Design of Pressure Injury Management Mobile Application Structure and User Interface (욕창관리 모바일 어플리케이션 구조 설계 및 사용자 인터페이스 구현)

  • Purpose: This study aimed to design user interfaces of a mobile application for managing pressure injury patients in a long-term care hospital based on the user's needs. Methods: To reflect users' needs in the mobile application, the user interfaces in this study were designed in five steps: brainstorming and mind mapping, persona and scenario, needs list and priority, a draft version of flow chart and user interfaces and expert review. These steps were conducted with a step nurse at a long-term care hospital, a professor who majored in nursing informatics, a professor who had lots of research experiences about pressure injury and a wound ostomy continence nurse. Results: Two personas, scenarios and needs' lists were derived. Listed Needs included the followings; Accurate staging of pressure injury; Appropriate management by staging; Acquisition of professional knowledge about pressure injury; Acquisition of easy pressure injury information through text, picture and video; and Sharing pressure injury information in unit. The structure, menus and features of the pressure injury mobile application were visualized with user flow based on two personas' scenarios and needs' lists. Conclusion: Our study suggests and visualizes the key features of the 'Pressure Injury Guide', a pressure injury management mobile application for nurses in a long-term care hospital, which can be utilized by nurses, application developers, and related researchers.

A Study for the Enhancement of Accessibility to Community Home Nursing Care Services - The Home Nursing Care Program of Seoul Nurse Association - (지역사회에서의 가정간호 접근성 제고 방안 - '서울시간호사회' 가정간호사업 분석을 토대로 -)

  • Recently, there has been an increasing need for long-term care and comprehensive health care services in community settings. The Ministry of Health and Welfare introduced the Hospital-Based Home Nursing Care Program in 2000. Before this initiative, there was a Home Nursing Demonstration Center, affiliated with the Seoul Nurse Association, had offered home nursing services with the financial support from the local government. since 1993, the Center's nursing staff has been engaged in a general hospital in an effort to provide home nursing care services within Korea's health care system. The purpose of this study was to analyze and identify characteristics of community-based home nursing care supplied by a community-based home nursing team engaged in a general hospital. Also. visit nursing care services provided by public health centers were evaluated in terms of accessibility and supply versus demand, to enhance the accessibility of low-income patients living in Seoul to home nursing care services. Data were collected from home nursing insurance reimbursement claims submitted by the community-based home nursing care team from March 1 to October 30 in 2001 and a questionnaire survey on home-visit nursing services of 25 public health centers in Seoul. The subjects consisted of 197 patients and 12 public health centers. The result were as follows. First, medical institution's community-based home nursing care program was better in technical quality than health-center-based home-visit nursing care. In addition. the pattern of the subject patients was similar to that of hospital-based home nursing care program. Second, there was a high demand for community-based home nursing care while only a small number of home-visiting nurses served at public health centers in Seoul. As a result, many patients could not receive adequate care. Finally, we suggest that community-based home nursing care program should be introduced in the national health system to meet the at-home care needs of severely ill low-income patients. Furthermore, to better utilize home nursing and visit-nursing care resources and offer continued care for patients in community settings, an efficient referral network should be built among related institutions. This would require improvement of reimbursement system and amendment of the law related to health insurance system and community-based home nursing care services.

A Study on the Actual Conditions of Health Education in Elementary Nurses Teachers (초등양호교사 보건교육실태에 관(關)한 조사연구)

  • The purpose of this study was to survey the current status of informal health education which elementary school nurses have performed in these days. The questionaire survey in this study was obtained from 154 (74.0%) out of 208 elementary school nurses in Kyungbuk province. This study is about their health educational conditions, contents, attitudes. The results are summarized as follows. 1. General characteristics of respondents 1) As for age distribution, thirties with 56.5% is larger than any other group 2) As for school size, below 18 classes with 48.1% is the highest 3) As for the place in which the work, country side (below county) with 65.7% is the highest 4) 89.6% of total respondents have graduated from nurse college 5) Their total career below 10 years is 68.2% 6) Their clinical career below one year is 35.7% 2. Actual conditions of health education 1) The rate of respondents who execute health education is 75.3% out of 154 respondents 2) Health education does not execute because the task of school nurses is busy. This percentage is 55.3% 3) Respondents who draw up a yearly health education plan are 84.5% 4) Weekly health education classes with 1~2 times are 79.3% and teaching time with 89.7% is physical class. 5) 75.0% of total respondents don't use the textbook in health education and the reason is that the contents of nextbook are poor. 6) In health education, teaching aids with VTR, slide flims, charts etc. are 80.2% 7) 82.8% of total respondents don't evaluate the result of teaching 3. Analysis of contents of health education and attitudes of school nurses. 1) The rate of private sanitation for keeping health with 64.7% is high among the contents 2) 90.9% of total respondents demand health education and 92.9% of then demand its independence of a subject. 3) The degrees of school principals' help and understanding are 90.3%, and those of teachers are 89.6% 4) The degrees of school nurses' confidence are high at 35.1% 5) Regular health education classes a week of a school nurse with 3~4 times are 40.9% 6) The greatest difficulty in health education with 70.1% is lack of teaching aids.

