The Effect of Banhabaekchulcheonma-tang on Benign Paroxysmal Positional Vertigo: A Systematic Review Using the CNKI Database (양성 돌발성 두위 현훈에 대한 반하백출천마탕(半夏白朮天麻湯)의 효과 : 체계적 문헌고찰 - CNKI 검색을 중심으로)

  • Objectives: This study examines the effect of Banhabaekchulcheonma-tang on Benign Paroxysmal Positional Vertigo (BPPV) through a systematic review of randomized controlled trials (RCTs) in the Chinese National Knowledge Infrastructure Database (CNKI). Methods: RCTs were included that tested the effect of Banhabaekchulcheonma-tang on BPPV. A study of the literature in the CNKI was performed for papers published from January 1, 2011 to June 30, 2021. Results: A total of eight RCTs met the inclusion criteria. All studies identified the effect of Banhabaekchulcheonma-tang in the treatment of BPPV. The effectiveness of treatment was higher in the intervention group than in the control group, as shown through evaluation indicators, including the total effective rate, dizziness handicap inventory (DHI), and blood lipids. Conclusion: Treatment with Banhabaakchulcheonma-tang was found to be effective in treating BPPV. However, due to the low quality of available studies, the significance of this conclusion is somewhat limited. Further clinical studies are needed on the development of diagnosis and treatment methods for BPPV based on Korean medicine.

  • Brady-arrhythmia is defined as an arrhythmia in which the heart rate slows to less than 60 beats per minute. Brady-arrhythmia reduces cardiac output and causes changes in blood flow. As a result, dizziness occurs because not only ischemia occurs in the central nervous system, but also blood flow in major organs decreases. The insertion of an artificial pacemaker is known as almost the only treatment alternative for patients with brady-arrhythmia with symptoms. This study reports a case of 85 year old male diagnosed with brady-arrhythmia complaining presyncope type of dizziness. The patient underwent complex Korean medicine treatments by herbal medicine, acupuncture and moxibustion. The daily average value of pulse rate, K-DHI score, NRS score and the patient's subjective expression of symptoms were used as evaluation tools. This study suggested a significant improvement in symptoms through complex Korean medicine treatments without the use of anti-arrhythmic drugs or insertion of an artificial pacemaker in the patient with brady-arrhythmia complaining dizziness.

  • 최근 기후변화로 인해 가뭄의 규모 및 발생빈도가 증가하고 있는 추세이며, 불특정 다수의 지역에 장기간에 걸친 가뭄 발생이 증가하고 있다. 이에 본 연구는 선제적 가뭄 대응 및 관리를 위해 기후변화에 따른 가뭄 현황과 원인을 분석하고, 전국을 대상으로 일괄적인 가뭄평가를 수행하여 지자체별 가뭄대책 수립이 시급한 지역을 확인하기 위한 가뭄취약지도를 구축하였다. 가뭄취약성 평가는 가뭄대응능력, 노출도, 민감도, 보조수원능력을 기반으로 하여 평가 방법론을 개발하였다. 가뭄 대응능력은 지역별 현재 급수체계를 고려한 물수지 분석을 통해 용수공급 가능일수를 산정하고 점수화하였다. 노출도는 가뭄의 발생빈도와 심각한 정도를 나타내는 가뭄빈도와 가뭄심도를 활용하여 가뭄위해성지수(DHI)를 산정하고 계수화 하였다. 민감도는 생활용수와 공업용수 이용량을 합산하여 생공용수 이용량을 산정하고 계수화 하였다. 마지막으로 보조수원능력은 지역별 주 수원의 기능을 보조하여 가뭄에 대응할 수 있는 지하수 개발 가능량과 저수지 용량을 합산하여 해당지역 생공용수 수요량으로 나눈 값을 계수화 하였다. 4가지의 평가인자를 통한 가뭄취약성 평가는 I-IV등급까지 총 4개의 등급으로 평가된다. I등급은 가뭄취약지도제작 시급성 및 가뭄발생빈도가 낮고, 가뭄의 규모와 피해가 작으며, 용수공급능력이 충분한 지역이며, II등급은 가뭄취약지도의 제작이 다소 시급하며, 가뭄발생빈도가 낮고, 가뭄의 규모와 피해가 작으며, 용수공급능력이 다소 부족한 지역이다. III등급은 가뭄취약지도의 제작이 시급하며, 가뭄발생빈도가 높고, 가뭄의 규모와 피해가 크며, 기존 용수공급 능력이 부족한 지역이며, IV등급은 가뭄취약지도의 제작이 시급하며, 가뭄발생빈도가 높고, 가뭄의 규모와 피해가 크며, 용수공급 능력이 매우 부족하여 별도의 대책이 필요한 지역이다. 개발된 가뭄취약성 평가 방법을 시범적으로 충남 8개 시군에 적용하여 평가 해본 결과, 가뭄대응능력이 가뭄취약성에 가장 중요한 인자로 나타남에 따라 가뭄 대응 및 관리에 있어 가뭄대응능력, 즉 용수공급 가능일수를 높이는 것이 가장 중요한 것으로 판단되었다.

