A forty-eight-year-old female patient with ascending aortic aneurysm with aortic insufficiency underwent a modified Bentall operation. The ascending aorta and the aortic valve were replaced with a composite graft containing a St. Jude valve. The coronary orifices were anastomosed to the tubular Dacron prosthesis by means of a second smaller Gore-Tex tube, and a fistula between the aneurysmal sac and the right atrial appendage was created to drain oozing from the prosthesis. The postoperative course was uneventful and the patient was discharged without complication. She is doing well on the 14 months follow-up.

We report a case of a 45-year-old woman with Budd-Chiari syndrome caused by the obstruction of the inferior vena cava just below the diaphragm. Transatrial dilatation or membranotomy was not possible due to the severe fibrotic obliteration of the inferior vena cava. Instead, cavoatrial bypass with a Dacron graft[20 mm-Vascutek] was performed under the median sternotomy and median abdominal incision.The postoperative course was uneventful and generalized symptoms were much improved. During the following period[6 month] the graft patency was maintained with no recurrence of symptoms.

The incidence of syphilitic aortic aneurysm was decreased now a day. We experienced a case of huge syphilitic ascending aortic aneurysm from just above portion of aortic annulus to about 1 cm below innominate artery without aortic insufficiency. Surgical correction was done by replacement of ascending aorta with woven Dacron graft under cardiopulmonary bypass. Postoperative course was uneventful and discharged 15 days after surgery with good condition.

We have recently operated 6 year-old male patient who had double outlet right ventricle with dextrocardia, situs inversus, pulmonary stenosis and 2 ventricular septal defects. Rastelli operation was performed by internal baffling with Cooley woven Dacron and tunnel grafting with Hancock valved conduit between the right ventricle and the main pulmonary artery.His postoperative course was uneventful. He was discharged in good conditions on P.O.D. #33.

We present a case of ruptured abdominal aortic aneurysm caused by blunt injury. The patient was 23-year-old soldier injured by a motor vehicle accident. Injuries sustained a contused abdominal aorta. At the time of aortic repair, the involved segment formed huge pseudoaneurysm, and which had intimal tear. Aorto-iliac graft replacement was carried out with a woven-dacron Y-graft prosthesis, which restored satisfactory circulation to both lower limbs. The postoperative course was uneventful.

Repair of incomplete endocardial cushion defect was accomplished in two cases in September 1973 in this department. In each case a low atrial septal defect and a cleft of mitral septal cusp were found. Interrupted suture repair on the cleft mitral cusp and closure of the atrial septal defect with dacron patch were done successfully. Rigg-Kyvsgaard bubble oxygynator and Sigma motor were utilized for extracorporeal circulation. Follow up study, six months after discharge, revealed satisfactory results.

Between Jan. and Oct. 1978, mitral valve Replacement had been performed on eight patients using porcine aortic valve mounted on flexible polypropylene or Egyloil wire and Dacron covered stent. Mitral valve replacement was performed on five, mitral valve replacement with tricuspid valve replacement on two, and mitral valve replacement with tricuspid valve annuloplasty on 1 patient using five Carpentier and five Hancock Xenograft. Overall operative deaths were 2 cases. All other 6 cases did well after surgery and improved. No systemic thromboembolism occurred without anticoagulant-No valve failure took place. The results are satisfactory.

We experienced one case of total anomalous pulmonary venous connection to coronary sinus with secondum type ASD. The case was 21 years old woman with mild dyspnea on exertion and ill looking appearance. The disease was confirmed by 2D echocardiogram and cardiac catheterization. Under the conventional cardiopulmonary bypass, we repaired the anomaly by use of Dacron patch. The postoperative course was uneventful and discharged with excellent general condition and has been good during follow-up.

Since diaphragmatic injuries are difficult to diagnose, those that missed may present with latent symptoms.Delayed diaphragmatic hernia is very rare and occurs commonly after penetrating or blunt trauma. Recently we had experience with two cases of delayed diaphragmatic hernia after left nephrectomy.They were operated by herniolysis and closure of diaphragm using prosthetic patch[Dura, Dacron through the left thoracotomy. Postoperative courses were uneventful.

A patient with benign superior vena cava syndrome caused by the thrombus and fibrotic membrane in superior vena cava is described. Surgical treatment of superior vena cava syndrome remains controversial still. After endvenectomy and thrombectomy of superior vena cava, angioplasty with use of Gore-Tex patch and bypass graft using 10mm diameter Dacron vessel graft from left innominate vein to right atrial appendage were performed. The early postoperative course was uneventful with achievement of good decompression. But 12 months later, the symptoms of superior vena cava syndrome were reoccurred.