A survey of private physician's opinion on utilization of nursing manpower (간호사 및 간호조무사 활용에 관한 개업의사의 의견조사)

  • The medical insurance system has been adopted in 1989. Since then, the utilization of medical care services has increased rapidly. It appears that the medical institution needs more nursing manpower such as nurses and nurse aides to meet an increased demand for medical care for the residents. However, the medical care clinics run by a medical practitioners has a more shortage of nursing manpower than hospitals. The purpose of this survey was to analyze the current employment status and to obtain an opinion on the improvement of utilization such as recruitment and retention of nursing manpower. The questionnaire sent to the private physician and the response rate was 28.7% ; 87 out of 300 physicians. 82.6% of the respondents employed nurses aides rather than nurses and an average number of employees per clinic was 2.5 persons. Most physicians had difficulties in recruiting nursing manpower. It took more than one month for replacement on average, therefore, they were suffering from giving good continual care for the patient and also high resignation of nursing personnel. The low wage, long working hours, hard work and lack of incentives are the biggest reasons for the difficulty in recruiting and retaining of the nursing personnel obtained for the clinics survey. The above mentioned problems would be solved in the near future by changing those difficulties. In the meantime, the private physicians are marking an effort to retain the nursing personnel by keeping a relationship of the private physician's cooperatives.

The Relationship Between Emotional Labor and Job Stress of Clinical Nurses in a Hospital (일개 병원 임상간호사의 감정노동과 직무스트레스에 관한 연구)

  • Purpose: The purpose of this research is to understand the relationship between emotional labor and job stress for clinical nurses. Method: The information for this research has been gathered in method of questionnaire for 202 nurses working in a hospital from January 30 to February 10, 2006. Results; The emotional labor for clinical nurses showed 2.43 on a scale of 4 and the job stress showed 49.84 out of 100. The correlation between the emotional labor and job stress showed that the emotional labor was higher, job stress was also higher. The result was that the factors that influenced the emotional labor were health status in a subjective point of view, working department, and the personality. The factors that influence job stress were self-efficacy, working department, and health status in a subjective point of view. Conclusion: The result of the research showed that emotional labor and job stress for clinical nurses were high and the strategy for reduction in those two areas is needed. Especially, diversified plans for job demand and occupational climate in an area of job stress that is correlated to emotional labor will be needed to minimize the emotional labor and job stress.

Comparative Study on the Nurses' Job Satisfaction between the Oriental Medicine Ward and the General Ward (한방병동과 일반병동 간호사의 직무만족도의 비교연구)