Comparison of IMRT and VMAT Techniques in Spine Stereotactic Radiosurgery with International Spine Radiosurgery Consortium Consensus Guidelines (International Spine Radiosurgery Consortium Consensus Guidelines에 따른 Spine Stereotactic Radiosurgery에서 IMRT와 VMAT의 비교연구)

  • Stereotactic body radiation therapy (SBRT) is increasingly used to treat spinal metastases. To achieve the highest steep dose gradients and conformal dose distributions of target tumors, intensity-modulated radiation therapy (IMRT) and volumetric-modulated arc therapy (VMAT) techniques are essential to spine radiosurgery. The purpose of the study was to qualitatively compare IMRT and VMAT techniques with International Spine Radiosurgery Consortium (ISRC) contoured consensus guidelines for target volume definition. Planning target volume (PTV) was categorized as TB, $T_{BPT}$ and $T_{ST}$ depending on sectors involved; $T_B$ (vertebral body only), $T_{BPT}$ (vertebral body+pedicle+transverse process), and $T_{ST}$ (spinous process+transverse process). Three patients treated for spinal tumor in the cervical, thoracic, and lumbar region were selected. Eacg tumor was contoured by the definition from the ISRC guideline. Maximum spinal cord dose were 12.46 Gy, 12.17 Gy and 11.36 Gy for $T_B$, $T_{BPT}$ and $T_{ST}$ sites, and 11.81 Gy, 12.19 Gy and 11.99 Gy for the IMRT, RA1 and RA2 techniques, respectively. Average fall-off dose distance from 90% to 50% isodose line for $T_B$, $T_{BPT}$, and $T_{ST}$ sites were 3.5 mm, 3.3 mm and 3.9 mm and 3.7 mm, 3.7 mm and 3.3 mm for the IMRT, RA1 and RA2 techniques, respectively. For the most complicated target $T_{BPT}$ sites in the cervical, thoracic and lumbar regions, the conformity index of the IMRT, RA1 and RA2 is 0.621, 0.761 and 0.817 and 0.755, 0.796 and 0.824 for rDHI. Both IMRT and VMAT techniques delivered high conformal dose distributions in spine stereotactic radiosurgery. However, if the target volume includes the vertebral body, pedicle, and transverse process, IMRT planning resulted in insufficient conformity index, compared to VMAT planning. Nevertheless, IMRT technique was more effective in reducing the maximum spinal cord dose compared to RA1 and RA2 techniques at most sites.

  • The present study was conducted to investigate effects of auto milking system (AMS) on milk yield and milk composition in dairy cow using dairy herd improvement (DHI) record. DHI records for 147 cows were compared between a year before and after AMS use. AMS significantly increased daily milk yield compared with the conventional milking system (30.4kg vs 34.3 kg for CMS and AMS, respectively). There were no significant differences in the contents of milk fat (3.7% vs 3.8% for CMS and AMS) and milk protein (3.2% vs 3.2% for CMS and AMS) between CMS and AMS. But the yields of milk fat and milk protein were significantly higher for AMS than for CMS. Average somatic cell counts were 169,400/ml (1st grade) before AMS use but increased by 314,400/ml (2nd grade) after AMS use. Overall, AMS increased milk yield and the yields of milk fat and protein without affecting milk composition but also increased somatic cell count.

On Listening, Reflection and Meditation in Vedānta (베단따의 '듣기·숙고하기·명상하기'(문·사·수)에 관하여)

