탄력밴드를 이용한 등척성 엉덩관절 벌림 교각운동이 전방머리자세 환자의 족저압에 미치는 영향 (The Effect Of Isometric Hip Abduction Bridge Exercise Using Elastic Band on Foot Pressure in Patients with Forward Head Posture)

  • Background: In patients with forward head posture (FHP), the head is positioned forward, causing increased tension in the muscles and structures of the head, neck, and shoulders. This can result in joint dysfunction that may lead to abnormal afferent information. The purpose of this study was to investigate the effect on foot pressure through the isometric hip abduction (IHA) bridge exercise using elastic bands in patients with FHP. Methods: Twenty patients with FHP were randomly assigned to a study group that applied joint mobilization, soft tissue mobilization, a deep neck flexor strengthening exercise, and the IHA bridge exercise using an elastic band. A control group was also constituted, and members were given joint mobilization, soft tissue mobilization therapies, and the deep neck flexor strengthening exercise. Ten patients were assigned to each group. The static foot pressure and dynamic foot pressure of each patient were measured before and after the intervention, and the interventions for each group were applied twice a week for 4 weeks. Results: Both the study group and the control group showed significant differences in static and dynamic foot pressure before and after the interventions (p<.05). There were no significant differences in foot pressure between the study and control groups. Conclusion: : The results of this study revealed that there were no significant differences between the group doing the IHA bridge exercise using the elastic band and the control group. However, the intervention methods applied to both the groups were effective in improving the body center control of FHP patients.

호흡패턴이 머리목 굽힘 운동시 목빗근과 심부 목굽힘근의 근두께에 미치는 영향 (Influence of Breathing Patterns on the Thickness of Sternocleidomastoid Muscle and Deep Cervical Flexor Muscles During Craniocervical Flexion Exercise)

  • Background: The deep cervical flexor (DCF) muscles have a crucial role in the management of neck pain. For preventing neck pain by activation of the DCF, craniocervical flexion (CCF) is an effective exercise. However, sternocleidomastoid (SCM) muscle is considered to affect negatively the activation of the DCF. SCM muscle which is an accessory muscle for respiration is activated differently depending on types of breathing patterns. It's not certain that breathing patterns affect the SCM and DCF muscles thickness during CCF exercise. Objects: The purpose of this study was to investigate the influence of breathing patterns on the SCM and DCF muscles thickness during CCF exercise. Methods: Forty-five subjects participated in this study, and they were classified according to their breathing pattern, as follows: costodiaphragmatic breathing (CDB) and upper costal breathing (UCB) groups. Ultrasonographic imaging of the SCM and DCF muscles was performed during five incremental levels of CCF during tidal breathing and expiration. Results: There was a significant interaction between the breathing pattern and the phase of CCF for percentage of SCM muscle thickness changes (p<.05). In phase 1 CCF, a percentage of SCM muscle thickness changes was increased in the UCB group than in the CDB group (p<.05). There was an increase in DCF muscles thickness with each additional CCF phase (p<.05). Conclusion: Recruitment of SCM muscle was increased in the UCB group while performing CCF with a low intensity. There were no significant differences on DCF recruitment between the breathing pattern groups. Higher CCF exercise intensities elicited a higher DCF recruitment.

Effects of Cervical Stabilization Exercise Using Pressure Biofeedback on Muscle Tone, Muscle Endurance and Craniovertebral Angle in Collage Students with Forward Head Posture

  • 이 연구는 앞쪽머리자세를 가진 대학생을 대상으로 바이오피드백을 적용한 목뼈 안정화 운동중재가 뒤통수근육의 긴장도, 깊은목굽힘근 지구력 및 머리뼈 척추각에 어떠한 영향을 미치는지 알아보고자 실시되었다. 연구 대상자는 바이오피드백 목뼈 안정화 운동군(BCSEG) 12명과 대조군(CG) 12명을 선정하였으며, 8주간 주 3회, 50분간 실시하였다. 이 연구 결과 바이오피드백을 적용한 목뼈 안정화 운동 후 뒤통수밑근은 BCSEG에서 경직도와 근 긴장도가 유의하게 감소하였고(p<.01), 깊은목굽힘근은 BCSEG에서 근지구력(p<.01)과 머리 척추각(p<.01) 유의하게 증가하였다. 이상의 결과를 종합해 보면 바이오피드백을 적용한 목뼈 안정화 운동은 뒤통수밑근과 깊은목굽힘근의 근기능 향상시킴으로써 머리 척추각도 개선을 한 것으로 생각된다. 압력 바이오피드백을 적용한 목뼈 안정화 운동은 FHP를 개선을 위한 중재방법으로 제안될 수 있다.

