The Occurrence of Renal Scarring in Children with Unilateral Vesicoureteral Reflux (일측성 방광 요관 역류 환아에서 신반흔의 발생)

  • Purpose : We evaluated the occurrence of renal scarring in children with unilateral vesicoureteral reflux(VUR), and the relationships between renal scar formation and risk factors such as VUR, duration of fever, acute-phase reactant, age, and sex. Methods : We retrospectively analyzed the data of 35 children newly diagnosed with unilateral vesicoureteral reflux after urinary tract infection(UTI) in Wallace Memorial Baptist Hospital between January 1996 and December 2004. Ultrasonography, Erythrocyte sedimentation rate(ESR), and C-reactive protein(CRP) were performed initially. Voiding cystourethrography(VCUG) was performed 1 to 3 weeks after treatment with UTI. $^{99m}Tc$-dimercaptosuccinic acid(DMSA) scan was performed 4 to 6 months after treatment. Results : Scintigraphic renal damage was present in 29 percent of the refluxing and in 3 percent of the nonrefluxing kidneys(P<0.05). The severity of VUR was significantly correlated with renal scar formation(P<0.05). The duration of fever before treatmen($5.0{\pm}1.3$ vs $2.6{\pm}1.3$) and prolonged fever of over 5 days were significantly different between renal scar group and non-renal scar group(P<0.05). ESR($56.3{\pm}23.8$ vs $27.9{\pm}18.1mm/hr$, P<0.05) and CRP($12.8{\pm}7.3$ vs $3.9{\pm}3.8mg/dL$, P<0.05) at the diagnosis of UTI in the renal scar group were higher, compared to those of the non-renal scar group. There were no significant differences in age and sex between the two groups. Conclusion : The presence and grade of VUR, the duration of fever before treatment, prolonged fever over 5 days, ESR, and CRP were risk factors for renal scarring, irrespective of age and sex. Diagnosis and management of VUR, in children with UTI, is important to prevent renal scars.

Usefulness of serum procalcitonin test for the diagnosis of upper urinary tract infection in children (소아 상부 요로감염의 진단을 위한 혈청 procalcitonin 검사의 유용성)

  • Purpose : It is difficult to make a distinction between lower urinary tract infection(UTI) and acute pyelonephritis(APN) during the acute phase of febrile UTI due to nonspecific clinical symptoms and laboratory findings, especially among young children. We measured the serum procalcitonin(PCT) in children with UTI to distinguish between acute pyelonephritis and lower UTI, and to determine the accuracy of PCT measurement compared with other inflammatory markers. Methods : Serum samples were taken from children who admitted with unexplained fever or were suspected of having UTI. 51 children(mean $12.2{\pm}11.4$ months) were enrolled in this study. Leukocyte counts, erythrocyte sedimentation rates(ESR) and C-reactive protein(CRP) were also measured. Renal parenchymal involvement was assessed by $^{99m}Tc$ DMSA scintigraphy in the first 7 days after admission. PCT was measured by immunoluminometric assay. Results : PCT values were significantly correlated with the presence of renal defects in children with UTI(n=16)($5.06{\pm}12.97{\mu}g/L$, P<0.05). However, PCT values were not significantly different between children with UTI without renal damage(n=18) and children without UTI(n=17). Using a cutoff of $0.5{\mu}g/L$ for PCT and 20 mm/hr for ESR, 20 mg/L for CRP, sensitivity and specificity in distinguishing between UTI with and without renal involvement were 81.3 percent and 88.9 percent for PCT 87.5 percent and 72.2 percent for ESR, and 87.5 percent and 55.6 percent for CRP, respectively. Positive and negative predictive values were 86.7 percent and 84.2 percent for PCT and 60.9 percent and 81.8 percent for CRP, respectively. Conclusion : In febrile UTI, PCT values were more specific than CRP, ESR and leukocyte count for the identification of patients who might develop renal defects.