  • The three means of listening, reflection and meditation (${\acute{s}}raava{\d{n}}a$, manana and $nididhy{\bar{a}}sana$) which are central devices of practice in $Ved{\bar{a}}nta$ philosophy should be understood not as a continuative step but as a methodological extension on condition of having one and the same purpose. In other words, the three means should be interpreted in a listening-oriented manner, in which the process has to be methodologically extended to reflection and meditation only when the direct knowledge on the reality is not gained in listening. This kind of interpretation can be more justified by displaying significant characteristics of Indian philosophy implied in the three means. It can be easily said that $Ved{\bar{a}}nta$ belonging to the liberation-centric tradition is a project of 'regaining essential self' through which the self becomes essential self by knowing that self. In this case the listening-oriented interpretation coincides with the basic teachings of $Ved{\bar{a}}nta$, since listening alone can be a sufficient means for obtaining knowledge of the original self. Further, as the project of 'regaining essential self' is carried out by the three means, these can be called a sort of 'event' that is carried out according to the scenario of $Ved{\bar{a}}ntic$ metaphysics. In this case listening is a course of comprehending the scenario of event participated by oneself, and that participant can accomplish the project by way of listening the scenario alone judged as somewhat more effective for liberation. However, in the later $Ved{\bar{a}}nta$ there arises a meditation-oriented interpretation of which three means are regarded not as a methodological extension but as a continuative step, because of the emphasis on meditation under the lasting influence of other philosophical systems. This is a result of epistemic desire that tries to convert what is heard to what is specially perceived or what is given to what is accepted. It may be said that this interpretation emphasizing the phased transition from the indirect to the direct of knowledge is an attempt to rationalize the repetitive delay of event as the actual failure of project. Furthermore, an assertion of the later $Ved{\bar{a}}nta$ which refers the fourth means called $sam{\bar{a}}dhi$ is based on the logic that the self-realization is possible apart from and outside the text, and accordingly it is incompatible with an assertion of the early $Ved{\bar{a}}nta$ that the self-realization is a reproduction as it is of the scenario guided by the absolute text. After all, the standard interpretation on the three means in $Ved{\bar{a}}nta$ have to be the listening-oriented, but not be the meditation-oriented or the $sam{\bar{a}}dhi$-oriented.

Evaluation of Tangential Fields Technique Using TOMO Direct Radiation Therapy after Breast Partial Mastectomy (유방 부분 절제술 후 방사선 치료 시 TOMO Direct를 이용한 접선 조사의 선량적 유용성에 관한 고찰)

  • Purpose: Investigation of the clinical use of tangential fields technique using TOMO direct in comparison to conventional LINAC based radiation therapy after breast partial mastectomy. Materials and Methods: Treatment plans were created for 3 left-sided breast cancer patients who had radiation therapy after breast partial mastectomy by using wedged tangential fields technique, field in field technique (FIF), TOMO Direct, TOMO Direct intensity modulated radiation therapy (IMRT) under the normalized prescription condition ($D_{90%}$: 50.4 Gy/28 fx within CTV). Dose volume histogram (DVH) and isodose curve were used to evaluate the dose to the clinical target volume (CTV), organ at risk (OAR). We compared and analyzed dosimetric parameters of CTV and OAR. Dosimetric parameters of CTV are $D_{99}$, $D_{95}$, Dose homogeneity index (DHI: $D_{10}/D_{90}$) and $V_{105}$, $V_{110}$. And dosimetric parameters of OAR are $V_{10}$, $V_{20}$, $V_{30}$, $V_{40}$ of the heart and $V_{10}$, $V_{20}$, $V_{30}$ of left lung. Results: Dosimetric results of CTV, the average value of $D_{99}$, $D_{95}$ were $47.7{\pm}1.1Gy$, $49.4{\pm}0.1Gy$ from wedged tangential fields technique (W) and FIF (F) were $47.1{\pm}0.6Gy$, $49.2{\pm}0.4Gy$. And it was $49.2{\pm}0.4$ vs. $48.6{\pm}0.8Gy$, $49.9{\pm}0.4$ vs. $49.5{\pm}0.3Gy$ Gy for the TOMO Direct (D) and TOMO Direct IMRT (I). The average value of dose homogeneity index was W: $1.1{\pm}0.02$, F: $1.07{\pm}0.02$, D: $1.03{\pm}0.001$, I: $1.05{\pm}0.02$. When we compared the average value of $V_{105}$, $V_{110}$ using each technique, it was the highest as $34.6{\pm}9.3%$, $7.5{\pm}7.9%$ for wedged tangential fields technique and the value dropped for FIF as $16.5{\pm}14.8%$, $2.1{\pm}3.5%$, TOMO direct IMRT as $7.5{\pm}8.3%$, $0.1{\pm}0.1%$ and the TOMO direct showed the lowest values for both as 0%. Dosimetric results of OAR was no significant difference among each technique. Conclusion: TOMO direct provides improved target dose homogeneity over wedged tangential field technique. It is no increase the amount of normal tissue volumes receiving low doses, as oppose to IMRT or Helical TOMO IMRT. Also, it simply performs treatment plan procedure than FIF. TOMO Direct is a clinical useful technique for breast cancer patients after partial mastectomy.