머리-목굽힘 운동 시 앞쪽머리자세와 목굽힘근 근두께 간의 상관성 연구 (The Correlation Between Forward Head Position and Neck Flexor Thickness During Cranio-Cervical Flexion Exercise)

  • Background: As the increase of forward head position, we studied the change of thickness of deep cervical flexor (DCF) compared of with sternocleidomastoid (SCM). we measured and analysed the change of thickness of the neck flexor for forward head position and cranio-cervical flexion exercise (CCFE). Methods: Using a cross-sectional design, we conducted the study selecting 35 healthy adults (12 males, 23 females). We measured the craniovertebral angle (CVA), instructed them to perform the CCFE, and measured the DCF and SCM using ultrasonography during the contraction and relaxation period. Results: Intra-reliability of SCM is .96, longus capitis is .92 and longus coli is .97. we compared according to the change of forward head position, Correlation of DCF is .841, and DCF/SCM is .754 by significant positive correlation. At the comparison of CCFE and Resting muscle thickness, SCM and DCF is .00, DCF/SCM is .68. Conclusion: There is a strong positive correlation between the change amount of DCF and DCF/SCM as the increase of CVA.

방사선 사진상 경추 후만을 보인 만성 경항통 환자의 만곡 이상 치험 2례 (Two Cases Report of Chronic Neck Pain Patients with Kyphotic Cervical Curvature Measured by Radiography)

  • We researched two patients complaining about chronic neck pain with kyphotic cervical curvature. We assumed that the patients' symptom was caused by weakened deep neck flexor and activated superficial muscles around neck. So, acupuncture therapy, SCENAR therapy and self exercise with wooden neck pillow were used to treat the patients. We measured their pain by numerical rating scale (NRS) and neck disability index (NDI) before and after treatment. And cervical curvature was evaluated by Cobb method (C1-C7) and Ishihara Index. As a result, NRS and NDI significantly reduced and cervical curvature was also improved. Therefore, we conclude that acupuncture therapy with SCENAR therapy and self exercise using wooden neck pillow is an effective treatment to reduce chronic neck pain with kyphotic cervical curvature. But there is a limit on this study due to insufficient number of cases and absence of control group. Further studies will be needed.

The Effects of Modified Chin Tuck Exercise on the Cervical Curvature, the Strength and Endurance of the Deep Cervical Flexor Muscles in Subjects with Forward Head Posture

  • Purpose : The purpose of this study was to investigate the effects of chin tuck exercise (CTE) on the craniovertebral angle (CVA), strength and endurance of deep cervical flexor (DCF) muscles in subjects with forward head posture(FHP). This study was performed on 30 subjects with FHP. Method : Thirty subjects were divided into two groups; modified CTE (n=15), conventional CTE (n=15). Both of the group performed the exercise 4 times a week for 6 weeks. The subjects performed CTE in two different methods; modified CTE, with device designed that help keep cervical lordois curve, and coventional CTE, without using device. The CVA was measured using Image software version. A pressure biofeedback unit was used to measure the strength and endurance of the DCF muscles. The data was analyzed by the paired t-test for comparing before and after changes of variables in each group and the independent t-test for comparing the between groups. Result : There was statistically significant difference of before and after strength and endurance of DCF muscles in modified CTE (p<0.05). There was statistically significant difference of before and after only endurance of DCF muscles in conventional CTE (p<0.05). There was statistically significant difference of between the two group in strength of DCF muscles (p<0.05). Conclusion : Muscle strength to stabilize the spine plays an important role in maintaining a good posture. Therefore, we suggest that the application of CTE with a device designed to maintain the lordotic curvature in the neck is likely to yield better outcomes in FHP subjects in future studies.

깊은목굽힘근 강화운동과 등뼈 관절가동운동이 뇌졸중 환자의 전방머리자세와 목통증에 미치는 영향 (The Effect of Deep Neck Flexor Exercise and Thoracic Mobilization Exercise on Forward Head Posture and Neck Pain in Chronic Stroke)

  • 본 연구는 깊은목굽힘근운동과 등뼈 관절가동운동이 뇌졸중 환자의 통증, 전방머리자세에 미치는 영향을 알아보고자 한다. 연구대상자는 36명은 사전측정 후 무작위로 깊은목굽힘근군(DNFE), 등뼈 관절가동운동군(TROM), 대조군으로 각각 12명씩 배정되었다. 중재 전후에 통증(VAS), 두개척추각도(CVA), 두개회전각도(CRA), 전방머리내밈자세(FHP), 등뼈관절가동범위(TROM)를 측정하였다. 그 결과. DNFE 군과 TROM 군은 중재 전후 VAS, CVA, CRA, FHP, TROM에서 유의한 차이를 보였으며(p<.05), 두 군은 대조군에 비해 VAS, CVA, CRA, FHP, TROM에서 보였다(p<.05). 결론적으로 깊은목굽힘근운동과 등뼈 가동범위운동이 뇌졸중 환자의 전방머리자세와 목통증 회복에 효과적이었다. 그러므로 임상에서 뇌졸중 환자 중재프로그램에 깊은목굽힘근운동과 등뼈 가동범위운동을 적극적으로 활용되기를 바란다.

