Risk Factors and Features of Critically Ill Patients with Deep Vein Thrombosis in Lower Extremities (하지 심부정맥혈전증을 가진 중환자의 특성 및 발생 영향 요인)

  • Purpose: The purpose of this study was to identify the features, risk scores and risk factors for deep vein thrombosis in critically ill patients who developed deep vein thrombosis in their lower extremities. Methods: The participants in this prospective descriptive study were 175 adult patients who did not receive any prophylactic medication or mechanical therapy during their admission in the intensive care unit. Results: The mean age was 62.24 (${\pm}17.28$) years. Men made up 54.9% of the participating patients. There were significant differences in age, body mass index, and leg swelling between patients who developed deep vein thrombosis and those who did not have deep vein thrombosis. The mean risk score was 6.71(${\pm}2.94$) and they had on average 4.01(${\pm}1.35$) risk factors. In the multiple logistic regression, body mass index (odds ratio=1.14) and leg swelling (odds ratio=6.05) were significant predictors of deep vein thrombosis. Conclusion: Most critically ill patients are in the potentially high risk group for deep vein thrombosis. However, patients who are elderly, obese or have leg edema should be closely assessed and more than one type of active prophylactic intervention should be provided.

  • Objective: This study reports on the clinical effects of combinatorial Hyulbuchuko-tang on a deep-vein thrombosis patient.Method: The deep-vein thrombosis patient was treated using combinatorial Hyulbuchuko-tang and pharmacopuncture (water-soluble carthami flos, WCF) three times a day for 33 days. The effects of deep-vein thrombosis were measured using a 0-10 numerical rating scale (NRS) and external appearance.Result: Improvements in the NRS from 7 to 2 were observed after the combinatorial Hyulbuchuko-tang treatment.Conclusions: From these results, combinatorial Hyulbuchuko-tang may be considered to show improvement for patients with deep-vein thrombosis.

Deep Vein Thrombosis after Total Knee Replacement in a Patient undergoing Korean Medical Rehabilitation (슬관절 전치환술 후 한방재활치료 중 발생한 심부정맥 혈전증 보고)

  • Objectives The objective of this study is to report the deep vein thrombosis (DVT) after total knee replacement (TKR) in a patient undergoing Korean medical rehabilitation. Methods A female patient who had total knee replacement (TKR) was treated by Korean medical rehabilitation with protocol. After fifteen days treatment, deep vein thrombosis (DVT) was suspected. Therefore, CT venography was performed. Results After Korean medical rehabilitation, pain was decreased. But also occurred deep vein thrombosis (DVT) as a complication. Conclusions We present a case of deep vein thrombosis (DVT) after total knee replacement (TKR) in a patient undergoing Korean medical rehabilitation. There is a limit that this study hasn't sufficient number of cases. Further studies will be needed.

  • Abdominal compartment syndrome is one cause of deep vein thrombosis of lower extremity. Although prophylactic dose of anticoagulation agent is safely started after 24~48 hours without the evidence of active bleeding, there may be bleeding complication related to invasive procedure which trauma victims undergo. Inferior vena cava filter should be considered in the treatment plan of this complex situation.

  • 1. Objectives The purpose of this study is to evaluate the effects of Sasang Constitutional medical diagnosis and treatment on Soyangin Edema caused by Deep Vein Thrombosis. 2. Methods The Subject is a 75 years old woman who has a left lower limb edema caused by Deep Vein Thrombosis, we had diagnosed her as Soyangin and prescribed Sasang Constitutional Medicine. We evaluated with length of patient's calf, thigh, ankle circumference and pitting edema, skin flare after medication. 3. Results Lower limb edma of a Soyangin patient responded well to the treatment with Dojeokganggi-tang(導赤降氣湯). 4. Conclusions This case study shows an efficient result by using Dojeokganggi-tang in treatment of Soyangin edema patient.

  • A 5-year-old boy was admitted to the hospital with a high fever and abnormal gait. Magnetic resonance imaging showed extensive swelling of the quadriceps and adductor muscles around the right hip and anterior thigh. A duplex scan demonstrated a thrombus from the external iliac vein to below the popliteal vein. The blood culture revealed Streptococcus pyogenes. The patient was immediately treated with antibiotics and anticoagulants. Follow-up sonography demonstrated complete disappearance of the venous thrombosis. We report a case of streptococcal myositis complicated by deep vein thrombosis.

  • Antiphospholipid syndrome (APS) is an acquired systemic autoimmune disorder characterized by a combination of clinical criteria, including vascular thrombosis or pregnancy morbidity and elevated antiphospholipid antibody titers. It is one of the causes of deep vein thrombosis and pulmonary embolism that can be critical due to the mortality risk. Overall recurrence of thromboembolism is very low with adequate anticoagulation prophylaxis. The most effective treatment to prevent recurrent thrombosis is long-term anticoagulation. We report on a 17-year-old male with APS, who manifested blue toe syndrome, deep vein thrombosis, pulmonary thromboembolism, and cerebral infarction despite adequate long-term anticoagulation therapy.

  • Adventitial cystic disease of the vein is a rare vascular disease. Herein, we report a case of adventitial cystic disease of the common femoral vein that was initially misdiagnosed as deep venous thrombosis in a 41-year-old male who presented with leg swelling. We focused on the imaging findings and compared them with those of deep venous thrombosis.

  • A 69-year-old man was admitted to our hospital for deep vein thrombosis, and he had experienced two episodes of left leg swelling: one happened after an 11 hour-air trip from Europe in June 2007, and the other developed 3 days after an operation for a herniated lumbar disc in July 2007. We treated him for a deep vein thrombosis of the deep femoral vein after admission. However, we arrived at the final diagnosis of angioleiomyosarcoma after the operation for the tentative diagnosis of deep vein thrombosis. We removed the firmly engorged deep femoral vein on the second operation. We report here on a case of angioleiomyosarcoma that arose from the deep femoral vein.

  • Purpose: To report on a patient with subarachnoid hemorrhage and deep vein thrombosis treated with a combination of Korean and Western medicine. Methods: A patient diagnosed with subarachnoid hemorrhage and deep vein thrombosis was treated with herbal medicine (加減淸熱瀉濕湯 (Gagam-cheongyeolsaseub-tang)), acupuncture (GV2, GB20, LI11, LI4, TE5, GB34, ST36, SP9, ST38, GB39, SP6, LR3), moxibustion (CV6, CV9), and Western medicine for deep vein thrombosis and subarachnoid hemorrhage. To evaluate edema, one of the classic symptoms of deep vein thrombosis, we measured leg circumference at a specified time. Results: After taking Gagam-cheongyeolsaseub-tang and being treated with acupuncture, moxibustion, and Western medicine, the patient showed improvement in leg edema. The left calf circumference decreased from 37.5 cm to 34.5 cm. Conclusions: Korean medicine, acupuncture, moxibustion, and Western medicine may provide effective treatment for deep vein thrombosis in patients with subarachnoid hemorrhage, but further study is needed.
