일부 복지관 장애인치과 내원 환자들의 서비스 만족도 및 진료 후 변화 인식도 연구 (A study on quality of handicapped inpatients' service satisfaction at special dental clinic and their transformed perceptions toward to the dental treatment services)

  • Objectives : The purpose of this study is about discovering the basic references to find the ways to vitalize handicapped dental clinics. Methods : The study was analyzed by the satisfactions of those patients who took the advantages of using the dental care at B-welfare center and also their transformed perceptions after the services are influenced. The analysis was the questionnaire consisting of 100 items and survey data. Results : 1. The satisfaction of the dental service for the disabled was respectively high which was the average of 4.49. 2. Their satisfactions of receiving kindness services by volunteers and employees were the highest in the entire research of successful handicapped dental services and it was 4.78 overall. 3. The satisfaction of using handicapped dental clinic has the higher range of female users than male's. In the mean time treatment details of the dental care and the satisfaction towards to the volunteers and faculty at the center show the statistical significance gap. 4. The oral care service after experiencing the dental clinic for handicapped relived their discomforts of using the regular dental clinic which shows their highest satisfaction as it is the point of 4.75. 5. the change perception after dental treatment for handicapped has the higher range of females than men's and solving the problems of mouth reference and discomfort of using regular clinics show the statistical significance gap. 6. In the change perception after having dental treatment for handicapped the thought of the possibility of periodical dental care shows the highest perception when the number of visiting is usually shorter and it shows the statistical significance. Conclusions : According to the satisfaction of those inpatients who use free dental care services that belong to dental clinics for handicapped in a part of Seoul welfare centers human services were appeared as the most important factor due to their advantages of taking services from volunteers and staff members. On the other hand to enhance the medical treatment information and environment which showed the weakest factors each inpatient should be specifically specialized for their needs and also further study on plans which enhance their perceptions toward to a better quality of oral-related life is required after using dental treatment service.

장애인 입소시설과 특수학교의 구강건강관리 실태에 대한 조사 연구 (ORAL HEALTH CARE STATUS IN INSTITUTIONS AND SPECIAL SCHOOLS FOR THE DISABLED PEOPLE IN KOREA)

  • 전국의 123개 장애인 입소시설과 102개 특수학교를 대상으로 구강건강관리 실태를 파악하여 장애인의 구강건강관리 계획을 수립하는 데 사용할 수 있는 근거를 마련하고자 하였다. 설문지 조사법으로 연구하였다. 정기구강검진은 특수학교에서 연 2회 실시하고 있었으나, 장애인 입소시설에서는 56.8%에서 시행하였다. 우식 예방 프로그램은 장애인 입소시설의 69.1%, 특수학교의 86.3%에서 시행하였으며 프로그램의 종류는 입소시설에서 규칙적 칫솔질이 58.1%, 특수학교에서 예방교육이 65.7%로 가장 많았다. 불소를 이용한 우식예방은 입소시설과 특수학교에서 각각 8.6%, 11.8%에서 시행하고 있었으며, 열구전색은 각각 6.8%, 6.9%에서 시행하였다. 치과치료를 시행하고 있는 입소시설과 특수학교에서는 각각 84.2%, 39.2%이었고, 입소시설에서 치료를 받기 위해 이용하는 곳은 치과의원(60.2%), 보건소(16.8%), 자원봉사자(15.3%), 대학병원 (1.0%) 등이었다. 치료를 위해 치과의원에 방문하여 치료를 거부당한 경험이 있는 응답자는 17.7%이었다.

장애인치과학 수업 전후의 치의학 전문대학원 학생들의 장애인에 대한 인식 변화 연구 (STUDENT ATTITUDES TOWARD PEOPLE WITH DISABILITIES : A COMPARISON BETWEEN BEFORE AND AFTER CLASS OF SPECIAL CARE DENTISTRY)

  • Patients with disabilities are a significant segment of Korea population. However, they encounter more challenges when seeking dental care. So, dentists and students who are future dental providers should have a responsibility to provide oral care services for them. In this study, we hypothesize that the education of special care dentistry may influence positive attitudes of the disabled. The attitudes are influenced by demographic variables such as gender, religion, experience of volunteer activities and marital status. Therefore, the objective of this study was to explore the change of attitudes of students towards people with disabilities between before and after class of special care dentistry. 72 students enrolled at the Kyung Hee University were surveyed. All participants attended class of Special Care dentistry in the third grade. The survey was conducted two times, just before class and after class. The attitudes of students were assessed using the well-documented ATDP (Attitude Toward Disabled Person)-O(Original) scale and MAS (Multidimensional Attitudes Scales Toward Persons with Disabilities) scale through the methodology of questionnaires. The results indicated that gender, experience of volunteer activities and marital status did not affect their ATDP and MAS score. In religious affiliation, only, there were statistically significant difference in student attitudes between before and after class of Special care dentistry. It means that habitual religious activity may offer stronger effects on student's attitude than short-term education. Therefore, long-term and specific educational experiences are needed to promote positive attitudes for people with disability.

