The Comparison of Protective Effects of Adenosine Included Cardioplegia According to Adenosine Dosage (심정지액 속에 포함된 아데노신의 용량에 따른 심근보호 효과 비교)

  • Background: Adenosine is secreted by myocardial cells during myocardial ischemia or hypoxia. It has many beneficial effects on arrhythmias, myocardial ischemia, and reperfusion ischemia. Although many investigators have demonstrated that cardioplegia that includes adenosine shows protective effects in myocardial ischemia or reperfusion injury, reports of the optimal dose of adenosine in cardioplegic solutions vary. We reported the results of beneficial effects of single dosage(0.75 mg/Kg/min) adenosine by use of self-made Langendorff system. But it is uncertain that dosage was optimal. The objective of this study is to determine the optimal dose of adenosine in cardioplegic solutions. Material and Method: We used a self-made Langendorff system to evaluate the myocardial protective effect. Isolated rat hearts were subjected to 90 minutes of deep hypothermic arrest(15$^{\circ}C$) with modified St. Thomas' Hospital cardioplegia including adenosine. Myocardial adenosine levels were augmented during ischemia by providing exogenous adenosine in the cardioplegia. Three groups of hearts were studied: (1) group 1 (n=10) : adenosine - 0.5 mg/Kg/min, (2) group 2(n=10): adenosine -0.75 mg/Kg/min, (3) group 3 (n=10) : adenosine -1 mg/Kg/min. Result: Group 3 resulted in a significantly rapid arrest time of the heart beat(p<0.05) but significantly slow recovery time of the heart beat after reperfusion(p<0.05) compared to groups 1 and 2. Group 2 showed a better percentage of recovery(p<0.05) in systolic aortic pressure, aortic overflow volume, coronary flow volume, and cardiac output compared to groups 1 and 3. Group 1 showed a a better percentage of recovery(p<0.05) in the heart rate compared to the others. In biochemical study of drained reperfusates, CPK and lactic acid levels did not show significant differences in all of the groups. Conclusion: We concluded that group 2 [adenosine(0.75 mg/Kg/min) added to cardioplegia] has better recovery effects after reperfusion in myocardial ischemia and is the most appropriate dosage compared to group 1 and 3.

Surgical Treatment of the Aortic Dissection (대동맥박리증의 외과적 치료)

  • From September 1992 to May 1996, 38 patients ranging in age from 23 to 78, were operated for aortic dissection at Asan medical center There were 21 men and 17 women. The underlying aortic pathology were acute aortic dissection in 23, chronic aortic dissection in 15. Eight patients had Martian syndrome. In 34 cases of DeBakey type I, II patients, femoral artery and vein and/or right atrial auricle were used as cannulation site. With deep hypothermic c rculatory arrest (esophageal temperature 12 $\pm$ 2.5$^{\circ}C$) and retrograde cerebral perfusion of cold oxygenated blood through SVC, we replaced the ascending aorta and the part of arch if necessary. The mean duration of the total circulatory arrest time was 25 $\pm$ 1.7 mintstuts. In 4 cases of DeBakey type III patients, we replaced descending thoracic aorta or thoracoabdomlnal aorta without shunt or bypass under normothermia with an average 30: 1.5 minutesaortic cross clamp time. One death(2.6%) occurred on the twenty-second postoperative day owing to asphyxia related to ulcer bleeding. Postoperative complications were myocardial infarction with transient left peroneal palsy in 1 case, transient lower extremity weakness in 1 case and prolonged ventilatory support in 1 case. Two patients required reoperation due to retrograde extended dissection and aortic insufuciency. There was no late death with an average 25 months follow-up period.

The Mid Term Clinical Result and the Risk Factor Analysis of Isolated Aortic Valve Replacement (단독 대동맥판막 치환술의 중기 성적과 그 위험인자에 대한 분석)

