An effective patient training for deep inspiration breath hold technique of left-sided breast on computed tomography simulation procedure at King Chulalongkorn Memorial Hospital

  • Purpose: To observe the effectiveness of the practical instruction sheet and the educational video for left-sided breast treatment in a patient receiving deep inspiration breath hold (DIBH) technique. Two parameters, simulation time and patient satisfaction, were assessed through the questionnaire. Methods: Two different approaches, which were the instruction sheet and educational video, were combinedly used to assist patients during DIBH procedures. The guideline was assigned at least 1 week before the simulation date. On the simulation day, patients would fill the questionnaire regarding their satisfaction with the DIBH instruction. The questionnaire was categorized into five levels: extremely satisfied to dissatisfied, sequentially. The patients were divided into four groups: not DIBH technique, DIBH without instruction materials, the DIBH with instruction sheet or educational video, and DIBH with both of instruction sheet and educational video. Results: Total number of 112 cases of left-sided breast cancer were analyzed. The simulation time during DIBH procedure significantly reduced when patients followed the instruction. There was no significant difference in simulation time on the DIBH procedures between patient compliance via instruction sheet or educational video or even following both of them. The excellent level was found at 4.6 ± 0.1 and 4.5 ± 0.1, for patients coaching via instruction sheet as well as on the educational video, respectively. Conclusion: Patient coaching before simulation could potentially reduce the lengthy time in the simulation process for DIBH technique. Practicing the DIBH technique before treatment is strongly advised.

Liver dose reduction by deep inspiration breath hold technique in right-sided breast irradiation

  • Purpose: Deep inspiration breath hold (DIBH) is a well-established technique that enables efficient cardiac sparing in patients with left-sided breast cancer. The aim of the current study was to determine if DIBH is effective for reducing radiation exposure of of liver and other organs at risk in right breast radiotherapy (RT). Materials and Methods: Twenty patients with right-sided breast cancer were enrolled in this study. Three-dimensional conformal RT plans were generated for each patient, with two different computed tomography scans of free breathing (FB) and DIBH. Nodes were contoured according to the Radiation Therapy Oncology Group contouring guidelines. Dose-volume histograms for the target volume coverage and organs at risk were evaluated and analyzed. Results: DIBH plans showed significant reduction in mean liver dose (5.59 ± 2.07 Gy vs. 2.54 ± 1.40 Gy; p = 0.0003), V20Gy (148.38 ± 73.05 vs. 64.19 ± 51.07 mL; p = 0.0003) and V10Gy (195.34 ± 93.57 vs. 89.81 ± 57.28 mL; p = 0.0003) volumes compared with FB plans. Right lung doses were also significantly reduced in DIBH plans. Heart and left lung doses showed small but statistically significant improvement with application of the DIBH technique. Conclusion: We report that the use of DIBH for right-sided breast cancer significantly reduces the radiation doses to the liver, lungs, and heart.

Feasibility Study of Deep Inspiration Breath-Hold Based Volumetric Modulated Arc Therapy for Locally Advanced Left Sided Breast Cancer Patients

