Factors Influencing Quality of Life of Elderly People with Dementia (치매노인의 삶의 질에 영향을 미치는 요인)

  • Purpose: The purpose of this study was to identify the factors that affect quality of life in elderly people with dementia. Methods: The participants for this descriptive correlational study were 97 elderly patients with dementia who were attending a daycare center or were being cared for at home. Data were collected from the participants using the Mini-Mental Status Examination-Dementia Scale, the Geriatric Depression Scale (Short Form), Activity of Daily Living (ADL) and Instrumental Activities of Daily Living (IADL) Scale. Results: There were statistically significant differences in quality of life of elders with dementia according to mobility, using dementia day-care center, and cohabitation. In multiple regression analysis, depression, activity of daily of living, using dementia day-care centers and cohabitation were significant predictors of quality of life and the four variables explained 60% of the variance in quality of life. Conclusion: Findings indicate that to enhance the quality of life for elders with dementia, depression needs to be decreased and regular use of daycare service recommended. Further, various programs enhancing physical ability need to be developed and used with these elders.

The Survey on the Awareness, Interest and Need of Occupational Therapy Students for the Dementia Care Center (작업치료 전공 학생들의 치매안심센터에 대한 인식, 관심 및 요구도 조사)

  • Objective : The purpose of this study was to investigate the awareness, interest, and needs of the occupational therapy students for the dementia care center. Methods : We conducted an online questionnaire survey of 492 occupational therapy students nationwide from July 1 to 31, 2018. General characteristics and perceptions, interests and needs were analyzed by descriptive statistics. One way ANOVA and chi-square test were conducted to compare the differences between the items according to grade and age. Results : Lower awareness was found in National Responsibility for Dementia($2.61{\pm}1.0$), Dementia Care Center($2.84{\pm}0.9$) and role of an occupational therapist in the Dementia Care Center($2.94{\pm}0.9$) of the occupational therapy students. Interest to the employment was investigated between middle and high($3.42{\pm}0.9$). Education was not sufficient($2.98{\pm}0.8$), and the adequacy of fieldwork was moderate($3.07{\pm}0.8$). Conclusion : The results of this study can be served as basic data for enhancing competence as a specialist in the dementia care center. Also, it is expected to be a help to raise awareness of the occupational therapy students.

A Qualitative Case Study of the Narrative therapy group work on the elderly with palsy to solve conflict with the elderly with dementia (치매노인과의 관계개선을 위한 중풍노인 이야기치료 집단상담 사례 연구)

  • The purpose of this study is to explore the process for the elderly with palsy to solve conflicts with the elderly with dementia in day care center through the narrative therapy group work. For this, the researcher enacted the Narrative therapy group work on the 12 elderly with palsy for seven sessions, and analysed these data through qualitative case study method. The results of this study are as follows. First, the elderly with palsy became to recognize the elderly with dementia as 'the people suffering from dementia' rather than 'the problem', and tried to care them rather than to blame. Second, they became to recognize themselves as 'component people' rather than 'the victim'. Third, they grew intimate and collaborated with themselves. Fourth, they participated actively in day care center. It was discussed connection between these changes and narrative therapy skills, such as to separate problem from people, to respect client as expert for one's problem, to seek unique outcome. The recommendation is suggested that it is important to enlarge choice to participate for long-term care service consumer and to apply narrative therapy on the elderly.

