Cytokeratin-positive Cells in the Bone Marrow of Patients with Gastric Cancer (위암 환자의 골수에서 발견된 Cytokeratin 양성세포의 임상적 의의)

  • Purpose: Controversy still exists over in the prognostic significance of microscopic tumor cell dissemination in patients with cancer. This study evaluated the prognostic implication of isolated tumor cells in the bone marrow of patients with gastric cancer. Materials and Methods: Four hundred nineteen (419) patients who underwent surgery for gastric cancer between June 1998 and July 2000 were enrolled in the study. Bone marrow aspirate was obtained from the iliac crest before removal of the primary tumor. Mononuclear cells were isolated and stained with AE-1/AE-3 PAN-CYTOKERATIN. Results: Cytokeratin-positive cells were found in the bone marrow of 219 patients (52.3%). The incidence varied significantly with the depth of invasion (P=0.021) and the stage (P=0.026). The five-year survival rate of patients with cytokeratin-positive cells was 74.1% and that of patients without cytokeratin-positive cells was 81.1%(P=0.2481). There were no significant differences in the recurrence rate and the site of recurrence according to whether or not cytokeratin-positive cells were present in the bone marrow. Conclusion: The presence of cytokeratin-positive cells in the bone marrow of patients with gastric cancer did not predict outcome and recurrence. Therefore, it cannot be used as a prognostic factor.

  • Progressive dysphagia in a 53 year old man was caused by a giant polypoid tumor in the lower intrathoracic esophagus. Radical transthoracic esophagectomy and esophagogastrostomy were carried out. Microscopic examination of the tumor revealed a true carcinosarcoma, composed of a mixture of basaloid squamous cell carcinoma and chondrosarcoma with multiple cartilagenous productions. Carcinoma metastases were found in the subcarinal and perigastric lymph nodes. Immunohistochemically, squamous area displayed strong positive to cytokeratin, and basaloid area showed positive immunoreaction to high molecular weight cytokeratin (34${\beta}$E12). Spindle cell sarcoma reacted to vimentin and smooth muscle actin. Chondrosarcomatous area reacted to vimentin and S-100 protein. He received postoperative chemotherpy and radiotherapy. He has been free of disease for 11 months.

  • Immunohistochemical staining for keratin proteins may be useful as a diagnostic parameter in head and neck neoplasm. Our study evaluates the keratin antibody staining properties of normal tissues and squamous cell carcinoma of the vocal folds from surgical procedures performed on 27 cases. In normal epithelia, low molecular weight cytokeratins were strongly positive in basal layer but apparently reduced in suprabasal layers and completely negative in superficial layer. In invasive squamous cell carcinomas, low molecular weight anti-ck Ab were positive in all carcinoma cells of poorly differentiated carcinomas. On the other hand, high molecular weight anti-ck Ab were positive in almost carcinoma cells of well differentiated carcinomas.

  • Background : The most common anterior mediastinal tumors originate from the thymus. Among them, thymic carcinomas occur as an early local invasion and wide spread metastases. However, when squamous cell carcinoma in the thymus or mediastinum is identified, an occult primary lung cancer must be excluded because the histologic types resemble those found more typically in the lung. CD5 and cytokeratin immunohistochemical staining is useful in evaluating biopsy samples from those tumors. Squamous cell carcinoma of an unknown primary origin in the mediastinum is a rare occurrence and there are only a handful of case reports. Here we describe a case with an anterior mediastinal mass of squamous cell carcinoma with unknown primary origin. A resection of the mediastinal mass without an association with the lung was performed. Immunohistochemical stallings were positive using cytokeratin 13, and negative using CD5 and cytokeratin 7. This was followed by chemotherapy for presumed thymic carcinoma.

  • A mass was detected in the oral cavity from a 18-year-old female miniature poodle dog. Grossly, the mass was soft to hard, red to purple, and $1.5{\times}1.5{\times}1cm$ in size. Histopathologically, the mass was composed of hyperplastic gingival epithelium, well-vascularized stroma, closely packed pleomorphic cells, and numerous giant cells with multiple nuclei and abundant eosinophilic cytoplasm. Immunohistochemically, tumor cells were positive for alkaline phosphatase and cytokeratin 7, but not positive for CD68 unlike in human. The mass was diagnosed to peripheral giant cell granuloma in oral cavity through typical clinical and histopathological features.

