  • Objectives : The aim of this study was to survey the present utilization of pattern identification(PI) by chronic disease and using PI system in the clinical field among Korean medicine doctors. Methods : This survey was conducted from Oct. 1 to Oct. 31, 2014 by face-to-face interview using structured questionnaire. The subjects were 400 Korean medicine doctors who worked in hospitals or clinics. The questionnaire consisted of whether use clinical experience by chronic disease and the utilization of PI, the utilization of PI systems, the PI utilization rate, and the correlation between utilization PI and so on. General elements consisted of sex, age, clinical experience, place of work, district and so on. Results and Conclusions : This study revealed that clinical experience by chronic disease used 24.0-90.8%. The most chronic diseases were chronic fatigue and chronic pain, and least disease was cancer. Experience of the utilization of PI among Korean medicine doctors who have clinical experience was 87.8-97.1%. Also, the most utilized disease was chronic gastritis & gastric ulcer, and least disease was dyslipidemia. In this case, the most common cases were using the one PI between the utilization of PI systems, and the utilization ratio of Visceral PI was the highest. Phi coefficient between the PI systems in chronic diseases divided into positive correlation and negative correlation. The correlation of dementia between Triple energizer PI and Six excesses PI was the highest at positive correlation, and the frequency of occurrence between Defense, Qi, Nutrient and Blood PI was the highest at positive correlation. The correlation of osteoarthropathy between Meridian and Collateral PI and Sasang Constitutional Medicine was the highest at negative correlation. Also the frequency of occurrence between Qi, Blood, Fluid and Humor PI and Sasang Constitutional Medicine was the highest at negative correlation. We hope that additional studies on systematic PI research of chronic disease which needed to be in korean oriental medicine and meet the needs of clinical consumer continue based on this study.

  • In recent years, there were frequent exogenous contagious diseases in Eastasia like SARS(severe acute respiratory syndrome), Avian influenza, Swine influenza, MERS etc. But there are various interpretations about their pathological differentiations and lead to controversy to diagnosis and medicinal use. So there needs universal and consistent understanding methods. Several conclusions are obtained from the research on differentiation theories of various epidemic diseases. Essential elements of differential diagnostic system are pathogen, characters and matters of disease and loci, especially three yin and three yang has close affinity with constitutional features or body shape. Binding these 3 categories, an integrated differentiation 3 dimensional coordinates are made. Out of these, each elements of 3 pathogen-axial lines are related with names of exogenous disease, and those of 3 feature-axial lines are related with 8 principal patterns. And those of 3 locus-axial lines implicating therapeutic method are related with steps and location of exterior and interior, 3 yin 3 yang, Defense, Qi, Nutrient and Blood, five viscera and six bowels and tissues. Additionally, 3 lines of each axis consist of factors which have their own affinity each other, so classification of pathogen, feature, locus of disease has layered interconnectedness. This classification system is included in constitutional features of individual patient. Afterwards, these cognitive structure can be used as a general theory guiding method of therapy, prevention and aftercure healthcare.

This purpose of this study was to examine the trend of Hwabyung research and to serve as a guide for the future 'Hwabyung' study. The results of studies were as follow. In the design of research, clinical studies are definitely dominant. As for the fields of research, the concept of Hwabyung was studied more frequently than the others. As for the authorship. psychiatrists prevailed but nursing professionals are on the increase. Research subject in clinical and survey study, patient was definitely dominant and the place of study was almost always a hospital (75%), data collection was higher on interview (58.3%). Hwabyung was considered unique culture bounded syndrome related to Korean culture. Frequent etiologic factor of Hwabyung were a husband's extra-marital affair, conflicts between houses wives and mothers-in-laws, and financial loss and suffering. • Objectives : This paper examines major symptoms representation in COVID-19 patients as groundwork for development of an effective clinical data collection format in Korean Medicine. Methods : Major symptoms representation in COVID-19 related papers published worldwide were collected. Corresponding symptoms in Korean Medicine were then examined, followed by discussion of symptomatic features that require further consideration in regards to a more systematic clinical data collection. Results : Of 256 papers, most papers listed fever and cough while symptoms such as difficulty breathing, diarrhea, muscle pain, headache, nausea, fatigue, chest pain, phlegm, nasal discharge were also mostly listed. Clinical representations could be categorized into general symptoms, throat symptoms, chest symptoms, head and facial symptoms, gastrointestinal symptoms, musculo-skeletal and cutaneous symptoms, psychiatric symptoms and sensory problems. Conclusions : Although each clinical representation could be likened to certain clinical representations of Korean Medicine, the variety of symptoms were too limiting and lacking in detail to be applied in the pattern identification[辨證] of Korean Medicine. For effective clinical data collection and analysis in the future, symptom change according to time, comparison between location, climate and ethnicity, existence of interior symptoms when diagnosing exterior symptoms, deficiency-excessiveness of blood patterns, consciousness levels, etc., need to be considered in establishing criteria for symptom evaluation.

  • Objective : This case series study was designed to assess the effect of Gamisungmagalguntang for chronic urticaria identified as "Wind-Heat" pattern by retrospective methods. Methods : We treated 19 patients for chronic urticaria with Gamisungmagalguntang(加味升麻葛根湯), who visited to care chronic urticaria at the department of Ophthalmology, Otolaryngology and Dermatology of Korean Medicine at Kyunghee University Korean Medical Center from 1st January 2013 to 31st August 2013. We analyzed information and body conditions of 19 patients. And by using Urticaria Activicy Score(UAS), frequency and number of wheals, itch severity and total score on a 4-point(0-3) scale were assessed in 19 patients. The change of each criterion and total scores between baseline and follow up were analyzed using paired t-test and Wilcoxon test(p<0.05). Results : 1. UAS improvement was statistically significant(p<0.001). Effects of symptom improvement were excellent(31.58%) that means symptom remission and good(42.10%) that means symptom improved over 50%. 2. Heat sensitivity as suspected provocation factor could be a marker for the Wind-Heat pattern urticaria. 3. Among the Wind-Heat pattern urticaria patients, normal digestion(52.63%) and no thirst(78.95%) were higher than abnormal conditions. Whereas about sweat item, abnormal conditions(57.89%) were higher than normal. That means abnormal sweat conditions could be a marker for a defense qi(衛氣) dysfunction of the Wind-Heat pattern urticaria. Conclusions : Our findings suggest that Gamisungmagalguntang could be effective on the Wind-Heat pattern urticaria.