치과위생사의 경증 장애인 환자 진료 고려사항에 대한 지식 측정 도구의 타당도와 신뢰도 검증 (Testing the validity and reliability of an assessment tool for dental hygienists' consideration of patients with mild disabilities)

  • Objectives: The purpose of this study was to test the reliability and validity of an assessment tool for dental hygienists' consideration of patients with mild disabilities. Methods: Thirty-seven preliminary questions were compiled through literature review, and thirty-two questions were selected through content validity testing by eight experts. Two hundred and three dental hygienists completed the assessment tool as an online survey. The construct validity, internal consistency, and test-retest reliability of the tool were investigated using the SPSS 26.0 program. The construct validity was assessed using exploratory factor analysis. The internal consistency was assessed using Cronbach's alpha. The intraclass correlation coefficient (ICC) value with 95% confidence intervals was calculated for test-retest reliability. Results: Ten items and four factors were selected through exploratory factor analysis. The internal consistency reliability of the tool was found to be Cronbach's alpha=0.658, and test-retest reliability was found to be ICC=0.812. Conclusions: An assessment tool of care for patients with mild disabilities by dental hygienists is a reliable and valid tool suitable for the job characteristics of dental hygienists. It can be used in clinical practice and research.

Effects site concentrations of propofol using target-controlled infusion in dental treatment under deep sedation among different intellectual disability types

  • Background: We aimed to assess the dose needed to achieve the propofol effect-site concentration using target-controlled infusion in intellectually disabled patients and to detail the most effective method for achieving a safe level of consciousness without hemodynamic changes as well as detail any resulting adverse effects. Methods: We performed a retrospective review of sedation service records of 138 intellectually disabled patients (51, mental retardation; 36, autism; 30, brain lesion, 12 genetic diseases, 9 dementia) aged over 15 years and weighing over 30 kg. These patients had received propofol via target-controlled infusion in the special care dental clinic of Seoul National University Dental Hospital from May 2008 to September 2018 for restorative treatment (112), minor surgery (13), prosthodontics (7), periodontics treatment (5), and implant (1). Results: For all groups, the duration of dental treatments was $43{\pm}18$ minutes, total sedation time was $73{\pm}23$ minutes, and total BIS values was $57{\pm}12$. The propofol maintenance dosage values for each group were: mental retardation, $3{\pm}0.5(2-4){\mu}g/ml$; autism, $3.1{\pm}0.7(2-5){\mu}g/ml;$; brain lesion, $2.8{\pm}0.7(1.5-5){\mu}g/ml;$; genetic disease, $2.9{\pm}0.9(1-4){\mu}g/ml;$ and dementia $2.3{\pm}0.7(1-3.4){\mu}g/ml;$. Conclusions: The dementia group needed a lower dosage to reach a safe, effective propofol effect-site concentration than the other groups. Since there were no complications, deep sedation is a great alternative to general anesthesia for dental treatment of intellectually disabled patients.

한국 치과대학 및 치의학 전문대학원의 장애인치과학 교육 현황 (CURRENT STATUS OF SPECIAL CARE DENTISTRY EDUCATION OF DENTAL SCHOOLS IN KOREA)

  • Special care dentistry, also known as special needs dentistry, is concerned with the oral health of people who have intellectual disability, or who are affected by other medical, physical, or psychiatric issues. Dental schools must educate dental students for the competency in managing and treating individuals with special health care needs. The purpose of this study is to identify the current status of special care dentistry education in Korean dental schools. A questionnaire relating to the education of special care dentistry was sent to eleven dental schools in Korea via email. The result turned out that eight out of eleven schools had classroom teaching of special care dentistry, while only four had practical teaching programs. Nine schools had dental clinics for the disabled, in which two of them had special care clinics within the Pediatric dentistry clinic, and other seven in separate clinical sites. Despite the increased interest in special care dentistry, education including classroom teaching and practical teaching was still insufficient and needed more development. This study implicated a small but valuable understanding of special needs dentistry education in Korea.

Prognosis following dental implant treatment under general anesthesia in patients with special needs

  • Background: This study retrospectively investigated outcomes following dental implantation in patients with special needs who required general anesthesia to enable treatment. Method: Patients underwent implant treatment under general anesthesia at the Clinic for the Disabled in Seoul National University Dental Hospital between January 2004 and June 2017. The study analyzed medical records and radiographs. Implant survival rates were calculated by applying criteria for success or failure. Results: Of 19 patients in the study, 8 were males and 11 were females, with a mean age of 32.9 years. The patients included 11 with mental retardation, 3 with autism, 2 with cerebral palsy, 2 with schizophrenia, and 1 with a brain disorder; 2 patients also had seizure disorders. All were incapable of oral self-care due to serious cognitive impairment and could not cooperate with normal dental treatment. A total of 27 rounds of general anesthesia and 1 round of intravenous sedation were performed for implant surgery. Implant placement was performed in 3 patients whose prosthesis records could not be found, while 3 other patients had less than 1 year of follow-up after prosthetic treatment. When the criteria for implant success or failure were applied in 13 remaining patients, 3 implant failures occurred in 59 total treatments. The cumulative survival rate of implants over an average of 43.3 months (15-116 months) was 94.9%. Conclusion: For patients with severe cognitive impairment who are incapable of oral self-care, implant treatment under general anesthesia showed a favorable prognosis.

