Dental IQ and Oral Health Care Status of Elementary School Students (초등학생의 Dental IQ 수준과 구강보건 관리실태)

  • Objectives: Oral health care program for the elementary school children (ESOHCP) should be met the first priority because dental caries are highly sensitive and prevalent among them. This study was performed to enhance the efficiency of ESOHCP, and their oral health promotion. Methods: The Subjects were 346 students (3rd grade 163, 6th grade 185) of one elementary school in Busan. Dental IQ and actual oral health status; the decayed, mixing and filling teeth were checked through questionnaire and oral health examination from April to June 2002. Dental IQ was presented out of one hundred, and data analysis was done using SAS (ver 8.1) program. Results: The students' cognitive level about the causative and preventive factor of dental caries was relatively high. However, the practical aspects of preventive behavior - tooth brushing; three times per day (20%), three minutes per time (22%), oral health examination; one time per three months (10%) left much to be desired. And, only 27% of the subjects had experienced in school oral health education. The mean level of dental IQ was 79 out of 100, and 51 % and 42% of them had the decayed and teeth with filling, respectively, with the rate of DMFT 82%, DT 43%, FT 57%. The level of dental IQ was higher in case of having his (her) own tooth brush (p=0.072), standard tooth brushing (p<0.001) three times per day, post meal, three minutes per time, present experience of oral health examination (p<0.001) and dental clinic visit (p<0.001). The grade of caries was more serious in case of 6th grade (p=0.059), an absence of his (her) own tooth brush (p= 0.090), present experience of oral health examination (p=0.021), and an absence of regular dental clinic visit (p=0.003). The frequency of oral health examination (γ= 0.620), tooth brushing; times per day (γ=0.445), post meal (γ=0.355), expending times per brushing (γ=0.352), right cognition to the treatment of caries (γ=0.401), positive attitude to dental treatment (γ=0.387), the frequency of dental clinic visit for the past one year (γ=0.152) showed significant correlation with dental IQ. In the multivariate analysis, dental IQ was influenced by the frequency of oral health examination, right cognition to the cause of caries, times of teeth brushing per day, right cognition to the treatment of caries, etc., with adjusted R2=0.857. Conclusion: Though the students' cognitive level about the causative and preventive factor of dental caries was high, the practical aspects of preventive behavior left much to be desired. Ant the current ESOHCP considered to be still inefficient. However, frequent dental visits had apparent correlation with hish level of dental IQ. The specialized and practical program should be needed for the efficient ESOHCP. Harmonized effort from educational, health and dental society is essential.

Comparison vibration characteristics of several wireless endodontic handpieces (여러 근관치료용 무선 핸드피스의 진동양상 비교)

  • Purpose: Wireless endodontic handpieces (WEH) are widely used in dental clinics due to their convenience and portability. This study aimed to compare the vibration magnitudes and patterns generated by five WEH. Materials and Methods: Vibration acceleration of five WEH (X-Smart IQ, E connect S, Endo A Class, ENDOIT, and TRAUS ENDO) in the rotary and reciprocating motion was measured with accelerometer The average vibration acceleration was analyzed using the t-test, Welch's ANOVA test, and Dunnett T3 test at P < 0.05. Results: In all WEH, the average vibration acceleration in reciprocating motion was significantly higher than that in rotary motion (P < 0.001). In rotary motion, repeated vibration graphs of constant amplitude were obtained without sudden changes in the magnitude of vibration, and the average vibration acceleration value was high in the order of X Smart IQ, Endo A Class, ENDOIT, E Connect S, and TRAUS ENDO (P < 0.001), there was no statistically significant difference between X Smart IQ and Endo A Class. In reciprocating motion, a vibration graph was obtained in which large amplitude peaks appear at specific points within one cycle are repeated. The average vibration acceleration value was highest in the order of X Smart IQ, E Connect S, Endo A class, ENDOIT, and TRAUS ENDO (P < 0.001). Conclusion: Regardless of the type of WEH, greater vibration occurred in the reciprocating motion than in the rotary motion (P < 0.001). In the reciprocating motion, there was a difference in vibration for all handpieces (P < 0.001).