  • Tooth formation is a complex developmental process that is mediated through a series of reciprocal epithelial-mesenchymal interactions. Several signal pathways and transcription factors have been implicated in regulating molar crown development, but relatively little is known about the regulation of root development. It was reported that NFI-C knockout mice showed abnormal root formation with normal crown. The aims of this study are to elucidate how the NFI-C regulate the determine of root shape and odontoblasts differentiation. We carried out immunohistochemistry using cytokeratin to investigate the role of Hertwig's epithelial root sheath and DSPP mRNA in-situ hybridization to conform the nature of root dentin during root development in NFI-C knockout mice. Cytokeratin reacted with all the HERS cells and the continuity of cytokeratin positive cells between the HERS cells and enamel epithelium was lost in the cervical region both wild and K/O types. After root dentin deposition cytokeratin positive-HERS cells showed irregularity and loss of polarity in the cervical region in K/O type. DSPP mRNA was strongly expressed in odontoblasts of crown and root dentin in wild type mice, whereas expression of DSPP mRNA was restricted in odontoblast of crown dentin in the K/O type. During root formation in NFI-C knockout mice, HERS normally grow out of the crown but fail to induce odontoblast differentiation in root portion. These results suggest that NFI-C may play important roles in odontoblast differentiation during root dentin formation.

  • Oncocytic carcinoid는 매우 드문 종양으로 단지 몇 례만이 문헌상 보고되고 있다. 본 저자들은 좌폐 상엽에 발생한 oncocytic carcinoid 1례를 치험하였는데 환자는 56세된 여자환로 특이한 증상은 없었다. 흉부 전산화 단층촬영상 좌폐상엽에 3$\times$3.5 cm 크기의 동종의 종괴를 보이고 있었다. 수술전 검사후 외과적절제술을 시행하였는데 육안적소견상 4$\times$3cm 크기의 경계가 명확한 황갈색의 종괴였으며, 병리조직학적 검사상 괴립성 호산성 세포질이 풍부한 양상을 보였으며 면역조직학적 검사에서는 cytokeratin에 양성반응을 보였으며 EMA와 chromograin에 국소적인 양성반응을 보여, 폐에 발생한 oncocytic carcinoid로 진단하였다. 수술후 환자의 상태는 양호하였으며, 현재 외래추적관찰중이다.

  • Epithelioid sarcoma has been well characterized as a distincitve entity. In the classic form, it occurs in the distal extremities of young adults. However, in the proximal form, it was found mostly in the pelvis, perineum, and genital tract of young to middle-aged adults. Morphologically, the tumor cells had promient epithelioid or rhabdoid features with marked cytologic atypia, and grew in a multinodular pattern. Immunohistochemically, cytokeratin, epithelial membrane antigen, and vimentin were positive in the tumor cells. This reports dealt with 32-year-old male lesion on his perineum. The tumor shows typical features of proximal type epithelioid sarcoma. We also discussed pathological differential diagnosis and prognosis on this particular disease.

  • A 6-year-old castrated male domestic short hair cat with the clinical signs of anorexia and vomiting was admitted to the local animal hospital. Abdominal radiography and ultrasonography revealed renomegaly and severe hydronephrosis in the right kidney. Surgically excised right kidney was submitted for diagnosis. On the cut surface, two milky white masses and severe dilation of renal pelvis were observed. Most of the neoplastic masses were composed of uniform well differentiated tubules lined by a single layer of cuboidal to columnar cells and projected papillae into the lumen. The neoplastic cells were strong positive for cytokeratin (CK) MNF116, but negative for CK 7. Based on the clinical and gross findings, histopathology and immunohistochemistry, this case was diagnosed as papillary renal adenoma with hydronephrosis.

  • Chordoma is a slow-growing malignant neoplasm arising from the remnants of the primitive notochord. It accounts for 1 to 4% of all malignant bone tumors. It occurs exclusively along the spinal axis. Authors experienced four cases of chordoma occurred in the sacrococcygeal region. There were two male and two female patients, with a mean age of 63.5 years(range, 57~75 years). Tissue was obtained by wide excision in two patients, by incisional biopsy in one patient and by needle biopsy in the other. Adjuvant radiation therapy was performed on all the patients after their biopsy. The mean diameter of the tumors was 7.6cm(range, 5.5 to 13.0cm). Grossly, tumor was multiobulated, soft and myxoid gelatinous mass. Microscopically, the tumor showed lobulated feature divided by fibrous septa within it. There were physaliphorous cells with vacuolated bubbly cytoplasm. And small uniform, round, and non-vacuolated tumor cells were also present. On immunohistochemical stain, all the cases were immunoreactive for cytokeratin, epithelial membrane antigen(EMA) and vimentin, respectively. One of the 4 cases was positive for S-100 protein. All the cases were negative for CEA.
