Factors Predictive of Treatment by Australian Breast Surgeons of Invasive Female Breast Cancer by Mastectomy rather than Breast Conserving Surgery

  • Background: The National Breast Cancer Audit Database of the Society of Breast Surgeons of Australia and New Zealand is used by surgeons to monitor treatment quality and for research. About 60% of early invasive female breast cancers in Australia are recorded. The objectives of this study are: (1) to investigate associations of socio-demographic, health-system and clinical characteristics with treatment of invasive female breast cancer by mastectomy compared with breast conserving surgery; and (2) to consider service delivery implications. Materials and Methods: Bi-variable and multivariable analyses of associations of characteristics with surgery type for cancers diagnosed in 1998-2010. Results: Of 30,299 invasive cases analysed, 11,729 (39%) were treated by mastectomy as opposed to breast conserving surgery. This proportion did not vary by diagnostic year (p>0.200). With major city residence as the reference category, the relative rate (95% confidence limits) of mastectomy was 1.03 (0.99, 1.07) for women from inner regional areas and 1.05 (1.01, 1.10) for those from more remote areas. Low annual surgeon case load (${\leq}10$) was predictive of mastectomy, with a relative rate of 1.08 (1.03, 1.14) when compared with higher case loads. Tumour size was also predictive, with a relative rate of 1.05 (1.01, 1.10) for large cancers (40+ mm) compared with smaller cancers (<30 mm). These associations were confirmed in multiple logistic regression analysis. Conclusions: Results confirm previous studies showing higher mastectomy rates for residents of more remote areas, those treated by surgeons with low case loads, and those with large cancers. Reasons require further study, including possible effects of surgeon and woman's choice and access to radiotherapy services.

Prospective randomized clinical trial of hydrophilic tapered implant placement at maxillary posterior area: 6 weeks and 12 weeks loading

  • PURPOSE. Early loading of implant can be determined by excellent primary stability and characteristic of implant surface. The implant system with recently improved surface can have load application 4-6 weeks after installing in maxilla and mandible. This study evaluated the effect of healing period to the stability of hydrophilic tapered-type implant at maxillary posterior area. MATERIALS AND METHODS. This study included 30 patients treated by hydrophilic tapered-type implants (total 41 implants at maxilla) and classified by two groups depending on healing period. Group 1 (11 patients, 15 implants) was a control group and the healing period was 12 weeks, and Group 2 (19 patients, 26 implants) was test group and the healing period was 6 weeks. Immediately after implant placement, at the first impression taking, implant stability was measured using Osstell Mentor. The patients also took periapical radiographs after restoration delivery, 12 months after restoration and final followup period. The marginal bone loss around the implants was measured using the periapical radiographs. RESULTS. All implants were survived and success rate was 97.56%. The marginal bone loss was less than 1mm after 1 year postoperatively except the one implant. The stabilities of the implants were not correlated with age, healing period until loading, insertion torque (IT), the diameter of fixture and the location of implant. Only the quality of bone in group 2 (6 week) was correlated with the stability of implant. CONCLUSION. Healing period of 6 weeks can make the similar clinical prognosis of implants to that of healing period of 12 weeks if bone quality is carefully considered in case of early loading.

  • In order to solve the problems such as overload, single point of failure, non-optimized data path, and network scalability in conventional central mobility management protocols, distributed mobility management schemes have been continually studied in and around the IETF. In this paper, a network-based partially-distributed mobility management mechanism, pDMMv6, is suggested and the performance comparison with traditional protocols such as PMIPv6 and MIPv6 is made through simulation under the various user traffic environment. The simulation results include UDP packet delivery ratio, end-to-end packet delay, binding delay for registration signaling, CPU utilization in each node, and response delays in several server-client TCP applications such as web browsing, e-mail, telnet remote login, FTP file up/down-load, and database access.

  • This paper proposes an interactive VOD system to serve truly interactive VCR services using multicast delivery, client buffer and web-caching technique which implements the distributed proxy in Head-End- Network(HNET). This technique adopts some caches in the HNET that consists of a Switching Agent(SA), some Head-End-Nodes(HEN) and many clients. In this model, HENs distributively store the requested video under the control of SA. Also, client buffer dynamically expands to support various VCR playback rate. Thus, interactive services are offered with transmitting video streams from network, HENs and stored streams on buffer. Therefore this technique makes the load of network occur In the limited area, minimizes the additional channel allocation from server and restricts the transmission of duplicated video contents

AN EFFECT OF Nd : VAG LASER IRRADIATION ON THE MICROHARDNESS OF ROOT SURFACE (Nd : YAG layer 조사가 치근면의 미세경도에 미치는 영향)

  • Root caries is very frequently developed on exposed root surface after periodontal surgical treatment. In order to determine the anti-caries effect of Nd : YAG laser irradiation on periodontally exposed root surface, 40 mandibular molar teeth that had been extracted due to excessive periodontal destruction were used as the experimental teeth. All teeth were treated by the same procedure as conventional periodontal root treatment, ie thorough scaling, root planing and root conditioning with tetracycline HCl(100mg/ml, 5min.). Within middle one third of root, mesial half surface(20) or distal half surface(20) was randomly irradiated at various power of 1.0W, 2.0W, 3.0W and 4.0W for 60 seconds by non-contact(5mm) delivery of a pulsed Nd : YAG laser(EN.EL.EN060, Italy). The microhardness was measured by Vikers microhardness tester(Wilson, USA) at 2mm/second of jog speed under 100gm load. The difference of microhardness between irradiated side and non-irradiated side was statistically analyzed ANOVA and Duncan's method. Following results were obtained ; 1. The microhardness(Knoop hardness number) was significantly higher in laser irradiated surface than non-irradiated surface(p<0.05). 2. There was no significant difference in microhardness between experimental groups classified by different laser power(p>0.1). The results suggest that Nd : YAG laser irradiation on exposed root suface after periodontal therapy may inhibit the root caries development by enhancing surface microhardness.