Development and Testing of Day Care Program for Demented Elders (주간보호센터 중심의 재가 치매노인을 위한 프로그램의 개발 및 효과 검증 연구)

  • Purpose: The purpose of the study was to develop the day care program for the demented elders and to determine the effects of multiple activity therapies on the physical, emotional, cognitive, social function and physiological indicators of dementia elders in day care center. Method: A total of 13 elders with mild dementia in the data care center were participated. Manual and guideline of data care program was developed. Data care program was composed of movement/exercise, foot massage, music therapy, horticultural therapy, and art therapy. Data care program was performed by trained intervener for 32 weeks. Instruments were physical functioning such as hand grip strength., vital capacity, chiar stand test, sit and reach test, one leg standing, Berg' Balance scale, 6m Walk Velocity, stair climb, ADL, GDS-K, MMSE-K, Integrative Social Functioning Scale, cortisol, Ig A and Ig G. These were measured at the baseline, 16-week, and 32-week post-intervention by the trained nurses. Data were analyzed as Wilcoxon signed rank test. Results: For Hand grip, balance, and vital capacity, there was a significant difference between baseline and 16-week post-intervention(p<.05). For one leg standing, 6m velocity, there was a significant difference between 16-week post-intervention and 32-week post-intervention(p<.05). For Stair climb, there was a significant difference between baseline and 32-week post-intervention(p<.05). For depression and social functioning, there was no significant difference among baseline, 16-week, and 32-week post-intervention. For cognitive functioning, there was a significant difference between baseline and 32-week post intervention(p<.05). There was a significant difference of cortisol, and Ig G between baseline and 32-week post-intervention(p<.05). Conclusion: It would appear that physical fitness, functional performance, ADL, depression, social and cognitive functioning, and physiological indicator of community dwelling elders with dementia can be improved through day care program using multiple stimuli.

The Influence of Fall-Related Knowledge and Fall Prevention Self-Efficacy of Care-Givers Working in Long-term Care Hospitals with Older Adults with Dementia on Fall Prevention Behaviors and Fall Management Behaviors (요양병원 요양보호사의 치매노인에 대한 낙상 지식, 낙상예방 자기효능감이 낙상예방행위 및 낙상관리행위에 미치는 영향)

  • Objectives: The purpose of this study was to investigate the influence of fall-related knowledge and fall prevention self-efficacy of care-givers working in long-term care hospitals on fall prevention behaviors and fall management behaviors for older adults with dementia. Methods: Participants were 125 care-givers working in long-term care hospitals. Data were collected with structured questionnaires from August 7 to 14, 2018. Data were analyzed by t-test, ANOVA, $Scheff{\dot{e}}$ test, Pearson's correlation coefficients, and multiple regression analysis using SPSS 18.0. Results: Fall prevention self-efficacy influence fall prevention behaviors(${\beta}=.55$, p<.001), while fall-related knowledge impacted fall management behaviors(${\beta}=.43$, p<.001). Conclusions: The results suggest that an education program for fall prevention of older adults with dementia in long-term care hospitals should focus on improving the fall-related knowledge and fall prevention self-efficacy of care-givers to increase both the fall prevention behaviors and fall management behaviors of care-givers.

  • The purpose of this study was to understand the adult day care center as a place for the elders and adults and to develop an initial understanding of the services and activity programs for adult day care for the cognitively-impaired in the U.S.A. The data were collected from 12 Adult Day Care Centers(ADC) from 2001 to 2002 by personal interview and the documents about the programs. ADC programs provided primarily social, recreational, and health activities and services in a group setting. Centers offered participants to socialize, enjoy peer support, and receive health and social services in a stimulating and supportive environment that promotes better physical and mental health. For this purpose those centers offered not only professional health care, occupational.speech.physical therapies but also socio-recreation and therapeutic-recreation services.

Application of the Development Model of Day Care Center for Dementia (치매주간보호사업 개발모형의 적용)

