A Clinical Case of Systemic Psoriasis with Korean Medical Treatment (전신성 건선 환자의 한방 치료 임상 보고 1례)

  • Objectives : The purpose of this study is to report the Sik-yang-seng(食養生) therapy with Korean Medicine and acupuncture on a chronic psoriasis patient who is occurred to latrogenic cushing's syndrome. Methods : The patient who has been diagnosed psoriasis had symptoms with latrogenic Cushing's syndrome. The patient was treated with Sik-yang-seng(食養生) therapy including Korean Medicine and acupuncture. Evaluation was done by PASI score, itching score, statistics and pictures. Results : After treatment, the PASI score of patients with Sik-yang-seng(食養生) therapy including Korean Medicine and acupuncture was 1.8 which means 95.5% improvement. Conclusions : Korean medical treatment performed with Sik-yang-seng(食養生) therapy can help recovery of Biwi(脾胃)function therefore can increase effect of the treatment of psoriasis and latrogenic Cushing's syndrome.

  • Small cell lung cancer(SCLC) is frequently associated with paraneoplastic syndromes, which occur in approximately 20% of patients at presentation. Clinical Cushing's syndrome secondary to ectopic ACTH production is uncommon, occurring in approximately 5% of all SCLC patients. However, biochemical evidence of hypercortisolism can be detected in up to 50% of patients. Patients with Cushing's syndrome from ectopic ACTH production show hypertension, weakness, hyperglycemia, and hypokalemic metabolic alkalosis, but differ from patients with classic Cushing's disease in that symptoms develop more rapidly. Ectopic ACTH production is associated with a poor response to chemotherapy, short survival, and a high risk of treatment-related complications. We report a case of Cushing's syndrome associated with ectopic corticotropin production in 59-year-old male patient with extensive stage of SCLC.

  • Objectives : The purpose of this study is to report the improvement of a patient diagnosed as iatrogenic Cushing syndrome treated with herbal medicine. Methods : We treated a patient diagnosed as iatrogenic Cushing syndrome with s Wiryeong-tang gagam and Sinbeopbanha-tang for 10days. While being treated with herbal medicine, the patient got acupuncture treatment two times a day. Results : After treatment, weakness of lower limb and tremor got improved. Vomiting and disgusting also got better. Conclusion : Using herbal medicine may improve various symptoms of iatrogenic Cushing syndrome effectively.

  • Thymic carcinoid tumor associated with Cushing's syndrome is a rare disease with a poor prognosis. Thymic carcinoid with Cushing's syndrome caused by CRH (corticotropin-releasing hormone) production is even rarer. We report a 58-year-old woman with a huge anterior mediastinal mass. Five months after thymectomy the patient was readmitted with symptoms of generalized edema and dyspnea. Recurrence and metastases were discovered and Cushing's syndrome diagnosed.

  • Neuroendocrine tumor in thymus is rare and has poor prognosis due to frequent recurrence and distant metastasis. Approximately half of thymic carcinoids are hormonally active and Cushing\`s syndrome is seen in 33% of affected patients. Treatment of choice is surgical excision of tumor and role of chemotherapy and radiotherapy is controversal. We report 2 cases of thymic neuroendocrine carcinoma associated with Cushing\`s syndrome.

The Clinical Usefulness of NP-59 Scintigraphy in Adrenal Cortical Diseases (NP-59 부신 신티그라피의 임상적 유용성에 관한 연구)

  • $^{131}I-6{\beta}$-iodomethyl-19-norcholesterol(NP-59) has an advantage to assess adrenal dysfunction caused by adrenal cortical disorders. The aim of this study is to evaluate the clinical usefulness of NP-59 scintigraphy in each adrenal disease. Ten patients who did eleven NP-59 adrenal scintigraphies at Dong-A University Hospital from March 1990 to December 1996 were selected as the subject. Among the subject there were 5 cases of Cushing's syndrome, 2 cases of incidentaloma, 1 case of metastatic adrenal tumor, liver cirrhosis with hirsutism and hypertension respectively. Among 5 cases of Cushing's syndrome, there were 2 cases of Cushing's disease, 2 cases of adrenal adenoma and 1 case of adrenal carcinoma. There are no disagreement between clinical diagnosis and scan finding in Cushing's syndrome. In 2 incidentaloma cases, even though one is interpretated as a functioning tumor, both of 2 cases could avoid unnecessary biopsy according to scintigraphy result. One case of hirsutism, clinically adrenal originated, revealed the normal scintigraphic finding after dexamethasone suppression scan. It could suggest that the etiology of hirsutism was extra-adrenal origin. One case of hypertension took the study to exclude the possibility of primary aldosteronism. Normal suppression scan finding revealed that primary aldosteronism did not exist in this case. In conclusion, NP-59 scintigraphy was very useful in diagnosis and differential diagnosis of Cushing's syndrome and it could avoid unnecessary biopsy in the incidental adrenal tumor.

Histone deacetylase inhibition attenuates hepatic steatosis in rats with experimental Cushing's syndrome

  • Cushing's syndrome (CS) is a collection of symptoms caused by prolonged exposure to excess cortisol. Chronically elevated glucocorticoid (GC) levels contribute to hepatic steatosis. We hypothesized that histone deacetylase inhibitors (HDACi) could attenuate hepatic steatosis through glucocorticoid receptor (GR) acetylation in experimental CS. To induce CS, we administered adrenocorticotropic hormone (ACTH; 40 ng/kg/day) to Sprague-Dawley rats by subcutaneous infusion with osmotic mini-pumps. We administered the HDACi, sodium valproate (VPA; 0.71% w/v), in the drinking water. Treatment with the HDACi decreased steatosis and the expression of lipogenic genes in the livers of CS rats. The enrichment of GR at the promoters of the lipogenic genes, such as acetyl-CoA carboxylase (Acc), fatty acid synthase (Fasn), and sterol regulatory element binding protein 1c (Srebp1c), was markedly decreased by VPA. Pan-HDACi and an HDAC class I-specific inhibitor, but not an HDAC class II a-specific inhibitor, attenuated dexamethasone (DEX)-induced lipogenesis in HepG2 cells. The transcriptional activity of Fasn was decreased by pretreatment with VPA. In addition, pretreatment with VPA decreased DEX-induced binding of GR to the glucocorticoid response element (GRE). Treatment with VPA increased the acetylation of GR in ACTH-infused rats and DEX-induced HepG2 cells. Taken together, these results indicate that HDAC inhibition attenuates hepatic steatosis through GR acetylation in experimental CS.

  • Even though nocardiosis is one of opportunistic infections, and can occur in Cushing's syndrome, it rarely occurs in patients with Cushing's disease. Herein, a case with Cushing's disease in whom nocardiosis had manifested as a pulmonary lesion, which after percutaneous needle aspiration, empyema and a breast abscess were also noted.

  • While the rising epidemic of obesity is primarily attributed to a sedentary lifestyle, poor dietary habits, and the aging of the population, secondary causes of obesity generally go undetected and untreated. These include endocrinological disorders, such as Cushing's syndrome, polycystic ovary syndrome, hypothalamic obesity, hypogonadism, and hypothyroidism, as well as genetic, syndromic, and drug-related obesity. We present an overview of the major disorders associated with obesity, highlighting the pathophysiologic mechanisms and discussing the diagnostic and treatment strategies that are most helpful to practicing physicians in recognizing and treating these generally under-detected and undertreated disorders.
