Patients with disabilities have difficulties tolerating in-office dental treatment due to limitations relating to cooperation and/or physical problems. Therefore, they often require general anesthesia or sedation to facilitate safe treatment. When deciding on dental treatment under general anesthesia, the plan should be carefully determined because compared to general patients, patients with disabilities are more likely to experience anesthetic complications because of their underlying medical conditions and potential drug interactions. Clinicians prefer simpler and more aggressive dental treatment procedures, such as extraction, since patients with impairment have difficulty maintaining oral hygiene, resulting in a high incidence of recurrent caries or restorative failures. This study aimed to review the available literature and discuss what dentists and anesthesiologists should consider when providing dental treatment to patients with severe disability under general anesthesia.

Patients with mental disability who are classified as dental severely disabled, have poor oral health status and many difficulties in taking health services. The purpose of this study was to investigate factors of influencing in visiting oral health care services. After receiving approval from institutional review board in Seoul National University, 39 participants were recruited and the oral health examination and questionnaire were taken. Data were analyzed using one-way ANOVA, independent t-test, hierarchical linear regression for predicting influence of each participants' characteristics on oral examination time and care time. In regression model, the higher the disability grade and the lower the degree of cooperation, the more oral examination time increased. However possibility of communication was not significantly influenced. Also, the higher the disability grade and the lower the degree of cooperation, the more oral care time increased. Background: Patients with autism spectrum disorder (ASD) present challenges in dental treatment cooperation owing to deficits in communication skills and social interaction. Behavioral guidance, sedation, and general anesthesia may be employed to ensure the quality of dental care for individuals with ASD. This study aimed to examine the trends in dental treatment for patients with ASD who visited the Department of Pediatric Dentistry at Dankook University Jukjeon Dental Hospital, an oral health center for the disabled in the Gyeonggi region, over the past 10 years. Methods: This study utilized the order communication system to gather data on sex, age, cooperation level, number of quadrants treated, and administration of sedation or general anesthesia for patients with ASD who visited the Department of Pediatric Dentistry at Dankook University Jukjeon Dental Hospital between January 2013 and December 2022. Results: The total number of patients with ASD increased annually, possibly due to an increase in ASD prevalence and the hospital's designation as a center for disabled oral health. General anesthesia was predominant before 2017, with a shift towards N2O-O2 sedation. The most common age group for sedation or general anesthesia was 6-9 years, with a higher prevalence in males than in females. Notably, N2O-O2 and midazolam sedation resulted in better cooperation and fewer treated teeth than general anesthesia. Conclusion: This study highlights the evolving trends in dental treatment for individuals with ASD, indicating a shift towards outpatient methods, particularly N2O-O2 sedation. The sex distribution aligns with national statistics, emphasizing a higher prevalence of ASD in males than in females. These findings underscore the need for further research to establish evidence-based guidelines for optimal dental care strategies tailored to the unique needs of individuals with ASD.
Objectives: This study aimed to review the definition, qualification conditions, accreditation field, and system of Japanese recognized dental hygienists. It is expected that it will be used as basic data for the system introduction of the Korean advanced dental hygienists. Methods: From May to November 2020, a literature review was conducted on Japanese dental hygienists and Japanese certified dental hygienists. Results: The lifelong education system training courses to become Japanese certified dental hygienists consisted of basic training, special training, and specific training. Each training session lasted for 15 h, and the number of training hours required to complete the course was 30 h. The training items for the field of recognition A were prevention of lifestyle disease, home care, oral function management, rehabilitation for dysphagia, prevention of diabetes, and oral management by medical and dental partnership. The training items for the field of recognition B were dentistry for the disabled, dentistry for the aged, community oral health, and oral health care. Conclusions: The Japanese recognized that dental hygienist system is valuable as a demonstration model in introducing the Korean advanced dental hygienist system.

