  • This paper presents an optimal design for the SPECT reconstruction filter, based on a physical limit of SPECT lesion detection capability. To increase the performance of the filter on lesion detectability, the filter design was focused on increasing the local SyW ratio of a threshold lesion, that was determined by minimum detectable lesion size (MDU) from SPECT lesion detectabllity contrast-detail curve. The proposed filter showed flexible window characteristics of resolution recovery and noise smoothing for MDLSs in the resolution-limited and photon-limited regions, respectively, compennting for the relative impact of the main limitation factors on threshold detectability. The simulated results showed good adaptability of the proposed filter to the changes in physical parameters of photon counts, object contrast, and detector system resolution.

Structural Dynamic System Reconstruction for Modal Parameter Estimation

  • We as modal parameter estimation technique by developing a residual based system reconstruction and using the system matrix coordinate transformation. The modal parameters can be estimated from and residues of the system transfer functions expressed in modal coordinate basis, derived from the state space system matrices. However, for modal parameter estimation of multivariable and order structural systems over broad frequency bands, this non-iterative algorithm gives high accuracy in the natural fre- and damping ratios. From vibration tests on cross-ply and angle-ply composite laminates, the natural frequencies and damping ratios on be estimated using tile coordinates of the structural system reconstructed fro the experimental frequency response. These results are compared with those of finite element analysis and single-degree-of-freedom curve-fitting.

Anatomic reconstruction for acromioclavicular joint injuries: a pilot study of a cost-effective new technique

  • Background: The treatment for acromioclavicular joint injuries (ACJI) ranges from a conservative approach to extensive surgical reconstruction, and the decision on how to manage these injuries depends on the grade of acromioclavicular (AC) joint separation, resources, and skill availability. After a thorough review of the literature, the researchers adopted a simple cost-effective technique of AC joint reconstruction for acute ACJI requiring surgery. Methods: This was a prospective single-center study conducted between April 2017 and April 2018. For patients with acute ACJI more than Rockwood grade 3, the researchers performed open coracoclavicular ligament reconstruction using synthetic sutures along with an Endobutton and a figure of 8 button plate. This was followed by AC ligament repair augmenting it with temporary percutaneous AC K-wires. Clinical outcomes were evaluated using the Constant Murley shoulder score. Results: Seventeen patients underwent surgery. The immediate postoperative radiograph showed an anatomical reduction of the AC joint dislocation in all patients. During follow-up, one patient developed subluxation but was asymptomatic. The mean follow-up period was 30 months (range, 24-35 months). The mean Constant score at 24 months was 95. No AC joint degeneration was noted in follow-up X-rays. The follow-up X-rays showed significant infra-clavicular calcification in 11 of the 17 patients, which was an evidence of a healed coracoclavicular ligament post-surgery. Conclusions: This study presents a simple cost-effective technique with a short learning curve for anatomic reconstruction of acute ACJI. The preliminary results have been very encouraging.

수축을 이용한 파이프 곡면의 복원 (Pipe Surface Reconstruction Using Shrinking)

  • 구조화되어있지 않은 점 집합으로부터 파이프 곡면을 복원해 내는 알고리즘을 기술한다. 파이프 곡면은 척추 곡선과 스위핑 구의 반지름으로 정의된다. 수축과 동적 최소자승법에 의해 점 집합은 얇은 곡선 형태로 변환된다. 이 얇은 곡선 형태의 점 집합은 쉽게 척추곡선으로 근사될 수 있다. 얇은 점 집합과 원래 주어진 점 집합의 대응하는 두 점간의 거리로 파이프 곡면을 정의하는 스위핑 구의 반지름을 구할 수 있다.

곡선 부-분할 보간과 Neighbor Embedding 기반의 복합 초고해상도 기법 (Hybrid Super Resolution Based on Curve Subdivision Interpolation and Neighbor Embedding)

  • Curve subdivision interpolation reconstructs edge well with low complexity, however it lacks of ability to recover texture components, instead. While, neighbor embedding is superior in texture reconstruction. Therefore, in this paper, a novel Super Resolution technique which combines curve subdivision interpolation and neighbor embedding is proposed. First, edge region and non-edge regions are classified. Then, for edge region, the curve subdivision algorithm is used to make two polynomials derived from discrete pixels and adaptive weights are adapted for gradients of 4 different sides to make smooth edge. For non edge region, neighbor-embedding method is used to conserve texture property in original image. Consequently results show that the proposed technique conserves sharp edges and details in texture better, simultaneously.

