TREATMENT OF FUSED PERMANENT MANDIBULAR LATERAL AND CENTRAL INCISORS: REPORTS OF CASES (유합된 하악 영구 중절치와 측절치의 치료에 관한 증례보고)

  • Developmental dental morphological anomalies are fusion, gemination, twinning, concrescence and etc. They may cause many problems in conservative, periodontal and esthetic aspects. Fusion is a condition where two separate tooth buds unite at some stage in their development to form a bifid crown. If tooth contact occurs early, at least before the start of calcification, the two teeth may be completely unites to form a single large tooth. If tooth contact occurs after the time when a portion of the tooth crown has completed its formation, there may be union of the roots only. In fusion the dentin is always confluent. Fusion teeth is more common in the deciduous than in the permanent dentition. Fused teeth are relatively rare, and are mostly mandibular anterior teeth. Fusion of normal and supernumerary teeth or between normal teeth may occurs. The exact etiology factor of fusion is unknown, but genetic and environmental factors seems to be related. A variety of complications and subsequent treatments have been suggested on this teeth. Periodontal conditions may arise due to a groove formed at the line of fusion of the two teeth. Crowding or Protrusion with potential for malocclusion or delayed eruption of adjacent teeth. Fusion teeth appear in the anterior region, they usually cause esthetic problems. Treatments vary depending on the problem, the location, and the extent of fusion. Treatment of fused teeth has been reported from endodontic, orthodontic, periodontic, surgical and multidisciplinary. This report presents the esthetic improvements by separation of two clinical crowns. Bone reduction or endodontic treatments are not required.

KNOWLEDGE AND ATTITUDE OF MOTHERS WHO HAVE INFANTS ABOUT BABY BOTTLE TOOTH DECAY AND DENTAL CARIES PREVENTION (유아 모친의 우유병우식증 및 치아우식증 예방에 대한 지식 및 태도 조사)

  • The purpose of this study was to investigate the knowledge and attitude of a group of mothers who had infants about baby bottle tooth decay(BBTD) and dental caries prevention. The sample consisted of 261 mothers who had infants from 6 to 36 months from certain areas of Dae-jeon city. The mothers were asked to answer a questionnaire which included a series of questions regarding general information cencerning the infant and his mother, knowledge about BBTD and methods of prevention using fluoride, knowledge about behavioral risk factors contributing to caries of the infant, attitude toward oral health care of the infant, opinions regarding the effectiveness of methods to prevent dental caries and sources of oral health information. he results of this study were as follow. 1. 85.4% of the respondents thought that prolonged bottle feeding would harm the teeth of the infant, but 77% of the respondents had never heard of baby bottle tooth decay. 2. Levels of knowledge about methods of prevention using fluoride were relatively low and there were significant relationships between the levels of this knowledge and the educational level of the respondents(P<0.05). 3. Only 7.3% of the respondents thought that prolonged breast feeding would harm the teeth of the infant. 4. Overall oral health care of the infants was relatively insufficient. 5. Only 28.7% of the respondents selected either when the first tooth erupt or deciduous dentition complete as the time a infant should first be seen by a dentist. 6. 64.6% of the respondents thought that toothbrushing should be instituted once the first tooth appears. 7. Among the six methods of preventing caries in infants, respondents ranked the effective of fluoride lower than making regular dental visits and reducing intake of sugared foods. 8. Most frequently cited sources of information about dental health were newspapers, magazines and books(65%), followed by friends, neighborhoods and families (55%).

AMELOBLASTIC FIBROMA IN MIXED DENTITION : A REPORT OF 2 CASES (법랑모세포섬유종 환아에 관한 증례보고)

  • Ameloblastic fibroma is a rare benign tumor, accounting for only 2.5% of odontogenic tumors. It occurs during the period of tooth formation between the ages of 5 and 20 years with the average age being about 15. There is no gender predilection. In the majority of cases, the lesion arises in the mandible, presenting the swelling of jaw and the failure of tooth eruption. In this report, the main concern of the patient was the failure of eruption of lower permanent and deciduous molars. Radiographic investigation showed a radiolucency surrounding the crown of unerupted teeth. Surgical intervention and histopathologic study revealed the lesion to be ameloblastic fibroma. After the surgery, no evidence of residual tumor or recurrency was found. These patients are scheduled for the long-term continuing evaluation of the eruption of adjacent teeth and successor with radiographic study.

  • The ectopic eruption is defined as abnormal eruption which gives to displacement of the teeth and abnormal root resorption of adjacent teeth. The prevalence of ectopic eruption is reported to vary 2~6%, most of them are in the maxilla. Etiologic factors include narrow maxilla, large maxillary teeth, inclined eruption path of the first molar, retruded position of the maxilla and hereditary factor. Irreversible ectopic eruption where the second primary molar is lost often causes mesial tipping and rotation of the permanent molar, unfavorable occlusion and space deficiency for the second premolar. Ectopically erupted teeth should be treated early to maintain normal development of the dentition, harmony of facial growth and occlusal support. The method of the treatment are classified as follows : appliances that is positioned at the contact point for unlocking and the distal movement, fixed appliance that is connected to more than one tooth, and occlusion guiding method after disking or extraction of the second deciduous molar. A case report of a patient with bilaterally ectopic eruption of maxilla and mandible first permanent molar was present. Also, the patient who had experienced the chronic myelogenous leukemia, show various dental developmental complications. The ectopic eruption was treated with a Halterman appliance that was a effective way of correcting of ectopic eruption of the permanent first molar.