Urinary Tract Infection and Vesicoureteral Reflux in Children (소아에서의 요로감염과 방광요관역류에 관한 고찰)

  • Urinary tract infection (UTI) in children has been known to be a cause of renal damage, leading to scar formation, hypertension and renal failure. And vesico-ureteral reflex (VUR), frequently accompanying UTI in young children, has been incriminated as the main factor causing scar formation. This retrospective study has been undertaken to see the relationship among UTI, VUR and renal scar formation. Study population consisted of 291 children (boy 134, girl 42) with UTI, who have been admitted to the Pediatric Department of Kyungpook University Hospital during 6 1/2 year period from January 1990 to June 1996. VUR was diagnosed by VCUG and renal scar by ultrasonogram, DMSA scan (or DMSA SPECT) and IVP. The following result were obtained. Sexual difference showed male predominance (male to female, 134:42) below 1 year of age, and female predominance (male to female, 11:35) over 5 years of age were rioted. VUR has been found in 64 children (22%) and the degree of reflux, classfied by the method proposed by 'International Reflux Study in Children', were as follows ; Grade I : 4.0%, Grade II : 3.0%, Grade III : 2.7%, Grade IV : 5.8% and Grade V : 6.2%. There was no sexual difference E.coli was the most predominant infecting agent occurring in 167 children (57%), and end-stage renal failure was diagnosed at the time of first admission in 5 children with Grade V VUR. Renal scar has been noted in 49 out of 582 kidneys (8.4%), and the incidence of scar foramation according to the degree of VUR were as follow ; Grade 0 (No reflux) : 1.2%, Grade I : 6.7%, Grade II 27.3%, Grade III 29.4%, Grade IV : 57.1%, and Grade V : 100%. In summary, present study shows that renal scar formation in UTI has close correlation with the severity of VUR occurring more frequently in severe reflux, so that early diagnosis and proper treatment of UTI and VUR is of paramount importance in preventing renal damage in children with UTI.

Clinical Characteristics and Prognostic Factors of Vesicoureteral Reflux (방광요관역류의 임상적 특징 및 예후인자)

  • Introduction: Persistent vesicoureteral reflux (VUR), a major cause of urinary tract infection (UTI) in children, can result in serious renal complications, such as reflux nephropathy and chronic renal failure. We evaluated the clinical characteristics and prognostic factors of VUR. Methods: From December 1993 to May 2011, we examined 117 children with vesicoureteral reflux who were admitted to the Department of Pediatrics and Urology, Chungbuk National University hospital for a UTI. The patients were managed medically or surgically. Results: Male patients had a slightly higher prevalence of VUR than female patients (55%). The degrees of the 161 refluxing ureters, as classified by the International Reflux Study Committee, were as follows: grade I, 15 ureters; grade II, 32 ureters; grade III, 54 ureters; grade IV, 26 ureters; grade V, 34 ureters. One hundred and sixty-one renal units (115 cases) underwent a 99m TC-DMSA renal scan, and 62% showed abnormal findings. The incidence of renal cortical defects showed a direct correlation with the severity of VUR. Ninety-four refluxing ureters were followed up medically, and 66 ureters (67%) either disappeared or improved. However, 9 refluxing ureters persisted. The spontaneous resolution rate of VUR seemed to be higher in younger patients with lower grades of reflux, and without renal cortical defects. Sixty-seven refluxing ureters (41%) were treated surgically, 62 refluxing ureters (92%) disappeared, and 5 refluxing ureters (8%) persisted. Conclusion: The incidence of renal cortical defects in patients with UTIs was 62% (in a 99m TC-DMSA renal scan), and showed a direct correlation with the severity of VUR. The spontaneous resolution rate seemed to be lower in the patients with higher grades of VUR, older age (over 4 years old) and diffuse renal cortical defects.

Treatment for Urinary Tract Infection of Children in Korea (우리나라 어린이 요로 감염의 치료 행태)

