  • Purpose: The present study aimed to evaluate the clinical benefit of additional toothbrushing accompanying non-surgical periodontal treatment on oral and general health in patients with type 2 diabetes mellitus (T2DM). Methods: We conducted a doubled-blind randomized controlled trial in 60 T2DM patients between June 2013 and June 2014. The patients were randomly assigned to the scaling and root planing (SRP) group; the scaling and root planing with additional toothbrushing (SRPAT) group, in which additional toothbrushing was performed by toothpick methods; or the control group. Microbiological and oral examinations were performed for up to 12 weeks following treatment. Non-surgical treatment was conducted in the experimental groups. The SRP group received scaling and root planing and the SRPAT group received additional toothbrushing with the Watanabe method once a week from the first visit through the fifth visit. The primary outcomes were changes in haemoglobin A1c (or glycated haemoglobin; HbA1c) levels, serum endotoxin levels, and interleukin-1 beta levels. Periodontal health status was measured by periodontal pocket depth, the calculus index, and bleeding on probing (BOP). Results: Both the SRP and SRPAT groups showed improvements in periodontal health and HbA1c, but the SRPAT group showed significantly less BOP than the SRP group. Furthermore, only the SRPAT group showed a statistically significant decrease in serum endotoxin levels. Conclusions: Non-surgical periodontal treatment was effective in improving HbA1c and serum endotoxin levels in T2DM patients. Furthermore, non-surgical treatment with additional tooth brushing had a more favourable effect on gingival bleeding management. • Objectives: The aim of this study is to evaluate the association between shiftwork and skeletal muscle mass index in a single university health check-up. Methods: We used data from 98,227 workers who answered in a special interview on health check-up at a local university hospital from 2014 to 2020. Pearson correlation analysis was conducted for comparing the association between skeletal muscle mass index and demographic and hematological variables in shiftwork and non-shiftwork groups. Mixed linear model analysis after controlling demographic and hematological variables was used to analyze the difference of skeletal muscle mass index between groups at every visit for seven years. Results: In linear regression analysis, the variables most significantly correlated with skeletal muscle index in both groups were shiftwork(p=0.049), BMI(p<0.001), hypertension(p=0.024), platelet(p<0.001), total protein (p<0.001), AST(p=0.028), ALT(p=0.003), ALP(p<0.001), total cholesterol(p=0.002), triglyceride(p=0.019), BUN (p=0.001), creatinine(p<0.001), and uric acid(p=0.002). After the adjustment for demographic and hematologic variables, the skeletal muscle mass index at every visit was decreased both in the shiftwork group and non-shiftwork group. The slope of the shiftwork group was -0.240 and non-shiftwork group -0.149, showing a significant difference (p<0.001). Conclusions: In the shiftwork group, the skeletal muscle mass index showed a tendency to decrease markedly over time compared to the non-shiftwork group. It is presumed that shift workers' skeletal muscle health was adversely affected by changes in the biological clock due to changes in wake-up and sleep patterns, and changes in food intake.

  • Objectives: This study is designed to verify the effects of Mannyeon-hwan (MNH) on acetic acid-induced colitis in rats. Methods: Colitis was induced in male Sprague-Dawley rats weighing approximately 250 g by injecting acetic acid through the anus. The rats were classified into four groups: normal group, acetic acid (AA) group, AA+MNH (L) (low concentration) group, and AA+MNH (H) (high concentration) group. The body weight, visual evaluation of the colonic mucosa, anatomical histological changes, and changes in the expression of cytokines in the colon tissue were compared and analyzed. Results: Compared with the normal group, weight loss was observed in mice induced with colitis. Compared with the AA group, weight loss recovery occurred in the AA+MNH (L) and AA+MNH (H) groups, and significant changes were observed after the sixth day. In the visual evaluation of the colonic mucosa, a significant decrease in damage indicators was observed in the AA+MNH (L) and AA+MNH (H) groups compared with the AA group. In terms of anatomical histological changes and changes in the expression of tumor necrosis factor-α and interleukin-6 in colon tissue, a significant decrease was observed in the AA+MNH (L) and AA+MNH (H) groups compared with the AA group. A more pronounced decrease was observed in the AA+MNH (H) group compared with the AA+MNH (L) group. Conclusion: The effects of MNH on colitis were confirmed through research. MNH can be used as a first-line treatment for patients complaining of colitis who visit oriental medicine clinics.

  • Objectives: The aim of this study is to evaluate the association between vibration exposure and skeletal muscle mass index through a single university health check-up. Methods: We used data from 134,067 male subjects who received a general health check-up or vibration exposure health check-up out of the 1,515,322 people who underwent medical check-up at a local university hospital from 2002 to 2018. Pearson correlation analysis was conducted for comparing the association between skeletal muscle mass index and demographic and hematological variables in both groups. Mixed linear model analysis after controlling demographic and hematological variables was used to analyze the differences in skeletal muscle mass index between groups at every visit over 10 years. Results: In the Pearson correlation test, the variables that showed different results when comparing the two groups were C-reactive protein (p=0.001) and glycated hemoglobin (p=0.002) in the vibration exposure group and erythrocyte sedimentation rate (p<0.001) and vitamin D (p<0.001) in the general group. After the adjustment of demographic and hematologic variables, the skeletal muscle mass index at every visit was markedly decreased in the vibration exposure group (p<0.001). Conclusions: In the vibration exposure group, the skeletal muscle mass index showed a tendency to decrease markedly over time compared to the general health check-up group, which showed that C-reactive protein and glycated hemoglobin would have an influence on skeletal muscle index in male workers exposed to vibration.