환축추 회전 아탈구 환자에 대한 도수치료 효과 (Effect of Manual Therapy on a Patient With Atlantoaxial Rotatory Subluxation)

  • Background: An 8-year-old girl had severe neck pain and stiffness after trauma. CT scan showed atlantoaxial rotatory subluxation (AARS). She had conservative treatment because she did not have neurological symptoms and spinal basilar artery dysfunction. Conservative therapy was halter traction twice for 4 weeks. However, pain and stiffness persisted. She had been recommended to have surgery from her physician, but she received manual therapy for non-surgical procedures. Methods: The joint mobilization, muscle energy technique, motor control exercise, and deep neck flexor (DNF) endurance exercise were applied as manual therapy and 10 session for 2weeks. Results: Clinical outcomes were measured at initial baseline, after 2 weeks, and after 6weeks. Active range of motion was completely restored after 6weeks and numeric pain rating scale was completely reduced after 2 weeks. The strength of neck flexor muscle recovered to normal after 2 weeks, and the DNF endurance was improved to 25 seconds after 2 weeks and to 42 seconds after 6weeks. Motor control capacity recovered to 30 ㎜Hg after 2 weeks. Conclusions: This case report describes the immediate and short-term clinical outcomes for a patient presenting with symptoms of neck pain following AARS. Clinical rationale and patient preference aided the decision to incorporate manual therapy as a treatment for this patient. Manual therapy has shown a successful recovery in AARS patients, more research is needed to validate the inference of this case report.

거북목을 가진 사람에게 메켄지 폄운동과 어깨안정화운동이 앞쪽머리자세에 미치는 영향 (Effect of Shoulder stabilization exercise and McKenzie Exercise on Forward Head Posture)

  • 본 연구의 목적은 거북목을 가진 사람에게 메켄지 폄운동과 어깨안정화운동이 앞쪽머리자세에 미치는 영향에 대해 알아보는 것이다. 연구대상자는 30명의 거북목을 가진 평범하고 건강한 사람들로 이루어져 있다. 대상자는 어깨안정화운동과 메켄지 폄운동으로 나누어(인원=30, 남자 15명, 여자 15명) 무작위로 나누어 6주 동안 실시되었다. 본 연구는 마름근, 아래등세모근의 운동프로그램과 깊은목굽힘근, 위등세모근, 어깨올림근의 신장프로그램을 통한 목뼈의 머리척추각(Cervical Vertebra Angle) 값과 각 근육별 근전도의 차이를 알아보았다. 앞쪽머리자세와 경우에는 목과 등근육의 활성도가 감소하고 피로도는 증가함을 보였다. 이러한 결과는 목과 머리의 불안정한 상태를 안정시키기 위해서는 위등세모근, 깊은목굽힘근, 어깨올림근의 신장과 마름근, 아래등세모근의 운동을 통해 거북목의 개선에 긍정적인 변화를 줄 것으로 보인다. 이러한 근육들의 불균형한 수축과 과도한 긴장으로 근육의 경직과 피로가 누적되며 이러한 상태가 계속 유지되게 되면 만성화상태가 되어 근육의 불균형으로 인해 통증을 유발할 수 있으며, 피로의 누적으로 인하여 자극에 대한 반응과 근력을 약화시켜서 목 디스크가 발생할 확률이 높아질 것으로 사료된다.

젊은 성인에서 머리-목 굽힘근 운동 시 시선과 압력이 목긴근과 목빗근의 근두께에 미치는 영향 (The Effect of Gaze Directions and Pressure Levels on longus colli and Sternocleidomastoid Thickness during Cranio-cervical flexor Exercise in Young Adults)

  • 이 연구는 정상 성인에서 머리-목 굽힘 운동 시 시선의 각도와 압력이 깊은목굽힘근의 두께변화에 미치는 영향을 알아보고자 하였다. 본 연구에는 27명이 참여하였다. 머리-목굽힘 운동하는 동안 4가지(0°, 20°, 40°, 60°)의 시선과 5가지(20mmHg, 22mmHg, 24mmHg, 26mmHg, 28mmHg)의 압력에서 목긴근과 목빗근의 근 두께는 초음파 영상을 이용해 측정하였다. 시선 각도 및 압력변화에 따른 목긴근과 목빗근의 두께 변화를 비교 분석하기 위하여 반복측정분산분석을 수행하였다. 본 연구결과, 목긴근은 20과 22mmHg에서 0°와 20°, 0°와 40°, 0°와 60°은 유의한 차이를 보였으며, 20°에서 가장 근활성도가 높았다(p<.05). 목빗근은 28mmHg에서 0°와 20°, 20°와 40°, 40°와 60°은 유의한 차이를 보였으며, 20°에서 가장 낮은 근활성도를 보였다(p<.05). 본 연구에서 머리-목 굽힘 운동 시에는 시선의 방향을 20°로 설정하는 것이 목긴근의 활성화를 높이고, 목빗근의 근활성도를 낮추어 운동의 효과를 얻을 수 있다는 것을 찾았다. 본 연구의 결과를 바탕으로 임상환경에서 머리-목 굽힘 운동시에 시선 방향을 설정하여 운동의 효과를 증대시키길 바란다.
