Large Defect May Cause Infectious Complications in Cranioplasty

  • Objective : Cranioplasty is necessary to repair the cranial defect, produced either by decompressive craniectomy or removal of the contaminated depressed skull fracture. Complications are relatively common after cranioplasty, being reported up to 23.6%. We examined the incidence and risk factors of infectious complications after cranioplasty during last 6 year period. Methods : From January 2000 to December 2005, 107 cranioplasties were performed in our institution. The infectious complications occurred in 17 cases that required the removal of the bone flap. We examined the age of the patients, causes of the skull defect, timing of the cranioplasty the size of the defect, and kinds of the cranioplasty material. The size of the skull defect was calculated by a formula, $3.14{\times}long\;axis\;{\times}short$ axis. The cranioplasty material was autogenous bone kept in a freezer in 74 patients, and polymethylmetacrylate in 33 patients. Statistical significance was tested using the chi-square test. Results : The infection occurred in 17 patients in 107 cranioplasties (15.9%). It occurred in 2 of 29 cases of less than $75\;cm^2$ defect (6.9%), and 6 in 54 cases of $75{\sim}125\;cm^2$ defect (11.1%). Also, it occurred in 9 of 24 cases of more than $125\;cm^2$ defect (37.5%). This difference was statistically significant (p <0.01). Conclusion : During the cranioplasty, special attention is required when the skull defect is large since the infection tends to occurr more commonly.

Analysis of Major Factors of Window Work in Construction Phase Considering Recurrence of Defects in the Maintenance Phase (유지관리단계의 하자 재발생을 고려한 창호공사 시공단계의 중점관리요소 분석)

  • As the construction standards for energy-saving eco-friendly housing have recently been strengthened, the proportion of window work has increased with the demand for high-efficiency housing. Windows have high frequency of use, and there is the potential for many defects to occur depending on the characteristics of construction. According to a government agency's survey of defects in public rental apartment housing, defects in the windows work accounted for the highest portion of complaints received. Accordingly, related previous studies were considered, and it was found that the existing studies in Korea lacked research that reflected the construction characteristics of window work and the importance of maintenance. In addition, existing overseas studies considered both the constructor and the resident's position, considering the cost aspect together, and showed a trend of structuring the relationship between defects and causes. Therefore, this study will analyze the causes of defects that can occur in the construction phase of the windows work, reflect the construction characteristics, and derive major factors that consider the importance of maintenance based on the possibility of recurrence after repairing defects. Ultimately, this research will contribute to preventing defects in the construction phase and reducing maintenance costs by presenting a highly effective defect management plan through selecting the major factors for each defect type that can be intuitively judged by analyzing the causal relationship between defect types and causes.

Various Methods of Reconstruction in Nasal Defect (코 결손 부위에 따른 다양한 재건)

  • Purpose: Nasal defect can be caused by excision of tumor, trauma, inflammation from foreign body reaction. Nose is located in the middle of face and protruded, reconstruction should be done in harmony with size, shape, color, and textures. We report various methods of nasal reconstruction using local flaps. Methods: From March 1998 to July 2008, 36 patients were operated to reconstruct the nasal defects. Causes of the nasal defects were tumor (18 cases), trauma (11 cases), inflammation from foreign body reaction (5 cases) and congenital malformation (2 cases). The sites of the defects were ala (22 cases), nasal tip (8 cases) and dorsum (6 cases). The thickness of the defects was skin only (5 cases), dermis and cartilagenous layer (7 cases) and full-thickness (24 cases). According to the sites and thickness of the defects, various local flaps were used. Most of alar defects were covered by nasolabial flaps or bilobed flaps and the majority of dorsal and tip defects were covered by paramedian forehead flaps. Small defects below $0.25 cm^2$ were covered with composite graft or full-thickness skin graft. Results: The follow-up period was 14 months. Partial flap necrosis was observed in a case, and one case of infection was reported, it was improved by wound revision and antibiotics. Nasal reconstruction with various local flaps could provide satisfactory results in terms of color and texture match. Conclusion: The important factors of nasal reconstruction are the shape of reconstructed nose, color, and texture. Nasolabial flap is appropriate method for alar or columellar reconstruction and nasolabial island flap is suitable for tip defect. The defect located lateral wall could be reconstructed with bilobed flap for natural color and texture. Skin graft should be considered when the defect could not afford to be covered by adjacent local flap. And entire nasal defect or large defect could be reconstructed by paramedian forehead flap.

