Congenital Hemagiopericytoma in the Flexor Digitorium Profundus Muscle of the Distal Forearm - A Case Report - (전완 원위부 심 수지 굴근에 발생한 선천성 혈관 외피세포종 - 증례 보고 -)

  • Hemangiopericytoma is a rare malignant vascular tumor that usually occurs in adults. The occurrence of these tumors in infants, known as congenital or infantile hemangiopericytoma, is even rare and their behavior may be more benign than the adult type. Despite of the generally good prognosis associated with this neoplasm, a complete surgical excision has so far been recommended to avoid recurrence, because no definite criteria for determining whether or not the tumor will regress spontaneously have been established to date. We describe a 1-day-old male neonate with congenital hemangiopericytoma, presenting with a left forearm mass at birth. Wide resection was performed at 65 days of age and hemangiopericytoma was diagnosed by histology. There was no tumor recurrence during 32 months of follow-up.

Analysis of Physical Activity Measured by International Physical Activity Questionnaire and Actigraph Accelerometer, and Participation Intention for Physical Activity of Breast Cancer Survivors (국제신체활동설문지(IPAQ)와 액티그래프 가속도계를 이용한 유방암 생존자들의 신체활동량과 신체활동 프로그램 참여 의도)

  • Purpose: This study aimed to analyze physical activity as measured by the International Physical Activity Questionnaire (IPAQ) and an actigraph in breast cancer survivors, as well as to identify their intention to participate in a physical activity program. Methods: Breast cancer patients who had been diagnosed for more than six months (N=135) at a university hospital participated from June 2012 to May 2013. Physical activity was measured using the Korean version of the IPAQ-Short Form and Actigraph GT3X plus an accelerator for seven consecutive days. Data analyses were conducted using the SPSS WIN 19.0 program. Results: Mean total physical activity was 2298.21 metabolic equivalent task (MET)-min/week as assessed by IPAQ and 150,140.57 counts/day as measured by an actigraph. There were statistically significant correlations between moderate physical activity from IPAQ and light intensity of physical activity from the actigraph (r=.735, p<.001), vigorous physical activity from IPAQ and vigorous intensity of physical activity from the actigraph (r=.871, p<.001), total physical activity from IPAQ and light intensity of physical activity from the actigraph (r=.825, p<.001), respectively. Most (80.7%) cancer survivors reported a positive attitude toward physical activity and 57.8% expressed a willingness to participate in a physical activity program. More than half (60%) of the subjects preferred walking, 80.6% preferred more than 30 minutes of exercise, and 57.1% wanted to engage in physical activity three times a week and preferred home-based activities. Perceived barriers included fatigue, lack of strength and pain. Conclusion: It is necessary to consider intensity, personal preferences, and patient-perceived barriers when developing physical activity programs for breast cancer survivors.

Comparative Study for the Anesthetic Efficacy between Articaine HCl and Lidocaine HCl during the Surgical Extraction of Bilateral Mandibular Impacted Third Molars (하악 매복 제3대구치 발치 시 Articaine HCl과 Lidocaine HCl의 국소마취 효능에 대한 비교연구)

  • Background: Articaine, a new amide-type local anesthetic, was recently commercially available in Korea. The purpose of this study was to compare the anesthetic efficacy between articaine HCl and lidocaine HCl for the surgical extraction of bilateral mandibular impacted third molars. Patients and Methods: Forty young and healthy patients with bilateral impacted third molars were selected with permission. Randomly, one side of impacted third molar was operated under local anesthesia using 4% articaine and the other side under 2% lidocaine after 1 or 2 week recovery time. Intraoperative pain was evaluated via 0-10 cm visual analog scale (VAS) by both the patient and operator immediately after the operation. After 1 day and 7 days, the complications of local anesthesia were checked. Results: The pain VAS scores in articaine group, evaluated by both the patient and operator, were lower than those in lidocaine group, but they were not statistically significant (P = 0.44 and 0.54, respectively). The incidence of complications of local anesthesia between articaine and lidocaine was similar. Conclusion: In this study, articaine showed at least equal efficacy compared to lidocaine. The further dose-controlled study with more pharmacokinetic parameters will be needed.