APPLICATION OF AN ORAL SCREEN (Oral Screen의 임상 적용에 대한 고찰)

  • The oral screen is a functional appliance, suitable for the treatment of developing malocclusion associated with aberrant muscular patterns. The better muscle balance between tongue and the buccinator mechanism can be established, and the reestablishment of normal growth and development can be achieved. The oral screen can be used for the correction of the following conditions : (1) thumbsucking, tongue thrusting and lip biting, (2) mouth breathing, (3) mild distocclusion with premaxillary protrusion, (4) open bites in deciduous and mixed dentition, and (5) incompetent lips. The patient should wear the oral screen every night and also during the day whenever possible. The effects of oral screen can be elevated through lip seal exercise : the lips should be kept in contact all the time to improve the lip seal. In the presented two cases, the patients were considered mouth breathers and to have incompetent lips, and one patient with maxillary incisal protrusion and the other with open bite. They were instructed to wear the oral screen with lip seal exercise. After wearing the appliance for 1 and 2 years respectively, mouth breathing was decreased and lip length and strength were increased, the maxillary incisors were retruded and open bite reduced.

  • Incidence of crown-root fracture due to traumatic injury, have been reported 3% in the permanent dentiton, 2% in the deciduous dentition. There are two treatment methods for crown-root fractured teeth with pulp exposure, when the fracture line was located under the alveolar crest. One way is the extrusion by orthodontic force the other way is intra-alveolar transplantation which occlusally repositioning of apical fragment in the alveolar socket. Since intra-alveolar transplantation has introduced in 1970s, it was practiced as alternative to orthodontic extrusion. As the result, this method may thoughted that had a good prognosis. As a result of trauma, completely crown-root fracture was occured in the maxillary right central incisor in this case. We couldn't reposition the deepest fracture line above the alveolar crest by the conventional surgical extrusion, because apical fragment was too short. Thus, after extraction of apical fragment, we repositioned it to the socket following demineralized freezed dried bone graft, which possible to support the apical fragment. At the 15-month recall examination, the root still showed normal mobility and there was not observed any in flammatory or replacement root resorption in the periapical radiograph.

A STUDY ON AN INTERPRETATION OF THE ORTHOPANTOMOGRAPH IN THE MIXED DENTITION (Orthopantomograph에 의(依)한 혼합치열상(混合齒列像)의 해석(解析)에 관(關)한 연구(硏究))

  • Plaster models were constructed and orthopantomographs were taken for 86 male and 70 female primary school pupils, whose upper and lower permanent 4 incisors and 1st molars were completely erupted without crowding; whose deciduous canines and molars were found almost uniformly even without any visible tooth fractures, dental caries or restorations on proximal surfaces of the teeth. Certain reference points on the orthophantomograph were set up and measured and the values were compared with actual or predicted values from the models. The following results were obtained: 1. In regards to available space, the values from the orthopantomograph were greater than the values from the models by a mean of 3.24% on the upper and 10.06% on the lower for males; 3.05% on the upper and 10.01% on the lower for females. 2. In regards to total mesiodistal widths of permanent canine, 1st and 2nd premolars, the values from the orthopantomograph were greater than the presumed values based on the size of lower permanent 4 incisors from the models by a mean of 18.50% on the upper and 24.09% on the lower for males; 14.54 on the upper and 20.51% on the lower for females. 3. Comparing the magnified values of total mesiodistal widths of permanent canine, 1st and 2nd premolars with those of available space, the regression constants of regression equation (Y = a + bX) between them were a=3.2336, b=0.6533 on the upper and a=5.0138, b=0.3290 on the lower for males; a=2.5994, b=0.6521 on the upper and a=3.0113, b=0.6586 on the lower for females. 4. The correlation coefficients between the magnified values of available space and permanent canine, 1st and 2nd premolars were moderately positive as 0.6474 in the upper and 0.505 on the lower for males; 0.6493 on the upper and 0.6183 on the lower for females. 5. In regards to magnified values of the available space from the orthopantomographs there were no significant difference between sexes, (P>0.05) but of the total mesiodistal widths of permanent canine, 1st and 2nd premolars, a significant difference between sexes was found.(p<0.01).

