Derivation of core competencies of advanced practice dental hygienists in the elderly using the Delphi method (델파이 방법을 이용한 노인전문치과위생사 핵심역량 도출)

  • Objectives: This study derived the core competencies for developing a curriculum for advanced practice dental hygienists in the elderly. Methods: A Delphi survey was conducted thrice times with 12 subjects engaged in dental hygiene-related activities for the elderly. Results: The item with the highest average in each domain was 'understanding the elderly (mean=5.00)' in basic qualities and attitude competency, 'understanding the oral health characteristics of the elderly (mean=5.00)' in the job competency, and 'development and evaluation of oral health programs for the elderly (mean=4.75)' in job support competency. Conclusions: The core competencies of dental hygienists specializing in the elderly are divided into 3 'basic qualifications and attitude competencies', 'job competencies', and 'job support competencies' 27 sub-competencies are presented.

Development and evaluation of standards for clinical dental hygiene practice (국내 표준 임상치위생실무지침의 개발 및 타당성 평가)

  • Objectives: The purpose of this study was to develop applicable standards for clinical dental hygiene practice in Korea and to evaluate their validity. Methods: Based on the standards for clinical dental hygiene practice developed in the United States and Canada, the standards were adapted to be applicable in Korea. The validity of the standards was evaluated by a self-writing questionnaire among 14 professors and 10 clinicians using a developed tool for evaluating the standards. A focus group interview was additionally conducted for clinicians to increase the validity of the standards. Descriptive statistics and Mann-Whitney test were performed using SPSS 25.0. To analyze the content of the focus group interviews, content analysis was conducted. Results: The standards for clinical dental hygiene practice consisted of five elements of professionalism for dental hygienists and a total of 28 items to perform the five stages of dental hygiene process of care (assessment, diagnosis, planning, implementation, and evaluation) and included conceptual meaning, clinical significance, and application methods with examples for each item. Conclusions: The standards for clinical dental hygiene practice developed in this study could contribute to standardizing dental clinical practices provided by dental hygienists. It is necessary to consistently improve the standards that are highly practical, to prevent oral diseases and maintain oral health of the public, based on the results of this validity evaluation.

Clinico-statistical Analysis of Cooperation and Anesthetic Induction Method of Dental Patients with Special Needs (장애인 환자의 치과치료를 위한 전신마취 시 협조도와 마취 유도 방법에 대한 통계적 고찰)

  • Background: This study aimed to evaluate the cooperative levels of dental patients requiring general anesthesia during dental treatments. Anesthetic induction methods for patients were also recorded and analyzed using descriptive statistics. Methods: Total 566 patients who visited Seoul National University Dental Hospital Clinic for Persons with Disabilities were reviewed on pre-anesthetic review and anesthesia records. The cooperative levels of patients were graded by 4 levels and induction methods used for the patients during general anesthesia application were analyzed. Results: More than half of patients(55.8%) were willing to receive the anesthetic induction(cooperative level 1), 18.6% were minimally cooperative(level 2), 20.8% needed physical restraint prior to induction(level 3), and 4.8% was poorly cooperative and induction procedure was performed under an unconscious condition after ketamine intramuscular injection(level 4). There was no gender difference in cooperative levels(P=0.11). Patients over 30 years revealed better cooperation levels compared to other age groups(P<0.05). For patients of level 1, 53.5% were anesthetized in a way of intravenous induction, while 77.1% out of patients of level 3 were anesthetically induced through inhalation method. Conclusion: Many dental patients with special needs were not cooperative to receive anesthetic induction. Additional behavioral support may be applied to poorly cooperative patients for the safe and successful clinical outcome.

Development of human rights indicators for dental hygiene students (치위생(학)과 학생들의 인권 실태 지표 개발: 치과의료기관 임상실습을 중심으로)

  • Objectives: This study aimed to develop human rights indicators through reliability and validity tests in order to measure the human rights situation of dental hygiene students who experience clinical practice at dental clincs. Methods: The basic framework of questions was constructed through literature review. Nine experts were tested for validity of the contents of the experts twice. The main survey was conducted on 121 students in the 3rd and 4th grade who were enrolled in the department of dental hygiene located in Seoul, Gyeonggi and Gangwon-do. Validity was tested through exploratory factor analysis, and reliability was tested through internal reliability coefficient and test-retest method. Results: The cronbach's α value was 0.734, and the Kappa coefficient was 0.584. The result of the reliability and validity test was composed of 11 questions and 3 factors. Conclusions: During the practice of a dental institution, the validity and reliability of the indicators that can grasp the human rights status of dental hygiene students were tested. It is expected that the indicators of this study will be provided an opportunity to understand the human rights situation of dental hygiene students and to improve problems such as human rights violations.