  • Purpose : Urinary tract infections (UTIs) of children require prompt and correct diagnosis and treatment to reduce the risk of renal damage. As a first step to improve the outcome of UTI in Korea, we investigated the practical variations in the methods of diagnosis, treatment, and evaluation of children with UTI and UTI prevention. Method :A questionnaire related to the individual policy on UTI diagnosis. treatment, imaging test, and prevention was submitted to 26 experts. Result Majority of the experts used bag-collected urine specimen for infants and mid-stream urine specimen for children for urinary culture. With a negative result of culture study, they diagnosed UTI when there was pyuria, positive results of the nitrite test, or bacteriuria. 80 $\%$ of experts prescribed prophylactic antibiotics after upper tract UTI. Operative treatment of vesicoureteral reflux (VUR) was indicated for children older than one or two years old with high-grade VUR, refractory breakthrough infections, or recurrent UTIs. Most of them performed kidney ultrasonography on the diagnosis of UTI and more than half of them evaluated children treated of UTI with vesicocystourethrogram and/or DMSA scintigraphy. Majority did not recommend circumcision. Half of the experts were screening siblings of VUR patients. Conclusion : Considering the variations exposed through this study systematic guidelines for management of children with UTI in Korean would be necessary. (J. Comparison of the Measurement of the Injection Rate of Radioactive Drugs Using 99mTc in Nuclear Medicine (핵의학과에서 99mTc를 이용한 방사성의약품의 투여율 측정 비교)

  • This study was conducted by SPECT test at the Department of Nuclear Medicine at Daegu P Hospital from June 1 to October 31, 2019. A 3-way injection material was mounted among inpatients, and a syringe that was administered with radiopharmaceuticals using a 99mTc labeled compound was secured. We tried to find a way to calculate the dose rate of each radiopharmaceutical and increase the dose rate. As a result of measuring the radioactivity of radio-pharmaceuticals using 99mTc, the average dose rate of 60 syringes of all 6 radiopharmaceuticals was 93.26±7.34%, and the average dose rate of 99mTc-DMSA was 77.72%, 15.54% lower than the total. As a way to increase the dosing rate, the average dose rate diluted twice with the remaining amount of syringe after administration using normal saline increased to 95.37±6.99%, and the average dose rate diluted three times increased to 96.32±6.86%. The corresponding sample t-test to compare the pre- and post-dose rates at 1 dilution and 2 and 3 dilutions. As a result of the dilution and 2 dilutions, the probability of significance was 0.013, which was significantly higher than the dilution(p<0.05). The probability of significance for dilution 1 and dilution 3 was 0.016, which was significantly higher than in one dilution(p<0.05). The sum of the average dose rate using the experimental 3-way line was the highest with 98.85±1.42% of 99mTc, 99mTc-ECD 98.82±1.26%, 99mTc-Mebrofenin 98.82 ± 1.16%, 99mTc-HDP 98.74 ± 1.91%, 99mTc -MIBI was 98.69 ± 1.48%, and 99mTc-DMSA was the lowest with 86.47 ± 4.74%. When the number of dilutions was 5 times using 0.5 cc of normal saline and when the number of dilutions was 5 times using 1 cc of normal saline, when the number of dilutions was 5 times using 0.5 cc of normal saline and 1 cc of nomal saline When the number of dilutions was 5 times and the syringe volume was 0.5 cc, there was a statistically significant difference (p<0.05). There was a statistically significant difference when the number of dilutions was 5 times using 1 cc of nomal saline and the number of dilutions was 5 times using 1 cc of normal saline, and the syringe volume was 0.5 cc (p<0.05).

Incidence of Vesicoureteral Reflux in Asymptomatic Siblings of Patients with Reflux (방광요관역류 환아의 무증상 형제 자매의 역류 빈도 조사)

  • Purpose : Vesicoureteral reflux(VUR) is defined as a retrograde flow of urine from the bladder into the upper urinary tract. It has been shown to predispose patients to hypertension, renal scarring, and end-stage renal failure if not recognized and treated. The observation that VUR occurs in siblings of children with reflux at a significantly higher rate than the general pediatric population has been recognized for many years and VUR was detected in 26% to 51% of siblings of patients with VUR. The purpose of this study is to determine the incidence of VUR in asymptomatic siblings of children with VUR and to see if this form of screening would be practical. Methods : We retrospectively reviewed the records of 28 patients with VUR and their siblings. The total number of asymptomatic siblings investigating VUR were 28 persons. All patients and siblings were evaluated for VUR by a voiding cystourethrography and all patients and siblings with VUR were performed $^{99m}Tc$ 2,3-dimercaptosuccinic acid renal scan. Results : A total of 28 patients(14 boys, 14 girls) with VUR were studied; the mean patient age was 2.7 years(range 1 month to 8.4 years). The total number of asymptomatic siblings investigating VUR were 28 persons(17 boys, 11 girls) and the mean age was 3.3 years(range 2 months to 7.4 years). Renal scar was detected in 20 of 28(71.4%) patients with VUR. VUR was noted in three of 28(10.7%) siblings and renal scar was detected in one of three siblings with VUR. Conclusion : In this study, the predictive value of a positive family history alone in identifying VUR was 10.7%. This incidence suggests more investigation of asymptomatic siblings and continued study of this group of patients at risk is needed for clarifying the family screening of patients with VUR.

