A Survey on the Ostomate Education Materials (장루 보유자 교육용 자료에 대한 조사 연구)

  • Ostomates have suffered from many difficulties due to their physical, psychosocial handicaps and changes of life style to include ostoma management that influences their daily and quality of life. An appropriate nursing education for ostoma management is very important. Practical educational materials needs revision because those were developed by pharmaceutical companies and hospital institutions. The purpose of this study is to provide more practical and reasonable education materials for ostomates by doing analysis and survey of educational materials now being used. We surveyed 8 types of educational materials used in 23 university hospitals and medical centers in Seoul ; four of them were developed by department of nursing and the remainder by an Ostomy Company Data, collected from July 14, 1997 through July 31, 1997 were analyzed. The results are as follows ; 1. The analysis of education guide, on ostomate included 14 subcategories : introduction, structure and function of gastrointestinal tracts, definition of stoma, types of ostomy, definition of peristalsis, methods of defecation management, selection of instrument, resolution of problems and general situations following surgery, daily life, where to ask for help, explanations for terms, information about where to buy instrument, explanations for enterostomal therapist, a matter of consultation with doctor, etc. 2. Introduction contained specific contents on practical ostomate management that ostomates would experience through their lives. Ostomate education guides were developed 3 hospitals except one which missed this point. 3. Most ostomate education guides, except one hospital, helped ostomates to understand their physical structure changes with specific explanations on gastrointestinal tracts with figures. 4. Six institutions did not talk about the definition of peristalsis. 5. All institutions, except two, helped ostomates to understand types of ostomy with figures. 6. More detailed explanations on natural defecation are needed. The benefits and pitfalls of natural defecation should be more specified. 7 No psychosocial difficulties of ostomy management were addressed. 8. The efficiency of enema can be better understood through all explanations with figures. Some institutions did not mention items about definition, benefits, pitfalls of enema, sequency of enema, how to wash, cautions performing and enema, skin management, cleaning instrument after enema proper time to spend. 9. There were no detailed contents and what to do in case of not being able to do enema. 10. Only one educational material mentioned emotional aspects after the surgrey. 11. Most institutions explained subcategory of daily life but did not provide specific contents on the difficulties of physical, psychological, and sociocultural controls. 12. The subcategory of ureterostomy education guides included explanations on normal structure and function of urinary tracts, types of ureterostomy, how to manage skin, usage and types of instrument, commercial urostomy, how to manage instrument, daily life, introduced the general contents. However, more specific explanations were needed.

  • Constipation is very common phenomenon during childhood that accounts approximately 3% of general pediatric outpatient visits and up to 25% of visit to the pediatric gastroenterologist. The diagnosis of functional constipation is based on the historical features and absence of physical abnormalities. Most infants and children with chronic constipation require no diagnostic tests. Constipation and subsequent fecal retention behavior often begins soon after experience of painful defecation. The effective management of childhood chronic constipation consist of education, prolonged support by physicians and parents, medications, and long term follow up.

Neuronal Vacuolation in a Pekingese (Pekingese에서의 Neuronal Vacuolation)

  • A 6-month-old female Pekingese was euthanized due to poor progrosis after 1 month history of neurologic signs that include depression, ataxia, urination and defecation difficulty. At necropsy, no significant gross abnormalities were noted Histologically, neuronal vacuolation was noted in the brain, primarily cerebellum and occasionally in the brain stem area. Neuronal necrosis and secondary axonal swelling were also observed. Differential diagnoses were able to rule out other diseases which can induce neuronal vacuolation such as lysosomal storage disease, prion infection, and postvaccinal change.

A Study of the Evaluation of the Spinal Cord Injuries (척수 손상 환자에 관한 실태 조사)

