장애인 구강건강관리인력에 따른 구강환경관리능력 지수 비교 (The comparison of Patient Hygiene Performance(PHP) Index according to the number of Oral Health Care worker with Disabled)

  • Objectives: 현재 장애인의 구강건강은 구강건강전문인력인 치과위생사가 아닌 사회복지사가 책임지고 있는 실정이다. 장애인 복지시설에 상주하는 사회복지사에게 올바른 구강건강 관리방법을 제공함으로써 장애인 구강건강의 형평성 제고를 도모하고자 한다. Methods: 2019년 4월 13일부터 2019년 4월 20까지 서울시 송파구에 위치한 'o' 장애인 복지시설에 거주하는 1급 지적장애인을 대상으로 식후 치과위생사 4명과 사회복지사 4명이 잇솔질 시행 후 구강환경관리능력지수(PHP Index)를 측정하여 비교하였다. 사전에 사회복지사 4명에게 잇솔질(회전법)을 교육하였으며, 잇솔질 방법과 검사 측정 도구를 통일하였다. 대상자의 구강보건지식을 평가하기 위해 자기기입식 설문지를 사용하였고, 구강보건지식점수 및 구강환경관리능력지수(PHP index) 점수의 평균, 대상자의 일반적 특성에 대해 Microsoft Office Excel ver.2016 을 이용하여 빈도 및 백분율 산출하였다(N%). Results: 치과위생사와 사회복지사의 구강보건지식 조사에서 모든 문항에서 치과위생사가 높은 점수를 나타냈다. 치과위생사와 사회복지사가 지적장애인을 대상으로 잇솔질(회전법) 실시 후 구강환경관리능력지수(PHP Index)를 비교했을 때 연령별, 성별 모든 분류에서 치과위생사의 구강환경관리능력지수(PHP Index)가 낮게 나타났다. 2차에 걸쳐 치과위생사와 사회복지사가 지적장애인을 대상으로 잇솔질(회전법) 실시 후 구강환경관리능력지수(PHP Index)를 비교했을 때 1차, 2차 모두 치과위생사의 구강환경관리능력지수(PHP Index)가 낮게 나타났다. Conclusions: 이상의 결과를 종합해 볼 때, 장애인들의 구강건강관리를 위해 치과위생사가 사회복지사보다 구강건강관리능력이 더 효율적인 것으로 나타났다. 따라서 장애인들의 입소시설에 치과위생사가 구강건강관리전문 인력으로 배치되어 장애인 및 사회복지사의 교육을 담당할 수 있는 환경이 조성되어야하며, 치과위생사를 양성하는 대학의 교육과정에도 장애인의 특성을 이해하는 교과목이 개설되어 전문성이 제고되어야 한다.

치과위생사에 관한 일부 의료종사자의 인지도 조사 (Recognition of Health Care Workers for Dental Hygienists)

  • This study involved an investigation of perception of dental hygienists based on a self-reported survey of a sample of 230health care personnel working at long-term care hospitals located in Gyeonggi Province, Korea. The primary objective was to provide basic data for establishing various policies related to building up a positive image of dental hygienists as a profession. The results were as follows. 79.1% of the respondents recognized dental hygienists as a profession. Among the titles for a dental hygienist, "teacher" was the most common with 47.4% of all. Female respondents and respondents who were able to discern between dental hygienists and nursing assistants were more likely to perceive dental hygienists positively than male respondents and ones who were not, respectively. Among social perceptions of dental hygienists was there a negative finding, that is, there was no appropriate title to address dental hygienists. This negative finding indicates that there is a vital need to enhance the perception of who are dental hygienists as a professionals level at the individual as well as institutional level.

근로자의 구강보건에 대한 인식과 실천정도 조사 - 일개 사업장을 중심으로 - (A study on awareness and practice about the oral health of workers)

  • Objectives : The purpose of this study was to examine the oral health awareness and practice of workers. Methods : The subjects in this study were 200 workers in a working place located in Taean-gun, Chung-Nam Province. The data for this study was obtained by means of a self-administered questionnaire. Results : 1. An average of awareness for the oral health of workers was $2.60{\pm}0.44$ points out of 4. They were most aware of the item that toothbrushing should be done after meals($3.06{\pm}0.42$), and the second most widely recognized item was that toothbrushing should be done before bedtime($2.73{\pm}0.60$). 2. An average of practice for the oral health of workers was $2.54{\pm}0.32$ points out of 4. They did the best was brushing their teeth after meals($2.97{\pm}0.33$), followed by cleaning the tongue during toothbrushing($2.91{\pm}0.45$). 3. Awareness of oral health was significantly higher in such groups, including the female, college graduates, unmarried(p<0.05). 4. Practice of oral health was significantly higher in such groups, including the female, college graduates(p<0.05). Conclusions : To reform and improve of the system, incremental oral health care system for workers is needed. and oral health education is needed to increase the motivation of workers to control their basic dental disease.

