소아 루프스 신염의 임상양상 및 치료결과 (Clinical Manifestation and Treatment Outcome of Lupus Nephritis in Children)

  • 목적 : 전신성 홍반성 낭창(Systemic lupus erythematosus, SLE)은 여러장기를 침범하는 자가면역성 질환으로 신장의 손상이 본 질환의 예후를 좌우하는 주요원인이다. 특히 소아에서 루프스 신염은 성인보다 그 빈도가 높고, 증상이 심하므로, 신생검은 효과적인 치료의 계획을 위해서 중요한 수기이다. 이에 저자는 소아연령에서 루프스 신염의 임상적 병리학적 특성 및 치료방법에 대해 전반적으로 고찰하여 예후의 향상을 기대하고자 한다. 방법 : 1990년 1월부터 2002년 9월까지 소아과에서 전신성 홍반성 낭창으로 진단받은 63례의 환아중 신생검을 시행하여 루프스 신염으로 진단되었던 40례를 대상으로 의무 기록을 후향적으로 고찰하였다. 결과 : 환아의 남녀비는 1:3이었고 진단당시 평균발병 연령은 12.1(2-18)세였다. ARA 기준중에서는 형광 항핵항체(95.0%), 항dsDNA항체(87.5%), 나비모양 홍반(80.0%)의 순이었다. 가장 흔한 신장증상은 단백뇨와 현미경적 혈뇨(75.0%), 신증후군(55.0%), 현미경적 혈뇨 단독(15.0%)의 순이었고, 신생검상 27례(67.5%)에서 WHO Class IV 병변이 관찰되었고 3례에서 추적 관찰 신생검에서 조직소견이 바뀌었다. 치료는 prednisolone 단독 5례, prednisolone+azathioprine 9례, prednisolone+azathioprine+정맥cyclophosphamide 14례, prednisolone+cyclosporineA+정맥 cyclophosphamide 12례였고, 9례에서 혈장 교환술을 시행하였다. 환아들의 평균 추적관찰은 $51.8{\pm}40.5$개월이었고 사망은 4례에서 있었다. 사망과 관련된 위험인자로는 진단당시 성별이 남아일 때, WHO class IV의 조직소견, 급성 신부전을 동반할 때 의미있는 것으로 나타났다. 결론 : SLE 환아 중 루프스 신염의 빈도는 63.5%였으며 그중 67.5%가 예후가 불량한 WHO Class IV로 확인되었다. 따라서 신염의 초기에 적극적인 면역억제제 사용이 장기 예후 향상에 도움을 주리라고 생각된다. 하지만 소아기에 성장, 정신 사회적 발달, 생식기의 독성 등도 중요한 문제이므로 항상 적절한 치료를 위해 세심한 관심을 쏟아야 할 것이다.

급성 간질성 폐렴에서 조기 부신피질호르몬 치료의 효과 (Efficacy of Early Steroid Therapy in Acute Interstitial Pneumonia)

