This case report included 4 cases of traumatic diaphragmatic hernias, 3 cases of non-traumatic diaphragmatic hernias, and 3 cases of eventration of the diaphragm. Among the traumatic hernias, one case was in immediate phase of traumatic diaphragmatic rupture by traffic accident, 2 cases were in intermediate phase with chronic respiratory or vague gastrointestinal symptoms after traffic accident, and the other was developed after an operation, decortication for a chronic empyema with severe pleural calcifications, damaging the diaphragm. Three cases of nontraumatic diaphragmatic hernia were presented, including 2 cases of probable Bochadlek's hernia (Parents refused operation) and a case of Morgagni's hernia with severe gastrointestinal symptoms. And three cases of eventration of the diaphragm with symptoms were also reported. Results of all treated cases were excellent.
The author made clinical study of 100 cases of empyema in infancy and childhood that were treated at the Department of Chest Surgery, Busan University Hospital and Busan Children's Charity Hospital, from Jan 1962 to Nov. 1975. 1. In infancy and childhood, 62 cases out of my 100 cases of empyema were caused by .staphylococci and most of recent reports showed a gradual increase in number of staphylococcal empyema. 2. Most frequent lesion predisposing to empyema in infancy and childhood was pneumonia (72%), being remarkable in staphylococcal empyema (85.5%) to that of others. 3. Antibiotics sensitivity test for staphylococci revealed that the erythromycin was most susceptible (85. 5%). 4. The mortality rate was 6% in over all and the author believes that from the point of view of surgical treatment, failure of early continuous drainage on account of multiple thoracentesis for the early stage of empyema, and also early open thoracotomy procedure such as decortication were all the contributing factors to higher mortality in the empyema of infancy and childhood. 5. It may be concluded that the treatment of choice for empyema in infancy and childhood were early and prolonged continuous drainage of pus by closed thoracotomy with caution and administration of more susceptible antibiotics with nutritional support.
Pulmonary aspergillosis is relatively rare disease, most commonly presenting pre-existing cavitary disease. This investigation is designed to illustrate the clinical features, preoperative diagnosis and surgical role in the management of this disease. In retrospective review of opeative cases since Jan. 1988, total 16 cases were analysis. Peak incidence of age lies in the 3rd & 5th decade[81.3%].The common presenting symptoms were hemoptysis & blood tinged sputum[90.9%] & all cases had a history of treatment with antituberculous drugs, but the tuberculosis was revealed in only 3 cases in the pathologic studies on specimen after resection. The locations of lesion were upper lobes in 13 cases, lower lobe in 2 cases, and left whole lobe in 1 case. The operative procedures for treatment of aspergillosis were performed. The lobectomy was performed in 7 cases, lobectomy & segmentectomy in 2 cases, lobectomy & decortication in 2 cases, segmentectomy in 3 cases, bilobectomy & segmentectomy in 1 case and pneumonectomy in 1 case. There was one death due to respiratory insufficiency 21 days later,postoperatively, and empyemas & residual space problems were developed in 2 & 3 cases,respectively. The empyemas were treated with open thoracostomy and the dead spaces was filled with granulation tissues within 6 months. During follow up, there was no recurrence. In conclusion, surgical resection is the treatment of choice for symptomatic aspergillosis and needed in asymptomatic patient to prevent possible fatal sequelae in the future.
We have experienced a case of pleural aspergillosis. A 50 year old female complained of malaise, anorexia, coughing with sputum, and right sided pleuritic chest pain of two weeks' duration. About ten years ago, she had been treated for pulmonary tuberculosis with medication. Chest radiography showed right pyopneumothorax with cavitation in the rig t upper lung and Chest computed topography revealed right loculated pyopneumothorax with cavity formation suggesting bronchopleural fistula. Decortication and wedge resection with pleurectomy were performed. The postoperative course was satisfactory and has been in good condition up to now. Pleural aspergillosis is a very rare and potentially life-threatening disease, but we have had good results without significant complications by treatment with systemic antifungal drugs and surgical operation.
A Clinical analysis of 64 patients of thoracic empyema was done who received surgical intervention at Dept. of Thoracic Surgery of the Chosun University Hospital in the period of 3 years from September 1976 to October 1979. Following was the results: 1. Seven cases [10.9%] were under the age of 15 years, 16 cases [25%] was between 15-30 years and 41 cases [64.1%] was above the age of 30 years. A proportion of children and adult was 1:8. 2. Male and female ratio was 3:1. Right and left side pleural cavity ratio was 2.4:1. 3. Predisposing factors were pneumonia [35.9%] and pulmonary tuberculosis [28.1%]. 4. Most frequently encountered symptoms were dyspnea, cough, chest pain and fever in order. 5. Etiologic organisms were confirmed in 39 cases [86.7%] which requested in 45 cases. Staphylococcal infections were 11 cases and streptococcal, pneumococcal pseudomonas infection was infected in order. 6. Pneumothorax was associated with empyema on 21 cases [32.8%]; among those 13 cases [61.9%] were tuberculous in nature. 7. Sensitivity test was revealed that Minocin was most very sensitive drug, and next Erythromycin, Gentamycin and Penbrex in order. But most resistant drugs were Penicillin, Kanamycin, Streptomycin and Tetracycline in order. 8. Treatments were combined with antibiotics therapy and several surgical procedures for empyema. 26 cases [40.6%] were treated with closed thoracotomy drainage, 17 cases [26.6%] with open thoracotomy tube drainage and 9 cases decortication and 9 cases thoracoplasty. 9. 2 death cases occurred in 64 cases of thoracic empyema, and 79.7% cases were discharged with recovery and improvement.
