts of contact in the abutment screw thread were larger for assemblies with Gold-Tite screw, gold alloy screw. Torq-Tite screw than those with titanium screws. The findings of intimate contact between the screw and screw seat were seen in all samples, regardless of manufacturers. However, microgap between the head and lateral neck surface of the screw and the abutment could be dectected in all samples. The findings of intimate contact between the platform of the implant and the bottom of the abutment were consistent in all samples, regardless of manufacturers. However, microgaps between the lateral surface of external hex of the fixture and the abutment could be dectected in all samples. Conclusion : Considerable variations in machining accuracy and consistency were noted in the samples and the implant-abutment-screw interfaces were incomplete. From the results of this study, further development of the system will be required, including improvements in pattern design.

심한 치조골 소실이 있는 상악 무치악 환자에서 Pekkton telescopic attachment를 이용한 임플란트 피개의치 증례 (A case of maxilla implant overdenture using Pekkton telescopic attachment with severe alveolar bone resorption)

  • 심한 치조골 흡수를 동반한 편악 무치악 환자의 보철 수복시 대합치열의 상태를 고려하여 올바른 교합평면을 설정하고, 교합력이 균일하게 분산될 수 있는 치료가 제공되어야 한다. 임플란트 유지 피개의치는 소수의 임플란트로 치조골 흡수가 심한 완전 무치악 환자에게 총의치보다 우수한 유지와 안정성을 주는 치료이다. 최근 치과 영역에서 소개된 신소재 polyaryletherketone (PAEK) 기반의 폴리머(Pekkton ivory, Cendres + $M{\acute{e}}taux$ SA, Biel / Bienne, Switzerland)를 사용한 telescopic 어태치먼트는 전형적인 telescopic 코핑의 이점을 가지며, 내마모성이 뛰어나며, 기존의 임플란트 피개의치에 비해 가볍고 경제적인 장점이 있다. 본 증례에서는 상악 완전 무치악 환자에 임플란트를 5개 식립하여, Pekkton으로 내 외관이 제작된 텔레스코픽 어태치먼트를 활용한 임플란트 유지 피개의치를 제작하고, 하악은 임플란트 고정성 보철로 수복하였다. 심미적 및 기능적으로 만족스러운 결과를 얻었기에 이를 보고하는 바이다.

Screw joint stability according to abutment screw materials

  • Statement of problem. There have been previous studies about instability according to screw material by means of calculating preload in tightening screw or recording of the torque necessary to loosen screw after tightening screw. Purpose. The purpose of this study was to evaluate screw joint stability through the analysis of fitness at the mating thread surfaces between implant and screw after tightening screws made of different materials. Material and methods. In this study, screws were respectively used to secure a cemented abutment to a hexlock implant fixture; teflon coated titanium alloy screw and titanium alloy screw(Steri-Oss), gold-plated gold-palladium alloy screw and titanium alloy screw(Implant Innovation), gold screw and titanium screw(AVANA Dental Implant System). Each abutment screw was secured to the implant with recommended torque value using a digital torque controller. Each screw was again tightened after 10minutes. All samples were cross sectioned with sandpaper and polished. Then samples were evaluated with an scanning electron microscope analysis. Results. In titanium alloy screw, irregular contact and relatively large gap was present at mating thread surface. Also in teflon-coated titanium screw, incomplete seating and only partially contact was present at the mating thread surface. In gold-plated gold-palladium alloy screw, relatively close and tight contact without the presence of large gap was present by existing of gold coating at the mating thread surfaces. In gold alloy screw, relatively small gap between the mating components was seen. Conclusions. This result suggested that gold plated gold-palladium alloy screw and gold alloy screw achieved a greater degree of contact at the mating thread surfaces compared to titanium alloy screw and teflon-coated titanium alloy screw.

지르코니아의 생체적합성과 임플란트로서의 생체활성에 대한 연구: In vivo 실험 문헌 고찰 (A review of biocompatibility of zirconia and bioactivity as a zirconia implant: In vivo experiment)

  • 심미적 치료에 대한 요구가 늘어나면서 높은 강도와 심미성을 갖는 지르코니아의 요구도 증가하고 있다. 이러한 흐름에 비추어 지르코니아의 생체적 합성을 평가하는 것은 중요한 일이다. 이번 논문에서는 지르코니아의 생체적합성에 대한 in vivo 실험에 대한 문헌 연구를 진행하였다. In vivo 실험에서 연조직, 경조직에 대한 지르코니아의 생체적합성을 확인할 수 있었다. 다양한 실험동물 및 환자에서 진행된 연구의 대다수에서 지르코니아의 높은 생체적합성이 보고되었으며, 신생골 합성 및 골부착의 면에서 티타늄과 유사한 성질을 보였다. 한편, 지르코니아는 임플란트로도 활용할 수 있다. 임플란트로 활용하기 위해 HA (hydroxyapatite)를 처리하여 생체활성을 높이는 다양한 방식이 제안되고 있다. 하지만 기존의 티타늄 임플란트에 HA를 코팅하는 방식은 낮은 결합강도 및 HA의 변성으로 인한 문제점이 있었기 때문에 HA-지르코니아 composite, HA-coated 지르코니아, HA-지르코니아 functionally graded material (FGM) 또는 알루미나 개재 HA-지르코니아 등의 새로운 방식이 연구되고 있다. 이러한 방식들은 보다 높은 결합강도를 지니고 있으며, 높은 생체적합성을 보여주고 있다.