  • 연구배경 : 급성 호흡부전증후군(ARDS) 환자 중에서 폐생검에서 섬유증식의 조직소견을 보이고 감염의 증거가 없을 경우 부신피질호르몬 치료가 임상경과를 호전시키는 것으로 알려져 있다. 급성 간질성폐렴(AIP)의 폐조직소견이 ARDS의 미만성 폐포손상과 일치하므로 감염의 증거가 없는 AIP 환자들에서 조기 부신피질호르몬치료의 효과를 알아보았다. 방 법 : 1995년 5월부터 2002년 5월까지 단국대학교 병원에 입원한 AIP 환자 중에서 인공호흡치료 7일 이내의 조기에 부신피질호르몬 치료를 발은 환자들을 대상으로 부신피질호르몬 치료의 효과를 진료기록을 중심으로 후향적으로 조사하였다. AIP의 진단은 ARDS의 원인으로 얄려진 원인인자 없이 발병한 ARDS 환자중에서 조직소견상 섬유종식기의 미만성 폐포손상의 소견을 보였을 때 진단하였다. 대상환자들의 병리소견, 임상소견, 그리고 부신피질호르몬 치료에 대한 폐생리지수 등을 분석하였다. 결 과 : 5명(남자 3명, 여자 2명)의 AIP 환자들이 조기 부신피질호르몬 치료를 받았다. 방법으로는 methylprednisolone, 1-2 mg/kg을 매 6시간마다 경정맥으로 투여받았다. 인공호흡 시작시기부터 부신피질호르몬 부신피질호르몬 치료시작 시기까지의 기간은 $0.6{\pm}1.7$ 일이었으며, 인공호흡 시작시기부터 폐생검까지의 기간은 $1.8{\pm}1.4$일이었다. 모든 환자에서 폐조직소견상 섬유증식의 소견을 보였으며, 폐생검으로 인한 합병증을 일으킨 환자는 없었다. 4명(80%)의 환자에서 부신피질호르몬 치료로 뚜렷한 임상소견의 호전으로 기계호흡으로부터 이탈이 가능하였고, 부신피질 호르몬 치료 시부터 이탈까지의 기간은 $2.8{\pm}0.4$일이었으며, 부신피질호르몬 치료 후 $PaO_2/FiO_2$ 비는 치료 첫날의 $127.4{\pm}10.0$에서 치료 7일 째 $223.8{\pm}37.6$으로 호전되었다. 1명의 환자에서 (20%)에서 부신피질호르몬 치료에도 불구하고 사망하였다. 부신피질호르몬 치료로 인한 합병증을 보인 환자는 없었다. 결 론 : 본 연구의 결과 감염증의 증거가 없는 AIP 환자에서 조기 부신피질호르몬 치료가 효과가 있을 것으로 사료 된다. 그러나 대상 환자 수가 너무 적으므로 앞으로 대규모의 전향적 연구가 필요할 것으로 사료된다.

진안 평지리 이팝나무군(천연기념물 제214호)의 생육진단 및 관리방안 (A Study on the Growth Diagnosis and Management Prescription for Population of Retusa Fringe Trees in Pyeongji-ri, Jinan(Natural Monument No. 214))