The Corelation Analysis between Condition Evaluation Factors and Defect Index on the Concrete Retaining Wall (콘크리트 옹벽에 대한 상태평가 항목과 결함지수와의 상관관계 분석)

  • Although lots of safety inspection and precision safety diagnosis have been conducted on concrete retaining wall, there is no comprehensive analysis on the basis of the accumulated data associated with the statistic. Especially, the concentrated management is necessary on the evaluation items that cause critical damages for the efficient performance. In this regard, this study conducted a correlation analysis between the 18 condition evaluation items and defect index for the concrete retaining wall as well as how each item affects the final defect index as much as in the manual. As a result, correlation coefficient between sliding and overturning was 0.601, which means that they have a strong correlation, and the most influential item on defect index is the condition of drainage that scored the 0.750 correlation coefficient. In addition, as a result of regression analysis, the condition of drainage with the 0.683 correlation coefficient has a strong correlation with the defect index. If the condition evaluation items are integrated or readjusted based on the results of the statistical analysis in this study, the more efficient and accurate maintenance will be possible.

  • Defects in the coating process of plastic automotive components are caused by various causes and phenomena. The correlation between defect occurrence rate and meteorological and environmental conditions such as temperature, humidity, and fine dust was analyzed. The defect rate data categorized by type and cause was collected for a year from a automotive parts coating company. This data and its correlation with environmental condition was acquired and experimented by machine learning techniques to predict the defect rate at a certain environmental condition. Correspondingly, the model predicted 98% from fine dust and 90% from curtaining (runs, sags) and hence proved its reliability.

  • Nowadays, consumers' consciousness on me quality are very diversified and their pattern is changing from the price-priority to me quality-priority. So they prefer good-quality products without defects. Therefore quality is a core competence. To compete with me facts, it is very important to keep me the quality competence. To do so, one way is to introduce 100ppm quality innovation system. Many corporates introducing 100ppm improved defect rations(not only process defect ratios but also product defect ratios). In these respects, this study focuses on the comparison and measurement of performance by the analysis of quality system between 100ppm corporates and non-100ppm corporates. Second, this focuses on the finding out core competence reinforcement factors according to the introducing of 100ppm quality system to improve the performance. Third, this focuses on the persuading to introduce the 100ppm quality system to the non-100ppm corporates. Last, this focuses on the playing on the role of guidance to improve QM efficiency.

Angiogenic factor-enriched platelet-rich plasma enhances in vivo bone formation around alloplastic graft material

  • Although most researchers agree that platelet-rich plasma (PRP) is a good source of autogenous growth factors, its effect on bone regeneration is still controversial. The purpose of this study was to evaluate whether increasing angiogenic factors in the human PRP to enhance new bone formation through rapid angiogenesis. MATERIAL AND METHODS. In vitro, the human platelets were activated with application of shear stress, $20\;{\mu}g/ml$ collagen, 2 mM $CaCl_2$ and 10U thrombin/$1\;{\times}\;10^9$ platelets. Level of vascular endothelial growth factor (VEGF) and platelet microparticle (PMP) in the activated platelets were checked. In the animal study, human angiogenic factors-enriched PRP was tested in 28 athymic rat's cranial critical bone defects with $\beta$-TCP. Angiogenesis and osteogenesis were evaluated by laser Doppler perfusion imaging, histology, dual energy X-ray densinometry, and micro-computed tomography. RESULTS. In vitro, this human angiogenic factors-enriched PRP resulted in better cellular proliferation and osteogenic differentiation. In vivo, increasing angiogenic potential of the PRP showed significantly higher blood perfusion around the defect and enhanced new bone formation around acellular bone graft material. CONCLUSION. Angiogenic factor-enriched PRP leads to faster and more extensive new bone formation in the critical size bone defect. The results implicate that rapid angiogenesis in the initial healing period by PRP could be supposed as a way to overcome short term effect of the rapid angiogenesis.

  • We have calculated the d/p boundary values in which the low twist defect and the stripe domain defect can occur with dielectric constants and elastic constants and compared them with experimental data using the compensation factors. We estimated d/p margin for $250^{\circ}$ twist angle without experimental data and investigated qualitatively the reasonability of the behavior.

  • Recently, the arbitration system for defect disputes has been introduced to settle the disputes arising from defects in apartment buildings. However, the conciliation system did not reflect the current technical issues of defect disputes and the opinions of each party involved in the disputes. Moreover, it revealed more imperfections in the content and the process of the system itself. Therefore, this study aims to review the technical aspects of defect disputes, and suggest an alternative to the conventional conciliation system. This paper also discusses logical factors that can be addressed for the current subjective judgment. It is recommended that each party involved in the defect dispute build mutual trust in order to meet social needs, which is the underlying support for the revitalization of the institutional level.

  • In order to improve software quality, it is necessary to efficiently and effectively remove software defects in source codes. In the development field, defects are removed according to removal ratio or severity of defects. There are several studies on the removal of defects based on software quality attributes, and several other studies have been done to improve the software quality using classification of the severity of defects, when working on projects. These studies have thus far been insufficient in terms of identifying if there exists relationships between defects or whether any type of defect is more important than others. Therefore, in this study, we collected various types of software defects, standards organization, companies, and researchers. We modeled the defects types using an ANP model, and developed the weighted severities of the defects types, with respect to the general application software, using the ANP model. When general application software is developed, we will be able to use the weight for each severity of defect type, and we expect to be able to remove defects efficiently and effectively.