치과위생사의 감염성 폐기물 처리실태 (A research on the actual condition on Dental Waste Treatment of dental hygienes)

  • Dental personnels faced risks of infection in the clinic. For infection control, recognition and practice of dental personnels are important factor. This study was performed to investigate the recognition and practice of dental hygiene for infection control and infection waste control. A stratified convenience sample of dental hygienists in dental health-care settings. The major finding of the present study are as follows: 1. The existence of education about Standard Precaution and low of infection waste storage was higher dental hospital than dental clinic. 2. The degree of practice in the storage of dental wastes was low in absorbent cotton and body tissue exclude damage waste. And the degree of practice in the disposal of dental wastes was high in all three. 3. Practice in the storage of dental waste was higher dental hospital than dental clinic. 4. At the conclusion of this investigation, systematic refresher training of infection control should be prepared by campaign an various media, Dental health care workers should be encouraged to practice those action items from training. For successful implementation of infection control in every dental health-care settings, it is highly demanded as well that development of effective safe-guard tools, stategic support, and standardized action items against infection problems.

우리나라 치과 외래의료비 지출규모와 치과 외래의료비 지출에 미치는 요인 (Expenditure in ambulatory dental care and factors related to its spending)

  • This study estimates the total health expenditure of ambulatory dental care and explores the factors related to disbursements. The study used two waves of a 2008 Korea Health Panel (KHP) survey, of which each wave is composed of 7866 households and 24,659 persons. The KHP includes missing expanses of reimbursement data of the National Health Insurance (NHI), such as out-of-pocket, drugs, and private health insurance. The study estimates total monthly ambulatory dental expenditure and the sub-special categories of dental care. For influential factors analyses, the study exploits log-linear model with age, gender, education, job, equivalence income, the status of chronic diseases, means-tested benefit recipients, private insurance, and the composite deprivation index as independent variables. The total monthly outpatient health spending is estimated to be 102,468 won per household, and for dental, each household spends 31,115 won per month. Older age, means-test recipients, non-regular workers are more likely to spend less money on dental care, whereas private insurers, high income, and those who live in less deprived areas are more likely to spend more money for dental services. From the study we found that the KHP data are more suitable to estimate the total amount of health care markets, especially when the NHI coverage is low, such as for dental care in Korea.

치과의사와의 관계에서 치과위생사와 간호조무사의 직종 간 갈등 연구 (A study on conflicts between different occupational categories of dental hygienists and nursing assistances in terms of relationships with dentist)

  • The purpose of this study was to examine the state of conflicts among dental health care workers. A survey was conducted on 266 dental hygienists and nursing assistants who worked in dental institutions from September 12 to November 13, 2017, and SPSS(statistical package for the social science) version 20.0 was employed to analyze the collected data. The findings of the study were as follows: 1. The most common reason of the dental hygienists for turnover was working hours and heavy workload(24.6%), followed by pay (22.6%), conflicts with dentists(16.0%) and conflicts with colleagues (11.3%). The most dominant reason of the nursing assistants for turnover was pay(31.1%), followed by working hours(24.4%), heavy workload(17.8%), conflicts with dentists(15.6%) and conflicts with colleagues(8.9%). 2. The largest reason for unsuccessful communication with dentists was that heavy workload reduced the opportunity to communicate well(54.5%). The second biggest reason was that they couldn't communicate well though they had the opportunity(24.0%), and the third greatest reason was that they tended to lag behind dentists in terms of professional knowledge(16.9%). 3. The biggest reason for unsuccessful communication among the dental health care workers was that they didn't have a lot of chances to communicate well on account of heavy workload(41.0%). The second largest reason was the differences in professional knowledge(24.9%), and the third greatest reason was that they couldn't communicate well though they had the chance(23.7%). 4. The most dominant reason for conflicts with dentists was the difference in power(24.0%), followed by poor communication skills(22.1%) and a lack of mutual respect(18.1%). But the opinions of the nursing assistants were different from those of the dental hygienists, as they cited poor communication skills as the most common reason, which was followed by the difference in power and a shortage of understanding of each other's work. 5. The most common reason for conflicts among the dental health care workers was a shortage of communication and communication skills(22.9%), and the second most dominant reasons were a lack of mutual respect and poor understanding of each other's work(17.5%), followed by a lack of mutual respect(17.2%). 6. As to the ways of resolving conflicts with dentists, the most common case was making some mutual concessions to compromise (28.9%), followed by delivering opinions through the staff meeting (23.9%), resolving conflicts by candidly exchanging opinions(15.8%), avoiding each other in moderation(11.7%) and following the opinions or assertions of dentists(1.3%). 7. Concerning the conflict resolution methods among the dental health care workers, the most prevalent way was making some mutual concessions to compromise(36.4%), followed by resolving conflicts by candidly exchanging opinions(23.0%) and conveying opinions through the staff meeting(18.5%). 8. Regarding communication among the dental health care workers, the dental hygienists(3.53±.729) considered themselves to be better at communicating than the nursing assistants(3.29±.745) did(p<0.05), and the dental hygienists(3.45±.809) who thought there was respectful treatment among workers who were different in occupational categories found themselves to be better than the nursing assistants(3.21±.952) who had the same thought did(p<0.05). As a result of analyzing whether frequent job-related meetings occurred among the workers whose occupational categories were different, the dental hygienists(3.05±.975) perceived that there were more frequent meetings than the nursing assistants(2.67±.955) did (p<0.01).

