A case of pulmonic stenosis in a Shihtzu dog

  • A 3-year-old, intact female, Shih-tzu dog was presented with a 15-day history of vomiting,depression, and anorexia. On physical examination, systolic ejection murmurs with precordial thril atthe left heart base were detected. A diagnosis of congenital pulmonic stenosis (PS) was made mainlyfrom the thoracic radiography, electrocardiography, and echocardiography. On complete blood counts andconfirmed that main pulmonary artery was tremendously buldged and electrocardiography was suggestiveof severe right ventricular hypertrophy. Echocardiographic findings revealed the pulmonic valve stenosiscontaining valvular dysplasia and poststenotic dilation. On Doppler echocardiography, ejection velocityof the lesion accounted for 3.38 m/sec, meaning mild velocity through the stenotic area. The dog'sproblem and resulted in death. However, there has been no reliable relation between PS and CRF. Primarymalformation of pulmonic valve was confirmed at necropsy after death.

Sudden death caused by diaphragmatic rupture following rib fracture in a female sika deer (Cervus nippon) at a zoo

  • A one-year-old female sika deer died suddenly with no preliminary signs during exhibition at a zoo. At necropsy, the carcass was emaciated and had dried fur. Examination of the thoracic cavity revealed a diaphragmatic rupture measuring 2 cm in diameter and a fracture in the middle of the right eighth rib. The liver and lungs had irregular circular discolorations caused by diaphragmatic rupture and subsequent herniation. Dark-brown-colored ascitic fluid, hydrothorax, and yellowish hydropericardium were also observed. The cause of death was determined to be diaphragmatic rupture caused by a rib fracture, which led to respiratory imbalance and circulatory disorders.

Early Valve Replacement in Patient with Native Valve Endocarditis - Report of Seven Cases - (활동기 자가판 심내막염의 판막치환술: 7례 경험)

  • From October 1988 to November 1989, seven patients underwent valve replacement during the active phase of native valve endocarditis. There were 4 males and 3 females whose mean age was 41 years[range, 16 to 68 years]. Preoperative two-dimensional and Doppler echocardiography showed vegetations and severe valvular regurgitation in all patients. Blood cultures were positive in 4, and negative in 3 patients Organisms were alpha-hemolytic Streptococcus in 2, Staphylococcus epidermidis in 1, Erysipelothrix rhusiopathiae in 1 patient Valve tissue cultures were negative in all patients. Intravenous antibiotic therapy had been done for 3 to 18 days in 5 patients pre-operatively and was not done in 2 patients, Indications for operation were heart failure in h, and systemic emboli in 1 patient. The aortic valve was involved in 3, mitral in 1, and both aortic and mitral in 3 patients, One operative death[14.4%] occurred in patient with cardiogenic shock before operation. Late death occurred in one on 14 months after operation. The remaining 5 patients were followed up over a two year period in good condition. In conclusion, native valve endocarditis with severe heart failure must be considered for early operation.

  • The purpose of this study is to develop a estimation model of migration with only population data using the cohort-survival model which has been used for forecasting future population. The fluctuation of population can be bisected to the natural change which can be occurred by birth and death and the social change which means migration. The factors of the social change are usually very important for establishing rural policies. However, researches using migration data has limitations because the usage of them are restricted. For verifying a estimation model of migration, comparing estimated population in 2000 year and migration quantity between 1996 and 2000 of 25 gu with real values, using population data and death ratio from 1995 to 2000 of the 25 gu in Seoul. Result shows a reliable data that R-square of forecating population model is 0.9755 and migration is 0.9180. So these model are worth to estimate a population and migration quantity to restricted migration data.

  • Domestic construction industry has seen a negative image in the sense that various accident occurs frequently. Status of construction accidents in the case of domestic workers is reduced. However, if the situation of foreign workers is increasing every year. This study aims to create high-temperature exposure standard table for foreign workers and domestic workers in the analysis through the discomfort index of fatalities and using the WBGT index for reducing fatalities foreign workers to improve the image of the construction industry.

Postmortem Identification of Jejunal Volvulus in a Captive Striped Hyena (Hyaena hyaena) (동물원에서 사육중인 줄무늬하이에나에서 사후 확인된 공장염전 증례)

  • Jejunal volvulus was diagnosed in a captive 12-year-old male striped hyena at necropsy. The animal was presented with decreased appetite, dullness, vomiting, and mild abdominal distention intermittently for about two months. Then, the animal was found anorexia, lethargy, depression, red brown colored vomiting materials, and decreased defecation from 10 days before death. At necropsy, jejunal volvulus was found about 1 m in length, and coalescence was observed at the inner membranes of jejunum. The parenchyma of the kidney was congested, and the bladder was filled with hematuria. These conditions might cause shock by renal failure that led to death. To prevent volvulus in a captive striped hyena, it is needed to improve zoo environment and early diagnosis using ultrasonography and X-ray.

