CAD/CAM system과 전통적인 방법을 이용한 총의치 동시 제작 증례 (Fabrication of computer-aided design/computer-aided manufacturing complete denture and conventional complete denture: case report)

  • Computer-aided technology는 최근 치과 치료의 흐름이다. DENTCA$^{TM}$ CAD/CAM denture (DENTCA Inc.)는 상용화된 computer-aided design/computer-aided manufacturing (CAD/CAM) 의치 시스템 중 하나로, 한 번의 내원을 통해 의치 제작에 필요한 환자의 모든 정보를 획득하여, 이 정보를 컴퓨터에 저장하고 3D 프린팅을 통해 두 번째 내원 시에 의치 장착을 목표로 한다. 현재까지 여러 증례들은 총의치 제작에 대한 CAD/CAM system의 임상적 적용을 시험해 보았다. 본 증례는 두 명의 환자에서 DENTCA system을 이용한 의치와 전통적인 방법을 이용한 의치를 동시에 제작하여 DENTCA system의 효용성과 한계점을 고찰하였다.

Adherence of Candida to complete denture surfaces in vitro: A comparison of conventional and CAD/CAM complete dentures

  • PURPOSE. The goal of this study was to compare the adhesion of Candida albicans to the surfaces of CAD/CAM and conventionally fabricated complete denture bases. MATERIALS AND METHODS. Twenty discs of acrylic resin poly (methyl methacrylate) were fabricated with CAD/CAM and conventional procedures (heat-polymerized acrylic resin). The specimens were divided into two groups: 10 discs were fabricated using the CAD/CAM procedure (Wieland Digital Denture Ivoclar Vivadent), and 10 discs were fabricated using a conventional flasking and pressure-pack technique. Candida colonization was performed on all the specimens using four Candida albicans isolates. The difference in Candida albicans adhesion on the discs was evaluated. The number of adherent yeast cells was calculated by the colony-forming units (CFU) and by Fluorescence microscopy. RESULTS. There was a significant difference in the adhesion of Candida albicans to the complete denture bases created with CAD/CAM and the adhesion to those created with the conventional procedure. The CAD/CAM denture bases exhibited less adhesion of Candida albicans than did the denture bases created with the conventional procedure (P<.05). CONCLUSION. The CAD/CAM procedure for fabricating complete dentures showed promising potential for reducing the adherence of Candida to the denture base surface. Clinical Implications. Complete dentures made with the CAD/CAM procedure might decrease the incidence of denture stomatitis compared with conventional dentures.

  • Dental computer-aided design (CAD) and computer-aided manufacturing (CAM) technology have rapidly progressed over the past 30 years. The technology, which can be used in the dental laboratory, the dental office and the form of production centers, has become more common in recent years. This technology is now applied to inlays, onlays, crowns, fixed partial dentures, removable partial denture frameworks, complete dentures, templates for implant installation, implant abutments, and even maxillofacial prostheses. Dentists and dental technicians, who want to use these techniques, should have certain basic knowledge about that. This article gives an overview of CAD/CAM technologies, histories and how it applies in prosthetic dentistry.

CAD/CAM 전용 금속 합금과 주조용 합금의 세라믹 결합강도에 관한 연구 (A Study on the Metal-Ceramic Bond Strength of CAD/CAM Metal Disk Alloy and Casting Alloy)

  • Purpose: The purpose of this study was to evaluate bond strength of Metal Disk alloy and casting alloy. Methods: Metal specimens were divided into 4 groups for each alloy. Three point flexural test were used to measure the bond strength of CAD/CAM metal alloy and casting alloy. Statistical analysis was done using the Statistical Package for Social Sciences version 19.0 for Windows. As for the analysis methods, the study used Kruskal-Wallis test. Results: The average bonding strengths of Group 1 to porcelain was $36.7{\pm}9.90$ MPa, Group 2 to porcelain was $37.68{\pm}4.51$ MPa, Group 3 to porcelain was $36.43{\pm}6.57$ MPa, Group 4 to porcelain was $42.88{\pm}6.81$ MPa, Each group was not significantly different. Conclusion: Bond strength of Casting alloy is equal to or higher than bond strength of CAD/CAM Metal Disk alloy. Alloy clinical bond strength is 25 MPa, So CAD/CAM Metal Disk alloy can be used as dental material.

지르코니아 CAD/CAM block의 강도에 관한 연구 (A study on the Flexural strength of zirconia CAD/CAM block)

  • Recently, zirconia is widely used in the field of dental ceramics thanks to the proliferation of CAD/CAM systems. Accordingly, different types of zirconia block are being solid in the market. However, there are no precise, objective standards for properties of zirconia. This study concerns the flexural strength of zirconia ceramic for CAD/CAM block. The test specimens used for this study were ZirBlank(Acucera), ZirBlank shade(Acucera), VITA, Cercon(Densply) and Cerasys. The test results suggest that ZirBlank shade block shows the highest flexural strength and density among the zirconia blocks tested. Its flexural strength was $971{\pm}58MPa$ and its density was 99.89%. On the other hand, Cerasys block shows the lowest flexural strength of $576{\pm}36MPa$ and the lowest density of 94.85%. Given all, the density of the specimens is found to be directly proportional to strength and inversely to grain size.

