This case report describes a technique in which endodontic treatment and permanent indirect restoration were completed in the same clinical appointment with the aid of a computer-aided design/computer-aided manufacturing (CAD/CAM) system. Two patients were diagnosed with irreversible pulpitis of the mandibular first molar. After access preparation, root canals were located, irrigation was performed until bleeding ceased, and the coronal tooth structure was prepared for indirect restoration. Then, utilizing an interim 3-mm build-up of the endodontic access cavity, a hemi-arch digital scan was performed with an intraoral scanner. Subsequent to digital scanning, restoration design was performed simultaneously with the endodontic procedure. The root canals were shaped using the Race system under irrigation with 2.5% sodium hypochlorite followed by root canal filling. The pulp chamber was subsequently filled with a 3-mm-thick composite resin restoration mimicking the interim build-up previously utilized to facilitate block milling in the CAD/CAM system. Clinical try-in of the permanent onlay restoration was followed by acid etching, application of a 5th generation adhesive, and cementation of the indirect restoration. Once the restoration was cemented, rubber dam isolation was removed, followed by occlusal adjustment and polishing. After 2 years of follow-up, the restorations were esthetically and functionally satisfactory, without complications.
The purpose of this study was to evaluate the marginal sealing ability of 'Hi-Pol' Enamel Bond system. Using freshly extracted human teeth and 2% acquous methylene blue, the marginal leakage of dye in restorative materials such as 'Hi-Pol' Enamel Bond system, Hi-Pol, Adaptic, Nuva system, Epolite and Amalgam were investigated at 37℃ and under temperature cycling in range of 4℃-60℃. the results as follows; 1. All filling materials showed some degree of marginal penetration by 2% methylene blue dye. 2. 'Hi-Pol' Enamel Bond system revealed the least marginal penetration by 2% methylene blue as Nuva did at 37℃, but under temperature cycling in range of 4℃-60℃ it showed the greatest marginal penetration. 3. Hi-Pol restoration showed a tendency the similar to the other composite restoration. 4. Nuva system had the most effective marginal sealing ability either at 37℃ and under temperature cycling.

pH varies in the oral environment and on tooth surfaces. The surface of a plaque covered resinous restoration has a low pH. In human dental plaque a number of acids are produced by bacterial metabolism. The predominant acids are acetic, propionic and lactic acids. These acids induced a surface swelling of restorative resin. The swelling cause formation of pores inside the resin matrix from which organic substances can be released, resulting in a mass loss. The purpose of this study was to qualitatively and quantitatively analyse the leached monomers of dental composites after storage in acetate buffer solution as a function of time by means of high performance liquid chromatography(HPLC) / mass spectrometer and further determine whether pH and time has any influence on the degradation behavior of composite restoration.(omitted)
본 연구의 목적은 아말감과 복합레진의 수복 과정과 수복 후 상아세관액의 흐름(dentinal fluid flow, DFF)을 측정하기 위함이다. 치근을 절제한 제 3 대구치를 자체 제작한 미세 유체 흐름 측정장치에 연결한 후 1급 와동을 형성하여 아말감과 복합레진 수복을 시행하였다. DFF의 측정은 와동 형성부터 수복 후 30분까지 연속적으로 이루어졌고, 수복 후 3, 7일에 재 측정하였다. 주수 하에 와동 형성 시 DFF는 inward로, 끝난 후에는 outward로 바뀌었다. 아말감 충전 시 DFF는 inward로 바뀌었고 충전 완료 후 미약한 outward DFF를 보였다. 복합레진 수복의 경우, 산 처리 후 수세와 건조 시 각각 inward와 outward DFF를 보였고 primer도포 후 공기 분사는 급격한 outward, 소수성 본딩제를 적용하는 단계에서는 outward 였던 DFF가 감소하여 0에 가까워지거나 약간 inward 흐름을 보였다. 접착제와 복합레진의 광중합은 급격한 inward의 DFF를 일으켰다. 수복 후 30분, 3일, 7일째 수복 재료에 따른 DFF의 감소에 통계적으로 유의한 차이는 없었다 (p>0.05).