  • Background: The aims of this paper were to review the mid term clinical results and to analyze the preoperative risk factors of isolated aortic valve replacement (AVR). Material and Method: Between January 1992 and February 2003, 80 patients underwent isolated AVR. 58 were male and 22 were female patients, raging from 12 to 75 years of age (mean :$46.8{\pm}13.0$ years). 74 patients except one early death and 5 follow-up loss were contacted by OPD or by telephone. The mean duration of follow-up was $44.2{\pm}29.7$ months and the total cumulative period was 272.8 patient-year. Result: The complications in hospital occurred in 35 cases : 12 wound problems (11 superficial, 1 deep), 11 arrhythmias (9 temporary, 2 persistent), 3 low cardiac output, and so forth. The late deaths were 4 cases : the heart-related deaths were 2 cases ($0.7\%$ patient-year). Conclusion: The risk factors that influenced the early mortality and morbidity were older age (> 60 years)(p=0.04), poor preoperative NYHA functional class (> 3) (p=0.048), high preoperative serum creatinin level (> 1.2 mg/100 ml)(p=0.031), long operation time (aortic clamping time>90 min)(p=0.042). The same factors influenced the late mortality and morbidity. Freedom from valve-related complication was $86.4{\pm}5.3\%,$ actuarial survival rate were $96.8{\pm}2.3\%$ at 3 years and $90.8{\pm}4.6\%$ at 10 years.

  • The morphological, physical, and chemical properties of Sonjeong series derived from acidic crystalline rocks are presented. Also it deals with the genesis and classification of the Songjeong series. Morphologically these soils have brown to dark brown loam A horizons and yellowish red to red clay loam Bt horizons with moderate, medium subangular blocky structure and thin patchy clay cutans on the ped faces. C horizons are very deep, yellowish red to yellowish brown fine sandy loam or sandy loam with original rock structure. Physically distribution of particle size indicates that clay increases with depth up to argillic horizons but below the argillic horizons clay content decrease. The moisture holding capacity is fairly good in Songjeong soils. Chemically soil reaction is strongly to very strongly acid throughout the profile and content of organic matter is less than 1 per cent except A horizons. Cation exchange capacity ranges from 5 to 9 me/100g of soils and base saturation is less than 35 per cent throughout the profile. The natural fertility of Songjeong soils are usually low. It needs lime, organic matter, and heavy application of fertilizer for the crop land. These soils occur temperate and humid climate under coniferous, deciduous, and mixed forest vegetation. Songjeong soils are classified as Red-Yellow Soils. Characteristically Songjeong soils are similar to Red-Yellow Podzolic soils in the United States but lack of A2 horizons and are quite liket Red-Yellow Soils of the Japan. According to new classification system which is 7th approximation of USDA Songjeong soils can be classified as fine loamy, mesic family of Typic Hapludults and in the FAO/UNESCO project World Soil Map as Orthic Acrisols.

The Results of Combined Modality Treatment with Transurethral Resection, Cisplatin and Radiation Therapy for Invasive Bladder Cancer (침윤성 방광암에서 경요도적절제술 및 Cisplatin과 방사선의 병용치료의 효과)

  • Ten patients with deep muscle-invading bladder carcinoma (clinical stages T3a to T4b) who were not candidates for cystectomy were treated with combined modality treatment with transurethral resection, cisplatin chemotherapy and pelvic irradiation from 1989 through 1990, and were analyzed retrospectively. All patients were not candidates for cystectomy because the tumors were judged unresectable or they were not fit for a radical cystectomy. Of the patients 5 had clinical stage T3a, 3 stage T3b and 2 stage T4b disease. The minimum follow-up was 16 months. The complete response rate is 60$\%$ for all patients. The complete responses were achieved in 4 of 5(80$\%$) with stage cT3a, in 2 of 3(67$\%$)with stage cT3b and in none of 2(0$\%$) with stage cT4b. The partial responses were achieved in 2, so an overall response rate was 80$\%$. All six patients with grade I or II transitional cell carcinoma showed complete responses. Four patients with higher grade tumors showed partial responses in 2 and no response in 2, and all died of their bladder cancer. Six patients who showed complete responses after treatment are alive and only one of them showed a local recurrence 10 months after treatment. Distant metastases developed in 3 patients: lungs in 2(cT4b) of those who were never locally free of disease and spine in 1 patient (cT3b) among those with a partial response. Two patients died of metastases to lungs. During the follow-up diarrhea occurred in one which was improved after conservative treatment. On the basis of this analysis it is suggested that combined modality treatment seems to be a tolerable regimen and can be offered with a relatively high probability of success and conservation of bladder function in those with less advanced tumors by clinical stage and low grade.