  • This study was executed to develop the model of the format of the day care center which supplies the total service on the health care in the day time and social program related with the work and to evaluate the processing courses which was adjusted for 3 years from March 1999 to December 2001 in order to improve the quality of life of the family who experience dementia, dementia prevention, and the old and the burden of daily activities. The processing courses of this business model were evaluated to the confirmation on the health condition, application of the nursing processes, and the precise medical examination, and 20 sorts of social programs. The methods used in this model were the simplified MMSE-K, Ability for ,Daily Life(ADL), Instrumental Activities of Daily Living(IADL), and nursing process, and so on. The execution effects of this model are as follows. 1. It can contribute to the delay in the dementia process of the old people who are related with the dementia and maintenance and improvement of the health by confirming the stopping of the process of the dementia as the objective estimation method while the dementia symptom is maintained as the current condition through the medical and social total services. 2. The reduction of the burden for the daily care of the subject people and their families for the old people who are concerned with the dementia helped to improve the quality of life of the subject people and their family by enabling them to have jobs. 3. It enabled them to positively cope with the demand for the health and nursing of the local residents related to the old people. 4. It enabled them to enhance the recognition for the socialization of the local social organization and residents for the old people. 5. It enabled them to reinforce the related system among the local social organizations, and develop and provide the various social programs which are proper for the old people.

Development of Dementia-Care-Robot Integrated Program and Evaluation of Effectiveness -For the Elderly with Mild Dementia (치매케어로봇 통합 프로그램의 개발 및 효과검증 -경증치매노인을 대상으로)

  • The purpose of this study was to develop Dementia-Care-Robot integrated program based on group customization and to identify effects on IADL, depression, and attitudes toward robot. This study was nonequivalent control group pre-post test design. The subjects were 40 elders diagnosed with mild dementia participating in the dementia center daily program (20 in the experimental group and 20 in the control group). Each session lasted for 30 minutes, twice a week during 6 weeks. Data were collected from July 16 to November 7, 2019, and were analyzed using SPSS WIN 22.0 program to identify differences between groups for variables. After the program, the experimental group improved in IADL, depression, but there were no significant differences between the two groups. In attitude toward robot, the experimental group showed positive change to the robot, but the control group showed negative change to it, also there was a significant difference between the two groups. Findings indicate that integrated program using Dementia-Care-Robot has potential as a strategy to improve the symptoms of dementia and to delay the progression. It needs to be applied in the medical field through convergent approach of engineering.

The Effect of the Group Art Therapy using Recall on the Quality of Life and Emotion of the Elderly with Mild Dementia in Day Care Center (회상요법을 적용한 집단미술치료가 주간보호시설 경증치매노인의 삶의 질과 정서에 미치는 영향)

  • This study analyzed effect of the group art therapy with recall on the quality of life and emotion of the elderly with mild dementia in day care center. The study objects were the elderly from 75 to 96 years old and MMSE-K grade were 15-23 points with mild dementia in Jeollabuk-do day care center. The experimental group was treated group art therapy for 6 months, once a week, 60 minutes, total 30 times and the control group was treated none at the same duration. The study results had implications that group art therapy using recall method helped the elderly with mild dementia in day care center by actively embracing their life and expressing more stable emotion to get along their life.

Implementing a 1-Night, 2-Day Mental Health Healing Center Program for Dementia Patients and Their Caregivers

  • Objectives: This study aimed to assess the effects of a 1-night, 2-day mental health healing center program on the physical stress, autonomic nervous system health, brain activity levels, brain stress, concentration levels, and Patient Health Questionnaire-9 (PHQ-9) scores of dementia patients and their caregivers. Methods: Forty-eight dementia patients (average age 80 years, 14 males and 34 females) and 48 caregivers (average age 65.23 years, 14 males and 34 females) participated in the program. Pre- and post-assessments were conducted to measure the variables. Results: Dementia patients experienced reduced physical stress, increased brain activity levels, decreased brain stress, improved concentration levels, and a significant decrease in PHQ-9 scores (p<0.05). However, there was a tendency for a decline in autonomic nervous system health among dementia patients. A significant decrease in physical stress was seen in caregivers, but no other significant differences were observed. Conclusions: While the 1-night, 2-day mental health healing center program did not produce significant changes in the caregivers of dementia patients, it exhibited overall positive effects in dementia patients. Consequently, mental healing programs should be utilized effectively for dementia patients. Furthermore, investigating the significance of ongoing programs for the mental well-being of dementia caregivers is imperative, mirroring the attention given to dementia patient care.