치과 CAD-CAM용 임시 수복재의 색 안정성 비교 연구 (Comparative Study on the Color Stability of Temporary Prosthetic Materials for Dental CAD-CAM)

  • Purpose: This study was conducted to examine the color stability of temporary prosthetic materials for dental CAD-CAM. Method: 2 kinds of existing acrylic resin temporary filling materials and 2 kinds of polymer prosthetic materials for dental CAD-CAM were used for testing, and five specimens respectively were absorbed in soy sauce, red pepper paste, and coffee. A Chroma Meter was used as analysis equipment. The color change of the specimens was observed to calculate ${\Delta}E$ value throughout 2 days, 3 days, 4 days, and 5 days and this was analyzed and the following result was obtained. Results: Snap's ${\Delta}E$ value was bigger than Vipi block(p<0.05). Red pepper paste had the largest impact on the color change of resin(${\Delta}E$), followed by soy sauce and coffee(p<0.000). With the passage of time, ${\Delta}E$ value was inversely(-) related and decreased somewhat(p<0.000).

치과 CAD/CAM 가공방식에 따른 임시보철물의 내면 적합도 : 3차원 중첩 분석 (Internal evaluation of provisional restorations according to the dental CAD/CAM manufacturing method : Three-dimensional superimpositional analysis)

  • Purpose: The purpose of the present study was to compare the internal fit of two different temporary restorations fabricated by dental CAD/CAM system and to evaluate clinical effectiveness. Methods: Composite resin tooth of the maxillary first molar was prepared as occlusal reduction(2.0mm), axial reduction(1mm offset), vertical angle(6 degree) and chamfer margin for a temporary crown and duplicated epoxy die was fabricated. The epoxy dies were used to fabricate provisional restorations by CAD/CAM milling technique or 3D-printing technique. The inner data from all crowns were superimposed on the master die file in the 'best-fit alignment' method using 3D analysis software. Statistical analysis was performed using a Wilcoxon's rank sum test for differences between groups. Results: It showed that the internal RMS(Root Mean Square) values of the additive group were significantly larger than those of other group. No significant differences in internal discrepancies were observed in the temporary crowns among the 2 groups with different manufacturing method. Conclusion: All the groups had the internal fit within the clinical acceptable range (< $50{\mu}m$). The continuous research in the future to be applied clinically for the adaptation of additive manufacturing technique are needed.

  • Currently CAD/CAM technology has been used widely in dentistry. But it has mainly been focused on fabrication of fixed partial dentures and implant-supported prosthesis. DENTCA company uses new cutting edge of CAD/CAM technology to revolutionize denture production. With developing a CAD/CAM technology of DENTCA company, it is possible to make complete dentures with minimum visits to the clinic. The aim of this article is to introduce a new denture-making method by CAD/CAM.

  • With the explosive increase of esthetic demands by patients, many dental materials for the esthetic restoration have been introduced. Recently, zirconia based restorations are using for the cases of single crown, bridges, implant prostheses etc. Zirconia have superior mechanical properties and biocompatibility. Owing to the properties of high strength, zirconia has to be manufactured by CAD/CAM system. Dental CAD/CAM system is a futuristic treatment and technical system which makes it possible to produce the precision and uniform prosthesis and also standardize the treatments. This article introduces the characteristics of zirconia, fabrication procedure using CAD/CAM system and procedure for the cementation of zirconia based restoration.

Marginal and internal fit of nano-composite CAD/CAM restorations

  • Objectives: The purpose of this study was to compare the marginal and internal fit of nano-composite CAD-CAM restorations. Materials and Methods: A full veneer crown and an mesio-occluso-distal (MOD) inlay cavity, which were prepared on extracted human molars, were used as templates of epoxy resin replicas. The prepared teeth were scanned and CAD-CAM restorations were milled using Lava Ultimate (LU) and experimental nano-composite CAD/CAM blocks (EB) under the same milling parameters. To assess the marginal and internal fit, the restorations were cemented to replicas and were embedded in an acrylic mold for sectioning at 0.5 mm intervals. The measured gap data were pooled according to the block types and measuring points for statistical analysis. Results: Both the block type and measuring point significantly affected gap values, and their interaction was significant (p = 0.000). In crowns and inlays made from the two blocks, gap values were significantly larger in the occlusal area than in the axial area, while gap values in the marginal area were smallest (p < 0.001). Among the blocks, the restorations milled from EB had a significantly larger gap at all measuring points than those milled from LU (p = 0.000). Conclusions: The marginal and internal gaps of the two nano-composite CAD/CAM blocks differed according to the measuring points. Among the internal area of the two nano-composite CAD/CAM restorations, occlusal gap data were significantly larger than axial gap data. The EB crowns and inlays had significantly larger gaps than LU restorations.
