Any types of burn injury that involve more than deep dermis often require reconstructive treatment. In gluteal region, V-Y fasciocutaneous advancement flap is frequently used to cover the defect. However, in case of large burn wounds, this kind of flap cannot provide adequate coverage because of the lack of normal surrounding tissues. We suggest V-Y adipofascial flap using the surrounding superficially damaged tissue. We present the case of a patient who was referred for full-thickness burn on gluteal region. We performed serial debridement and applied vacuum-assisted closure device to defective area as wound preparation for coverage. When healthy granulation tissue grew adequately, we covered the defect with surrounding V-Y adipofascial flap and the raw surface of the flap was then covered with split-thickness skin graft. We think the use of subcutaneous fat as an adipofascial flap to cover the deeper defect adjacent to the flap is an excellent alternative especially in huge defect with uneven depth varying from subcutaneous fat to bone exposure in terms of minimal donor site morbidity and reliability of the flap. Even if the flap was not intact, it was reuse of the adjacent tissue of the injured area, so it is relatively safe and applicable.
Structually organized mono- and multilayers were developed on gold for the catalytic and affinity biosensing using hyper-branched dendrimers. For the catalytic biosensing interface, a new approach to construct a multilayered enzyme film on the electrode surface was developed. The film was prepared by layer-by-layer depositions of dendrimers and periodate-oxidized glucose oxidase. The voltammograms obtained from the GOx/dendrimer multilayered electrodes revealed that bioelectrocatalytic response is directly correlated to the number of deposited bilayers. From the analysis of voltammetric and ellipsometric signals, the coverage of active enzyme per layer during the layering steps was estimated, demonstrating the spatially-ordered multilayer formation. As an extension of the study, dendrimers having various degrees of ferrocenyl modification were prepared and used. The resulting electrodes were electrochemically characterized, and the density of ferrocenyl groups, active enzyme coverage, and sensitivity were estimated. For the affinity-sensing surrface, a biosensor system based on avidin-biotin interaction was developed. As the building block of affinity monolayer, G4 dendrimer having partial ferrocenyl-tethered surface groups was prepared and used. And the biotinylated and electroactive dendritic monolayer was used for the affinity-sensing surface interacting with avidin. Electrochemical characterization of the resulting biosensor was conducted using free enzyme in electrolyte in terms of degree of surface coverage with avidin and subsequent surface shielding.

In this paper, we present a distributed fault-tolerant topology control protocol that configure a wireless sensor network to achieve k-connectivity and (k+1)-coverage. One fundamental issue in sensor networks is to maintain both sensing coverage and network connectivity in order to support different applications and environments, while some least active nodes are on duty. Topology control algorithms have been proposed to maintain network connectivity while improving energy efficiency and increasing network capacity. However, by reducing the number of links in the network, topology control algorithms actually decrease the degree of routing redundancy. Although the protocols for resolving such a problem while maintaining sensing coverage were proposed, they requires accurate location information to check the coverage, and most of active sensors in the constructed topology maintain 2k-connectivity when they keep k-coverage. We propose the fault-tolerant topology control protocol that is based on the theorem that k-connectivity implies (k+1)-coverage when the sensing range is at two times the transmission range. The proposed distributed algorithm does not need accurate location information, the complexity is O(1). We demonstrate the capability of the proposed protocol to provide guaranteed connectivity and coverage, through both geometric analysis and extensive simulation.
The uncertainty of measurement in nitrate from burley leaf tobacco by continuous-flow analysis method was evaluated following internationally accepted guidelines. The sources of uncertainty associated with the analysis of nitrate were weight of standard and sample, purity of standard, dilution of standard solution, calibration curve, water content, etc. The calculation of uncertainty based on the GUM(Guide to the Expression of Uncertainty in Measurement) and EURACHEM/CITAC Guide. An expanded uncertainty was obtained by multiplying the combined standard uncertainty with a coverage factor (k) calculated from the effective degree of freedom. The concentration of nitrate from burley leaf tobacco was $2.09\%$ and the expanded uncertainty by multiplying by the coverage factor(k, 2.20) was $0.13\%\;at\;a\;95\%$ confidence level.
