  • Purpose: In Korea, the prevalence of dementia patients has increased, which makes the care burden of nurses important. The purpose of this study is to identify factors affecting the care burden of nurses caring elderly patients with dementia. Methods: A cross-sectional design was conducted using a convenience sample of 127 nurses from two hospitals and a nursing home in Korea. Participants completed questionnaires on knowledge of and attitudes toward dementia, social support, self-esteem, dementia problematic behavior (DPB), and professional caregiver burden index. The data were analyzed by using the t-test, ANOVA, Pearson correlation, and multiple regression with the SPSS/windows version 21.0 program. Results: The influencing factors for nurse burden include day shift, DPB, self-esteem, social support, which explain 28.0% of care burden of nurses. Conclusion: To reduce the burden of the nurses, there needs to be an administrative system that focuses on enhancing their self-esteem and social support. Active institutional support may be necessary for the nurses taking care of elderly patients with dementia.

The data was collected through in-depth interviews with participants using open-ended and descriptive questions about their coping experiences. All data were were audio-taped and transcribed. The data were collected from February, 2000 to February, 2001. The participants of this study were 17 women. Results: Caregiver's coping types varied according to 'family's support', 'the condition of caregiver's health', 'the relationship with dementia elderly', 'family perception of dementia', 'fixed idea of traditional female role' and 'economic state'. In this study, five types of coping strategies were emerged: active role allotment tyle, meaning grant tyle, devotion tyle, duty defense type, and pessimism type. Among these, those who belong to the types of active role allotment, meaning grant, and devotion, including caregiver's good health were found to adapt themselves to caregiver roles. Conclusion: The results of this study may be helpful for developing effective and individualized nursing strategies suitable for individual caregivers coping types. It is suggested to practice nursing mediation and to analyze the changes in nursing effect and family members' adaptation based on coping types of the well-adapted caregivers as we as on their affirmative coping strategy.

  • Little is known about the impact of family caregiving for the demented elderly in Korea. The purpose of this study was to identify the current state of development of family caregiving research for the demented elderly in Korea and to identify correlates of caregiver or health problems and burdens within the socio-political contexts of Korea. A critical review of 17 family caregivers was carried out. The review revealed that various caregiving impacts and correlates of caregiver burden or health problems have been studied in relation to demented elderly family caregiving. Family caregiving for the demented elderly is a very complex phenomenon and various factors were related to caregiver burden, or their emotional and physical health. Findings from studies reviewed have shown inconsistent, inconclusive, and contradictory results. Furthermore, several conceptual and methodological problems were identified in the studies reviewed: restricted conceptualization, unrepresentative study samples, inadequate sample size, inappropriate study design, absence of comparison groups, inappropriate psychometric properties, and uncontrolled confounding factors. More research, as well as directions for further research, is recommended to identify family caregiving the impact of for the demented elderly, and to clarify the factors that explain results.

  • The goal of the present research is to improve a quality of life of the elderly with a dementia. In this paper, it is realized by developing the dialog system that is controlled by three kinds of modules such as speech recognition engine, graphical agent, or database classified by a nursing schedule. The system was evaluated in an actual environment of a nursing facility by introducing it to an older male patient with dementia. The comparison study between dialog system and professional caregivers was then carried out at nursing home for 5 days in each case. The evaluation results showed that the dialog system was more responsive in catering to needs of dementia patient than professional caregivers. Moreover, the proposed system led the patient to talk more than caregivers did.

  • Purpose: The purpose of this study was to investigate the effect of Family Resilience Reinforcement Program (FRRP) for family caregivers of the elderly with dementia on the family resilience, caregiver burden, family adaptation, perceived health status and depression. Methods: FRRP was implemented for 60 minutes each time, once a week, for 8 weeks. According to 3 factors of Walsh's Family resilience theory, FRRP was organized and specified as redefining 'belief system' in 1 to 2 sessions, figuring out one's own 'organizational pattern' in 3 to 5 sessions, enhancing 'communication process' in 6 to 8 sessions. A total of 46 family caregivers were assigned into either the experimental group with FRRP or the control group. Data were collected from February 18 to April 12, 2013 at the dementia support centers, and the data of 36 participants were finally analyzed. Results: The experimental group reported statistically significant differences in family resilience (p=.002), caregiver burden (p=.012), family adaptation (p<.001), and perceived health status (p=.002) compared to those in the control group. No significant difference was found between the two groups in depression. Conclusion: In the light of these results, FRRP is considered to decrease caregiver burden and to influence family resilience, family adaptation and perceived health status positively. The developed FRRP is considered to be an efficient nursing intervention for strengthening family resilience of the given population. It warrants future research expanding the range of target population to those caregivers of the patients with other chronic conditions.

  • Purpose. The purpose of the study was to identify the factors, which predict care burden perceived by caregivers of elders with dementia. Methods: The participants in this descriptive survey were 92 caregivers who used one of six daycare centers located in Incheon. The data were collected by questionnaires composed of items on general characteristics of the elders and caregivers, care burden, caregiver fatigue, and functional status of the elders. Results: General characteristics of the elders associated with care burden were age, gender, religion, and the presence of a spouse. Features of caregivers related to care burden were education, relationship with elderly, amount of rest, intention to care, monthly family income, and perceived family economic status. There were significant correlations among care burden, fatigue of caregiver and functional status of the elders. In stepwised multiple regression analysis, significant influencing factors were identified as caregiver fatigue, functional status of the elders, intention to care, relationship with the elders, amount of rest and elder's gender. Those variables explained 46% of variance of care burden. Conclusion: Functional status of elders and amount of rest were significant predictors and are supported by other previous studies. Future interventions for caregivers need to be focused on the slowing down of functional status of elders and increasing of number and length of caregiver's rest periods.

Conflicts in the process and the results of conflicts in the care process with hospital managers are listed based on case studies. More specifically, the efforts and passion of researchers to understand the problem behaviors of the elderly with dementia and to devise and verify practical management methods based on this are urgently needed. It will be necessary to secure data of various case studies in clinical practice through research methods.