  • 천연기념물 제214호 진안 평지리 이팝나무군의 고사(枯死) 및 쇠약(衰弱) 요인의 명확한 진단(診斷)과 처방(處方)을 통한 천연기념물로서의 가치 존속(存續) 방안을 강구하고자 시도된 본 연구의 결과는 다음과 같다. 첫째, 1968년 13그루의 천연기념물 지정 이후, 1973년을 시작으로 근년까지 계속적으로 고사가 진행되어 현재는 3주만 생존하고 있다. 특히 2010년 이후 마령초등학교 담장 후퇴 등 정비과정에서 복토된 토양의 일부 제거에도 불구하고, 계속적으로 고사가 이루어진 것으로 확인된다. 둘째, 지정목 중 1 3번 생존목 또한 고사된 가지가 많고, 잎이 왜소하며, 엽량이 적었다. 1번목은 수세가 '극히 불량'하고 다량의 가지가 이미 고사하였으며, 금년에는 단지 2개 가지에서만 개화가 이루어졌으며 엽량 또한 현저히 적은 상태이다. 2번목 또한 '불량'상태로 잎이 작고 엽밀도도 낮으며 수형의 변형이 이루어지는 등 수세가 쇠약한 상태이다. 현존하는 가장 큰 1번목은 논토양으로 추정되는 점질토가 복토되어 있어 우려를 더하고 있다. 셋째, 평지리 이팝나무군의 토성 분석결과, '미사질양토[微砂質壤土, Silt loam(SiL)]'로 밝혀짐에 따라 미사(Silt)의 성분비가 높음을 알 수 있다. 또한 1번목 북측의 토양산도는 pH 6.6으로 다른 부위의 흙과는 편차가 컸는데 이는 원지반 토양이 아닌 외부에서 반입된 토양으로 복토되었음에 기인한 것으로 판단된다. 더불어 유기물함량은 적정범위보다 높게 나타났는데 이는 보호관리를 위한 시비가 계속적으로 이루어진 결과가 반영된 것이라고 판단된다. 넷째, 진안 평지리 이팝나무군의 고사 및 생육 불량의 근본적 원인으로는 복토(覆土)로 인한 심각한 생리저하의 만성적 증후군으로 판단된다. 이는 신규로 식재된 후계목 또한 일부 고사된 것에서도 그 원인을 찾을 수 있다. 다섯째, 1차적으로 기존 고사의 원인으로 추정되는 복토 부분에 대한 점진적 제거가 시급하다. 무엇보다도 근계부에 복토된 점질토양의 제거를 건의한다. 복토부를 제거한 후 굵은 모래로 치환하고 뿌리의 호흡 개선을 위해 유공관을 설치한다. 그리고 고사한 4번 고사목과 5 6번의 고사흔적인 밑둥은 제근하고, 하층식생은 예초한다. 생존목은 상부 고사지를 제거, 수피 이탈부는 외과수술을 시행하는 한편 생장점 아래에서 전정하여 맹아의 발생을 유도해야 할 것이다. 여섯째, 지하부 뿌리를 확인하여 부패근 절단 및 토양호흡 개선방안을 마련한다. 근계 전반에 대한 추적 굴취 등으로 썩은 뿌리부를 단근하여 새 뿌리의 발생을 유도한다. 일곱째, 이후 잡초 발생과 답압 억제 그리고 토양 보습효과 유지를 위한 멀칭을 시도한다. 또한 지속적 영양공급을 위한 무기양료 엽면시비 및 영양제 나무주사, 무기양료의 토양관주를 고려한다. 향후 모니터링과 예찰방안을 마련하여 계속적으로 변화상을 체크해야 할 것이다.

기능적 단심실 환자에 대한 심장내 외측통로 폰탄술식의 중기 수술성적 (Mid-term results of IntracardiacLateral Tunnel Fontan Procedure in the Treatment of Patients with a Functional Single Ventricle)

