Primary malignant mucosal melanoma of the maxillofacial area

  • Objectives: We aimed to collect and report data from all patients who have been diagnosed with mucosal malignant melanoma to obtain the epidemiology and principles of current treatments. Materials and Methods: Between January 2008 and December 2018, 20 patients underwent surgery or follow-up observations at Yonsei University Dental Hospital. The patients' clinical information was reviewed retrospectively. Results: Seventeen of 20 patients had undergone definitive surgery, while only 6 patients received adjuvant radiotherapy or systemic therapy. Eight of 20 patients, including those that had recurrent lesions, were provided immunotherapy. The 3-year survival for all stages was 50%, with a local recurrence rate of 75% and a metastasis rate of 65%. Conclusion: The overall survival of patients receiving surgical treatment was longer than that of patients who did not undergo surgical resection. Eight of 20 patients received immunotherapy as the first-line regimen at our clinic, and those patients exhibited longer overall survival compared to patients in reported keynote studies.

  • Odontogenic keratocyst (OKC) accounts for 3-11% of all odontogenic cysts. OKC is a benign intra-osseous odontogenic tumor, but what makes this cyst special is its aggressive behavior and high recurrence rate. OKC is relatively aggressive compared to other odontogenic cysts, but its malignant transformation is considered extremely rare. Squamous cell carcinoma associated with odontogenic keratocysts have rarely been reported in the medical literature. We recently experienced a case of a 63-year-old man finally confirmed with squamous cell carcinoma of the mandible, which was initially diagnosed as a benign odontogenic keratocyst. Surgical resection was performed as definitive treatment. Therefore, we present this unique case with a review of the literature.

  • The definitive and effective method for tetanus prevention has contributed to decreased incidence and mortality of tetanus infections in developed countries. On the other hand, there has been a recent increase in incidence due to the lack of awareness of the disease and lack of adherence to preventive measures. Tetanus infections can be fatal. Therefore, prevention must be the utmost priority, followed by aggressive surgical management along with supportive care. The authors present a case of a 59-year-old male patient diagnosed with a generalized tetanus infection from a foot injury. The patient recovered successfully after aggressive surgical debridement and amputation of the infected toe at the metatarsal level, followed by two months of supportive care.

  • A public health concern emerging from a zoonotic disease. Monkeypox is caused by the orthopoxvirus specie Monkeypox virus (MPXV). Monkeypox was identified as the most common orthopoxvirus infection in humans following the eradication of smallpox. Monkeypox has a similar clinical presentation to smallpox. The MPXV is now considered a high-threat pathogen that causes a serious public-health problem. The continuous spread of Monkeypox virus from West Africa to all other places around the world throughout 2018 to 2022, have raised concerns that MPXV could have emerged to acquire the immunological and ecological niche vacated by smallpox virus. This review highlights the current knowledge about Monkeypox evolution, infection biology, and epidemiology around the world since from 1970 to 2022, with a focus on the human, viral, and cellular factors that influence virus emergence, infection, spread, and maintenance in nature. This paper also discusses the current therapeutic options for Monkeypox treatment and control. Under the right conditions, with limited smallpox vaccination and very little orthopoxvirus immunity in some areas of the world, MPXV could become a more efficient human pathogen. Finally, the review identified knowledge gaps, particularly in terms of identifying a definitive reservoir host for monkeypox and proposes future research endeavors to address the unanswered questions.

Importance of 3-dimensional imaging in the early diagnosis of chondroblastic osteosarcoma

  • The aim of this report is to present a case of chondroblastic osteosarcoma located in the right maxillary premolar region of a 17-year-old female patient. The initial clinical presentation and 2-dimensional (2D) radiographic methods proved inadequate for a definitive diagnosis. However, a cone-beam computed tomography scan revealed a hyperdense, heterogeneous lesion in the right maxillary premolar region, exhibiting a characteristic "sun-ray" appearance. To assess soft tissue involvement, a medical computed tomography scan was subsequently conducted. A positron emission tomography scan detected no metastasis or indications of secondary tumors. T1- and T2-weighted magnetic resonance imaging showed signal heterogeneity within the lesion, including areas of low signal intensity at the periphery. Histological examination conducted after an incisional biopsy confirmed the diagnosis of highgrade chondroblastic osteosarcoma. The patient was then referred to an oncology department for chemotherapy before surgery. In conclusion, these findings suggest that early diagnosis using 3-dimensional imaging can detect chondroblastic osteosarcoma in its early stages, such as before metastasis occurs, thereby improving the patient's prognosis.