Objectives: To determine the retentive strength and failure mode of undercut composite post, glass fiber post and polyethylene fiber post luted with flowable composite resin and resin-cement. Materials and Methods: Coronal parts of 120 primary canine teeth were sectioned and specimens were treated endodontically. The teeth were randomly divided into 6 groups (n = 20). Prepared root canals received intracanal retainers with a short composite post, undercut composite post, glass fiber post luted with flowable resin or resin-cement, and polyethylene fiber post luted with flowable resin or resin-cement. After crown reconstruction, samples were tested for retentive strength and failure mode. Statistical analysis was done with one-way ANOVA and Tukey tests (p < 0.05). Results: There were statistically significant differences between groups (p = 0.001). Mean bond strength in the undercut group was significantly greater than in the short composite post (p = 0.030), and the glass fiber post (p = 0.001) and the polyethylene fiber post group luted with resin-cement (p = 0.008). However, the differences between the undercut group and the groups with flowable composite as the luting agent were not significant (p = 0.068, p = 0.557). Adhesive failure was more frequent in the fiber post groups. Conclusions: Although the composite post with undercutting showed the greatest resistance to dislodgement, fiber posts cemented with flowable composite resin provided acceptable results in terms of retentive strength and fracture mode.
Among developmental anomalies of tooth shape, fusion and gemination are comparatively common. Developed with different etiologies, both fusion and gemination show similar clinical features. Therefore, many clinicians have difficulty diagnosing those morpho-anatomic anomalies. The purpose of this study is to report malformed right maxillary first molar in a 20-year-old female. With the aid of computed tomography (CT), the tooth was diagnosed as fusion with supernumerary tooth and dental caries lesion was detected. After performing direct pulp capping, the tooth was permanently restored with microhybrid composite resin using direct method to alter union groove into smooth surface for improving oral hygiene management. Until 6 months of follow-up visits, patient's chief complaint was resolved and tooth is still vital. In conclusion, identifying exact anatomy, conservative treatment and improving oral hygiene are essential in managing unusual morphologic anomalies of tooth.
PURPOSE. The aim of the study was to evaluate the effect of abutment shade, ceramic thickness, and coping type on the final shade of zirconia all-ceramic restorations. MATERIALS AND METHODS. Three different types of disk-shaped zirconia coping specimens (Lava, Cercon, Zirkonzahn: ${\phi}10mm{\times}0.4mm$) were fabricated and veneered with IPS e.max Press Ceram (shade A2), for total thicknesses of 1 and 1.5 mm. A total of sixty zirconia restoration specimens were divided into six groups based on their coping types and thicknesses. The abutment specimens (${\phi}10mm{\times}7mm$) were prepared with gold alloy, base metal (nickel-chromium) alloy, and four different shades (A1, A2, A3, A4) of composite resins. The average $L^*$, $a^*$, $b^*$ values of the zirconia specimens on the six abutment specimens were measured with a dental colorimeter, and the statistical significance in the effects of three variables was analyzed by using repeated measures analysis of variance (${\alpha}$=.05).The average shade difference (${\Delta}E$) values of the zirconia specimens between the A2 composite resin abutment and other abutments were also evaluated. RESULTS. The effects of zirconia specimen thickness (P<.001), abutment shade (P<.001), and type of zirconia copings (P<.003) on the final shade of the zirconia restorations were significant. The average ${\Delta}E$ value of Lava specimens (1 mm) between the A2 composite resin and gold alloy abutments was higher (close to the acceptability threshold of 5.5 ${\Delta}E$) than th ose between the A2 composite resin and other abutments. CONCLUSION. This in-vitro study demonstrated that abutment shade, ceramic thickness, and coping type affected the resulting shade of zirconia restorations.
Objectives: This study aimed to evaluate the interface between a calcium silicate cement (CSC), Biodentine and dental adhesives in terms of sealing ability. Materials and Methods: Microleakage test: 160 standardized class II cavities were prepared on 80 extracted human molars. The cavities were filled with Biodentine and then divided into 2 experimental groups according to the time of restoration: composite resin obturation 15 minutes after Biodentine handling (D0); restoration after 7 days (D7). Each group was then divided into 8 subgroups (n = 5) according to the adhesive system used: etch-and-rinse adhesive (Prime & Bond); self-etch adhesive 2 steps (Optibond XTR and Clearfil SE Bond); self-etch adhesive 1 step (Xeno III, G-aenial Bond, and Clearfil Tri-S Bond); and universal used as etch-and-rinse or self-etch (ScotchBond Universal ER or SE). After thermocycling, the teeth were immersed in a silver nitrate solution, stained, longitudinally sectioned, and the Biodentine/adhesive percolation was quantified. Scanning electron microscopic observations: Biodentine/adhesive interfaces were observed. Results: A tendency towards less microleakage was observed when Biodentine was etched (2.47%) and when restorations were done without delay (D0: 4.31%, D7: 6.78%), but this was not significant. The adhesives containing 10-methacryloyloxydecyl dihydrogen phosphate monomer showed the most stable results at both times studied. All Biodentine/adhesive interfaces were homogeneous and regular. Conclusions: The good sealing of the CSC/adhesive interface is not a function of the system adhesive family used or the cement maturation before restoration. Biodentine can be used as a dentine substitute.