  • Background: The purpose of this study was to assess the feasibility of deep inspiration breath-hold (DIBH) based volumetric modulated arc therapy (VMAT) for locally advanced left sided breast cancer patients undergoing radical mastectomy. DIBH immobilizes the tumor bed providing dosimetric benefits over free breathing (FB). Materials and Methods: Ten left sided post mastectomy patients were immobilized in a supine position with both the arms lifted above the head on a hemi-body vaclock. Two thermoplastic masks were prepared for each patient, one for normal free breathing and a second made with breath-hold to maintain reproducibility. DIBH CT scans were performed in the prospective mode of the Varian real time position management (RPM) system. The planning target volume (PTV) included the left chest wall and supraclavicular nodes and PTV prescription dose was 5000cGy in 25 fractions. DIBH-3DCRT planning was performed with the single iso-centre technique using a 6MV photon beam and the field-in-field technique. VMAT plans for FB and DIBH contained two partial arcs ($179^{\circ}-300^{\circ}CCW/CW$). Dose volume histograms of PTV and OAR's were analyzed for DIBH-VMAT, FB-VMAT and DIBH-3DCRT. In DIBH mode daily orthogonal ($0^{\circ}$ and $90^{\circ}$) KV images were taken to determine the setup variability and weekly twice CBCT to verify gating threshold level reproducibility. Results: DIBH-VMAT reduced the lung and heart dose compared to FB-VMAT, while maintaining similar PTV coverage. The mean heart $V_{30Gy}$ was $2.3%{\pm}2.7$, $5.1%{\pm}3.2$ and $3.3%{\pm}7.2$ and for left lung $V_{20Gy}$ was $18.57%{\pm}2.9$, $21.7%{\pm}3.9$ and $23.5%{\pm}5.1$ for DIBH-VMAT, FB-VMAT and DIBH-3DCRT respectively. Conclusions: DIBH-VMAT significantly reduced the heart and lung dose for left side chest wall patients compared to FB-VMAT. PTV conformity index, homogeneity index, ipsilateral lung dose and heart dose were better for DIBH-VMAT compared to DIBH-3DCRT. However, contralateral lung and breast volumes exposed to low doses were increased with DIBH-VMAT.

The impact of continuous positive airway pressure on radiation dose to heart and lung during left-sided postmastectomy radiotherapy when deep inspiration breath hold technique is not applicable: a case report

  • Deep inspiration breathing hold (DIBH) compared to free-breathing (FB) during radiotherapy (RT) has significantly decreased radiation dose to heart and has been one of the techniques adopted for patients with breast cancer. However, patients who are unable to make suitable deep inspiration breath may not be eligible for DIBH, yet still need to spare the heart and lung during breast cancer RT (left-sided RT in particular). Continuous positive airway pressure (CPAP) is a positive airway pressure ventilator, which keeps the airways continuously open and subsequently inflates the thorax resembling thoracic changes from DIBH. In this report, authors applied CPAP instead of FB during left-sided breast cancer RT including internal mammary node in a patient who was unable to tolerate DIBH, and substantially decreased radiation dose the heart and lung with CPAP compared to FB.

좌측 유방암 환자의 방사선치료 중 환자의 호흡과 심장 위치 분석을 통한 Deep inspiration breath-hold(DIBH) 기법의 재현성 평가 (Reproducibility Evaluation of Deep inspiration breath-hold(DIBH) technique by respiration data and heart position analysis during radiation therapy for Left Breast cancer patients)