Alumina composite membranes were prepared by chemical vapor deposition (CVD) using aluminum-tri-isopropoxide as a precursor. Porous alumina supports were used in deposition, which were in disk shape with mean pore diameter of 0.1 ${\mu}{\textrm}{m}$ and prepared by slip-coasting process. film deposition morphology on porous support was simulated through depositing alumina film on polycrystalline silicon pattern, and its step coverage observed by SEM showed one deviated from uniform step coverage. N2 permeability through composite membranes and the pressure dependence decreased as the deposition time increased. Initially, the N2 permeability of the top layer was tend to decrease rapidly, and then the degree of decrease in N2 permeability was tend to diminish with deposition time. The N2 permeability increased with heat treatment temperature and the crack was generated in top layer at 100$0^{\circ}C$.
Background: Korea has gradually expanded the coverage of medical care services in its national health insurance system. On April 1, 2018, it implemented a policy that expanded the coverage for an ultrasonography in the upper abdomen. In this study, we aimed to investigate the effect of the policy on the utilization of the ultrasonography in the upper abdomen in tertiary care hospitals. Methods: Using the dataset of the Health Insurance Review and Assessment Service, we explored changes in the utilization of the ultrasonography in the upper abdomen in tertiary care hospitals from July 1, 2017 to November 30, 2018 through the difference-in-difference (DID) mixed-effects-model method. Facility factor, equipment factor and personnel factors, type of hospital, the total amount of medical care expenses, and geographic region were considered as control variables. Results: On average, the utilization of the ultrasonography in the upper abdomen increased by 228% after the coverage expansion policy. However, the results of DID mixed-effects-model method analysis showed that the utilization increased by 73%. As for the number of beds, the utilization was higher with a group of 844-930, 931-1,217, and 1,218 or greater compared with a group of 843 or fewer, while the utilization of the number of ultrasonic devices was lower with a group of 45-49 compared with a group of 44 or fewer. The utilization decreased with the number of interns and the number of nurse assistants. Besides, relative to Seoul, the utilization was lower in the other metro-cities and provinces. Conclusion: The coverage expansion policy in the national health insurance system increased service utilization among people. Future research needs to investigate the degree to which such coverage expansion policy reduces the unmet medical care needs among the deprived in Korea.
Purpose: The integrity of interproximal hard/soft tissue has been widely accepted as the key determinant for success or degree of root coverage following the connective tissue graft. However, we reason that the gingival biotype of an individual, defined as the distance from the interproximal papilla to gingiva margin, may be the key determinant that influence the extent of root coverage regardless of traditional classification of gingival recession. Hence, the present study was performed with an aim to verify that individual gingival scalloping pattern inherent from biotype influence the level of gingival margin following the connective tissue graft for root coverage. Methods: Test group consisted of 43 single-rooted teeth from 21 patients (5 male and 16 female patients, mean age: 36.6 years) with varying degrees of gingival recession requiring connective tissue graft; 20 teeth of Miller class I and 23 teeth of Miller class III gingival recession, respectively. The control group consisted of contralateral teeth which did not demonstrate apparent gingival recession, and thus not requiring root coverage. For a biotype determination, an imaginary line connecting two adjacent papillae of a test tooth was drawn. The distance from this line to gingival margin at mid-buccal point and this distance (P-M distance) was designated as "gingival biotype" for a given individual. The distance was measured at baseline and 3 to 6 months examinations postoperatively both in test and control groups. The differences in the distance between Miller class I and III were subject to statistical analysis by using Student.s t-test while those between the test and control groups within a given patient were by using paired t-test. Results: The P-M distance at 3 to 6 months postoperatively was not significantly different between Miller class I and Miller class III. It was not significantly different between the test and control group in a given patient, either, both in Miller class I and III. Conclusions: The amount of root coverage following the connective tissue graft was not dependent on Miller's classification, but rather was dependent on P-M distance, strongly implying that the gingival biotype of a given patient may play a critical impact on the level of gingival margin following connective tissue graft.