  • The incidence of spinal cord injury increase due to traffic accident, industrial accident and leisure sports. Spinal cord injury damages motor and sensory function below the injury level, also affects autonomic functions associated with voiding and defecation. Sexual dysfunction and psychosocial, vocational maladaptations are also some of the unwanted consequences of injury. The purpose of this study is look for means to prevent and to manage complications in spinal cord injury through investigation and analysis. The subjects of this study in spinal cord injured patients were admitted to the department of physical therapy, Kwangju christian Hospital, Nam Kwang Hospital, Chun Nam university Hospital and Cho Sun university Hospital, from April, 1, 1995 to March, 31, 1996. The results are as follows: 1. The subjects comprised 96 cases of spinal cord injury, ranging from 17 to 85(mean-40.8 yrs) and included 72 males and 24 females. Among these patients, 58 were cervical injury, 20 were thoracic injury and 18 were lumbar injury. 2. As for a major causative of spinal cord injury were traffic accident(59.4%), fall down (27.1 %), and motocycle(4.2%).. 3. The bladder control were taken by indwelling cathetar(41.7%), Crede maneuver(37.5%) and self voiding(16.7%). The bowel control were taken by all aid(61.5%), assitance(32.3%) and self defecation(6.2%). 4. Possible of sexual function were 35 cases (47.9%). 5. The device of transfer used wheel chair(69.8%) and bed(16.7%). 6. The patients with higher cord lesion got more serious pain than lower cord lesion. Also the patients with higher cord lesion got a serious spasticity. 7. The incidence of decubitus ulcer among 96 patients were in case 46(47.9%). The largest group of the pressure sore sites were sacral portion(82.0%), less than 1 month of onset occured a large numbers(50%). Incidence of pressure sore by spasticity occured many patients in case of mild or moderate. Incidence of pressure sore by pain occured many patients in case of severe pain.

A Comparative Study on The Effect of Cupping and Moxibustion Therapy on Treating Constipation of Stroke Patients (부항-뜸 병행치료와 뜸 단독치료가 뇌졸중 환자의 변비 완화에 미치는 효과 비교)

  • Objectives : This comparative study aimed to identify the effect of the Buddeumi(cupping-and-moxibustion device) therapy and moxibustion-only therapy on treating constipation of stroke patients who were admitted in the hospital of oriental medicine. The research design was a nonequivalent control group pretest-posttest design. Methods : The subjects of the study consisted of 23 stroke patients, and they were divided into two groups : one was the experimental group of 11 patients and the other was the control group of 12 patients. The Buddeumi therapy and the moxibustion-only therapy were given respectively. Each Buddeumi and moxibustion-only therapies on the 天樞(ST25, right & left) in abdomen were administered to the each group for 40 minutes a total at 10 times. The posttest included the frequency of defecation with the Constipation Assessment Scale every day for one week after the experiment. The data were analyzed by the $x^2$ test, t-test, Fisher exact test, and repeated measures ANCOVA using SPSS/WIN 12.0 program. Results : The results were as follows : First, Hypothesis 1 : 'The patients who are given the Buddeumi therapy in the experimental group have a lower Constipation Assessment Scale than the control group.' was supported(F=4.367, P=0.049). Second, Hypothesis 2 : 'The patients who are given meridian acupressure in the experimental group have a higher frequency of defecation per week than the control group.' was supported(F=0.947, P=0.034). Accordingly, 'The patients who are given the Buddeumi therapy in experimental group have a higher effect on constipation than the control group.' was supported. Conclusion : The Buddeumi therapy reduced the constipation assessment scale of stroke patients and increased the frequency of their defacation per week. Therefore, the Buddeumi therapy could be an effective treatment for constipation of stroke patients.

Effects of by-product feed-based silage on feeding, rumination, and excretion in growing Hanwoo heifers

  • This study investigated the effects of feeding by-product feed (BF)-based silage on the behavior of growing Hanwoo heifers. Twelve Hanwoo heifers (13.2 months-old, 315 kg body weight; four heifers per pen) were assigned to three diets: a rice straw (RS) diet (concentrate mix and free access to RS), a RS and BF-based silage (RSBFS) diet (concentrate mix and free access to RS and BF-based silage), and a BF-based silage (BFS) diet (concentrate mix and free access to BF-based silage). Behavior was recorded for 5 days using camcorders. Compared to the RS group, the BFS group showed 21.7% higher dry matter intake, shorter feeding, rumination, and chewing times, as well as longer resting time (p < 0.05). Although all groups exhibited similar drinking, urination, and defecation frequencies, the BFS group exhibited higher feeding rates, rumination efficiency, and chewing efficiency than the RS group (p < 0.05). Compared to the BFS group, the RSBFS group showed higher $peNDF_{8.0}$ intake (15.2% vs. 25.0% dry matter intake), longer feeding and sitting times, lower defecation frequency (p < 0.05), and similar rumination efficiency. In conclusion, complete replacement of conventional RS with BF-based silage reduced rumination and chewing activity in growing Hanwoo heifers, and BF-based silage feeding with large-particle straw is an effective approach in improving heifer behavior.