AN EXPERIMENTAL STUDY ON THE EFFECT OF THE MANDIBULAR RETRACTIVE FORCE ON THE MANDIBULAR CONDYLE OF GROWING YOUNG DOG (하악 후방 견인력이 성장기 유견의 하악과두에 미치는 영향에 관한 실험적 연구)

  • The purpose of this study was to evaluate the effects of the mandibular retractive force on the mandibular condyle of growing dog. The experimental animals were six mongrel dogs of two-month old. Their deciduous dentition were completed. Two of them was used as control group, and experimental group was composed of remaining four. Head band and chin cup were made of cotton tape, and hooks are fabricated on the chin cup and had band for closed coil. Mandibular retractive force was 100g/side and chin cap appliance was used for 14 hours/day during night. Experimental group were sacrificed at 2, 4, 6, 8 weeks from beginning of the experiment. Right TMJ was prepared for histologic study and left TMJ was examined grossly for disc, fossa, and condyle. The conclusions are: 1. Two-month old control animal showed active cartilaginous growth on the mandibular condyle, therefore showed thick proliferative and hypertrophic zones. Remodeling process in the condyle head was observed in which there were bone resorption on the anterior surface and bone apposition on the posterior surface. 2. Four-month old control animal showed marked reduction of hypertrophic zone but the condylar bone remodeling was more pronounced. 3. In experimental group, there are marked reduction of hypertrophic zone at 4 weeks from beginning of experiment, and hypertrophic zone disappeared at posterior-superior portion of condyle in 6-week experimental animal. 8 week experimental animal showed slight recovery of hypertrophic zone. 4. In experimental group, bone deposition was increased at anterior surface of condyle, and bone resorption was increased at posterior surface of condyle. 5. In control group, the glenoid fossa and surrounding bone showed mainly bone apposition. But experimental group showed bone resorption at anterior surface of articular eminence and increased bone apposition at posterior surface of postglenoid spine. 6. No marked traumatic change was seen but 4 weeks and 8 weeks experimental animal showed flattening of posterior surperior condylar surface. Bone marrow of condyle showed minute focal bleeding in 2 weeks and 4 weeks experimental animal, and congestion and depression of hematopoietic bone marrow during all experimental period.

PREVENTION OF DENTAL CARIES BY XYLITOL GUM IN PRE-SCHOOL CHILDREN DURING 12-MONTHS (자일리톨 껌 저작에 의한 유치 우식증 예방효과 비교분석)

  • The effect of xylitol and sorbitol chewing gums on dental caries levels among 5-year old kindergarteners was investigated. Three kindergartens the operations of which were partly subsidized by municipal government of Daegu city, Korea, were chosen as the study sites. The child base of the kindergartens were considered demographically and ethnically similar, representing middle-income families. The subjects were examined at their own kindergarten by the two dentists. Participation in the program was voluntary. Over 12 months, 123 participants chewed xylitol chewing gums(X group; 42 subjects), sorbitol chewing gum(S group; 42 subjects), or did not receive chewing gum as a control group(C group; 39 subjects). Consumption of xylitol and sorbitol was 4.5 to 5.0 g/day/subject, consumed in five daily chewing episodes of 5 min. Oral examination, Dentocult-SM test and interproximal dental plaque collection were completed at baseline and 12 months later. The dmfs of group C increased 59.2%, but group S increased 33.4% and group X increased only 31.3% during 12 months study period. The caries prevention ratio was 47.1% at group X and 43.6% at group S. There also appeared the reduction of caries activity at group X(1.39), group S(1.50) than control group(1.79). Compared with groups S and C, there was a statistically significant reduction of S. mutans in interproximal plaque in group X. The results suggest that xylitol chewing gum can prevent dental caries of deciduous dentition, and may be a little more effective than a sorbitol-containing product in controlling some caries-associated parameters in kindergarten-age subjects.

SURGICAL APPROACH TO THE INFRAOCCLUDED TEETH BY USING SPACE REGAINING TREATMENT (저위교합 유구치에서의 공간확장술을 이용한 외과적 접근)

  • Infraclusion may be defined as teeth that stop their relative occlusal movement in the dental arches during or after the period of active eruption and then remain under the occlusal plane. Delayed exfoliation, malocclusion, increased susceptibility to dental caries and periodontal disease of both the neighboring teeth and retained molar, and dislocation of the successor are the consequencces of infraclusion of primary molars. Therefore, early diagnosis and appropriate treatments are necessary. The therapeutic approach of the infracluded teeth varied from preservation to extraction. The teeth with simple infraclusion without any signs of interference with occlusal and jaw development may be examined periodically with follow-up check and radiographically. However, if the infracluded tooth interferes with normal eruption of successor or shows any sign of delayed resorption, or the tipping of adjacent teeth or supraeruption of opposing teeth is expected, the teeth inflicted should be extracted and appropriate measures should be provided in order to maintain the normal development of occlusion and dentition. The adjacent teeth which have been collapsed over a infracluded deciduous teeth can disturb the arch length perimeter. In such cases, surgical approach might be necessary, although it would be difficult when teeth are severly leaned. However, an easier surgical access have been obtained by space regaining procedures, in young patients whose arch length has been shortened due to the infracluded teeth.