A Study on the Relationship Between Halitosis and Dentures (불쾌감을 유발하는 구취와 의치장착의 상호관계)

  • The main purpose of this study was to observe the relationship of denture and halitosis and to investigate the influence with oral health indexes. Also we examined the contribution factors to subjective awareness of halitosis by denture condition. A total sample of 103 elderly people were answered questionnaire and all collected survey data were processed for data analyses. Based on general characteristics, the study of the halitosis showed higher score which is older age, oral health attitude, and living together. The result showed that the use of denture, dental intelligent quotient (dental IQ) and type of denture were significantly correlated with halitosis. These results indicated that halitosis was affected by denture and oral health index. According to these findings, it is necessary to develop and apply the oral health promotion program which is include the management of halitosis especially for elderly person.

Adhesion between heat-pressed lithium disilicate veneer and zirconia framework: Shear bond strength evaluation (열가압 리튬 디실리케이트 전장도재와 지르코니아 하부구조의 전단결합강도 평가)

  • This study examined the shear bond strength between the zirconia core and pressed lithium disilicate veneering ceramics. The Schmitz-Schulmeyer test method was used to investigate the core-veneer shear bond strength of industrially manufactured zirconia core ceramic (Zirtooth, HASS, Gangneung, Korea) and pressed veneer ceramic (IPS e.max Zirpress, Vita PM9, GC Initial IQ, HASS Rosetta SM) (N=40). Data were statistically analyzed using one-way ANOVA and Tukey's test (a=0.05). The fractured surfaces of the specimens were examined to determine the failure pattern using a digital microscope. The mean ± SD shear bond strength in MPa were 16.69±3.11, 14.21±3.63, 11.17±2.92, and 27.90±5.71 for IPS e.max Zirpress, VITA PM9, GC Initial IQ, and HASS Rosetta SM, respectively. The average shear bond strength was largest for HASS Rosetta SM, followed by IPS e.max Zirpress, Vita PM9, and GC Initial IQ(p<0.05). The digital microscopy examination of the fracture surface showed adhesive and cohesive failure in pressed lithium disilicate veneering ceramics. The use of lithium disilicate veneer ceramic produced a significantly higher shear bond strength.

A Study on the Effect of Oral Health Education on the Mentally Retarded Children (정신지체장애인의 구강보건교육 효과에 관한 연구)

  • The purpose of this study was to develop effective oral health education programs for mentally retarded children and promote their oral health, by offering oral health education for 45 mentally retarded children between age 6 to 20, tracking the change of their knowledge depending on the frequency of education, and examining the educational effect before and after oral health education. The children with mental retardation attended a special school for idiots in Gweonseon-gu, Suwon, Kyonggi Province, being able to take training(IQ 25-49). The education program was designed to be suitable for their cognitive power after consultation with a special school teacher. A teacher provided the same education seven times, once a week, and an interview was held with each of them to assess their correct answer rate. The findings of this study were as below: 1. The repeated oral health education served to have the children with mental retardation acquire better knowledge about harmful food for the teeth, what had to be done after eating cookies or candies between meals, the right time for toothbrushing, the concept of dental caries, and how to cope with dental caries(p<0.01). But after that education was offered four times, the frequency of that education made no difference. 2. The repeated oral health education increased, their knowledge on the role of the teeth and the right choice of toothbrush(p<0.01), yet there was no significant difference in their knowledge about oral health behavior, because they had already been familiar with that. 3. As a result of investigating the change of their oral health know-ledge before and after oral health education according to the type of handicap, the type of handicap made no significant difference to the change of their oral health knowledge. 4. The oral health education for the children with menial retardation had a significantly different effect on their knowledge about harmful food for the teeth, what had to be done after eating between meals, the right time for toothbrushing, the role of the teeth. the right choice and use of toothbrush, how to do toothbrushing, and fluorine(p<0.01).