성인에 비해 치료를 견별 수 있는 시간이 상대적으로 짧은 소아 환자에 있어 시술 시간은 치료 결과의 중요한 변수로 작용하게 된다. 최근 소개된 유동성 복합 레진을 예방적 레진 수복술에 사용하게 될 경우 전색재 도포 과정을 생략할 수 있어 시술 시간의 단축 편의성 등의 장점이 인정되나 미세누출 등에 관한 연구 결과는 미흡한 실정이다. 본 연구의 목적은 최근 소개된 유동성 복합 레진을 사용한 예방적 레진 수복물과 기존의 복합 레진과 전색재를 사용한 예방적 레진 수복물 간의 미세누출 및 기포발생 정도를 비교하고자 함이었다. 교합면이 건전한 상악 소구치 60개를 대상으로 고속용 1/2 round bur를 이용하여 교합면 열구를 따라 길이$3.0\times$폭$0.7\times$깊이2.0mm의 I급 와동을 형성하고 각 군 당 20개씩 무작위로 시편을 배분하고, 제조자의 지침에 따라 본딩재인 Scotchbond $Multipurpose^{TM}$(3M dental product, U.S.A.)를 적용하고 유동성 복합 레진 수복군인 I, II군에는 각각 $Revolution^{TM}$(Kerr, U.S.A.)과 Arabesk Flow(VOCO, Germany), 대조군인 III군(통상적 예방적 레진 수복군)에는 Restorative $Z-100^{TM}$(3M dental product, U.S.A.)과 $Concise^{TM}$(3M dental product, U.S.A.)를 제조자의 지시에 따라 충전, 적용하였다. 3,000회의 열 순환 및 색소 침투 후 시편을 절단하여 컴퓨터에 영상을 입력하고 색소의 침투 길이 및 침투 비율을 계산하고 미세누출 평가 기준을 이용하여 각 시편에 대한 미세누출 정도를 평가하였다. 또한, 각 군별로 특징적으로 나타난 기포의 발생 양상을 별도의 평가기준에 따라 평가하였다. 미세누출은 수복재와 법랑질의 계면부에 국한되어 관찰되었고, 와동 내부까지 침투된 경우는 없었으며, 유동성 복합레진 수복군(I, II군)이 기존의 예방적 레진 수복군(III군)에 비해 낮은 미세 누출 정도를 나타냈다(p<0.05). 미세누출 결과 외에, 대부분의 충전물 내에서 다양한 형태의 기포가 발견되었으며, I, II군 표본에서 III군에 비해 크기가 큰 기포가 더욱 많이 관찰되었다.

Objectives: To evaluate the influence of the restorative technique on the mechanical response of endodontically-treated upper premolars with mesio-occluso-distal (MOD) cavity. Materials and Methods: Forty-eight premolars received MOD preparation (4 groups, n = 12) with different restorative techniques: glass ionomer cement + composite resin (the GIC group), a metallic post + composite resin (the MP group), a fiberglass post + composite resin (the FGP group), or no endodontic treatment + restoration with composite resin (the CR group). Cusp strain and load-bearing capacity were evaluated. One-way analysis of variance and the Tukey test were used with ${\alpha}=5%$. Finite element analysis (FEA) was used to calculate displacement and tensile stress for the teeth and restorations. Results: MP showed the highest cusp (p = 0.027) deflection ($24.28{\pm}5.09{\mu}m/{\mu}m$), followed by FGP ($20.61{\pm}5.05{\mu}m/{\mu}m$), CR ($17.62{\pm}7.00{\mu}m/{\mu}m$), and GIC ($17.62{\pm}7.00{\mu}m/{\mu}m$). For load-bearing, CR ($38.89{\pm}3.24N$) showed the highest, followed by GIC ($37.51{\pm}6.69N$), FGP ($29.80{\pm}10.03N$), and MP ($18.41{\pm}4.15N$) (p = 0.001) value. FEA showed similar behavior in the restorations in all groups, while MP showed the highest stress concentration in the tooth and post. Conclusions: There is no mechanical advantage in using intraradicular posts for endodontically-treated premolars requiring MOD restoration. Filling the pulp chamber with GIC and restoring the tooth with only CR showed the most promising results for cusp deflection, failure load, and stress distribution.