혀의 재건을 위한 부피 및 표면적의 측정 (Estimation of Volume and Surface Area for Reconstruction of Tongue)

  • Purpose : Anterolateral thigh and radial forearm flap is the most important fasciocutaneous flap widely used for reconstruction of tongue. One important purpose of flap is replacing the volume of tongue but still there is no data about the surface area and volume to be reconstructed after glossectomy. In this paper, surface area and volume is estimated from the 3-dimensionally reconstructed MRI images to see which flap is more ideal and to give the reference value for reconstruction. Materials and Methods : With coronal MRI image, tongue including only the intrinsic muscle is delineated in every section and reconstructed 3-dimensionally and calculated the volume and surface area to be reconstructed according to the degree of glossectomy. This volume and surface area was compared with the volume of anterolateral thigh and radial forearm flap. Results : The volume and surface area to be reconstructed in hemiglossectomy was $39.0{\pm}4.0cm^3$ and $31.8{\pm}2.7cm^2$ respectively. The average thickness of anterolateral thigh flap is $9.4{\pm}2.8mm$ and that of radial forearm is $3.8{\pm}1.0mm$. Comparing the curve of tongue surface area and volume with the volume of flap, the anterolateral thigh flap has more ideal volume to replace the defect. Conclusions : The surface area and volume requested for reconstruction could be suggested and the anterolateral thigh flap has more ideal volume for reconstruction of glossectomy defect.

The Importance of a Conchal Bowl Element in the Fabrication of a Three-Dimensional Framework in Total Auricular Reconstruction

  • Background To construct a sophisticated three-dimensional framework, numerous modifications have been reported in the literature. However, most surgeons have paid little attention to the anatomical configuration of the concha and more to its deepness and hollowness, leading to unsatisfactory outcomes. Methods For a configuration of the concha that is definitely anatomical, the author further developed and employed the conchal bowl element, which has been used by several surgeons although the results have not been published elsewhere. The author constructed the conchal bowl element in one of three patterns according to the amount of available cartilages: one block, two-pieces, or a cymba bowl element only. A total of 20 patients underwent auricular reconstruction using a costal cartilage framework between 2009 and 2012. The 8 earliest reconstructions were performed without a conchal bowl element and the latter 12 with a conchal bowl element. The patients were followed up for more than 1 year. The aesthetic results were scored by evaluating characteristics involving the stability of the crus helicis, the conchal definition, and the smoothness of the helical curve. Results The ears reconstructed early without a conchal bowl element showed a shallow and one or two incompletely separated concha with an obliterated cymba conchal space. They also did not have a realistic or smooth curve of the helix because of an unstable crus helicis. However, ears reconstructed later with the concha bowl element showed a definite crus helicis, deep cymba conchal space, and smooth helical curve. Conclusions The construction of the conchal bowl element is simple, not time-consuming procedure. It is suggested that the conchal bowl element must be constructed and attached to the main framework for natural configuration of the reconstructed ear.

The supraclavicular artery island flap: a salvage option for head and neck reconstruction

  • Background: Some of head and neck cancer patients are in compromised general condition after ablation surgery and chemoradiation therapy, which makes secondary free tissue transfer quite challenging. Elderly cancer patients also have some risk for microvascular surgery with lengthened general anesthesia. In those cases, the pedicled flap vascularized by supraclavicular artery could be considered as an alternative to free flap. Despite several authors have demonstrated the clinical reliability of supraclavicular artery island flap (SCAIF), to date, SCAIF has not been widely used among reconstructive surgeon. In this article, we clarified vascular flow pattern and introduce simple surgical technique of SCAIF with a literature review. Case presentation: Three patients who had underwent previous neck surgery and adjuvant therapy received maxillofacial reconstruction using SCAIF. It required only a few landmarks, flap harvesting was carried out, and the elapsed time gradually decreased to 15 min with experiences. There were no remarkable morbidities in both donor and recipient sites. Conclusion: SCAIF exhibited minimal anatomic variations and short learning curve of surgical techniques, which might be valuable reconstruction modality for beginning surgeon. And it can be beneficial option for the patients with vessel-depleted neck, medically compromised status for lengthened general anesthesia and failed free tissue transfer.

A New Method for Reconstruction of Smooth Branching Surface from Contours

  • A new algorithm has been developed to construct surface from the contours having branches and the final smooth surface is obtained by the reversible Catmull-Clark subdivision. In branching, a particular layer has more than one contour that correspond with at least one contour at the adjacent layer. In the next step, three-dimensional composite curve is constructed from contours of a layer having correspondence with at least one contour at the adjacent layer by inserting points between them and joining the contours. The points are inserted in such a way that the geometric center of the contours should merge at the center of the contours at the adjacent layer. This process is repeated for all layers having branching problems. Polyhedra are constructed in the next step with the help of composite curves and the contours at adjacent layer. The required smooth surface is obtained in the proposed work by providing the level of smoothness.