  • 목 적 : Deep inspiration breath-hold(DIBH) 기법을 이용한 좌측 유방암의 방사선치료 중 환자의 호흡과 심장 위치의 변화에 대한 분석을 통해 DIBH 기법의 치료 재현성을 평가하고자 한다. 대상 및 방법 : 유방보존수술 후 DIBH 기법을 적용한 좌측 유방암 환자 3명을 대상으로 하였다. 전산화치료계획시스템(Eclipse version 10.0, Varian, USA)을 이용하여 자유호흡상태 Computed Tomography(CT)와 DIBH CT에서 동일한 전산화치료계획을 수립하여 심장의 체적과 선량을 비교하였고, RPM (Real-time Position Management) Respiratory Gating System (version 1.7.5, Varian, USA)의 데이터를 이용하여 환자의 호흡을 분석하였다. 치료 중 Electronic portal imaging device(EPID)를 이용한 Cine Acquisition 방법으로 영상을 획득하여 Varian 사의 Offline Review (ARIA 10, Varian, USA)에서 심장의 장축 거리를 측정하였다. 조사야 내 심장의 포함 정도를 비교 평가하기 위하여 동일한 3개(A, B, C)의 지점에서 길이 측정을 진행하였다. 결 과 : 자유호흡상태와 DIBH를 적용한 두 가지 전산화치료계획에서 심장평균선량 (6.82 vs 1.91 Gy), 심장의 V30 (68.57 vs $8.26cm^3$), V20(76.43 vs $11.34cm^3$)의 차이를 확인 할 수 있었다. 각 환자의 치료 중 DIBH 구간에서 호흡 진폭에 대한 표준편차의 평균값은 각각 ${\pm}0.07cm$, ${\pm}0.04cm$, ${\pm}0.13cm$로 분석되었다. 치료 중 획득한 영상에서 조사야 내에 포함되는 심장의 길이를 측정한 후 전산화치료계획에서의 심장의 길이와 비교한 결과 최대값은 0.32 cm, 최소값은 0.00 cm로 확인 되었다. 결 론 : 좌측 유방암 환자 치료 시 DIBH를 적용할 경우 심장에 들어가는 선량을 줄이기 위한 전산화치료계획을 수립하는데 용이할 뿐만 아니라, 실제 치료 중에도 심장의 위치에 대한 정확한 재현성과 심장 선량을 감소시킬 수 있다는 것을 확인할 수 있었다. 또한 EPID의 Cine acquisition 방법은 치료 선량 외에 추가적인 선량 없이 DIBH의 재현성 평가를 위한 매우 효과적인 방법이라고 생각된다.

Cardiac dose reduction with breathing adapted radiotherapy using self respiration monitoring system for left-sided breast cancer

  • Purpose: To quantify the cardiac dose reduction during breathing adapted radiotherapy using Real-time Position Management (RPM) system in the treatment of left-sided breast cancer. Materials and Methods: Twenty-two patients with left-sided breast cancer underwent CT scans during breathing maneuvers including free breathing (FB), deep inspiration breath-hold (DIBH), and end inspiration breath-hold (EIBH). The RPM system was used to monitor respiratory motion, and the in-house self respiration monitoring (SRM) system was used for visual feedback. For each scan, treatment plans were generated and dosimetric parameters from DIBH and EIBH plans were compared to those of FB plans. Results: All patients completed CT scans with different breathing maneuvers. When compared with FB plans, DIBH plans demonstrated significant reductions in irradiated heart volume and the heart $V_{25}$, with the relative reduction of 71% and 70%, respectively (p < 0.001). EIBH plans also resulted in significantly smaller irradiated heart volume and lower heart $V_{25}$ than FB plans, with the relative reduction of 39% and 37%, respectively (p = 0.002). Despite of significant expansion of lung volume using inspiration breath-hold, there were no significant differences in left lung $V_{25}$ among the three plans. Conclusion: In comparison with FB, both DIBH and EIBH plans demonstrated a significant reduction of radiation dose to the heart. In the training course, SRM system was useful and effective in terms of positional reproducibility and patient compliance.

좌측 유방암 방사선 치료에서 깊은 들숨 호흡법을 이용한 심장 선량 감소 평가 (Evaluation of Dose Reduction of Cardiac Exposure Using Deep-inspiration Breath Hold Technique in Left-sided Breast Radiotherapy)

