족삼음경의 락혈에 시술된 침 자극에 의한 NO, NOS, NE 발현 연구 (Studies on NO, nNOS, eNOS, iNOS and NE Expression by Acupuncture at SP4, KI4 and LR5)

  • Objectives : The acupuncture about acupoint affects the production of NO, NOS, and NE.Local action of acupuncture is important for acupuncture treatment. To prove this, the revelation degree of NO, NOS, and NE was observed by stimulating the acupuncture at the connecting point of SP4, KI4, and LR5 in the depths of Superficial layer, Middle layer and Deep layer. Methods : Needles were inserted into rats, on each right and left sides of the connecting point, SP4, KI4 and LR5 acupoints which are the stream points of the foot meridian. After insertion, needles were retained for three minutes. After the retention, rat was sacrificed via cardiac puncture, and tissues of each SP4, KI4 and LR5 point near meridian vessel was extracted to examine the changes in the expression of NO, NOS and NE. Results : In terms of the effect in NO production, there was significant increase in the Superficial layer, Middle layer and Deep layer at KI4. In terms of the effect in NE production, there was significant decrease in the Superficial layer at SP4 and increase in the Superficial layer, Middle layer and Deep layer at LR5. In terms of the effect in nNOS production, there was significant increase in the Superficial layer, Middle layer and Deep layer at SP4 also in the Superficial layer at KI4. In terms of the effect in eNOS production, there was a significant increase in the Superficial layer, Middle layer and Deep layer at SP4, KI4 and LR5. In terms of the effect in iNOS production, there was significant increase in the Superficial layer, Middle layer and Deep layer at SP4, KI4 and LR5. Conclusions : The effect of acupuncture applied at the connecting point of six meridians of the foot on the activities of NO, NOS and NE could be observed, and it can be induced from the effect of needle stimulation on disrupted local and systemic nervous responses.

최소 침(Minimal Acupuncture)이 침 임상연구에서 적절한 대조군인가? (Is Minimal Acupuncture an Adequate Control in Clinical Trials?)

  • Objective : Acupuncture, one of the Oriental medical treatment techniques in East Asia, is growing in popularity all over the world. Based on bibliographical and clinical data, the depth of needling, i.e. superficial or deep needling, has been considered to be a variable as potential modifiers of needling effects, such as localization, manipulation of the needle, or elicitation of DeQi. The purpose of this review is to summarize clinical trials using minimal acupuncture and to evaluate its appropriateness as a control. Methods : A survey of computerized literature searches for randomized controlled trials using minimal acupuncture revealed that a total of 10 studies were published until April 2008. Results : Results obtained from clinical trials showed that acupuncture were more effective than minimal acupuncture in 3 out of 10 trials, while others were no more effective than control. However, minimal acupuncture might neither be a inert placebo nor be indiscriminable. Conclusion : Minimal acupuncture as 'placebo' controls seems misleading and scientifically unacceptable.

급성 후방십자인대 완전손상의 침치료 호전 1례 (Case of Acute Posterior Cruciate Ligament Complete Injury of Knee Improve by Acupuncture Therapy)

  • The purpose of this case study is to show a case of acute posterior cruciate ligament(PCL) complete injury improved by conservative acupuncture therapy. The patient was treated for 100 days. The patient received deep interarticular electroacupuncture(EA) stimulation at S35, LE200, LE211 and treated by other acupoint stimulation, herb medication, cupping, physical treatments and exercise. We evaluated through MRI and Lysholm knee score. We suggest to treat acute posterior cruciate ligament(PCL) injury with acupuncture therapy meaning conservative therapy and would like to research acupuncture treatment plan for the further treatments.