The aims of this study were to evaluate dental caries status and to analyze the dental caries related factors which could explain the relationship between dental caries and disabled type, severity of handicap, and oral health behaviors among disabled children and adolescent in Korea. A total of 677 subjects aged from 6 to 18 were finally selected by stratified cluster sampling. They were examined by 13 trained dentists and interviewed with questionnaire. For the crude and adjusted associations, bivariate and multivariate analysis were performed using SPSS program(SPSS Inc, Chicago, USA). DMFT index were 0.90(ages 6-8), 2.32(ages 9-11), 3.63(ages 12-14) and 4.93(ages 15-17), respectively. The DMFT index was significantly related to age, frequency and dependency of toothbrushing, and mastication and functional disorder. Specialized preventive care program which accounted the age and toothbrushing instruction be developed and adapted to promote oral health status and behaviors of disabled children and adolescent in Korea.
Objectives : The purpose of this study was to examine the state of oral health care among special school personnels in an attempt to provide some information on the improvement of the oral health care of students with disabilities who would be under the first hand influence of school personnels. Methods : The subjects in this study were personnels who were selected by random selection in five different special schools located in the city of Jeonju, North Jeolla Province. A self-administered survey was conducted in person from July 5 to 14 after the purpose of this study was explained. Results : 1. Concerning their general characteristics, the level of oral health knowledge was high in the personnel whose career is 5 years more, and the younger personnels had a better oral health knowledge, and the men were more knowledgeable than the women. 2. As to oral health education experience, the rate of the respondents who ever received oral health education stood at 35.3 percent. In relation to the frequency of oral health education, the biggest group that accounted for 58.2 percent received that education once. As for the route of education, the largest group that represented 52.7 percent received that education at dental hospitals or clinics. In relation to satisfaction with oral health education, the greatest group that accounted for 38.5 percent were dissatisfied with that education. 3. As for an intention of receiving oral health education in the future, the biggest group that accounted for 60.9 percent intended to receive that education if they would have free time, and the largest group that represented 47.7 percent believed that oral health education should be conducted by dental hygienists. 4. Concerning their general characteristics, the level of oral health promotion behavior according to age in both bushing and supplies of oral health care was high in forties-1.89 point and 3.33 point, and that in regular visit to a dental clinic was the highest in twenties for 2.58 point, and that in dietary control was the highest in twenties for 2.59 point. 5. Their oral health knowledge had a significant positive correlation to their toothbrushing, regular dental clinic visit and dietary control that were the subfactors of oral health promotion behavior. 6. As for the impact of oral health promotion behavior on oral health knowledge, toothbrushing exerted the greatest influence on that(${\beta}$=0.306, p<0.001). Conclusions : Appropriate institutional measures should be taken to let dental hygienists who are expert in oral health care provide incremental oral health care for students and adults with disabilities in educational institutions and facilities for the disabled, and the development of oral health education programs is urgently required to offer systematic oral health education for not only students with disabilities but their teachers and guardians.
Objectives : The purpose of the study is to investigate the relationship between variables associated with oral health status in psychiatric patients and to provide basic information on oral health education program development for the psychiatric patients. Methods : Subjects were 151 psychiatric patients who filled out the structured questionnaire. The data were analyzed for independent t-test, one-way ANOVA and hierarchical multiple regression by using SPSS Win Program 18.0 version. Results : Higher DMFT index was shown in the older age(p<0.001) and low perceived oral health group(p<0.001). Higher DMFT index included no tooth brushing after breakfast(p=0.045), wrong tooth brushing method(p<0.001). Chewing discomfort(p=0.027) and denture(p=0.027) were closely related to higher DMFT index. Bread(p=0.033) and snack(p<0.001) increased DMFT index. Factors affecting oral health status were age, denture, snack and bread. The explanation power of the final model was 29.4%. Conclusions : Dietary control for the psychiatric patients is very important to prevent dental caries. Tooth brushing and use of oral hygiene devices can improve good oral health care. Furthermore, this result can improve the oral health care for the disabled people.
This study aims to evaluate the overall health of the disabled and oral health behavior to determine its relevance to DMFT index. Based on the National Health Nutrition Survey. The survey was conducted on disabled adults over 19years old age in 2013 the first of 6th phase. The data analysis included a general characteristics, disability factors, social economy factors, health factors, oral health behavior factors were cross-analyzed on a composite sample. The general linear model of the combined sample was analyzed to determine the factors affecting the DMFT index of the disabled. The analysis shows that the DMFT index is higher for men than for women, the higher age, the lower level of education, the worse condition of whole body(EQ-5D), the difficulty of Stomatognathic system, the uncomfortable of chewing and the worse oral health conditions subjectively. Therefore, the study objectively identified the overall oral health condition of the adult disabled in Korea, Through this study, a measure should be made to development of comprehensive oral health education program to promote oral health of disabled people and policy measures should be prepared for prevention and care of oral health of disabled people.

The spectrum of physically challenged patients who visit to a dental clinic for treatments are diverse: from ones of mild disabilities, who lead their lives in a similar way of ordinary people, to others who suffer from diseases that are difficult to be diagnosed, and need medical expertise due to systemic care or special techniques and instruments because of behavior management and difficulty of treatments. The patient in this case was 25-year-old autistic male patient who visited the clinic on account of overall treatment for multiple carious lesions. He could perform normal daily life to some degree under the care of a guardian. This case report describes predicaments of making a treatment plan resulted from lack of information about disabled patients, and difficulties of proceeding treatment procedures under general anesthesia. It is considered that sufficient consultation and history taking are needed before treatment for disabled patients.

In most persons with disabilities had poor oral hygiene because of less attention and ability to perform a dental care. So the increased prevalence and severity of dental disease were common oral state. Although most persons with disabilities need a adequate dental treatment, it is often very difficult to treat because of noncooperativity and involuntary muscle movements. Dental treatments under sedation and general anesthesia were make to provide a high-quality dental service because of decreased anxiety and fear associated dental treatment in persons with disabilities. The dental professionals must be able to select and apply the proper sedation methods in agreement with the characteristics of the disabilities, general conditions, sedation experience and capacity of dentist, type and time of dental treatment, equipments of dental clinic, consent of patient's protector. The proshodontic treatment procedures, such as abutment preparation, dental impression taking process, try-in process of prosthesis and adjustment of occlusion, are difficult even for patients without disabilities. Those procedures are more difficult to patients with disability because it's too hard to control breathing and muscle. In this report, we performed prosthetic dentistry procedures to three patients with disabilities under dental sedation and general anesthesia.