  • In our society today, a variety of medical caring system, along with a scientific development in the area of oriental medical science plus national demand, has increased the augmentation and the opening of oriental wards and hospitals (Han Bang), which has come to create an additional requirement of nursing activity at oriental medicine wards should be different from that of the general wards or the same as the other. In view of this, various studies need to be made in this connection. The purpose of this study is to comprehend the number of nurses who want to work at oriental medicine wards and measure job satisfaction in nursing as compared with those working at general wards so as to provide basic materials for future assignment and supplementary training for the nurses. An attempt was made to contribute toward nursing administration on the one hand improving nursing training course on the other hand. A total of 72 nurses currently working at two oriental medicine hospitals available in Seoul and 82 general nurses were selected for this study using the questionaire from December 1993 to January 1994. An instrument for the study was based on the measurement of work satisfaction developed by Paula L. Stamps including 37 questions complemented by Kim for revision (1993) and 14 questions regarding general characteristics and oriental medicine wards. The instrument to test dependability showed Cronbach's=0.7711. The collected data have been processed by computer package SPSS. General characteristics of the two groups and the matters involved in oriental medicine wards were calculated into real figure and percentage an similarities between the two groups were analyzed by t-test and F-test according to the characteristics of variables. The comparative test on work satisfaction among the two groups including general characteristics and work factors were conducted by t-test and F-test. The major findings as a result of the study are as follows : 1. As general characteristics, age group of $26{\sim}30$ years are more than any segment of age. As to marital status, the number of those in single status is slightly higher than the married. Approximately 80% of them are graduates of nursing schools and nursing colleges. They are mostly in service for one to three years. There is no significant difference between the two groups. 2. The number of those who want to work at oriental medicine wards represents 40.3% against 58.4%, being in favor of general wards. 3. The reason for service at oriental medicine wards is that "there is room for potential research" which happened to rank first, followed by "easy job," "good working atmosphere" and "growing interest in oriental medicine." 4. Work satisfactions among nurses who work at oriental medicine wards prove greater than that of nurses who work at general wards. 5. Work satisfaction between the two groups by work factors is reflected with significant difference statistically on task requirement, interaction and doctor-nurse relationships. 6. The general charcateristics and the work satisfaction by work factors prove that there are significant difference in age, marital status, education and the period of work. They tend to be more satisfied with the work as they grow in age. Significant differneces are found in the work factors such as autonomy, adiministration and professional job in the relationship with doctor-nurse. As to marital status, the married are more satisfied than the unmarried. There are significant differneces in the factors respecting administration and doctor-nurse college graduates are highly satisfied with task requirement. However, satisfaction with the professional level has proven the highest degree for those having master degree. The period of work and satisfaction : There are significant differneces in task requirement, administration, interactions, professional level and doctor-nurse relationships. As a general rule, the degree of satisfactions is in proportion to the lengrh of service. The following conclusions are drawn based on the fndings mentioned above. Even though the work satisfaction of the nurse who work at oriental medicine ward is relatively high, it is desired that personal consultation be given as to disposition of nurses when they are assigned to oriental medicine wards. It is also recommended that lectures on oriental medicine be conducted through supplementary training and/or basic nursing course in order that they may be motivated for ingenious activities with an increasing sense of self-esteem which will eventually enhance positive changes for the patients who are in need of oriental medicine nursing and for the medical teams. In addition, joint reseaches involvingclinical care and education should be in constant process for unique and scientific development for those who are subject to oriental medicine nursing care.

Relationship Between Job stress and Job Satisfaction Among Nurses in General Hospitals (종합병원 간호사들의 직무스트레스와 직무만족도와의 관련성)

  • The present study was intended to measure the relationship between job stress and job satisfaction among nurses working for a general hospitals. The self-administered questionnaires were given to 312 nurses during the period from February 25 to March 16, 2013. As a results, the level of job satisfaction of subjects was significantly lower in high group than low group in job demand, but it was significantly lower in low group than high group in job control, supervisor support and coworker support. In correlations, job satisfaction were found to be in a negative correlation with job demand, whereas in positive correlation with job control, supervisor and coworker support. In logistic analysis, the odds ratio of job satisfaction on job demand were significantly increased in the high group than in low group, but job control supervisor and coworker support were significantly decreased in the high group than in low group. In hierarchial multiple regression analysis, the affecting factors to the job satisfaction was selected variables such as educational background, work station, subject satisfaction of work, career choice motives, turnover experience, job stress contents(job demand, job control, supervisor support and coworker support). especially, job stress contents was significantly increased of explanatory power of job satisfaction. It suggested that the level of job satisfaction got more power by addition of job stress contents.

Management Strategy by Evaluation on Comprehensive Health Problem in the Community-dwelling Elderly of Korea (재가노인의 포괄적 건강문제 평가와 관리방안)

  • Purpose: The aim of this study was to define the health problem in the community-dwelling elderly of Korea and to compare differences of CAPs(Client Assessment Protocols) by characteristics. Method: Data was collected by visiting nurse from 556 elderly over 65 years in selected metropolitan areas. To evaluate the functional state of elderly in the community, I used "RAI-MDS HC; Residental Assessment Instrument Minimum Data Set-Home Care(2.0 version)" and established information exchange system among resources, by developing the data into a computer program. Results: The health problem of 'preventive health measures' was the largest(99.6%), and then 'health promotion(85.3%)', 'visual function(75.5%)', 'psychologic drug(68.9%)', 'pain(68.5%)', ‘social function(59.2%)', 'communication disorders(56.2%)', 'environmental assessment(53.2%)', 'depression & anxiety(46.9%)', 'oral health(43.4%)' followed. The number of health problems was average 10.16 in the community-dwelling elderly. Conclusion: The results suggest the need to emphasize the importance of assessment of the health problem of the elderly. We can apply it in the distribution of community resources and the development of service providing programs by figure out the health problem and resource in need for the elderly in the community.