Immediate implant placement and provisionalization of single implant in the esthetic region : Cases report (심미적 영역에서 즉시 임플란트 식립 및 임시 수복을 통한 단일치의 수복에 대한 증례 보고)

  • In the anterior maxillary area, dental implants for tooth replacement are challenging due to the need to satisfy high esthetic level as well as functionality. Immediate implant placement and provisionalization can dramatically reduce the edentulous period, and then fulfill patient's demand for esthetics. The aim of present case report is to demonstrate two cases that successfully restored single tooth with immediate implant placement and provisionalization in the anterior maxillary area. A 47 years old female was scheduled to replace her maxillary right central incisor due to crown-root fracture by trauma. Another 54-year-old female was planned to place dental implant following tooth extraction of maxillary right lateral incisor owing to continuous pus discharge despite repetitive treatments including apicoectomy. In these two cases, surgical and prosthetic procedures progressed in a similar way. After minimal flap elevation, atraumatic tooth extraction was performed. Implant was placed in proper 3-dimensional position and angulation with primary stability. Bone graft or guided bone regeneration for peri-implant bone defect was conducted simultaneously. Provisionalization without occlusal loading was carried out at the same day. Each definitive crown was delivered at 7 and 5 months after the surgery. Two cases have been followed uneventfully for 2 to 5 years of loading time. In conclusion, Immediate implant placement and provisionalization could lead to esthetic outcome for single tooth replacement with dental implant under proper case selection.

Hepatocyte Growth Factor is the Key Cytokine in Stimulating Potential Stem Cells in the Cord Blood into Hepatic Lineage Cells

  • Background: This study was designed to investigate the role of the hepatocyte growth factor (HGF) with regards to differentiation of somatic stem cells originating from the human umbilical cord blood (UCB) into hepatic lineage cells in vitro culture system. Methods: Mononuclear cells from UCB were cultured with and without HGF based on the fibroblast growth factor (FGF)-1, FGF-2, and stem cell factor. The cultured cells were confirmed by immunofluorescent staining analysis with albumin (ALB), cytokeratin-19 (CK-19), and proliferating cell nuclear antigen (PCNA) MoAb. ALB and CK-18 mRNA were also evaluated by reverse transcription-polymerase chain reaction. In order to observe changes in proliferating capacity with respect to the cultured period, CFSE with affinity to proliferating cells were tagged and later underwent flow cytometry. Results: In the HGF-treated group, cultured cells had a large oval shaped appearance with adherent, but easily detachable characteristics. In the HGF-non treated group, these cells were spindle-shaped with strong adherent characteristics. Expressions of ALB and CK-19 were evident in HGF-treated group compared to non-expression of those in to HGF-non treated group. Dual immunostaining analysis of the ALB producing cells showed presence of PCNA in their nuclei, and ALB and CK-18 mRNA were detected on the 21st day of cultured cells in the HGF-treated group. Conclusion: Our findings suggest that HGF has a pivotal role in differentiating somatic stem cells of human UCB into hepatic lineage cells in vitro.

A Case of Esophageal Perforation due to Foreign Body (식도이물에 의한 식도천공 1예)

  • Esophageal perforation occurred rarely, but it leads to a high mortality and morbidity. Main cause of esophageal perforation in Korea is instrumental perforation in patients with lye stricture of esophagus during diagnostic endoscopy or therapeutic bouginage. A case of 25-year-old male patient entered to outpatient clinic. Chief complaints were mild pain at swallowing, mild fever elevation and chest tightness with radiating pain to shoulder and interscapular region for 1 day. On the X-ray film with barium soaked cotton piece in esophagus, it was revealed that foreign body (fish bone) had been located on the level of 2nd vertebra with leakage of barium to mediastinum. The foreign body was removed with endoscopy in Dept. of otolaryngology, and cervical mediastinostomy was performed in Dept. of thoracic surgery.

BONE REGENERATION OF THE EXPERIMENTAL ENDODONTIC-PERIODONTIC COMBINED DEFECTS IN THE MANDIBLES OF THE DOGS (성견 하악의 치주-치근단 실험적 복합병소에서 골조직 재생에 관한 연구)

  • The endodontic-periodontic combined lesions have been difficult to get correct diagnosis and predictable treatment. This study was to make the experimental endodontic-periodontic combined defects in dogs for the study of the periodontal regeneration and to evaluate the efficacy of the enamel matrix protein and e-PTFE membrane in the experimental endodontic-periodontic combined defects. 5 mongrel dogs were used. The pulp chambers were opened and the plaque was inserted into the chambers to induce the periapical lesions on the mandibular second, third and fourth premolars of the dogs. 1 month later, the root canal treatments were done with gutta perch a and ZOE sealer. On the day of surgery, the periapical defects were standardized by trephine bur. The buccal dehiscence defects were made by the dental bur and bone chisels. The apicoectomy with retrofilling was done. The prepared roots were randomly selected for test and control groups. In the experimental groups, the enamel matrix derivative and e-PTFE membrane were used. Nothing was placed on the control group. Fluroscent labelling was used to evaluate the bone formation. After 4 and 12 weeks, the dogs were sacrificed and undecalcified sections were prepared and stained with toluidine blue. Those histologic sections were examined by fluorescent microscopy and light microscopy. The results were as follows. 1. In the control group, new bone was formed in the periapical defects and scarcely in the buccal dehiscence defects. New cementum was not detected at 4 and 12 weeks. 2. In the experimental groups, new bone, new cementum and periodontal ligament were found in the periapical and buccal dehiscence defects. The relative amount and the quality of the new bone, new cementum and periodontal ligament tissue that had formed on the experimental groups were superior to those of the control group. 3. The current observation implicated that e-PTFE membrane and enamel matrix protein could be the effective tools for the guided tissue regeneration of the endo-perio combined defects.