  • Amyotrophic lateral sclerosis(ALS) is a progressive disorder that causes degeneration of motor neurons of the brain and spinal cord. It is characterized by loss of motor neurons leading to muscle weakness, and sensation and mental function stay intact during the course of the disease. Diagnostic tests include magnetic response imaging(MRI), electromyogram(EMG), muscle biopsy, and blood tests. In order to a definitive diagnosis of ALS, damage must be evident in both upper and lower motor neurons. When three limbs are sufficiently affected, the diagnosis is ALS. There is no cure for ALS. We experienced one case of ALS, the patients was diagnosed as ALS by EMG and symptoms. We diagnosed her as ShinEumHur(SEH) and treated by Choakwiyeum(左歸飮). We report the change of her symptoms through oriental medical treatment.

Lymphaticovenular anastomosis for Morbihan disease: a case report

  • Morbihan disease (MD) is a very rare condition characterized by rosaceous or erythematous lymphedema on the upper two-thirds of the face. A definitive management strategy for MD is lacking, and treatment is challenging. Herein, we present a case of persistent bilateral eyelid edema treated by lymphaticovenular anastomosis (LVA) and lymph node-vein bypass surgery. The patient experienced persistent bilateral eyelid edema. Indocyanine green lymphography was performed, and the diagnosis of bilateral facial lymphedema was confirmed. On the right side, a preauricular lymphatic vessel was anastomosed to a vein. On the left side, lymphostomy on the preauricular lymph node was done, with anastomosis to the transected proximal end of the concomitant vein of the transverse facial artery. Furthermore, a preauricular lymphatic vessel was anastomosed to a vein. Eyelid edema decreased and progressively improved on both sides. The outcome of this case suggests that LVA and lymph node-vein bypass surgery are appropriate for treating persistent eyelid edema related to MD.

Effects of D-ribose Supplementation on Run-to-exhaustion Time and Antioxidative Capacity under Sea Level or High Altitude Condition (D-라이보스 섭취가 해수면환경과 고지대환경에서의 운동지속능력 및 항산화능력에 미치는 영향)

  • This study evaluated the effects of D-ribose supplementation on exercise time in a hypoxic chamber. Eight healthy young males participated in experiments under all four different conditions; placebo+normoxic, placebo+hypoxic, ribose+normoxic, and ribose+hypoxic. Subjects took 1 g per 10 kg body weight of ribose dissolved in drinking water 30 minutes before and immediately before running. We observed the run-to-exhaustion time, the maximum heart rate, and the changing pattern of the heart rate during exercise. The longest running time was achieved when subjects ran under normoxic condition with ribose supplementation. The shortest running time occurred when subjects ran under hypoxic conditions without ribose supplementation. We measured MDA and GPx to determine any changes in oxidative stresses or antioxidative systems. MDA was affected by the environmental conditions and the running time. The activity of GPx showed a significant difference only with the different environmental conditions of exercise. The results of this study indicate that ribose can be considered a possible ergogenic during exercise in specific conditions, but more detailed and well-controlled studies are needed to make a definitive conclusion.

Genetic determinants of periosteum-mediated craniofacial bone regeneration: a systematic review

  • Background: Periosteum-mediated bone regeneration (PMBR) is a recognized method for mandibular reconstruction. Despite its unpredictable nature and the limited degree to which it is understood, it does not share the concerns of developmental changes to donor and recipient tissues that other treatment options do. The definitive role of the periosteum in bone regeneration in any mammal remains largely unexplored. The purpose of this study was to identify the genetic determinants of PMBR in mammals through a systematic review. Methods: Our search methodology was designed in accordance with the PRISMA (Preferred Reporting Items for Systematic Review and Meta-Analysis) guidelines. We conducted a quality assessment of each publication, and evaluated the differences in gene expression between days 7 and 15. Results: A total of four studies satisfied the inclusion criteria. The subjects and tissues examined in these studies were Wistar rat calvaria in two studies, mini-pigs in one study, and calves and mice in one study. Three out of the four studies achieved the necessary quality score of ≥ 3. Gene expression analysis showed increased activity of genes responsible for angiogenesis, cytokine activities, and immune-inflammatory responses on day 7. Additionally, genes related to skeletal development and signaling pathways were upregulated on day 15. Conclusions: The results suggest that skeletal morphogenesis is regulated by genes associated with skeletal development, and the gene expression patterns of PMBR may be characterized by specific pathways.

Burning measure for burning mouth syndrome: a systematic review

  • This current systematic review aimed to evaluate the current evidence on the effect of topical capsaicin application to alleviate symptoms related to burning mouth syndrome (BMS). PubMed, Ovid SP, and Cochrane were searched from 1980 to 2022 to identify relevant literature. A total of 942 titles (PubMed, 84; Ovid SP, 839; Cochrane, 19) was retrieved, of which 936 were excluded based on the title and abstract. A total of 11 studies were further evaluated for full text analysis, of which 7 were excluded. As a result, 4 articles were included for qualitative synthesis of data. Capsaicin as a mouthwash can have potential application in the treatment of symptoms related to burning mouth. The quality of available studies is moderate to low, and a well-designed randomized multicentric study comparing capsaicin with other active agents is planned to obtain more definitive conclusions.