뇌병변장애 1급 아동에서 발생한 중증유아기우식증 (SEVERE-EARLY CHILDHOOD CARIES IN CHILDREN WITH GRADE 1 NEUROLOGICAL DISORDER)

  • 뇌병변장애인은 뇌성마비, 외상성 뇌손상, 뇌졸중 등 뇌의 기질적 병변에 기인한 신체적 장애로 보행 또는 일상생활의 동작 등에 상당한 제한을 받는 사람으로 정의된다. 최근에 뇌병변 장애아동의 DMFT는 비 장애아동과 유사하거나 낮다는 연구가 있었다. 그러나, 본 증례는 중증유아기우식증이 나타난 뇌병변장애 1급 아동들을 치료한 것으로, 아동들의 공통점을 통해 중증유아기우식 우식증의 특별한 원인을 찾고자 하였다. 뇌병변으로 인한 일차적인 운동장애와 이차적인 치과치료를 받기 어려울 정도의 쇠약함, 잦은 입원, 당분이 많은 음식과 약물의 섭취, 구강위생관리 소홀, 늦은 치과 내원, 협조도 저하 등이 우식 유발인자를 높이고, 우식 예방인자를 낮추어 치아우식증을 유발할 수 있는 것으로 사료되었다. 뇌병변 장애아동을 치료할 때, 치과의사는 개인적인 우식 위험도 평가를 통해 철저한 우식 예방프로그램을 만들고, 부모와 의학적 주치의는 구강관리의 중요성을 인식하여 조기 치과 내원 및 홈케어에 신경을 써야 할 것이다.

지적장애인의 구강건강관리를 위한 생활재활교사의 치위생 중재에 관한 연구 (A study on dental hygiene management for mentally retarded person's oral health care of life rehabilitation teacher)

  • 구강보건 관련 취약계층에 해당하는 거주 장애인의 개인위생과 구강위생 업무수행의 어려움의 정도 및 구강보건 수행환경을 알아보고자, 경기도 장애복지 시설 생활재활교사에게 설문조사를 실시하여 총 111부를 기술통계와 상관분석으로 분석한 결과는 다음과 같다. 첫째, 거주 장애인의 개인위생은 자발적인 칫솔질, 칫솔질 중요도 인식, 자신의 칫솔 찾기와는 정적 상관을 보였다. 둘째 거주 장애인의 구강위생 수행과 관련한 어려움의 정도는 자신의 칫솔 찾기, 치과 의료기관의 구강검사 여부에서 보통이상을 수행할 수 있는 것으로 나타났고, 자발적 수행정도에서는 잇몸 맛사지, 설탕 함유 식품 자제순으로 잘하지 못하는 것으로 나타났다. 셋째, 자발적인 칫솔질을 잘 수행할수록 올바른 칫솔질 방법 등에 대한 교육을 적절하게 받고 있는 것으로 나타났으며, 3분 이상 칫솔질과 회전법 칫솔질을 스스로 잘 수행 할수록 구강관리용품을 적절하게 지원받고 있는 것으로 나타났다. 또한 혀 닦기를 잘 수행할수록 담당 장애인의 수는 적절한 것으로 나타났다. 따라서 거주 장애인이 개인위생을 실천하기 어려운 경우, 구강위생 실천에 도움을 줄 수 있는 생활재활교사 및 장애인에게 지속적인 치위생 중재 프로그램이 필요할 것으로 보인다.

QLF-D를 이용한 장애 아동 보호자의 구강위생관리 교육 : 증례보고 (PARENTS EDUCATION OF ORAL HYGIENE USING QLF-D IN PATIENTS FOR SPECIAL HEALTH CARE NEEDS)

  • 본 증례는 Pallister-Killian 증후군 및 뇌성마비, 외상성 뇌손상이 있는 환자의 보호자에게 구강위생 관리 교육 및 동기부여를 시행하기 위한 방법 중 QLF-D를 사용한 경우이다. 치과치료에 대한 접근성이나 협조도에 어려움이 있는 장애 아동에서 치아 우식의 조기탐지 및 회복에 대한 객관적인 진단 장비, 보호자 교육 및 동기부여에 대한 효과적인 시각적 교육 자료로서 QLF-D가 유용하게 사용될 수 있다. 더 나아가 구강 건강 증진을 통한 장애 아동의 삶을 질 개선을 기대할 수 있다.