Qualitative Analysis of Nurses′ Metacommunicative Behaviors in a Pediatric Unit (아동간호사의 상위의사소통 행위)

  • The purpose of this study was to identify metacommunicative behaviors between nurses and patients in a pediatric unit. The research method included observation using videotaping. Data were collected from December, 2001 to February, 2002. Total six nurses, and eight patients and their mothers in a pediatric unit participated in this study. The interactions were videotaped under the participants' consent. The participants were observed for total 8 hours over 2-day period. Special episodes which were identified as metacommunicative behaviors in the taped interactions were transcribed. Transcription included verbal and nonverbal interactions. Selected episodes were classified using Mitchell's definition. Each classified definitions were named, and categorized by its purpose. The results were as follows: Nineteen metacommunicative behaviors which used frequently by nurses-approaching, mediating eye level, eye contact, touching, encouraging, turnabout, mimic voice, giving choices, friendly demand, expansion, tagging, repeating and confirming, identification, reflection, baby talk, symbolization, description of acts, relaxed posture, turning away- were identified and organized into four categories. They were call for attention, facilitating response, empathy, and tension release. In conclusion, nurses in this study used metacommunicative behaviors frequently and these behaviors were effective in interacting with children. It is suggested that any educational programs to teach communication skills to nurses need to include techniques on metacommunicative behaviors. This will help nurses to be more sensitive to different characteristics of their patients.

The Demands on Parish Nursing Services by Pastors in Busan (부산지역 목회자의 교구간호사업 요구조사)

  • The purpose of this study was to provide the basic resources for developing a parish nursing program. We did this by investigating what demands were made on the parish nursing service by the parishes or churches under review. The subjects of this study were 96 pastors located throughout the City of Busan. NP (New paragraph) $\gg$ We conducted our research by utilizing a modified version of the study created by Hwang (2000) and by using the help of prior research and professionals gathered from the parish nurse questionnaire by Djupe (1990). The data in this study were collected from July 1 to Oct. 31, 2001, using the questionnaire method. The Data were analyzed by: (a) frequency: (b) percentage: (c) mean: (d) standard deviation, and x^2-test$ with SPSS/PC program. The study has found the follows: 1. For subjects making demands on the parish nursing service, 95.8% were in need of using the services of parish nurses. On the demands of parish nursing service for the subjects, 95.8% the necessity of parish nursing services. And they answered by their intention of asking for parish nursing service practice. 2. Of the subjects under review, 71.9% were part-time workers and 28.1% were full-time employees 71.9% of part time and 28.1% of full times duty. In terms of the method of pay for work, 41.8% were pay free or freelance while 51.2% were on salary. And engagement intention of parish nurse as preacher was 88.5%. 3. The demand for nursing services in various categories were as follows. (a) hospice care: (4.02 1.11), (b) health screening: (3.98 1.09), (c) home visiting: (3.97 1.16), (d) group health education: (3.81 1.12), (e) organization of volunteer groups: (3.75 1.12), (f) individual health education: (3.75 1.14), (g) advice on choosing hospital or hospital consultation: (3.69 1.21) and (h) individual counseling: (3.51 1.31). 4. In terms of the specific services rendered by parish nurses. our study found that services were needed for the following: physical symptom management; preparation before death in spiritual preparation for death, blood pressure check in health examination, home visiting where the patient makes phone call, management of chronic disease in group health education, disease management in individual health education, advice on choosing hospital, or hospital consultation: and physical problems in individual counseling. 5. With respect to whether there was a correlation between what church a pastor came from and the types of demands made, there was NO significant difference found. 6. In relation to the characteristics of the subjects and their church and the hope demands (duty pattern and method of payment and engagement intention of parish nurse as preacher) for parish nurses, these had non significant differences. In conclusion, the perception of parish nursing service is very high. Moreover, we found that there is a great demand for well ordered parish nursing services to promote the health of each congregation. Before doing so, it would be better to make things known and to consider the relevant characteristics shown in the researched results.