  • 방사선 치료를 진행한 오른편 유방암 환자에 비해 왼편 유방암 환자의 방사선 치료 시 왼편 유방의 인접 주요 장기인 심장에 전달되는 선량에 의한 심장 질환의 발병 및 기타 질환의 발병으로 인한 높은 치사율과 관련된 예후가 보고되고 있다. 방사선 치료에서 computed tomography (CT) 영상을 획득 하는 방법 중 deep inspiration breath hold (DIBH) 기법은 들숨 상태에서 일정 시간 동안 환자의 호흡을 정지시키고 영상을 획득 하는 방법으로 심장과 흉곽 사이의 거리가 최대가 되게 한다. 따라서 본 연구에서는 DIBH 영상 획득 기법을 활용하여 왼편 유방암 환자의 방사선 치료 시 DIBH 기법을 적용한 CT 영상을 토대로 심장과 왼편 유방까지의 거리 계산 및 심장에 전달되는 피해 선량을 정량화 함으로써 왼편 유방암 환자의 방사선 치료 시 DIBH 기법의 유용성을 평가하고자 하였다. Free breathing (FB)와 DIBH 기법을 적용한 여성 유방암 환자의 CT 영상을 각 10세트를 획득하고, 50 Gy를 28번으로 분할하여 처방하였으며, 쐐기 필터(wedge filter)를 이용한 대향 2문 접선 조사를 적용했다. 심장과 왼편 유방까지의 거리는 각 장기의 중심 좌표를 획득하고, 각 중심좌표 간의 거리를 계산하였다. DIBH 기법의 경우, 일반적인 FB 기법을 적용 했을 때보다 심장과 왼편 유방 사이의 거리가 평균 1.43 mm 증가하였으며, 통계적 유효성을 확인할 수 있었다. 심장에 전달된 피해 선량의 경우, 최대 선량 기준으로 크게는 3,555 cGy 가량 감소함을 확인할 수 있었다. 각 영상마다의 거리 및 심장의 피해 선량에 대한 정도의 차이는 있었지만, DIBH 기법을 적용하였을 경우, 심장과 왼편 유방까지의 거리의 증가 및 피해 선량 감소 등의 경향성을 확인할 수 있었다. 본 DIBH 기법은 기존 방사선 치료 과정 중 추가적인 시간 소모가 적고, 쉽게 적용할 수 있다는 장점이 있으며, 임상에서의 적용으로 여성 유방암 환자의 불필요한 심장 피해 선량 전달을 감소 시킬 수 있을 것으로 사료된다.

The deep inspiration breath hold technique using Abches reduces cardiac dose in patients undergoing left-sided breast irradiation

  • Purpose: We explored whether the deep inspiration breath hold (DIBH) technique using Abches during left-sided breast irradiation was effective for minimizing the amount of radiation to the heart and lung compared to free breathing (FB). Materials and Methods: Between February and July 2012, a total of 25 patients with left-sided breast cancer underwent two computed tomography scans each with the DIBH using Abches and using FB after breast-conserving surgery. The scans were retrospectively replanned using standardized criteria for the purpose of this study. The DIBH plans for each patient were compared with FB plans using dosimetric parameters. Results: All patients were successfully treated with the DIBH technique using Abches. Significant differences were found between the DIBH and FB plans for mean heart dose (2.52 vs. 4.53 Gy), heart V30 (16.48 vs. $45.13cm^3$), V20 (21.35 vs. $54.55cm^3$), mean left anterior descending coronary artery (LAD) dose (16.01 vs. 26.26 Gy, all p < 0.001), and maximal dose to $0.2cm^3$ of the LAD (41.65 vs. 47.27 Gy, p = 0.017). The mean left lung dose (7.53 vs. 8.03 Gy, p = 0.073) and lung V20 (14.63% vs. 15.72%, p = 0.060) of DIBH using Abches were not different significantly compared with FB. Conclusion: We report that the use of a DIBH technique using Abches in breathing adapted radiotherapy for left-sided breast cancer is easily feasible in daily practice and significantly reduces the radiation doses to the heart and LAD, therefore potentially reducing cardiac risk.

Deep Inspiration Breath Holding을 적용한 유방암 세기변조방사선치료 시 위치잡이오차 분석을 통한 선량 평가 (Dosimetric Comparison of Setup Errors in Intensity Modulated Radiation Therapy with Deep Inspiration Breath Holding in Breast Cancer Radiation Therapy)