Fibrothorax is the end stage of chronic pathologic processes of pleura such as hemothorax, empyema, or tuberculous effusion. The pleural space become adherent and obliterated, and the lung parenchyma is covered by a thick, fibrous, unexpandable "peel", so the lung function is diminished markedly with impaired ventilation and oxygenation. Constrictive pericarditis is often accompanied fibrothorax, also cardiac and hemodynamic function is deteriorated. Surgical relief of these fibrous peels causes remarkable improvement in pulmonary function, cardiac and hemodynamic function, and subjective symptoms. We experienced a case of bilateral fibrothorax combined with constrictive pericarditis which occured 3 years after bilateral tuberculous effusion. Decortication and percardiectomy were done at the same time through bilateral submammary thoracotomy with sternal transection. Comparing postoperative Peripheral venous pressure, Circulation time, Pulmonary function test, Arterial blood gas analysis, Subjective symptoms with preoperative conditions showed noticeable improvement.provement.

Although Dieulafoy lesion can occur in any part of the gastrointestinal tract, its occurrence in the rectum is rare. Rectal Dieulafoy lesions have been associated with advanced age, renal failure, burns, liver transplantation and cirrhosis. Here, we report on a case of massive bleeding from a rectal Dieulafoy lesion after lung decortication surgery in a 57-year-old male patient with alcoholic cirrhosis. Although rare, a rectal Dieulafoy lesion should be included in the differential diagnosis of massive lower gastrointestinal bleeding in a patient with cirrhosis.
A 51-year-old woman was transferred from a private hospital for persistent massive left pleural effusion. Available examination methods did not reveal the cause of the disease. The pleural effusion was confirmed as chylothorax by thoracentesis and chest computed tomography. Previous tube drainage and pleurodesis had failed. Therefore we decided on an operative approach. A left thoracotomy revealed nothing abnormal except for the oozing lymph from the mediastinal pleura, which was sutured by 4-0 prolene. Decortication and pleurodesis were done at the same time. Postoperative course was uneventful and no recurrent pleural effusion was recognized for 3 months. Adult Idiopathic unilateral chylothorax with unknown etiology is rare, but this case was successfully treated with an operative method.
Piezoelectric surgery is a novel surgical approach used in orthodontic treatment for rapid tooth movement. This paper presents a case series wherein osseous outgrowths were observed in response to piezosurgery-assisted en-masse retraction. Sixteen patients requiring upper premolar extractions were treated with miniscrew-supported en-masse retraction and received minimally invasive decortication via piezosurgery. Computed tomography (CT) of the maxillary anterior region was performed to investigate the nature of the outgrowths. In 8 of the 16 patients, hemispheric or disc-shaped osseous outgrowths were observed on the sites where piezosurgery was performed during retraction. CT images revealed that these outgrowths were alveolar bone. This case series presents a previously unreported osseous response to piezosurgery-assisted tooth movement during orthodontic treatment. The response is mostly transient and is observed in 50% of the treated patients, suggesting a bone turnover that can be assessed clinically and radiographically.
The different products of neem (Azadirachta indica) are utilized for variety of purposes in industry, health and animal agriculture in the Indian subcontinent. The cake from seeds after oil extraction is a good source of nutrients (CP: 35-38%; EE: 4.5-5.5%; CF: 12-15%; Ca: 0.75%; P: 0.45% on DM), and in particular, the one out of its kernel is proteinaceous and is relatively balanced in its amino acid and mineral profile. But the cake is toxic and bitter to taste owing to triterpenoids (nimbin, salannin, azadirachtin), which restricts its safe inclusion in livestock diet. Several feeding trials with raw cake have revealed poor palatability and adverse performance among different categories of livestock and poultry. Internal organ changes included histological alteration in intestine, liver, kidney and distruption of spermatogenesis and ovarian activity. Ruminants appears to tolerate reasonably higher levels of the cake and to a limited low levels of dietary inclusion also proved to be tolerable in monogastric farm animals. Debitterization through solvent (hexane, ether) extraction, water washing, alkali (NaOH, 1.5, 2.5 or 3%, wt/wt) soaking and urea (1.5 or 3%, wt/wt) - ammoniation have been tried with appreciable success in improving the palatability and nutritive value of the cake. For enhanced utilization, decortication of neem seeds is to be done effectively at industrial level with maximum oil recovery. The resultant proteinaceous kernel by-product could be a cheaper unconventional protein supplement after suitable processing.