청냉연(TE11) 혈명에 대한 고찰 (Review on the Name of TE11)

  • Objectives : The acupoint TE11 (cheongnaengyeon; CNY) firstly appeared in the ≪A-B Classic of Acupuncture and Moxibustion (AB Classic)≫. The name CNY is known to be originated from an ancient deep pool described in the book of ≪Chuangtzu (Jangja in Korean)≫. Description of the name, however, was not found in a single form in the book of ≪Chuangtzu≫ and its annotated books which made a confusion. The aim of the present study is to review the name of TE11 in terms of its origin. Methods : We have compared printed editions of ≪Chuangtzu≫ and its annotated books and various acupuncture classics including ≪AB Classic≫, ≪Essential Prescriptions Worth a Thousand Gold for Emergencies (Essential Prescriptions)≫, ≪Illustrated Manual of Acupuncture Points of the Bronze Figure (Illustrated Manual)≫, ≪Collection of Gems of Acupuncture and Moxibustion (Collections of Gems)≫, and ≪Complete Compendium of Acupuncture and Moxibustion (Complete Compendium)≫. Results : The name of ancient deep pool was found in two different forms in the book of ≪Chuangtzu≫ and its annotated books. One was CNY, the other was Cheong Ryeong Yeon (CRY). The description in the ≪Chuangtzu≫, the source book, was CRY. In addition, the acupoint TE11 was described as CRY in the ≪AB Classic≫, Cheong Ryeong Cheon in the ≪Essential Prescriptions≫. Meanwhile, the point was described as CNY in the ≪Illustrated Manual≫, the ≪Collections of Gems≫ and the ≪Complete Compendium≫. Conclusions : Data suggest that the original description was CRY and a mix of CNY and CRY as a name of TE11 was used. The Korean standard name of TE11 should be changed as CRY.

Effect of Korean Medicine Treatments for Fat Pad Syndrome of Knee Joint: A Case Report

  • Fat pad syndrome is a knee joint condition/disease where acute or chronic inflammation occurs in the fat pad of the knee joint, and it is a cause of anterior knee pain. Fat pad syndrome usually occurs concurrently with other conditions/diseases to collectively cause anterior knee pain. No study to date has reported the treatment of anterior knee pain solely due to fat pad syndrome. Here, we report a case of fat pad syndrome of the knee joint as the sole cause of anterior knee pain in a 49-year-old woman who received integrated Korean medicine treatments (pharmacopuncture, acupuncture, herbal medicine, deep fascial meridian therapy, and chuna). Using patient-reported pain scale scores, the level of the patient's pain was relieved, and her mobility improved. Integrated Korean medicine treatments could be effective for patients who have fat pad syndrome of the knee joint.

경침 자가 운동 프로그램을 병행한 침 치료가 경추 만곡도와 심부 굴곡근 근력 변화에 미치는 영향 (Effect of Acupuncture Combined with Wooden Neck Pillow Self-exercise Program on Cervical Curvature Measurement Methods and Deep Flexor Muscle Strength)

  • Objectives This study was designed to identify the effect of acupuncture combined with wooden neck pillow self-exercise program (WSP). Methods 25 eligible subjects with chronic neck pain were recruited from September 2015 to August 2016. Subjects were treated by acupuncture twice a week for 4 weeks, also were educated WSP and exercised every day for 4 weeks. They were assessed using PI-NRS, NDI and the maximum muscle strength of deep cervical flexor muscles (DCF) before and after treatment. Also the Cobb's angle (C1-C7), Ishihara index, Park method were assessed for evaluating the radiographical changes. Variables were compared using paired t-test. And correlates analysis was used for analyzing relationship between differentials in the maximum muscle strength of DCF and differentials in cervical spine curvature measurement methods. Results Among the 25 subjects, 4 subjects were lost to follow-up or excluded in accordance with the criteria. Significant differences on NRS, NDI and the maximum muscle strength of DCF were seen after treatment (p<0.001). In radiological finding, cervical spine curvature measurement methods were significantly increased (p<0.001, p<0.05, p<0.05). Differentials in the maximum muscle strength of DCF significantly correlates to differentials in Ishihara index (R=0.479, p<0.05). Conclusions Acupuncture combined with WSP may decrease neck pain intensity and associated disability, and also increase the muscle strength of DCF and cervical lordosis, in patients with chronic neck pain. Especially, Improving the muscle strength of DCF has a relationship with improving Ishihara index. However, due to the design of this study, this treatment can not be compared with other treatments. Future randomized project should be needed.