Factors affecting modulation transfer function measurements in cone-beam computed tomographic images

  • Purpose: This study was designed to investigate the effects of voxel size, the oversampling technique, and the direction and area of measurement on modulation transfer function (MTF) values to identify the optimal method of MTF measurement. Materials and Methods: Images of the wire inserts of the SedentexCT IQ phantom were acquired, and MTF values were calculated under different conditions(voxel size of 0.1, 0.2, and 0.3 mm; 5 oversampling techniques; simulated pixel location errors; and different directions and areas of measurement). The differences in the MTF values across various conditions were evaluated. Results: The MTF 10 values showed smaller standard deviations than the MTF 50 values. Stable and accurate MTF values were obtained in the 0.1-mm voxel images. In the 0.3-mm voxel images, oversampling techniques of 11 lines or more did not show significant differences in MTF values depending on the presence of simulated location errors. MTF 10 values showed significant differences according to the direction and area of the measurement. Conclusion: To measure more accurate and stable MTF values, it is better to measure MTF 10 values in small-voxel images. In large-voxel images, the proper oversampling technique is required. MTF values from the radial and tangential directions may be different, and MTF values vary depending on the measured area.

Comparison of shear bond strength according to porcelain build-up methods (도재 축성 방법에 따른 금속 도재관의 전단결합강도 비교)

  • Purpose: This study compared the shear bond strength of heat pressed and feldspathic porcelain to metal. Through thermocycling, the clinical aspect of heat pressed porcelain fused metal was estimated. Materials and Methods: 90 non-precious metal specimens were made ($4{\times}4{\times}8 mm$) and divided to three groups. All spicimens were treated and built-up with the porcelain ($4{\times}4{\times}3 mm$) by 2 different methods according to group: Group I: $Inspiration^{(R)}$, Group II: Ivoclar, IPS $Inline^{(R)}PoM$, Group III: GC Initial IQ-One $Body^{(R)}PoM$. The half of each group's specimens were thermocycled. All specimens' shear bond strength were measured by Instron universal testing machine. Exact measuring point was far 1 mm from porcelain/metal interface to the porcelain side. For the statistical analysis, 2-way ANOVA was used. Results: In no-thermocycling specimens, the shear bond strength showed no statistical significance between each group (P > 0.05). In comparison between nothermocycling and thermocycling specimens in each group, the shear bond strength was decreased according to thermocycling, but there was no statistical significance (P > 0.05). In thermocycling specimens, there was no statistical significance between each group (P > 0.05). Conclusion: In feldspathic porcelain and other two types heat pressed porcelain, there was no statistical difference in the shear bond strength of porcelain to metal. The heat pressed porcelain seems to be clinically useful for the aspect of the shear bond strength.

A Case of Cockayne Syndrome with Focal Segmental Glomerulosclerosis (국소성 분절성 사구체 경화증(FSGS)을 동반한 Cockayne 증후군 1례)

  • Cockayne syndrome is a rare autosomal recessive disorder characterized by cachectic dwafism, mental retardation, loss of facial subcutaneous adipose tissue, microcephaly and photosensitive dermatitis. It is associated with renal abnormalities characterized by hyalinization of glomeruli, atrophy of tubules and interstitial fibrosis. To our knowledge, this is the first report of a case of Cockayne syndrome with FSGS in Korea. A 7-year old boy was admitted for evaluation of hypertension and proteinuria, which were detected 2 month ago. He was followed for short stature(<3 percentile), mental retardation(IQ 55), strabismus and dental caries since 3 years ago. He also showed microcephaly, a bird-like face and relatively large hands and feet. Laboratory findings showed decreased creatinine clearance($C_{Cr}$ 76.1 mL/min/$1.73m^2$) and proteinuria(1,548 mg/day). Renal biopsy demonstrated focal segmental glomerulosclerosis of the hilar type with large hyaline deposits, moderate tubular atrophy and interstitial fibrosis. His cardinal features, mental retardation, and renal biopsy findings were consistent with Cockayne syndrome. We report here a very rare case of Cockayne syndrome with FSGS presenting with proteinuria and hypertension.