  • Mobile IP is a simple and scalable global mobility solution. However, it may cause excessive signaling traffic and long signaling delay. So MN (Mobile Node) to the home network in order to reduce the number of location update signaling is necessary to reduce the delay. In this paper, the signaling overhead to be distributed evenly on the boundary of the regional network, each MN's dynamic mobility and traffic load is adjusted according to the PMIPv6 (Proxy Mobile IPv6) networks in the proposed dynamic regional mobility management scheme (dMMS). Each user in a distributed network system that offers the least amount of signaling traffic is tailored to the optimized system configuration. Signaling cost function in order to propose a new discrete analytical model is proposed, MN's mobility and packet arrival patterns. Location update and packet delivery costs to calculate the total average, the optimized area to determine the size of the network is proposed. The results of mathematical analysis, the proposed technique dMMS in terms of reducing the cost of the entire signaling were found to be excellent performance.

Stabilizing Technology of Pure Vitamin A using Triple Matrix Capsulation

  • In order to get stabilized pure retinol in skin care cosmetics, developing the three layered matrix bead capsules were studied. This study relates to make a cosmetic composition using the three layered matrix capsule that could increase the stability of the active ingredient. A primary encapsulation, vitamin A (pure retinol) of active ingredient was perfectly capsulated into water-in-oil (Water-in-Oil: W/O) emulsion vesicle using PEG-10 dimethicone copolyol emulsifier. A secondary encapsulation of multiple emulsion of the water-in-oil-in-water (W/O/W) emulsion blending W/O emulsion using sucrose distearate of surfactant was developed using homogenizing emulsifying system. Pure retinol of active ingredient was stably capsulized to inside the W/O/W-multiple emulsion in order to load the triple matrix capsulation. By coating it with a polymer matrix base, encapsulated in the triple layered type, which were developed bead encapsulation of 2~10mm uniformly size. To show beautifully appearance capsulated bead type, these finish particles in this triple matrix layer were developed as a gold, green, dark brown, silver and blue color were encapsulated in the bead types. Structural particle certification of triple matrix layer was observed through SEM analysis. Stability of pure retinol was remained stable more than 99.7% for 30 days at $42^{\circ}C$ incubating conditions compared with non-capsule. This technology was applied in different formulations such as various sizes and colors that by applying the skin care cosmetics. In the future, this technology to encapsulate an unstable active ingredient, we expect to be expanded this application in the food and drug as a time delivery system.

A Performance Comparison of On-Demand Routing Protocols for Application Services in MANET (MANET에서 응용 서비스 데이터 유형에 따른 요구기반 라우팅 프로토콜의 성능비교)

  • Recenty, there has been great interest in MANET from various areas. In this paper we focus on performance analysis of on demand routing protocols surf as DSR, AODV, and TORA in MANET. We have conducted several simulations concerned with application service data such as sensor, text. voice, and video data. And then, we have evaluated the performance of three protocols in a pre-designed ad hoc network, which is consisted of 20 nodes. As a result, we have obtained quantitative data for packet delivery fraction, average end to-end delay, routing load, channel utilization from upper layer and supportable routing protocol for application service data. The results can be used for designing specific-purposed ad-hoc networks.

A Dual Beaconing Scheme for Effective Context Awareness in Vehicular Ad Hoc Networks (차량 애드혹 네트워크에서 효과적인 상황 인지를 위한 이중 비콘 전송 기법)

  • Beacon transmission is one of the fundamental mechanisms in vehicular ad hoc networks. Variety services including context awareness can be embodied by exchanging beacons in each vehicle. In this paper, we propose dual beacon transmission instead of traditional single beacon transmission for beaconing mechanism. dual beacon transmission divides beacon into two functions and operates two beaconing process independently. We verified the superiority of dual beacon transmission by comparing beacon packet delivery ratio with single beacon transmission. In conclusion, dual beacon transmission can reduce channel load and have both wider communication range and effective context awareness. In addition, implementing dual beacon transmission has less complexity and appliable with conventional adaptive beaconing schemes.

A Traffic and Link Quality Based Congestion Control Scheme for Reliable Sensing Data Delivery in Wireless Sensor Networks (무선 센서 네트워크에서 신뢰성 있는 센싱 정보 전달을 위한 트래픽 및 링크 품질 기반 혼잡 제어 기법)

  • It has been occurred many times that wireless sensor networks (WSNs) had congested areas because all the sensing data collected by multiple sensor nodes are delivered to one sink node. Typically, in order to control congested areas, it used to reduce the traffic by increasing the sensing period or discarding packets. However, those schemes have a disadvantage that it loses the reliability when delivering sensing data. Moreover, there are no schemes considering case of having poor quality of links between nodes in practical terms. In this paper, we suggest a scheme not to reduce the traffic but to distribute the traffic by changing routing paths depends on the traffic and the quality of links. Also, it can be seen that the reliability of delivering of the sensing data is improved with the experiments improving collection rates and shortening end-to-end delay.