하지 심부정맥혈전증으로 인한 부종 및 통증에 계지복령환 투약과 침 요법을 포함한 한의 치료를 시행한 증례 1례 보고 (Korean medicine treatment including oral administration of Gyejibongnyeong-hwan and acupuncture therapy for calf edema and pain due to deep vein thrombosis of lower leg: A Case Report)

  • Objectives: This study was aimed to report changes in clinical symptoms and signs after treatment with Korean medicine in patients who suffered from calf edema due to deep vein thrombosis (DVT). Methods: A 46-year-old male patient, who discharged home after receiving standardized treatment for acute DVT in the right leg, suffered pain and heat sense due to edema of the affected calf. Oral administration of herbal medicine (Gyejibongnyeong-hwan 4 g, twice daily) and acupuncture therapy were given to him for 6 weeks. The change in chief complaints, Villalta score, the right and left side difference of the circumference and the skin surface temperature of both calves, and blood level of D-dimer were observed before and after treatment. Results: The patient's chief complaints began to significantly improve from 2 weeks after treatment, and disappeared completely within 4 weeks. At the end of the treatment, a substantial decrease in the difference between the right and left calves in the circumference and skin surface temperature was observed. This effect was maintained even at the time of follow-up 3 months after the end of treatment, and the patient completely recovered indoor and outdoor life to the level before onset. Conclusions: This case suggests that Korean medicine treatment, including Gyejibongnyeong-hwan administration and acupuncture therapy, can be a viable option to improve edema and related clinical problems in the affected limbs due to DVT.

2도 화상 치험 1례 (One case of Second Degree Burn Wound)

  • Burn is a dermal injury mainly caused by heat. It occurs by flame, boiling water or liquid, steam, sparks, chemicals, radiation, and electricity. Burn is classified as four grades depending on how deep into the skin a person is burned. As the deep second-degree burns possibly leave scars, it can have various physical, psychological, and aesthetical effects with seriousness on the patient with a burn. This report is on the 50-year-old female patient who had a burn with boiling water and a mixture condition of superficial and deep second degree burn. The Korean medical treatments such as Herbal acupuncture, Herbal medication, Acupuncture, Herbal-ointment were used for 17 days to treat effectively the patient with a mixture of superficial and deep second degree burn. It is difficult to find out the report that treats a burn with Korean medical treatment. If more clinical treatments as in this report are proved to be effective, we are able to expect that Korean medical treatment should be a good treatment in burn.

방광루술후 발생한 심부정맥혈전증 환자 1례에 대한 임상적 고찰 (Clinical Investigation about Deep Vein Thrombosis after cystostomy;Case Report)

  • Objectives We got a good effect on one patient who diagnosed Deep Vein Thrombosis after cystostomy. Methods The patient was treated with Acupuncture, Herbal medication, Venesection. To evaluate the edema, pain of leg, Visual Analogue Scale(VAS) and the measurement of leg circumference are used. Results 1. The discrepancy of the leg circumference is decreased from 3.1㎝ to 1.6㎝. 2. The Visual Analogue Scale was decreased from 10 to 2-1. Conclusions In this case, the patient who diagnosed Deep Vein Thrombosis(DVT) after cystostomy treated by Acupuncture, Herbal medication, Venesection was improved. We are reporting this case.

Combined Korean Medicine Treatment of a Rare Case of Burst Fracture in an Elderly Patient with Kissing Spine

  • A burst fracture refers to the fracture of the anterior and middle vertebral columns which are moving into the spinal canal causing neurological impairments, generally requiring surgical treatment. We herein report a rare case of burst fracture with kissing spine in a 90-year-old man who had severe lower back pain that worsened during back extension. Considering the surgical treatment risk, he was hospitalized at a Korean medicine hospital for 85 days and underwent combined Korean medicine treatments including pharmacopuncture, herbal medicine, chuna, deep-fascia meridian therapy, walking practice, and abdominal breathing. Based on patient-reported scales, his pain was alleviated, and his physical function improved. Furthermore, his range of motion and walking time increased. This case report suggests that combined Korean medicine treatments could be an effective alternative for patients with burst fracture who have surgery risks.
