갑상선 낭종 환자에서 Ethanol 경화술의 효과 (The Effects of Cystic Tyroid Lesions by Ethanol-Instillation)

  • We analyzed the results of ethanol sclerotherapy in 47 patients with cystic thyroid lesion. Cytologic study showed all of the lesions to be benign. The patients were followed clinically and ultrasonically I month and 3 or more months after ethanol sclerotherapy. If the cystic lesions recurred, repeated treatment was offered. In 4 patients with pure cystic lesion, all of the patents had almost total resolution of the cystic lesions. In 43 patients with mixed cystic lesion, 16(37.2%) patients had almost total resolution of the cystic lesions, 10(23.3%) patients showed a decrease' in the cystic lesion of <50%, but 11(25.6%) patients it did not change in size. Overall, in 30(63.8%) patients the cystic lesion decreased in size. Eleven patients who were not effective by sclerotherapy or follicular proliferation by cytology were converted to surgery. Although no severe complication were observed, there were complication of severe pain in I patient and a drunken feeling in I patient. If the malignancy must be excluded by repeated cytologic examination of aspirated fluid, we consider instillation of ethanol into the cystic lesions of the thyroid to be a simple, safe, economical and effective treatment.

Adventitial Cystic Disease of the Common Femoral Artery: A Case Report and Literature Review

  • Arterial adventitial cystic disease is an uncommon type of non-atherosclerotic peripheral vessel disease. Most cases of arterial adventitial cystic disease occur in the popliteal arteries; however, fewer cases have been reported in the femoral arteries. A 59-year-old male patient visited the hospital with a complaint of a swelling on the lower extremity that had begun two months earlier. Suspecting deep vein thrombosis based on a physical examination and ultrasonography from another hospital, tests were performed. Magnetic resonance imaging (MRI) was performed for exact diagnosis because venous adventitial cystic disease was suspected by computed tomography venography. The MRI indicated venous adventitial cystic disease as well. Thus, a cystic mass excision was performed. In the end, a cystic mass compressing the common femoral vein that originated from the common femoral artery was diagnosed based on the macroscopic findings. This case is reported because blood circulation in the vein was impeded due to arterial adventitial cystic disease, and the symptoms improved after the cystic mass excision and polytetrafluoroethylene roofing angioplasty.

  • Cystic lesions of the neck are frequently encountered on clinical basis. The usual differential diagnosis includes branchial cleft, cystic hygroma, dermoid cyst, tuberculous lymphadenitis, benign detached goiterous thyroid cyst, and Cavitation epidermoid carcinoma. Most of cysts are benign and malignant cysts are rare. The most common form of cystic malignancy in the lateral neck is cystic degeneration of epidermoid carcinoma metastatic to cervical lymph nodes. Cystic neoplasia may result from malingnant changes occurring within the wall of a previously benign cyst Complete head and neck examination on the primary focus is important Especially, when palpation of thyroid is negative, thyroid carcinoma is easily overlooked. Recently, we experienced papillary carcinoma of the thyroid seen as lateral neck cyst. So we report this case with review of literatures.

성인에서 발견된 선천성 낭성 선종양기형 1예 (A Case of Congenital Cystic Adenomatoid Malformation(CCAM) of the Lung in Adult)

  • A congenital cystic adenoid malformation of the lung(CCAM) is characterized by an anomalous fetal development of the terminal respiratory structures, resulting in the adenomatoid proliferation of the bronchiolar elements and cystic formation. CCAM has been detected on the fetus, premature babies and stillborn as well as infants and children. An adult presentation of CCAM is extremely rare. When cystic lesions occur with a repeated infection, an evaluation of the cystic lesions requires a differential diagnosis of CCAM, sequestration, a lung abscess, a pneumatocele and a bronchogenic cyst. The definite treatment of CCAM is the surgical removal of the involved lobe. We report a case of a CCAM in a 24-year-old female with a brief review of the relevant literature.