  • 서울대학교 어린이병원 흉부외과에서는 1990년부터 1996년까지 104례의 기능적 단심실을 가진 선천성심기형 환아에 대하여 심장내 외측통로술을 이용한 완전폰탄술을 시행하였다. 환아의 연령 및 체중분포는 각각 평균 35.9(범위 10-72) 개월, 12.8 (범위 6.5-37.8) kg이었다. 술전진단은 삼첨판폐쇄증(18), 단심실연결을보이는 중복개구심실(53) 및 기타 기능적 단심실을 동반한 복잡심기형(33)이었다, 50례의 환아에 대하여 체폐동맥 단락술 (37), 폐동맥밴딩(13), 외과적 심방중격절제술(15), 동맥전환술(2), 대동맥하 누두부제거술(2), 총폐정맥이상연결증(2), 폐동맥-대동맥봉합술(Damus-Stansel-Kaye, 1) 등의 고식술이 시행되었다. 완전폰탄술식전 19례의 양방향성 체정맥-폐동맥단락술과 1례의 전(全)체정맥-폐동맥단락술(Kawashima procedure)이 진행되었다. 술전 혈역학소견상, 평균 폐동맥압/폐혈관저항은 14.6 (범위 5-28mmHg) / 2.2 (범위0.4-6.9)wood.unit였으며, 폐혈류/체혈류비가 평균 0.9 (범위0.3-3.0)였다. 이완기말심실압은 평균 9.0 (범위 3.0-21.0) mmHg였고 동맥혈의 산소포화도는 평균 76.0 (범위 45.6-88.0) %였다. 수술은 분계능(terminal crest) 2cm 외측으로 우심이부터 우심방-하공정맥경계부에 종절개를 가하고 하공정맥개구부부터 상공정맥개구부 또는 우심이까지 Gore-Tex 인조도관을 이용하여 외측통동을 형성시키는 방법으로 시행하였으며 필요한 경우 통로상에 4-5.5 mm 직경의 구멍을 만들어 주었다. 동시에 시행한 술식은, 폐동맥성형술(22), 심방중격절제술(21), 폐정맥이상연결증 교정(4), 영구적인 인공심박동기거치(3) 등이었고, 32례에 대하여 통로내 구멍을 만들어 주었으며 그중 1례는 조절형(adjustable)으로 시행하였다. 심방-폐동맥 연결 방법으로 시행한 폰탄술 후 4년후에 발생한 재발성 난치성 상심실형 부정맥환아 1례에 대하여 외측통로형의 변환 폰탄술식이 시행되었다. 수술 직후 혈역학 소견상 평균 폐동맥압, 이완기말심실압, 실온에서 동맥혈의 산소포화도가 각각 12.7 (8-21)mmHg, 7.6 (범위4-12)mmHg, 89.9 (범위68-100) %였다. 병원사망율은 6.7 (7/104) %였고 술후 합병증으로 지속적인 늑막삼출(11), 부정맥(8), 유미흉(9), 중추신경계손상(5), 감염 및 염증(5), 급성신부전(4)이 발생하였다. 평균 27.2 (범위1-85) 개월동안의 외래 추적결과 5명의 만기 사망이 있었다. 저자등은 본연구결과를 토대로 심장내 외측통동폰탄술식이 기능적 단심실 환자에 대하여 비교적 낮은 사망율 및 합병증과 우수한 혈역학으로 시행될수 있는 수술방법이란 사실을 입증하였다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The correlation between glucose concentration and parameters was the best at 610nm. 30535 T00401030535 ^x Radiation-induced fibrosarcoma tumors were grown on the flanks of C3H mice. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. To synthesize microspheres for biomedical application, "two stage shot growth method" was developed. The uniformity ratio of synthesized microspheres was always smaller than 1.05. And the surface charge density (or the number of ionizable functional groups) of the microspheres synthesized by "two stage shot growth method" was 6~13 times higher than that of the microspheres synthesized by conventional seeded batch copolymerization. As a previous step for biomedical application, adsorption experiments of bovine albumin on microspheres were carried out under various conditions. The maximum adsorbed amount was obtained in the neighborhood of pH 4.5. Isoelectric point of bovine albumin is pH 5.0, so experimental result shows that it shifted to acid area. In this BEAM, instantaneous change and average energy distribution over any arbitrary time interval of brain electrical activity could be observed and analyzed easily. Our department has developed an information system for routine hematology using down-sized computer system. We have used an IBM 486 compatible PC with 16MB main memory, 210 MB hard disk drive, 9 RS-232C port and 24 pin dot printer. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer. The authors suggests that this system based on down-sized computer could provide a progressive approach to total LIS based on local area network, and the implemented system could serve as a model for other hospital's LIS for routine hematology. 30609 T00401030609 ^x To develop an artificial bone substitute that is gradually degraded and replaced by the regenerated natural bone, the authors designed a composite that is consisted of calcium phosphate and collagen. To use as the structural matrix of the composite, collagen was purified from human umbilical cord. The obtained collagen was treated by pepsin to remove telopeptides, and finally, the immune-free atelocollagen was produced: The cross linked atelocollagen was highly resistant to the collagenase induced collagenolysis. We propose a simple adaptive algorithm to increase capability of noise cancelation in QRS complex detection with two stage adaptive filter. At the first stage, background noise is removed and at the next stage, only spectrum of QRS complex components is passed. Two adaptive filters can afford to keep track of the changes of both noise and QRS complex. Each adaptive filter consists of prediction error filter and FIR filter The impulse response of FIR filter uses coefficients of prediction error filter.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer.