Death due to Aspergillus Tracheobronchitis: An Autopsy Case (아스페르길루스 기관기관지염에 의한 사망: 부검 1예)

  • Aspergillus infection is the most common cause of death due to fungi in immunocompromised hosts. Aspergillus tracheobronchitis is an uncommon but severe form of invasive pulmonary aspergillosis, which is limited entirely or mainly to the tracheobronchial tree and can often be life-threatening. We report a case of a 54-year-old man who died from Aspergillus tracheobronchitis without an underlying disease. Autopsy revealed an extensive yellowish plaque adhering to the trachea and bronchial wall. The microscopic examination of the trachea and bronchus revealed septate branching hyphae of Aspergillus.

  • This report describes the fatal case of a 13-year-old middle school girl(MSG) whose cause of death might be attributed to a intoxication involving Venlafaxine(VEN). MSG's mother had the history of depression for 11 years. She and her daughter were found dead in the same room of their apartment, with hanged herself. We analyzed the postmortem of MSG which were requested to our institute for the cause of her death. We couldn't get any blood samples from MSG's postmortem. (omitted)

Delayed death after chlorfenapyr poisoning (클로르페나피르 중독 후 지연성 사망)

  • Chlorfenapyr is a widely used insecticide, that is very lethal if ingested. It exhibits delayed toxicity in which there are few symptoms at first which suddenly worsen after a few days. A 66-year-old female patient ingested about 90 mL of chlorfenapyr liquid hydrating agent (Chlofenapyr 10%) and showed stable vital signs with no specific symptoms and findings other than a mild fever, vomiting, and nausea. From the 3rd day of ingestion, creatine kinase was high, and rhabdomyolysis was suspected. From the 4th day of ingestion, pancreatic enzymes began to gradually increase. A diffusion-weighted image showed a multifocal high signal intensity in the white matter and corpus callosum area. On the 8th day after ingestion, she suffered a high fever and a heart attack and died. Thus, if a patient is suspected of taking chlorfenapyr, he/she needs active treatment and monitoring even if he/she does not exhibit any symptoms.

The Long-term Clinical Result of St. Jude Mechanical Valve Replacement (St. Jude 기계판막 치환술의 장기 임상성적)

  • Background: The St. Jude Medical prosthesis is one of the popularly used artificial prosthesis, therefore the National Medical Center reports the long-term clinical results of patients who underwent prosthetic valve replacement with St. Jude medical valve for 18 years. Material and Method: Between January, 1984 and June, 2002, a series of 163 consecutive patients who had Implanted St. Jude prosthesis at the National Medical Center were reviewed. Mean age was 42.9$\pm$15.1 years and male to female ratio was 69:94. The operative procedure comprised of 87 MVR, 30 AVR, 45 DVR, and 1 TVR. The reoperative procedure comprised of 21 MVR, 2 AVR, and 14 DVR. Follow-up rate was 96.9%, and cumulative follow-up was 823.8 patient-years. Result: Early mortality rate was 7.9% (13 patients), late mortality rate was 8.7% (13 patients) and late mortality due to valve related complication was 47% (7 patients). Actual survival rate at 10 and 18 years were 91.7$\pm$2.1% and 91.0$\pm$1.9%. Linearized Incidence was as follows: thromboembolism, 1.09%/ patient-year; anticoagulant related hemorrhage, 0.36%/patient-year; valve thrombosis, 0.24%/patient-year; paravalvular leakage, 0.12%/patient-year; and prosthetic bacterial endocarditis, 0.12%/patient-year linearized incidence of over all valve related complication was 1.94%/patient-year. Freedom from valve related complication at 10 and 18 years were 89.1$\pm$3.3% and 88.4$\pm$3.9%. Freedom from valve related death at 10 and 18 years were 95.1$\pm$1.2% and 95.1$\pm$1.0%. Valve related complication was related the age of patient, especially anticoagulant related hemorrhage was more common in patients over 60 years of age. Valve related complication, death were higher in DVR than AVR or MVR, and valve related death was higher in reoperation. There was no relationship between valve related complication or death and implant valve of size. Conclusion: The long-term clinical results of patients implanted with St. Jude Mechanical prosthesis was quite satisfactory with a low incidence of valve related complication and mortality.