  • The aim of this study was analyzed the setup error of breast cancer patients in intensity modulated radiation therapy(IMRT) with deep inspiration breath holding(DIBH) and was analyzed the dose distribution due to setup error. A total of 45 breast cancer cases were performed a retrospective clinical analysis of setup error. In addition, the re-treatment planning was carried by shifting the setup error from the isocenter at the treatment. Based on this, the dose distribution of PTV and OARs was compared and analyzed. The 3D error for small breast group and medium breast group and large breast group were 3.1 mm and 3.7 mm and 4.1 mm, respectively. The difference between the groups was statistically significant(P=0.003). DVH results showed HI, CI for the PTV difference between standard treatment plan and re-treatment plan of 14.4%, 4%. The difference in $D_5$ and $V_{20}$ of the ipsilateral lung was 5.6%, 13% respectively. The difference in $D_5$ and $V_5$ of the heart of right breast cancer patients was 6.8%, 8% respectively. The difference in $D_5$, $V_{20}$ of the heart of left breast cancer patients was 7.2%, 23.5% respectively. In this study, there was a significant association between breast size and significant setup error in breast cancer patients with DIBH. In addition, it was found that the dose distribution of the PTV and OARs varied according to the setup error.

Deep inspiration breath-hold (DIBH) 적용한 림프절이 포함된 왼편 유방암의 방사선 치료계획에 따른 주변 장기 선량 평가

  • 목 적: 왼편 유방암 환자의 경우 오른편 유방암 환자보다 심장과 폐 등 정상장기에 불필요한 선량이 일부 조사됨에 따라 부작용이 우려되고 있다. 이를 줄이기 위해 DIBH기법을 시행하고 있다. Conventional Radiation Therapy, Intensity Modulated Radiation Therapy, Volumetric Modulated Arc Therapy의 치료계획 방법에 따라 쇄골상 림프절과 내유 림프절을 포함한 왼편 유방암의 경우 주변 장기의 선량 값을 비교 평가하고자 한다. 대상 및 방법: 왼편 유방암 환자 중 쇄골상 림프절과 내유 림프절을 포함한 환자 8명을 대상으로 Free Breathing과 Deep inspiration breath-hold 기법을 적용하여 각각 CT-Simulation을 시행한다. 획득한 영상을 기반으로 체표윤곽을 그려 95 %$D_{max}$<110 %이 되도록 Conventional Radiation Therapy, Intensity Modulated Radiation Therapy, Volumetric Modulated Arc Therapy방법으로 계획하였다. Conventional Radiation Therapy는 쇄골상 림프절에 1문조사, 유방 부분에 접선 2문조사로 Field in Field 기법을 사용하였다. Intensity Modulated Radiation Therapy는 7개의 조사면으로 구성하였다. Volumetric Modulated Arc Therapy은 회전반경을 $290^{\circ}{\sim}179^{\circ}$으로 한 2 ARC를 이용하여 계획을 수행하였다. Eclipse의 선량체적용적을 참고하여 주변 정상 장기 선량을 분석하였다. 결 과: Deep inspiration breath-hold기법을 적용함으로 심장과 흉벽 사이의 간격은 평균 $1.6{\pm}0.6cm$ 증가하였다. 폐의 평균 선량은 $19.2{\pm}1.0Gy$로 Intensity Modulated Radiation Therapy에서 가장 작은 값이 나타났다. 심장의 $V_{30}(%)$$2.0{\pm}1.9$로 Intensity Modulated Radiation Therapy에서 가장 작은 값이었다. 좌전하행 관상동맥에서는 평균 선량이 $25.4{\pm}5.4Gy$로 Intensity Modulated Radiation Therapy에서 가장 작은 값으로 나타났다. 반대편 유방의 최대선량 값은 Intensity Modulated Radiation Therapy일 때 $29.7{\pm}4.3Gy$로 가장 작은 값으로 나타났다. 결 론: 주변 정상장기 선량의 값을 비교해 보았을 때, Intensity Modulated Radiation Therapy와 Volumetric Modulated Arc Therapy은 치료에 적용할 수 있는 값으로 나타났다. 이 중 Intensity Modulated Radiation Therapy가 적합한 치료계획 방법으로 사료된다.
