  • From April, 1981, to April, 1990, 20 male and 7 female patients ranging in age from 17 to 63, were operated on for aortic insufficiency with an aneurysm of the ascending aorta. Ten patients were in New York Heart Association functional class II, 7 in class III, and ten in class IV. The surgical treatment in all cases consisted of total replacement of the ascending aorta with composite graft containing a prosthetic aortic valve and reimplantation of the coronary arteries by an intermediate tube graft. In 15 patients an uncomplicated annulo-aortic ectasia existed, and in 12 an aortic dissection; three of the latter group were operated during the acute phase. 17 patients showed typical Marfan syndrome, and 3 patients showed severe ascending aortic aneurysm secondary to the aortic valve disease. The overall operative mortality was 7%[2 deaths]. Those 2 deaths occurred following emergency operation due to associated aortic dissection, but no death during elective operation. All survivors have been followed-up during a period ranging 1 to 108 month[average 34 months]. There was no late mortality. Among the survivors, clinical improvement is readily apparent[2,3 in class I, 2 in class II ]. In conclusion, the treatment of aortic insufficiency associated with an aneurysm of the ascending aorta by insertion of a composite graft and reimplantation of the coronary arteries through an intermediate Dacron tube is a reliable method with low mortality and excellent results.

Surgical Management of Ventricular Septal Defect in Infancy (영아기의 심실중격결손에 대한 이라완전교정술과 단계교정술의 비교)

  • Eighteen infants with a large ventricular septal defect[VSD] underwent primary surgical repair from January 1986 to December 1992. Operation was done because of failure to thrive, medically intractable heart failure, recurrent pneumonia, increased pulmonary vascular resistance[PVR]. Four patients[22.2%] died in the early postoperative period. Relief of heart failure and normalization of growth and weight gain was evident in all survivor. There was no late postoperative death. The results of primary surgical repair of VSD in infancy are compared with those of palliative pulmonary artery banding[PAB] and of VSD closure after PAB. Twenty-seven patients with isolated VSD or with VSD associated with atrial septal defect, patent ductus arteriosus, or coarctation of the aorta underwent initial palliative PAB. There were 3 early postoperative deaths[11.1%]. Severe elevation of PVR persisted in two patients. Closure of VSD and pulmonary artery debanding was done in twenty patients, with 2 early postoperative deaths[10.0%]. Placement of the PAB too close to the pulmonary annulus necessitated trasannular patching in one patient, but any problem caused by migration of the band was not developed. It is concluded that primary surgical repair of VSD in infancy is reasonable and that PAB is indicated only for those patients less than 6 months old with a complicated defect or in an emergency situation.

Non-organ Specific Cancer Preventive Effect of Korean Ginseng : A Prospective Study for 10 Years

  • A prospective study was conducted to evaluate the preventive effect of Korean ginseng against cancer in the population residing ginseng cultivation area, Kangwha-eup from August 1987 to December 1997. The participants consisted of 4,553 adults over 40 years old who completed a questionare on ginseng intake. During the surveillance period, 14.4% (656 of 4,553) of subjects had died, cancer accounting for 23.9% (157) of total deaths. The proportional hazard model of Cox was used to estimate relative risks when controlling simultaneously for covariates. Ginseng intakers had a decreased risk (relative risk(RR) =0.48, 95% confidence interval (Cl) : 0.34-0.66) for cancer compared with non-intakers. The RRs of cancer were 0.36 (95% Cl: 0.24-0.56) for multiple combination intakers. Among 24 red ginseng intakers, there were no cancer deaths. The RRs of ginseng intakers were 0.38 (95% Cl: 0.20-0.71) in gastric cancer and 0.29 (95% Cl: 0.15- 0.57) in lung cancer. These findings strongly suggested Korean ginseng have non-organ specific cancer preventive effects against cancer. Further research for clarifying the mechanisms of prevention and clinical trials on Korean ginseng must be conducted with worldwide collaborations

A study on the Causes Analysis and Preventive Measures by Disaster types in Construction Fields (건설현장에서의 사고유형별 원인 분석 및 재해예방대책에 관한 연구)

  • Recently, carelessness on safety management in construction sites has caused a sharper upward trend in accidents and disasters, compared to the past. Therefore, this study analyzes the causes of accidents that occurred on construction sites in the year of 2010, and proposes preventive measures. The construction industry had a heavy death rate with 279 out of 418 deaths from falls in all industrial sites, accounting for 66.7%. And the causation analysis found a particularly high incidence of temporary structure-related accidents, with falling off scaffolding as the most common cause of accidents. According to the causation analysis of fall-down accident cases, accidents induced by protrusion on construction sites are a big contributor resulting in the deaths of 312, accounting for 74.8%, which is deemed to be due to site disorderliness. And to remove floor protrusions and to secure workers' traffic line and safety, neat and thorough site arrangement is necessary. Against collision, efforts must be made to prevent equipment-pedestrian crashes on site, which necessarily requires emergency lamps, alarm bells, and safety poles and blocks to be installed, lighting systems to be improved, obstacles to be removed, site supervisors to be assigned to construction equipment works, and workers to be more intensely trained and supervised.

