Ebstein 기형에 대한 개심술 -12례 보고- (Open heart surgery of Ebstein's anomaly -Report of 12 cases-)

  • Ebstein's anomaly is a rare congenital cardiac malformation oand the ideal surgical correction seems controversial at present, and some problems are left unsolved in the surgical correction of this anomaly. Between June 1978 and June 1982, 12 patients with Ebsteins' anomaly underwent corrective open heart surgery at Seoul National University Hospital. Except for one patient, who had no ASD, all had a huge right atrium, secundum type ASD, and definite atrialized right ventricle. Typically, displaced tricuspid valve leaflets were found in all cases, but the degree of displacement and deformity were variable. In the point of NYHA functional classification, five were in class II, six were in class III, and one was in class IV. Ten patients were operated on by tricuspid valve replacement and pication. Two patients were operated on only by plication and annuloplasty techniques, and in all cases, ASD was closed. Postoperatively, four patients suffered from complete A-V block, and two of them died immediately. The remaining two patients took pacemaker generator implantation with good results. The other eight patients were in good condition. Tricuspid valve replacement using tissue valve and plication of the atrialized rght ventricle seems to be a good method of surgical correction for Ebstein's anomaly.

  • A wide variety of deformities can occur following repair of the cleft lip. Especially, cleft lip nasal deformities offer the severe psychologic, esthetic, and functional impairment. We must restore the deformities of alar cartilge, nasal tip, septum, columella, or pyriform aperture. The authors reconstructed the cleft lip nasal deformities using with the alar cartilage rearrangement, postauricular cartilage graft, and/or columellar lengthening. The 3 case reports are presented.

  • Oblique facial cleft is a rare congenital deformity. Its incidence has been reported as 0.24% of all reported cases of facial cleft. We report on a patient who had a left-sided oblique facial cleft with anopthamia, including lip and palate, nose alar base, and medial canthus. The patient also had a right-sided oblique facial cleft, which included lip and palate, nose alar base, medial canthus, and upper eye brow. Primary closure of the facial cleft was performed using multiple Z-plasty after excision of scar tissue.

  • Supravalvular aortic stenosis may be defined as an obstructive congenital deformity of the ascending aorta which originates just distal to the level of the origin of the coronary arteries. It may occur in several different anatomical form. Three type have been described; the hourglass, the hypoplastic and the membranous type, each term identifying the gross characteristic of the lesion causing by the aortic obstruction. Non cardiovascular condition commonly associated with supravalvular aortic stenosis are mental retardation, facial anomalies, hypercalcemia, etc. The diagnosis can be established preoperatively by left heart catheterization and selective angiography. Recently, we experienced a case of multiple localized supravalvular aortic stenosis involving, just above the sinus Valsalva and just proximal of the innominate artery. The surgical correction which was performed by a vertical incision across the each narrowing of aorta with replacement of diamond shaped double velour Woven Dacron patch under the CPB. He was discharged without any event.

  • Successful correction of the partial form of atrioventricular[AV] canal is now readily achievable. The most challenging technical features of this anomaly are the associated mitral valvular deformity and the frequent inability to obtain mitral competence. Residual mitral incompetence after repair of mitral cleft figures importantly in the causes of early and late failure. So, accurate and complete repair of the mitral valve cleft might be important. Determination of residual severe mitral regurgitation after repair is desirable to permit mitral valve replacement at the time. We have experienced one case of severe mitral regurgitation which was occurred 6 months after repair of partial form of A-V canal [ASD primum] in 22 years old virgin. Pre-op. left ventriculogram revealed severe mitral regurgitation in this patient. We preferred to perform mitral valve replacement with St. Jude valve in this patient at March 1984. Post-op. course was uneventful to now for 5 months and we report this case, review and discuss the literatures.

유돌염을 동반한 유돌골종 1예 (Mastoid Osteoma with Mastoiditis)

  • Osteomas are benign osteoblastic tumors that occur mainly in the fronto-ethmoid areas ofthe head and neck region. When they occasionally occur in the temporal bone, the external auditory canal is the most common site of origin; they rarely occur in the mastoid region. Moreover, mastoid osteoma with mastoiditis is an extremely rare entity in the temporal bone. Recently, the authors experienced a case of mastoid osteoma with mastoiditis in the left temporal bone. The mastoid osteoma was completely resected itself without a mastoidectomy, only for correction of the cosmetic deformity; the mastoiditis was not treated. Hence, the authors report the first case of a mastoid osteoma with mastoiditis in Korea, along with a review of the related literature.

  • Acquired Hallux varus is defined radiographically by a negative metatarsophalangeal angle and clinically by adduction of the hallux on the first metatarsal and most commonly occurs after hallux valgus surgery. It's the prevalence has ranged from 2% to 17%. We report a case of hallux varus resulted from weakening of support of lateral soft tissue and resection of an excessive amount of the metatarsal head during a bunionectomy after initial correction of hallux valgus. We corrected the hallux varus deformity using transfer of extensor hallucis brevis tendon with reconstruction os lateral capsule.

누두흉의 외과적 치료 -Metal bar를 이용한 수술치험 7예- (Operative treatment of pectus excavatum -7 cases used retrosternal metal bar-)

  • From Jan. 1983 to Dec. 1985, seven cases of pectus excavatum, six were male and one female, were underwent an operation at the Department of Thoracic and Cardiovascular Surgery, Kyungpook National University Hospital. The ages of patients ranged from 6 to 27 years. They all had symptoms of feeling inferiority about chest deformity. The concavity on the funnel chest varied in its extent, and the severity, which was measured by water volume filled into it, varied from 59.5cc/m2 to 129.9cc/m2. All but one patients were approached through a bilateral transverse submammary incision and one approached through a vertical midline incision. Successful surgical correction required resection of all deformed costal cartilages with transverse anterior osteotomy and internal fixation using retrosternal metal bar. No serious complication have followed the use of this technique, but minor complications such as serous accumulation, pneumothorax and strut migration have been experienced. All patients were satisfactory about the surgical results.

구순구개열 환자에 있어 직선봉합법의 역할 (Revisited Straight Line Technique for Unilateral Cleft Lip)

  • The variations of cleft lip deformities imposed the difficulty to the surgical correction for them. Straight line technique for cleft lip surgery has been ignored quite long since other techniques were developed. Initially the straight line technique was introduced and widely accepted because it is simple and easy to perform during the period of no adequate anesthetics. But it was abandoned for its several shortcomings such as tighten lip, vermilion notching, anatomical distortion, and wound contractures. Recently, some groups advocated the usefulness of straight line technique which has a significant modification from its original form. Additionally the variable degree of cleft lip deformity allows simple straight line closure for those patients. Here a case of simple straight line technique was presented and discussed for its reliability and plausible results as well.