  • Multiple Myeloma is malignant tumor that malignant proliferous plasma cell to originate from bone marrow invades bone multiply. Objectives : Therapy for Multiple Myeloma includes chemotherapy, radiation therapy and self-stem cell transplantation, but it has no effect for a majority of Multiple Myeloma patients. So we diagnosed it as Wei symptom, oliguria, or dysuria(遺尿) in Oriental medicine, and treated it using the Oriental medical system. Methods : The patient was treated using acupuncture, electroacupuncture, herbal acupuncture treatment, moxibustion, physical treatment and western medicine. We observed 12 kinds of symptoms in the patient when admitted to the hospital. Results : 1. Paraplegia, urination desire, voluntary urination, and other symptoms improved except for a period of complication. 2. Defecation desire, sensory disturbance of the body and lower extremities, self-made changes, maintenance of body posture, and other symptoms improved during admitting days. 3. Voluntary defecation, pains of the neck and lower extremities, and other symptoms had irregular changes during admitting days. Conclusions : This study demonstrates the necessity of having more clinical study about Mutiple Myeloma.

Defecation of Norovirus from the Oyster Crassostrea gigas by Depuration Following Translocation of the Growing Area (자연정화에 의한 양식굴(Crassostrea gigas) 중 노로바이러스 저감화)

  • The efficacy of depuration following growing area translocation for the defecation of norovirus was evaluated under experimental conditions using oysters Crassostrea gigas previously subjected to bioaccumulation of this virus at a waste treatment plant discharge site. Three trials were assayed in an open experimental system with a commercial oyster farm located in a shellfish growing area approved by the Korean Shellfish Sanitation Program. Real-time reverse-transcription polymerase chain reaction (RT-PCR) was used to quantify viruses in the digestive glands of oysters. The final viral loads in oysters after 12 days remained under the detection limit (10 copies/g digestive gland) of the real-time RT-PCR. This reduction trend showed two-phase removal kinetics, with an initial slow reduction or slight increase in viruses during the first 2 days of depuration and subsequent stabilization with 0.12 to 2.64 log unit norovirus copies/g digestive gland per 2 days of depuration for the remaining time.

Correlation between Colon Transit Time Test Value and Initial Maintenance Dose of Laxative in Children with Chronic Functional Constipation

  • Purpose: To evaluate the correlation between colon transit time (CTT) test value and initial maintenance dose of polyethylene glycol (PEG) 4000 or lactulose. Methods: Of 415 children with chronic functional constipation, 190 were enrolled based on exclusion criteria using the CTT test, defecation diary, and clinical chart. The CTT test was performed with prior disimpaction. The laxative dose for maintenance was determined on the basis of the defecation diary and clinical chart. The Shapiro-Wilk test and Pearson's and Spearman's correlations were used for statistical analysis. Results: The overall group median value and interquartile range of the CTT test was 43.8 (31.8) hours. The average PEG 4000 dose for maintenance in the overall group was $0.68{\pm}0.18g/kg/d$; according to age, the dose was $0.73{\pm}0.16g/kg/d$ (<8 years), $0.53{\pm}0.12g/kg/d$ (8 to <12 years), and $0.36{\pm}0.05g/kg/d$ (12 to 15 years). The dose of lactulose was $1.99{\pm}0.43mL/kg/d$ (<8 years) or $1.26{\pm}0.25mL/kg/d$ (8 to <12 years). There was no significant correlation between CTT test value and initial dose of laxative, irrespective of the subgroup (encopresis, abnormal CTT test subtype) for either laxative. Even in the largest group (overall, n=109, younger than 8 years and on PEG 4000), the correlation was weak (Pearson's correlation coefficient [R]=0.268, p=0.005). Within the abnormal transit group, subgroup (n=73, younger than 8 years and on PEG 4000) correlation was weak (R=0.267, p=0.022). Conclusion: CTT test value cannot predict the initial maintenance dose of PEG 4000 or lactulose with linear correlation.

  • Objectives It is important to decide if the patient is belong to cold syndrome or heat syndrome in Sasang Constitutional Medicine. The purpose of this study is to report some proof to determine cold and heat syndrome and characteristic of the patient who can be treated with Cheongsimyeonja-tang. Methods To evaluate the results of this treatment, muscular symptom was assessed by Visual Analogue Scale(VAS). The patient's general condition such as urinary and fecal discomfort is assessed by the progress notes. Results The patient did not improved when she was treated with Choweseuncheung-tang. But when she was treated with Cheongsimyeonja-tang, she got improved with not only her main problem such as muscular symptom, but also general condition such as indigestion, urinary & fecal discomfort and menopausal symptom. Conclusion This study suggests that number of defecation per day also can be the clue to decide heat or cold syndrome not only appearance of defecation. And the patients who can be treated by Cheongsimyeonja-tang is different from the patients who can be treated by Yeoldahansotang in the color of their face, personality and edema.