Exploring the role and mechanisms of diallyl trisulfide and diallyl disulfide in chronic constriction-induced neuropathic pain in rats

  • Background: Garlic oil is a rich source of organosulfur compounds including diallyl disulfide and diallyl trisulfide. There have been studies showing the neuroprotective actions of these organosulfur compounds. However, the potential of these organosulfur compounds in neuropathic pain has not been explored. The present study was aimed at investigating the pain attenuating potential of diallyl disulfide and diallyl trisulfide in chronic constriction injury (CCI)-induced neuropathic pain in rats. The study also explored their pain-attenuating mechanisms through modulation of H2S, brain-derived neurotrophin factor (BDNF) and nuclear factor erythroid 2-related factor 2 (Nrf2). Methods: The rats were subjected to CCI injury by ligating the sciatic nerve in four places. The development of neuropathic pain was measured by assessing mechanical hyperalgesia (Randall-Selittotest), mechanical allodynia (Von Frey test), and cold allodynia (acetone drop test) on 14th day after surgery. Results: Administration of diallyl disulfide (25 and 50 mg/kg) and diallyl trisulfide (20 and 40 mg/kg) for 14 days led to a significant reduction in pain in CCI-subjected rats. Moreover, treatment with these organosulfur compounds led to the restoration of H2S, BDNF and Nrf2 levels in the sciatic nerve and dorsal root ganglia. Co-administration of ANA-12 (BDNF blocker) abolished pain attenuating actions as well as BDNF and the Nrf2 restorative actions of diallyl disulfide and diallyl trisulfide, without modulating H2S levels. Conclusions: Diallyl disulfide and diallyl trisulfide have the potential to attenuate neuropathic pain in CCI-subjected rats possibly through activation of H2S-BDNF-Nrf2 signaling pathway.

Efficacy Test of Polycan, a Beta-Glucan Originated from Aureobasidium pullulans SM-2001, on Anterior Cruciate Ligament Transection and Partial Medial Meniscectomy-Induced-Osteoarthritis Rats

  • The object of this study was to assess the efficacy of Polycan from Aureobasidium pullulans SM-2001, which is composed mostly of beta-1,3-1,6-glucan, on osteoarthritis (OA)-induced by anterior cruciate ligament transection and partial medial meniscectomy (ACLT&PMM). Three different dosages of Polycan (85, 42.5, and 21.25 mg/kg) were orally administered once a day for 84 days to male rats a week after ACLT&PMM surgery. Changes in the circumference and maximum extension angle of each knee, and in cartilage histopathology were assessed using Mankin scores 12 weeks after Polycan administration. In addition, cartilage proliferation was evaluated using bromodeoxyuridine (BrdU). As the result of ACLT&PMM, classic OA was induced with increases in maximum extension angles, edematous knees changes, and capsule thickness, as well as decreases in chondrocyte proliferation, cartilages degenerative changes, and loss of articular cartilage. However, these changes (except for capsule thickness) were markedly inhibited in all Polycan- and diclofenac sodium-treated groups compared with OA control. Although diclofenac sodium did not influence BrdU uptake, BrdU-immunoreactive cells were increased with all dosages of Polycan, which means that Polycan treatment induced proliferation of chondrocytes in the surface articular cartilage of the tibia and femur. The results obtained in this study suggest that 84 days of continuous oral treatment of three different dosages of Polycan led to lesser degrees of articular stiffness and histological cartilage damage compared with OA controls 91 days after OA inducement, suggesting that the optimal Polycan dosage to treat OA is 42.5 mg/kg based on the present study.

Duodenal Duplication Cyst in a Korean Native Cattle (한우 송아지에서 발생한 십이지장의 중복낭종)

  • A congenital duplication cyst, which presents with symptoms of gastric outlet obstruction, is an unusual disorder in humans and a rare condition in animals. A 5-day-old male Korean native cattle was presented with anorexia and no defecation. On exploratory laparotomy, an oval shaped mass ($16{\times}7$ cm) was identified on the antimesenteric border of the proximal duodenum, with no communication between the intestinal lumen and the mass itself. Surgical correction for gastrointestinal tract patency was performed. The calf recovered well and was clinically normal three months after surgery. This case is the first report of a duodenal duplication cyst in a Korean native cattle.