  • Metastatic squamous carcinoma of the head and neck may rarely present as a cyst in the cervical region. The true incidence of metastatic cystic neck mass is unknown. It is difficult to differentiate metastatic cystic neck mass from congenital cystic neck mass with physical examination and fine needle aspiration biopsy. So the differential diagnosis is dependent on the age of the patient, and therefore in the patient over the 40 years of age, the possibility of a metastatic neck mass should be considered. We report a metastatic cystic neck mass which had been misdiagnosed as a branchial cleft cyst.

microRNA biomarkers in cystic diseases

  • microRNAs (miRNAs) are small non-coding RNAs that regulate gene expression by targeting the 3'-untranslated region of multiple target genes. Pathogenesis results from defects in several gene sets; therefore, disease progression could be prevented using miRNAs targeting multiple genes. Moreover, recent studies suggest that miRNAs reflect the stage of the specific disease, such as carcinogenesis. Cystic diseases, including polycystic kidney disease, polycystic liver disease, pancreatic cystic disease, and ovarian cystic disease, have common processes of cyst formation in the specific organ. Specifically, epithelial cells initiate abnormal cell proliferation and apoptosis as a result of alterations to key genes. Cysts are caused by fluid accumulation in the lumen. However, the molecular mechanisms underlying cyst formation and progression remain unclear. This review aims to introduce the key miRNAs related to cyst formation, and we suggest that miRNAs could be useful biomarkers and potential therapeutic targets in several cystic diseases.

  • During three months from December 1967 to February 1968, 1867 Korean native cows slaughtered in pusan abattoir were investigated for studying the incidence of cystic ovaries. The results obtained in this experiment are summarized as follows: 1. The average frequency of cystic ovaries in 1867 Korean cows was 17 cases, or 0.91%. 2. In 17 cases 1, or 0.3%, occurred in cattle from 1 to 3 years of age; 4 or, 1.02% in four; 7 or, 2.02% in five; 2 or, 0.63% in six; and 3, or 0.63% occurred between 7 and 9 years of age. 3. The incidence of cysts in the respective ovaries, the left ovary was cystic in 23.5% of the cases, the right one in 41.2% and both ovaries in 35.3%. 4. According to the histological findings of cystic ovaries, follicular cysts were 88.2% and luteal cysts 11.8%.

Cystic Salivary Duct Carcinoma Penetrated by Facial Nerve

  • Salivary duct carcinoma is a rare malignant salivary gland tumor that mainly has solid features. When it occurs in the parotid gland, it can invade the facial nerve and cause facial nerve paralysis. However, in our case, the salivary duct carcinoma exhibited cystic features on computed tomographic imaging, and the facial nerve passed through the cyst. Total parotidectomy with level-I to -III dissections was performed and nerve passing through the tumor was sacrificed. The patient received postoperative radiotherapy and was clinically and radiologically followed-up for every 3 months. Recurrence or distant metastasis was not reported. To the best of our knowledge, this is the first case involving a salivary duct carcinoma with cystic features and facial nerve invasion. Here, we report a first case of cystic salivary duct carcinoma of the parotid gland which uncommonly undergo cystic change and penetrated by facial nerve and successfully resected without causing facial nerve injury.

The Incidental Pancreatic Cyst: When to Worry About Cancer

  • Incidental pancreatic cystic lesions are a common challenge encountered by diagnostic radiologists. Specifically, given the prevalence of benign pancreatic cystic lesions, determining when to recommend aggressive actions such as surgical resection or endoscopic ultrasound with sampling is difficult. In this article, we review the common types of cystic pancreatic lesions including serous cystadenoma, intraductal papillary mucinous neoplasm, and mucinous cystic neoplasm with imaging examples of each. We also discuss high-risk or worrisome imaging features that warrant a referral to a surgeon or endoscopist and provid several examples of these features. These imaging features adhere to the latest guidelines from the International Consensus Guidelines, American Gastroenterological Association (2015), American College of Gastroenterology (2018), American College of Radiology (2010, 2017), and European Guidelines (2013, 2018). Our focused article addresses the imaging dilemma of managing incidental cystic pancreatic lesions, weighing the options between imaging follow-up and aggressive interventions.

Successful removal of remnant cystic duct stump stone using single-operator cholangioscopy-guided electrohydraulic lithotripsy: two case reports

  • Cholecystectomy is the best method for treating gallstone diseases. However, 10%-30% of patients who undergo a cholecystectomy continue to complain of upper abdominal pain, dyspepsia, or jaundice-this is referred to as postcholecystectomy syndrome. Cystic duct stump stones are a troublesome cause of postcholecystectomy syndrome. Conventionally, surgery is mainly performed to remove cystic duct stump stones. However, repeated surgery can cause complications, such as postoperative bleeding, biliary injury, and wound infection. As an alternative method of surgery, endoscopic retrograde cholangiopancreatography is sometimes used to remove cystic duct stump stones, although the success rate is not high due to technical difficulties. Recently, peroral cholangioscopy, which can directly observe the bile duct, has been suggested as an alternative method. We report two cases in which a cystic duct stump stone was successfully removed via a single-operator cholangioscopy, after failure with an endoscopic retrograde cholangiopancreatography.