근로자들의 건강증진 생활양식에 따른 구강건강 실천요인 (Relationship of health-promotion life style to the factors of oral-health practice among workers)

  • Objectives : The purpose of this study was to examine the influential factors for the oral-health practice of adult workers and the relationship of their health-promotion life style to the factors of oral-health practice in an attempt to determine the impact of health-promotion life style. Methods : The subjects in this study were 160 workers who worked in Seoul, North-Jeolla and South-Jeolla Province, and were at the age of 20 and up. The survey was conducted from February 1 to July 20, 2011. Results : As for the relationship of the general characteristics of the workers to the factors of their health-promotion practice, gender made statistically significant differences to toothbrushing female (3.24), and there were statistically significant differences in education/concern according to more than college(2.72) scored higher academic credential(p<0.05). The group of the workers whose health-promotion life style was better in terms of all the toothbrushing(3.35), use of oral hygiene supplies(2.19), regular dental-clinic visit(2.70), dietary control(2.84) and education/concern(3.20) scored higher, and there were statistically significant differences in the factors of education/concern(p<0.05). Their health-promotion life style had an impact on regular dental-clinic visit among the factors of oral-health practice(p<0.05), and there was a positive correlation between health-promotion life style and tooth brushing, dietary control(p<0.05), regular dental-clinic visit, education and concern(p<0.01) the factors of oral-health practice. Conclusions : Workers should be stimulated to get into sound life habits to change their behavior, and they should be urged to improve their preventive health care, to get a dental checkup and ultimately to promote their health as well. Companies should take measures to accelerate the oral health promotion of workers and provide more dental checkup programs for them to be more concerned about their oral health, to promote their oral health and to maintain the best oral health.

Do depression and its associated factors differ in women daytime and shift workers?: an analysis of the Korea National Health and Nutrition Examination Survey 2018

  • Purpose: This study examined health behaviors, use of health services, and depression among women who perform daytime and shift work in Korea, as well as factors related to depression. Methods: We conducted a secondary analysis using data from the 2018 data of the 7th Korea National Health and Nutrition Examination Survey. Data on women, 1,493 regular daytime workers and 322 shift workers, were analyzed. Results: Women shift workers (χ2=43.97, p<.001), had a lower education level (χ2=45.56, p<.001), and lower personal income (χ2=16.85, p=.030) than their daytime counterparts. A higher proportion of shift workers were unmarried (χ2=37.47, p<.001) and they typically worked fewer than 40 hours per week (χ2=69.94, p<.001). The depression score of shift workers was higher than that of daytime workers (t=2.85, p=.005). A higher proportion of shift workers also drank alcohol (χ2=6.49, p=.032) and smoked (χ2=30.79, p<.001). Over 8% of shift workers typically slept fewer than 5 hours per night (χ2=14.17, p=.024). It was confirmed that depression in women shift workers was affected by age, personal income, marital status, health status, and smoking status, in addition to cancer screening participation, unmet medical care needs, and unmet dental care needs. Conclusion: More attention should be given to the health needs of women working shifts. Health promotion programs specific for women shift workers are needed to improve their physical and mental health, encourage use of medical care services, and improve public health policies and systems.