Assessment of the Wetland Soil Development in Constructed Wetlands using the Soil Properties of a Reference Wetland (기준습지 토양특성을 활용한 인공습지의 토양발달 평가)

  • Changes in wetland soil properties of two constructed wetlands after their constructions were compared to those of a natural wetland to determine if they could be used for the evaluation of the success of constructed wetlands and the assessment of their functions. One natural wetland as a reference wetland and two constructed wetlands(treatment wetland and experimental wetland) with different contaminant inflow characteristics were selected for this study. Major physicochemical properties of wetland soil such as soil texture, water content, pH, CEC(cation exchange capacity), organic matter content, total nitrogen, and available phosphorus were monitored to investigate the effects of inundation and accumulation of organic matters and nutrients on the wetland soil development. There was a clear difference in soil texture between the natural wetland and the constructed ones, with the high sand content in the constructed wetlands as compared to the high clay content in the natural one. Gradual increases of silt and clay contents over time were observed in the constructed wetlands. The soil of the natural wetland was higher in water content and organic matter but lower in pH than those of the constructed wetlands. The pH of the constructed wetlands reached near neutral ranges after initial increase. CEC and nutrient concentrations of the constructed wetlands seemed to be affected mainly by outside inflows of organic matter and contaminants. Concentrations of organic matter and nutrients decreased over time in the experimental wetland where surface and deep soils with different characteristics were mixed during its construction, suggesting that changes in soil properties during wetland constructions may affect the development of wetland soils or wetland biogeochemistry. This study showed that changes in physicochemical properties of soils in constructed wetlands could be used to assess the success of constructed wetlands and their functions, and also the importance of reference wetlands for the appropriate assessment.

Long-term Changes of Bathymetry and Surface Sediments in the dammed Yeongsan River Estuary, Korea, and Their Depositional Implication (영산강 하구의 수심 및 표층 퇴적물 특성의 변화와 퇴적환경)

  • Long-term changes in bathymetry and grain size of surface sediments were investigated for understanding depositional sedimentary environments in the channelized Yeongsan River Estuary, Korea. The results revealed that an average depth of the estuary had decreased up to 2.1 m from 1982 to 2006, while it had increased to 0.3 m from 2006 to 2012. The rapid decrease of the water depth from 1982 to 2006 was due to the vast deposition of mud caused by the change of water course and flow velocity after the estuary was dammed. Meanwhile the increase of the water depth from 2006 to 2012 may be associated with multiple erosional processes, including a dredging at the southern part of the estuary and other erosions from the dike sluice expansion work. Considering the water-depth change and tidal-level variation in the study area, an depositional rate in the estuary is estimated to be 8~9 cm/yr for the last 2 decades (1982~2006). The sediments of Yeongsan River Estuary are largely composed of silt-clay mixtures: overall, silt is distributed mainly in the shallow area of the estuary edge, while clay is confined to the deep area of the estuary center. Mean grain size of the sediments is 6.0 Ø on average in 1997, 7.8 Ø on average in 2005 and 7.7 Ø on average in 2012, respectively, suggesting that the sediments became finer due to the increase of silt and clay contents in 1997~2005. Furthermore, several lines of evidences, including the comparison between the amounts of the sediment influx discharged from the Yeongsan River and the sediments in the estuary, and the changes in distribution pattern of silt and clay contents implying that they moved from offshore to estuary dike, indicate that the mud sediments are originated mainly from the offshore, not from the river.

Long-term Variations of Water Quality Parameters in Lake Kyoungpo (경포호에서 수질변수들의 장기적인 변화)