Background: Most studies on the national health insurance benefit expansion policy have focused on policy tools or decision-making process. Hence there was not enough understanding on how policies are actually implemented within the specific policy context in Korea which has a national mandatory health insurance system with a dominant proportion of private providers. The main objectives of this study is to understand the implementation process of the benefit coverage expansion policy. Unlike other implementation studies, we tried to examine both the process of implementation and decision making and how they interact with each other. Methods: Interviews were conducted with the ex-members of the Health Insurance Policy Review Committee. Medical doctors who implement the policy at the 'street-level' were also interviewed. To figure out major variables and the degree of their influences, the data were analyzed with Winter's Policy Implementation Model which integrates the decision making and implementation phases. Results: As predicted by the Winter model, problems in the decision making phase, such as conflicts among the members of committee, lack of applicable causal theories application of highly symbolic activities, and limited attention of citizen to the issue are key variables that cause the 'implementation failure.' In the implementation phase, hospitals' own financial interests and practitioners' dependence on the hospitals' guidance were barriers to meeting the policy goals of providing a better coverage for patients. Patients, the target group, tend to prefer physicians who prescribe more treatment and medicine. To note, 'fixers' who can link and fill the gap between the decision-makers and implementers were not present. Conclusion: For achieving the policy goal of providing a better and more coverage to patients, the critical roles of medical providers as street-level implementers should be noted. Also decision making process of benefit package expansion policy should incorporate its influence on the implementation phase.

Since the 1990s, the application of Kentucky bluegrass cultivars has increased in newly-constructed Korean golf courses as opposed to the previously-used zoysiagrass. However, there have been few reports studying the selection and characterization of these Kentucky bluegrass cultivars under Korean weather conditions. A total of 12 Kentucky bluegrass cultivars were tested for their adaptability in environmental stresses during summer conditions in Korea in order to select the best Kentucky bluegrass cultivars for use in Korean golf courses. This study found that two Kentucky bluegrass cultivars ('Midnight' and 'Bluestone') maintained their green throughout the summer season and had a high degree of root density, as compared to the other cultivars tested. These two cultivars also had a good rate of coverage in the early growth period. These characteristics make them suitable for application on sports fields and golf courses, which receive many divots and the frequent replacement of turfgrass sod caused by both biotic and abiotic stress. Two other cultivars, the 'Ginney' and 'Nuglade', also displayed good visual quality and high rate of coverage under summer conditions. In conclusion, the 'Midnight' and 'Bluestone' cultivars performed well in areas including the maintenance of their green color, the number of roots and the rate of coverage during the summer months. These characteristics are necessary for golf courses sports fields, which receive many divots and requirethe frequent replacement of turfgrass. further research on a range of Kentucky bluegrass cultivars, including a sensitivity test for many pathogens and recovery rates from a variety of stresses, is required in the near future.
The Korean real-time traffic responsive control systems, Cycle Offset Split Model of Seoul (COSMOS), employs a single theoretical model to estimate the degree-of-saturation (DS) on approaches. However, the deployment of the system has been accomplished without practical consideration of its field performance. This paper delivers a diagnosis study performed to find the relationships yet known on the DS values against the operational conditions unproved in theory but ordinarily observed in field practice. Based on the analysis of the historical log data (476,505 cycles) obtained from the COSMOS server, it was found; (1) full coverage of lane detections should perform better than the sample coverage of detection in ordinary conditions, (2) the sample coverage of detection perform better than the other case with an exclusive bus lane, (3) detection in which a shared lane is involved provide poor estimation of DS, (4) poor DS estimation when a detection lane is adjacent to a shared lane, and (5) the DS values obtained during a day can hardly be stable all time. The findings suggest traffic engineers a progressive direction to move forward for the next real-time traffic control systems.