Analysis of Voice and Swallowing Symptoms after Thyroidectomy in Patients without Recurrent Laryngeal Nerve Injury in Early Postoperative Period (반회후두신경 손상을 동반하지 않은 갑상선 절제술 환자에서 수술 초기의 음성 및 연하 기능의 변화에 대한 분석)

  • Background and Objectives : After thyroidectomy, many patients experience problems report such things as reduced voice range and vocal fatigue, swallowing problems without superior and recurrent laryngeal nerve injury. The purpose of this study was to evaluate voice and swallowing problems before and after thyroid surgery without laryngeal nerve injury. Materials and Methods : Ninety-three patients who underwent thyroidectomy without laryngeal nerve injury and completed the follow-up evaluations were studied between June 2013 and December 2015. Each evaluation was performed preoperatively, as well as 1 week, 1 month postoperatively. Analysis was performed including voice handicap index (VHI), dysphagia handicap index (DHI), and acoustic voice analysis. Results : Patients show significant variation of parameters in the fundamental frequency (F), maximal phonation time (MPT), shimmer, jitter and soft phonation index (SPI) early after operation, and most of them showed recovery of parameters after 1month of operation. Perceptive complaint of voice and swallowing also showed significant decreased after operation (p<0.005). After 1month of operation, MPT, highest frequency and frequency ranges still showed significant decreased parameters. Comparing acoustic and perceptive parameters of total thyroidectomy and lobectomy, there was no significant changes between them except highest frequency (p=0.042). Conclusion : The results from both subjective and objective evaluations show voice and swallowing disturbance after thyroidectomy even in the absence of laryngeal nerve and provide patients information about the recovery process after surgery. Highest frequency parameter showed most significant changes after operation.

An Iterative, Interactive and Unified Seismic Velocity Analysis (반복적 대화식 통합 탄성파 속도분석)

  • Among the various seismic data processing sequences, the velocity analysis is the most time consuming and man-hour intensive processing steps. For the production seismic data processing, a good velocity analysis tool as well as the high performance computer is required. The tool must give fast and accurate velocity analysis. There are two different approches in the velocity analysis, batch and interactive. In the batch processing, a velocity plot is made at every analysis point. Generally, the plot consisted of a semblance contour, super gather, and a stack pannel. The interpreter chooses the velocity function by analyzing the velocity plot. The technique is highly dependent on the interpreters skill and requires human efforts. As the high speed graphic workstations are becoming more popular, various interactive velocity analysis programs are developed. Although, the programs enabled faster picking of the velocity nodes using mouse, the main improvement of these programs is simply the replacement of the paper plot by the graphic screen. The velocity spectrum is highly sensitive to the presence of the noise, especially the coherent noise often found in the shallow region of the marine seismic data. For the accurate velocity analysis, these noise must be removed before the spectrum is computed. Also, the velocity analysis must be carried out by carefully choosing the location of the analysis point and accuarate computation of the spectrum. The analyzed velocity function must be verified by the mute and stack, and the sequence must be repeated most time. Therefore an iterative, interactive, and unified velocity analysis tool is highly required. An interactive velocity analysis program, xva(X-Window based Velocity Analysis) was invented. The program handles all processes required in the velocity analysis such as composing the super gather, computing the velocity spectrum, NMO correction, mute, and stack. Most of the parameter changes give the final stack via a few mouse clicks thereby enabling the iterative and interactive processing. A simple trace indexing scheme is introduced and a program to nike the index of the Geobit seismic disk file was invented. The index is used to reference the original input, i.e., CDP sort, directly A transformation techinique of the mute function between the T-X domain and NMOC domain is introduced and adopted to the program. The result of the transform is simliar to the remove-NMO technique in suppressing the shallow noise such as direct wave and refracted wave. However, it has two improvements, i.e., no interpolation error and very high speed computing time. By the introduction of the technique, the mute times can be easily designed from the NMOC domain and applied to the super gather in the T-X domain, thereby producing more accurate velocity spectrum interactively. The xva program consists of 28 files, 12,029 lines, 34,990 words and 304,073 characters. The program references Geobit utility libraries and can be installed under Geobit preinstalled environment. The program runs on X-Window/Motif environment. The program menu is designed according to the Motif style guide. A brief usage of the program has been discussed. The program allows fast and accurate seismic velocity analysis, which is necessary computing the AVO (Amplitude Versus Offset) based DHI (Direct Hydrocarn Indicator), and making the high quality seismic sections.