  • In order to identify long-term trends of water quality parameters in Lake Kyeongpo, Mann-Kendall test, Sen's slope estimator and linear regression were applied on data, with 15 parameters from three different sites and rainfall, monitored once in every two months from March to November during 1998~2013. Seasonal variation analysis only used Mann-Kendall test and Sen's slope estimator. Analysis result showed that salinity, transparency and nutrient variables (total phosphorus, dissolved inorganic phosphorus, total nitrogen, nitrate nitrogen, ammonia nitrogen) were only parameters having statistically significant trend. In linear regression analysis, salinity (surface and bottom layer of all sites) and transparency (only at site 1), were figured out with statistically significant increasing trend, while in non-parametric statistical method, salinity and transparency in all sites (surface, middle, deep) were figured out with statistically significant increasing trend. Water quality parameters showing statistically significant decreasing trends were dissolved oxygen (surface layer of site 1 and bottom layer of sites 2 and 3), total phosphorus (sites 1 and 2), dissolved inorganic phosphorus, total nitrogen, nitrate nitrogen and ammonia nitrogen in the linear regression analysis and, dissolved oxygen (bottom layer of all sites), total phosphorus, dissolved inorganic phosphorus, total nitrogen, nitrate nitrogen and ammonia nitrogen in the non-parametric method. Seasonal trend analysis result showed that salinity, turbidity, transparency and suspended solids in spring, salinity, transparency, nitrate nitrogen and suspended solids in summer and temperature, salinity, transparency and suspended solids in fall were the variables depending on the season with increasing trends. In general, rainfall during the research period showed decreasing trend. The significant reduction trends of nutrients in Lake Kyeongpo were believed to be related to lagoon restoration and water management project run by Gangneung city and under-water wear removal, but further detailed studies are needed to know the exact causes.

Hydro-meteorological Effects on Water Quality Variability in Paldang Reservoir, Confluent Area of the South-Han River-North-Han River-Gyeongan Stream, Korea (남·북한강과 경안천 합류 수역 팔당호의 수질 변동성에 대한 기상·수문학적 영향)

  • This study explored spatiotemporal variability of water quality in correspondence with hydrometeorological factors in the five stations of Paldang Reservoir located in the Han River during 4 years from May 2012 to December 2015. Variability of basic water quality factors were largely related with seasonal fluctuations of hydrology. Temperature stratification occurred in the deep dam station, and prolonged hypoxia was observed during the draught year. Nitrogen nutrients were increased with decreasing inflow in which changing pattern of $NH_4$ reversed to $NO_3$ by the effect of treated wastewater effluent. Phosphorus increase was manifest during the period of high inflow or severe drought. Chl-a variation was reversely related with both flow change and AGP(algal growth potential) variations. Our study demonstrated that water quality variability in Paldang Reservoir was largely attributed to both natural and operational changes of inflow and outflow (including water intake) based on major pollution source of the treated wastewater (total amount of $472{\times}10^3m^3d^{-1}$) entering to the water system from watershed. In the process of water quality variability, meteorological (e.g., flood, typhoon, abnormal rainfall, scorching heat of summer) and hydrological factors (inflow and discharge) were likely to work dynamically with nutrients pulse, dilution, absorption, concentration and sedimentation. We underline comprehensive limnological study related to hydro-meteorolology to understand short- and long-term water quality variability in river-type large reservoir and suggest the necessity of P-free wastewater treatment for the effective measure of reducing pollution level of Paldang drinking water resource.

Breeding of Middle Season Pear Cultivar 'Shinil' with Attractive Appearance for Chuseok Season (외관(外觀)이 수려한 추석(秋夕) 출하용 중생종 배 '신일(新一)' 육성(育成))

  • 'Shinil' pear cultivar (Pyrus pyrifolia Nakai) which was originated in a cross between 'Shinko' (non-patented, released in 1941) and 'Hosui' (non-patented, released in 1972) in 1978 was released as a middle season harvest variety. Its usual picking time coincided with 'Chuseok' season which is one of the most famous national holiday in Korea. The fruit showed high soluble solids content and good appearance. The cultivar was preliminarily selected in 1991, and its regional adaptability was evaluated in the name of 'Wonkyo Na-13' at 9 sites for four years from 1992, and finally selected and named in 1995. 'Shinil' is medium in tree vigor like 'Hosui' and spreading in tree habit as 'Niitaka', a leading cultivar in Korea, and consistently very productive. It has high resistance to black rot caused by Alternaria kikuchiana and pear necrotic spot caused by pear necrotic spot virus. Its full bloom is one day earlier than that of 'Niitaka' cultivar and harvest time is September 25 at Suwon area which is 3 days later than that of 'Hosui'. Fruit is round in shape with a deep medium stalk cavity and medium calyx basin and has attractive light yellow brown skin color. The fruit weight ranges between 300 and 400 g, which is similar to 'Chojuro', 'Shinko', and 'Hosui'. Soluble solid content is approximately at the level of 13-14 Brix, which is higher than that of 'Chojuro'. The flesh is cream-white, very juicy, and light grit with soft and fine texture.
