  • This study was attempted to evaluate the effectiveness of cold cardioplegic solution [group B] compared with that of intermittent aortic cross clamping [group A]. 1. In group A, two dogs recovered spontaneous heart beat without using defibrillator and one case was used defibrillator once. In group B, four dogs recovered the spontaneous heart beat even without defibrillator and in one animal the defibrillator was used three times. 2. There was no significant difference in serum electrolyte [Na+, Ca2+, Cl- ] between group, but K+ level was decreased in both group. 3. The group A showed more increasing in serum LDH activity than group B. 4. The postoperative myocardial ATP level was $2.35{\pm}0.10$ ${\mu}$M/Gm. of wet weight in control group, $1.46{\pm}0.16$${\mu}$M/Gm. of wet weight in group A [60.2% of control group], and $1.80{\pm}0.14$${\mu}$M/Gm. of wet weight in group B [76.8% of control group].

  • In designing defibrillator, several parameters such as patient's transthoracic impedance, output energy level, peak current, and time duration of current waveform must be considered to generate optimum electrical shocks on the heart. Patient's transthoracic impendence depends on the physical and health condition of patient. In this study, before the development of a defibrillator, the range of above parameters value as circuit elements was determined to derive optimal waveform by predicting and analyzing the performance of designed circuit by means of simulation with the software, P-Spice. The efficiency of parameter design was verified through the performance test with the developed defibrillator.

  • Purpose: This study aimed to assess defibrillator pad positioning by the general public and healthcare providers during basic life support (BLS) renewal education (RE). Methods: A total 130 subjects performed defibrillation using an automated external defibrillator (AED) equipped with electrode pads developed by the researchers and male resuscitation manikins, in order to assess electrode pad placement (EPP) before BLS RE. Subjects included 54 Healthcare providers (HP) and 76 members of the general public (GP). Results: The apex EPP was positioned in the risk area (RA), more than 5 cm distant from the area recommended, by 63.2% of the GP (n = 48). The apex EPP was positioned in the RA by 44.4% of the HP (n = 23). The distance from the apex (GP $5.27{\pm}2.21cm$, HP $4.18{\pm}1.71cm$) was statistically significant, using Student's t-test. Conclusion: For both GP and HP, apex EPP was significantly in error. Thus, to prevent defibrillation failures, EPP education and practice for GP and HP in all BLS education programs should be improved, and incorrect Korean cardiopulmonary resuscitation guidelines and related literature should be revised and corrected.

Method: This study targeted totally 76 undergraduates (39 people for experimental group, 37 people for control group) at J university where is located in J province, from June 8, 2009 to June 12. The analytical method was performed by using SPSS WIN 12.0 Version program. Results: In difference before and after education of the skill performance accuracy in AED between groups, the experimental group(2.89) was higher compared to the control group(2.75). In difference before and after education of knowledge in AED between groups, the experimental group(2.03) was higher compared to the control group(1.82). In difference before and after education of attitude toward AED between groups, the control group(2.14) was higher compared to the experimental group(1.85). In difference before and after education of self-confidence in AED between groups, the experimental group(1.88) was higher compared to the control group(1.76).

  • Cardiac resynchronization therapy (CRT) is a new treatment for refractory heart failure. However, most patients with heart failure treated with CRT are adults, middle-aged or older with idiopathic or ischemic dilated cardiomyopathy. We treated a 12-year-old boy, who was transferred after cardiac arrest, with dilated cardiomyopathy, left bundle-branch block, and ventricular tachycardia. We performed cardiac resynchronization therapy with a defibrillator (CRT-D). After CRT-D, left ventricular ejection fraction improved from 22% to 4 4% assessed by echocardiogram 1 year postoperatively. On electrocardiogram, QRS duration was shortened from 206 to 144 ms. The patient's clinical symptoms also improved. For pediatric patients with refractory heart failure and ventricular arrhythmia, CRT-D could be indicated as an effective therapeutic option.

  • In designing defibrillator several points must be considered such as patient's transthoracic impedance, output energy level, peak current, tine duration of current waveform. Patient's transthoracic impedance depends on patient respectively and the health condition of patient. In this study, before the hardware implementation of defibrillator we determine the range of parameter values of circuit elements to derive optimal discharge waveform by predicting and analyzing the performance of designed circuit.

  • Virtual training is a kind of training that proceeds as if it were a real situation. In recent years, there has been a growing demand for experiencing a situation in which a virtual reality technology has not been experienced directly in the real world due to the rapid development of the technology. Especially, safety education is very necessary in Korea where safety accidents are caused by many disasters. Therefore, simulation of disaster response training using virtual reality is more urgent than ever. Although the automatic defibrillator is the medical device that is most needed to rescue patients with cardiac arrest, few people know how to use it. Therefore, there are very few cases where the use of automatic defibrillators has saved the patient's life in Korea. The proposed Automated External Defibrillator virtual training system enables immersive and experiential training in real situations and effective training at low cost.

  • Purpose: The purpose of the study is to emphasize the importance of out-of-hospital cardiac arrest resuscitation. This resuscitation by paramedic is very effectively performed under the medical direction of the doctors. Methods: The cardiac arrest victim was 4 month old infant. Informed consent from the parents of the infant was received. CPR combined with 150J defibrillation was performed to the 4 month old infant. Results: We reported that the 4 month old infant survived the cardiac arrest. Out-of-hospital cardiac arrest infant survived after 150J automated external defibrillator and CPR performance. Conclusion: Specific operative protocol is important because the paramedic can apply the proper manual defibrillator effectively. It is important to extend the work scpoe of the EMT.

  • Purpose: The purpose of the study was to investigate knowledge and attitudes of students majoring in health-related fields toward automated external defibrillator (AED) and to identify the influencing factors of knowledge and attitudes in AED use among the students. Methods: Data were collected from 346 students and analyzed using descriptive statistics, t-tests, one-way ANOVA, post hoc $Scheff{\acute{e}}$ test, Pearson's correlation coefficient, and hierarchical multiple regression analysis, with SPSS Win 20.0 program. Results: Overall knowledge of AED in health-related major students, except paramedics and physical therapy was relatively low while attitudes were at moderate level. There were statistically significant differences in knowledge according to age, grade, major, cardiopulmonary resuscitation (CPR) awareness, exposure to AED and CPR education; and differences in attitude according to major, health status, health awareness, CPR awareness, exposure to AED, and CPR education. Significant factors explained 63% of knowledge on AED (F= 43.96, p<.001) and 37% of attitudes on AED (F= 15.84, p<.001). Conclusion: The study findings suggest that systemic education programs on AED should be implemented during undergraduate curriculum to improve survival rate of sudden cardiac arrest.