Analysis of distribution trend among students of dental hygiene departments and active hygienists by region (지역별 치위생(학)과 학생 및 활동 치과위생사 분포의 추세 분석)

  • Objectives: This study compared the number of graduates in each region for the past 6 years and the number of dental hygienists working in dental clinics by region to evaluate the trend of dental hygienists moving to work areas after graduation. Methods: Health care big data open system_medical manpower statistics, resident population and household status data by year, and education statistics service were used to calculate the number of dentists and dental hygienists, admission status by region, number of dental hygienists per 100,000 population, number of dental hygienists per number of dentists, and distribution of dental hygienists by region. Results: Although the number of active dental hygienists increased in the metropolitan area, the ratio of dental hygienists to dentists did not improve significantly. In addition, the number of students enrolled in provincial universities decreased, and there were fewer active dental hygienists than graduates in provincial areas. Conclusions: Although the number of active dental hygienists increased due to increase in the number of dental hygiene departments, it was found that rural areas did not have a significant impact on the availability of dental hygienists as the graduates moved to the metropolitan area.

A qualitative study on communication among dental hygienists in dental hospitals and clinics (치과 병·의원 내 치과위생사 간 의사소통에 대한 질적 연구)

  • Objectives: This study aimed to offer basic resources needed to solve conflicts and problems of communication among dental hygienists working in dental clinics by comprehending and analyzing their in-depth communication experiences. Methods: Individual depth interviews were conducted with 10 dental hygienists who had over one year of clinical experience and worked in dental clinics in Gwangju, from September 16 to October 15, 2020. Colaizzi's phenomenological analysis was performed. Results: The results of the interviews were grouped into four central categories: lack of communication due to differences in position, relationships they want to avoid, return to joint work, beginning of new communication. The participants of this study experienced difficulties in communicating with dental hygienists in the clinical field, but they overcame the conflicts by trying to understand others by putting themselves in others' shoes. Conclusions: This study is expected to contribute to providing good quality medical services to patients by promoting effective communication among dental hygienists.

  • Major cardiac complication such as acute myocardial infarction can occur unexpectedly in patients without risk factors. We experienced cardiac arrest due to an unexpected acute myocardial infarction in a patient without any risk factors during head and neck reconstructive surgery. The patient was diagnosed with acute myocardial infarction after return of spontaneous circulation. With immediate percutaneous coronary intervention, the patient recovered without complications.

  • In cases of a difficult intubation where numerous intubation methods, including laryngoscopy, have failed, yet oral intubation is still necessary, the method of tube exchange after fiberoptic nasal intubation may be attempted. Fiberoptic nasal intubation allows intubation to be performed relatively easily when the laryngeal view grade is poor. We report a case in which our attempt at oral intubation for total maxillectomy with laryngoscopy and fiberoptic oral intubation had failed due to an unexpected difficult airway; subsequently, we successfully completed the surgery by performing fiberoptic nasal intubation to secure the airway, followed by using a tube exchanger to exchange to an oral endotracheal tube.

  • Background: This study evaluates the existing cardiopulmonary resuscitation (CPR) knowledge and skills of 1 st year dental students and also compares their CPR performance skill with those of 4 th year students. Methods: Incoming 1 st year and 4 th year dental students were surveyed about the level of their CPR knowledge before conventional CPR instruction. After we trained them for CPR performance, we tested CPR performance skill to randomly selected 10 1st year and 4 th year dental students and compared their CPR performance. Results: Overall CPR knowledge was low for both 1 st and 4 th year dental students. Especially, only 1 among 89 fourth year dental students answered the right sequence of CPR revised in 2010. Overall CPR performance was relatively low for both 1 st and 4 th year dental students. Irrespective of previous experiences of CPR education, their CPR performance was not statistically different. Conclusions: Retraining of CPR is a necessity. Information about the existing CPR knowledge for dental students will aid in establishing the goal of future CPR training and improve the current deficiency.

Research on dental hygienists' clinical skill proficiency in core dental hygiene competency (치과위생사의 핵심기본치위생역량술에 대한 숙련요구도 조사)

  • Objective: The purpose of the study is to investigate clinical skill proficiency in core dental hygiene competency in dental hygienists. Methods: The study subjects were 208 dental hygienists in Seoul and Gyeonggido. A complete self-reported questionnaire was filled out by 171 dental hygienists. The questionnaire consisted of general characteristics of the subjects, assessment of core dental hygiene competency, implementation of core dental hygiene competency, and education of core dental hygiene competency. Data were analyzed by PASW Statistics 18.0 to carry out a frequency analysis, one-way ANOVA and Pearson's correlation. Results: Chief complaint confirmation($4.08{\pm}0.91$), scaling (ultrasonic scaler)($4.36{\pm}0.88$), and toothbrushing education($4.35{\pm}0.86$) were found to be the most important in the areas of dental hygiene assessment, implementation, and education, respectively. In dental hygiene education, statistically significant differences were identified by 'level of education,' 'working career,' 'type of workplace,' 'department,' and 'main task.' Conclusions: Respondents were found to think that their level of proficiency should be above average for most jobs. Proficiency in core competency was shown to be different according to 'general characteristics of dental hygienists,' 'type of workplace,' and 'department.'