Renal Problems in Early Adult Patients with Turner Syndrome

  • Purpose: This study aimed to evaluate the status of renal function and the presence of urinary abnormalities in early adult patients with Turner syndrome (TS). Methods: Sixty-three girls with TS, who are attending pediatric endocrine clinics in Busan Paik Hosp., were studied. Urine and blood chemistry tests were performed in every visiting times. Renal ultrasonography was performed in all patients at the initial diagnosis, and intravenous pyelography, DMSA renal scan and renal CT were also performed, if necessary. Results: Of the 63 patients, the karyotype showed 45,X in 32 (50.8%), mosaicism in 22 (34.9%) and structural aberration in 9 (14.3%). The renal function at the latest visit was shown as normal in all patients. Nephrotic syndrome had developed in one patient. Hematuria was observed in seven patients. Renal anomalies were observed in 20 of the 63 TS (31.7%). Of the 32 TS patients with 45,X karyotype, 13 (40.6%) had renal anomalies, while these were found in 7 (22.6%) of 31 TS patients with mosaicism/structural aberration. But there was no significant statistical difference between two karyotype groups. Conclusion: Based on this study, most of the patients with TS do not have any significant problems related to renal function until early adulthood, regardless of renal malformation or hematuria.

Experimental Studies on the Preparation of Some $^{99m}Tc$ Instant Labelling Kits (몇가지 $^{99m}Tc$ 즉석표식(卽席標識)킷트 제조(製造)를 위(爲)한 실험연구(實驗硏究))

  • Using stannous chloride, optimum conditions for $^{99m}Tc$ labelling of some scanning agents such as phytic acid (P A), dimercaptosuccinic acid (DMSA), and calcium diethylenetriaminepentaacetate (Ca-DPTA) were established. Methods of separation and identification of the labelled compounds were practiced by a paper- or thin layer- chromatography. Biodynamic studies of the compounds were also carried out. The results indicate that the molar ratios of the chelating agent and stannous chloride varies only with the concentrations of the chelating agents, and thus the amounts of the stannous chloride per labelling tube were nearly constant $(500\sim600{\mu}g)$ regardless the variation of the molar ratios. It suggests that the given experimental conditions require about $500{\mu}g$ of stannous chloride regardless of the chelating agents. Under alkaline pH, the labelling yields were drastically decreased due to the probable formation of colloidal tin compounds. Biodynamic data showed characteristic patterns with each compound indicating that they are all suitable for the relevant scanning applications.

Functionalization of Fe3O4 Nanoparticles and Improvement of Dispersion Stability for Seperation of Biomolecules (생체분자 분리를 위한 Fe3O4 나노입자의 표면수식과 분산 안정성 향상)

  • The surface of magnetite ($Fe_{3}O_{4}$) nanoparticles prepared by coprecipitation method was modified by carboxylic acid group of poly(3-thiophenacetic acid (3TA)) and meso-2,3-dimercaptosuccinic acid (DMSA). Then the lysozyme protein was immobilized on the carboxylic acid group of the modification of the magnetite nanoparticles. The magnetite nanoparticles are spherical and the particle size is approximately 10 nm. We measured quantitative dispersion state by dispersion stability analyzer for each $Fe_{3}O_{4}$ nanoparticles with and without surface modification. The concentration of lysozyme on the modified magnetite nanoparticles was also investigated by a UV-Vis spectrometer and compared to that of magnetite nanoparticles without surface modification. The functionalized magnetite particles had higher enzymatic capacity and dispersion stability than non-functionalized magnetite nanoparticles.