  • Objectives : Functional dyspepsia is a type of disease characterized by bothersome postprandial fullness, early satiation, epigastric pain and burning without structural gastrointestinal disease. The aim of this study was to analyze the characteristics of functional dyspepsia patients and evaluate changes in symptoms, quality of life, and electrogastrography parameters before and after oriental medical interventions. Methods : We recruited forty-six functional dyspepsia patients who visited gastroenterology clinic in the oriental medicine hospital of Kyung Hee University between November 2009 and February 2011. Patients were assessed for their frequency of dyspepsia (based on short form-Leeds Dyspepsia Questionnaire: SF-LDQ), quality of life (based on functional dyspepsia-related quality of life questionnaire: FD-QoL), gastric motility (based on electrogastrography: EGG) on the first visit. Then, the effect of oriental medical interventions was evaluated using EGG on the second visit. Results : The majority of patients had symptoms of nausea and indigestion. The largest decrease in EGG parameters was found in the indigestion group. The frequency of regurgitation and postprandial EGG power % bradygastria showed a significant correlation. Also, significant correlations were found between some items of FD-QoL and some EGG parameters. Compared to the EGG parameters before oriental medical interventions, some parameters after treatment had positive results, implying the improvement of gastric motility disorder. We also found improvement of EGG parameters in both digestant medicinal group and digestant combined with qi-tonifying medicinal group. Conclusions : The results of this study suggest that clinical application of EGG can be an objective diagnostic tool in functional dyspepsia patients visiting oriental medical hospital.

  • Purpose: Trauma systems have been shown to decrease injury-related mortality. The present study aimed to compare the mortality rates of patients with major trauma (injury severity score >15) treated before and after the establishment of a level I trauma center. Methods: During this 20-month study, participants were divided into pre-trauma center and trauma center groups, and trauma and injury severity score (TRISS) method was used to compare mortality rates during 10-month periods before and after the establishment of the trauma center (October 2013 to July 2014 vs. October 2014 to July 2015). Results: Of the 541 total participants, 278 (51.5%) visited after the establishment of the trauma center. The Z and W statistics indicated better outcomes in the trauma center group than in the pre-trauma center group (Z statistic, 2.635 vs. -0.700; W statistic, 4.640). The trauma center group also exhibited meaningful reductions in the time interval from the emergency department (ED) visit to emergency surgery (118.0 minutes vs. 142.5 minutes, p=0.020) and the interval from the ED visit to intensive care unit admission (202.0 minutes vs. 259.0 minutes, p=0.035) relative to the pre-trauma center group. Conclusions: The TRISS and multivariate analysis revealed significant improvements in survival rates in the trauma center group, compared to the pre-trauma center group.

  • Purpose: This study examined the risk factors of a delayed diagnosis of acute appendicitis in children undergoing an appendectomy. Methods: This retrospective study involved children aged below 18 years, who underwent an appendectomy. After dividing them into a delayed diagnosis group and nondelayed diagnosis group according to the time interval between the initial hospital visit and final diagnosis, the risk factors of delayed diagnosis were identified using logistic regression analysis. Results: Among 712 patients, 105 patients (14.7%) were classified in the delayed diagnosis group; 92 patients (12.9%) were diagnosed using ultrasonography (US), and both US and computed tomography were performed in 38 patients (5.3%). More patients in the delayed diagnosis group underwent US (P=0.03). Spring season and prior local clinic visit were significantly associated with a delayed diagnosis. Fever and diarrhea were more common in the delayed diagnosis group (fever: odds ratio [OR], 1.37; 95% confidence interval [CI], 1.05-1.81; diarrhea: OR, 1.94; 95% CI, 1.08-3.46; P<0.05). These patients showed symptoms for a longer duration (OR, 2.59; 95% CI, 1.78-3.78; P<0.05), and the admission course (OR, 1.26; 95% CI, 1.11-1.44; P<0.05) and C-reactive protein (CRP) levels (OR, 1.47; 95% CI, 1.19-1.82; P<0.05) were associated with the delayed diagnosis. Conclusion: To decrease the rate of delayed diagnoses of acute appendicitis, symptoms such as fever and diarrhea, seasonal variations, admission course, and CRP levels should be considered and children with a longer duration of symptoms should be closely monitored.

  • Objective : The purpose of this study was to figure out the radiologic findings and risk factors related to adjacent segment degeneration (ASD) after anterior cervical discectomy and fusion (ACDF) using 3-year follow-up radiography, computed tomography (CT), and magnetic resonance image (MRI). Methods : A retrospective matched comparative study was performed for 64 patients who underwent single-level ACDF with a cage and plate. Radiologic parameters, including upper segment range of motion (USROM), lower segment range of motion (LSROM), upper segment disc height (UDH), and lower segment disc height (LDH), clinical outcomes assessed with neck and arm visual analogue scale (VAS), and risk factors were analyzed. Results : Patients were categorized into the ASD (32 patients) and non-ASD (32 patients) group. The decrease of UDH was significantly greater in the ASD group at each follow-up visit. At 36 months postoperatively, the difference for USROM value from the preoperative one significantly increased in the ASD group than non-ASD group. Preoperative other segment degeneration was significantly associated with the increased incidence of ASD at 36 months. However, pain intensity for the neck and arm was not significantly different between groups at any post-operative follow-up visit. Conclusion : The main factor affecting ASD is preoperative other segment degeneration out of the adjacent segment. In addition, patients over the age of 50 are at higher risk of developing ASD. Although there was definite radiologic degeneration in the ASD group, no significant difference was observed between the ASD and non-ASD groups in terms of the incidence of symptomatic disease.