수도권 지역 산업체 근로자들의 주관적 구강건강인식과 치과치료가 근로 손실에 미치는 영향 및 관련 요인 (A Study on the Effect and Related Factors of Self-Perceived Oral Health Awareness and Dental Care on Work Loss of Workers in the Capital Region)

  • 본 연구는 산업체 근로자들이 주관적 구강건강에 대한 인식을 조사하고 구강질환치료에 적극적으로 참여하지 못하는 요인과 치과치료로 인한 근로 손실에 대한 영향 및 요인을 연구하여 산업체 근로자들의 구강건강증진 정책 마련과 구강병 예방을 위한 기초자료를 마련하고자 한다. 서울, 경기 인천 등 수도권 지역의 근로자 총 284명을 대상으로 설문 조사를 하였다. 산업체 근로자들의 주관적인 구강건강에 대한 인식은 건강한 편이라고 느끼는 대상자가 36.9%로 건강하지 못한 편(18.7%)보다 많았다. 구강병 치료경험이 있는 근로자가 82.0%, 치료가 필요한 구강증상이 있는 근로자가 63.7%, 치과치료로 인한 근로 손실 경험은 있다가 81.0%로 나타났다. 성별로는 여자가 남자보다, 연령이 많을수록, 미혼보다 기혼이 치과치료로 인해 결근이나 조퇴를 한 경험이 높게 나타났다(p<0.05). 근무기간별로는 20년 이상인 근로자와 하루 평균 근무시간이 많은 근로자일수록, 일주일에 초과 근무를 5~9시간 하는 근로자가 경험이 높게 나타났고, 비제조업보다 제조업 근로자가, 사무직보다는 생산직이, 정규직보다 비정규직이 치과치료로 인한 결근이나 조퇴 경험이 높게 나타났다. 주관적 구강건강인식에서 건강하지 못하다고 인식할수록 근로 손실이 높게 나타났고(p<0.001), 구강병 치료경험이 있는 근로자가 그렇지 않은 근로자보다 높게 나타났다(p<0.001). 치료가 필요한 구강증상이 있는 근로자가 그렇지 않은 근로자보다 높게 나타났다(p<0.01). 산업체 근로자의 치과치료로 인한 근로 손실에 영향을 미치는 요인은 월 평균 소득이 많을수록, 주관적 구강건강상태가 건강하지 못하다고 인식할수록 치과치료로 인한 결근이나 조퇴의 경험이 많았다(p<0.001). 이상의 결과를 통해 산업체 근로자들의 구강질환으로 인한 치과치료가 근로 손실로 이어지지 않도록 구강보건교육을 통해 구강건강에 대한 중요성을 인식시키고 조기 치료와 예방을 위한 근무환경 개선과 근로자들의 지속적인 구강건강증진정책 마련이 선행되어야 한다고 생각된다.

일부 사회복지시설 종사자의 구강건강상태에 따른 구강건강관련 삶의 질 (Oral health-related quality of life in social welfare workers according to oral health status)

  • Objectives: The purpose of the study was to investigate the influencing factors of oral health-related quality of life in social workers. Methods: A self-reported questionnaire was completed by 240 social workers in Gwangju by convenience sampling method. The questionnaire consisted of general characteristics of the subjects(age, monthly salary, smoking, alcohol consumption) and systemic health condition(systemic diseases, medication, oral health status, and stress). The factors associated with oral health-related quality of life included skin dryness, eye dryness, lip dryness, and nasal dryness. The subjective dry mouth consisted of 6 questions measured by visual analogue scale(VAS). Cronbach's alpha was 0.868 in the study. The data were analyzed for t-test, one-way ANOVA and multiple regression analysis using SPSS 18.0 program. Results: The oral health-related quality of life in social workers varied by age, oral health status, stress status, and halitosis. The oral health-related quality of life had positive correlations with the subjective perception of dry mouth and whole body dry symptoms. The health status(${\beta}=-0.410$) had the influence on the oral health-related quality of life, nasal dryness(${\beta}=0.230$), age(${\beta}=0.189$), and halitosis (${\beta}=0.162) in order. Conclusions: The oral health-related quality of life was closely related to the health status and nasal dryness. It is necessary to develop the quality of life improvement programs including oral health prevention and care program in the social workers.