  • Protecting an infant from a risk of accident was an absolute responsibility and duty of parents in the past. As increase in the ratio of entering kindergartens, the demand of all day program also increases. In infancy, safety concerns are higher than other periods since infants are curious about objects and environments and that have stronger impulse of the spirit of inquiry. Since baby's functioning is poor, an ability of balance maintenance and an exercise function are not sufficiently developed. Further more, they lack a cognitive power about self-control or situation. According to Korea Consumer Agency, safety accidents in infant institution have increased in 115% as 294 cases compared with 137 cases in 2004. Major causes of infant deaths are street accidents, drowning, and deaths from a fall in order. Considering this situation, we need to prevent and deal with accidents. Therefore this thesis grasp the situation and analyze an actual condition of an safety education about teachers who work in such institutions and infants who are educated in institutions to prevent from accidents and make up pleasant environments. Then, it will suggest all efficient safety education plans for infants to them.

Estimation of Future Death Burden of High Temperatures from Climate Change (기후변화로 인한 고온의 미래 사망부담 추정)

  • Objectives: Elevated temperatures during summer months have been reported since the early 20th century to be associated with increased daily mortality. However, future death impacts of high temperatures resulting from climate change could be variously estimated in consideration of the future changes in historical temperature-mortality relationships, mortality, and population. This study examined the future death burden of high temperatures resulting from climate change in Seoul over the period of 2001-2040. Methods: We calculated yearly death burden attributable to high temperatures stemming from climate change in Seoul from 2001-2040. These future death burdens from high temperature were computed by multiplying relative risk, temperature, mortality, and population at any future point. To incorporate adaptation, we assumed future changes in temperature-mortality relationships (i.e. threshold temperatures and slopes), which were estimated as short-term temperature effects using a Poisson regression model. Results: The results show that climate change will lead to a substantial increase in summer high temperature-related death burden in the future, even considering adaptation by the population group. The yearly death burden attributable to elevated temperatures ranged from approximately 0.7 deaths per 100,000 people in 2001-2010 to about 1.5 deaths per 100,000 people in Seoul in 2036-2040. Conclusions: This study suggests that adaptation strategies and communication regarding future health risks stemming from climate change are necessary for the public and for the political leadership of South Korea.

Suicide Mortality Across Broad Occupational Groups in Greece: A Descriptive Study

  • Background: Several studies have investigated the relationship between specific occupations and suicide mortality, as suicide rates differ by profession. The aim of this study was to investigate suicide mortality ratios across broad occupational groups in Greece for both sexes in the period 2000-2009. Methods: Data of suicide deaths were retrieved from the Hellenic Statistical Authority and comparative mortality ratios were calculated. Occupational classification was based on the International Classification of Occupations (ISCO-88) and the coding for Intentional self-harm (X60-X84) was based on the international classification of diseases (ICD-10). Results: Male dominant occupations, mainly armed forces, skilled farmers and elementary workers, and female high-skilled occupations were seen as high risk groups for suicide in a period of 10 years. The age-productive group of 30-39 years in Greek male elementary workers and the 50-59 age-productive group of Greek professional women proved to have the most elevated number of suicide deaths. Conclusion: Further research is needed into the work-related stressors of occupations with high suicide mortality risk and focused suicide prevention strategies should be applied within vulnerable working age populations.

Twenty three experiences of Fontan operation (Fontan 술식의 치험 23례에 관한 고찰)

  • The Fontan principle of redirecting systemic venous blood into the pulmonary arteries via a conduit was initially proposed for Tricuspid Atresia, but it and its modifications have now gained a much wider application than initially intended. From September 1978 to July 1983, Atriopulmonary anastomosis has been performed in 23 patients, 2 months to 19 years of age, Seoul National University Hospital. The diagnoses were Tricuspid Atresia [TA] in 13, Univentricular Heart [UVH] in 9 and one case of Double Outlet of Right Ventricle [DORV]. Previous procedures included two Glenn shunts and one Blalock-Taussl8 shunt. Among these 23 patients, 10 patients had Right Atrial to Pulmonary Artery conduit, with a valve in 7 and without in 3. The remaining 13 patients had direct anastomosis between RA to PA. There were 14 early deaths, 7 of 13 TA patients, 6 of 9 UVH patients and one DORV, and the total hospital mortality was 60%. But there were no later deaths, 9 of these 23 patients survived operation and are presently alive. The Fontan procedure can be done with an acceptable low mortality with good functional results for Tricuspid Atresia and other complex lesion in foreign hospital, but till now our results revealed much higher risks. For our good operative results and effective patients selection, we must clarify the exact condition of pulmonary arterial system and accumulate much more experiences and technique.

Comparative Acute Toxicity Studies on Woo Whang Chung Shim Won Suspension and Pill in Mice (우황청심원 환제 및 액제의 급성경구비교독성시험)

  • Acute toxic effects of the Woo Whang Chung Shim Won suspension and pills were studied with the doses of 60, 74, 90, and 110 ml/kg in mice. In this study, we daily examined number of deaths, clinical signs, body weights, and pathological findings for 7 days after administration of Woo Whang Chung Shim Won. All mice given the highest dose (110ml/kg) died at 24 hour after administration of Sam-Sung Woo Whang Chung Shim Won suspension (Byunbang, SS-BS and Wonbang, SS-WS), one male and two female mice given 90 ml/kg dose of SS-BS died, and a few mice given middle dose (74 ml/kg) of SS-WS died. In control group (110 ml/kg) treated with vehicle (SS-BS and SS-WS), three out of 5 males and two out of 5 females mice died during the study. However, in animals treated with Kwang-Dong Woo Whang Chung Shim Won suspension (KD-S) and pill (KD-P), deaths were not observed. In the clinical signs, increase of drooling and decrease of spontaneous motor activities were observed in the highest dose group (110 ml/kg). No significant dose-related change in body weight was observed. The results suggest that the toxic effect of SS-BS and SS-WS may be atttributed to the solution for the Woo Whang Chung Shim Won suspension.
