Six major outbreaks of highly pathogenic avian influenza (HPAI) occurred from 2003 to 2016 in Korea. Epidemiological investigations of each outbreak revealed that migratory birds were the primary source of the HPAI virus. During the last five years, the geographic transmission pattern of domestic HPAI seems to have extended from local to nationwide; therefore, it is necessary to identify specific locations in which poultry farms are at elevated risk for HPAI outbreak to enable targeted surveillance and other mitigation strategies. Here, a geographical information system (GIS)-based analysis was used to identify geographic areas at high risk for future HPAI incidents in Korea based on historical outbreak data collected between December 2003 and April 2016. To accomplish this, seven criteria were used to identify areas at high-risk for HPAI occurrence. The first three criteria were based on defined spatial criteria buffering of 200 bird migration sites to some defined extents and the historical incidence of HPAI outbreaks at the buffering sites. The remaining criteria were based on combined attribute information such as number of birds or farms at district levels. Based on the criteria established for this study, the most-likely areas at higher risk for HPAI outbreak were located in Chungcheong, Jeolla, Gyeonggi, and Gyeongnam provinces, which are densely populated poultry regions considered major poultry-production areas that are located along bird migration sites. The proportion of areas at risk for HPAI occurrence ranged from 4.5% to 64.9%. For the worst criteria, all nine provinces, including Jeju Island, were found to be at risk of HPAI. The results of this study indicate that the number of poultry farms at risk for HPAI outbreaks is largely underestimated by current regulatory risk assessment procedures conducted for biosecurity authorization. The HPAI risk map generated in this study will enable easy use of information by policy makers to identify surveillance zones and employ targeted surveillance to reduce the impact of HPAI transmission.
Gastric cancer (GC) is the fourth most prevalant cancer and the second leading cause of cancer-related mortality worldwide. As in other cancers gastric carcinogenesis is multifactorial involving environmental, genetic and epigenetic components. Epigenetic silencing due to hypermethylation of tumour suppressor genes is one of the key events in gastric carcinogenesis. This study was aimed to analyse the hypermethylation status of the E-Cadherin (CDH1) gene promoter in GCs in the ethnic Kashmiri population. In this study a total of 80 GC patients were recruited. Hypermethylation in tumour tissue was detected by methylation specific PCR (MS-PCR). Hypermethylation of CDH1 promoter was observed in 52 (65%) of gastric carcinoma cases which was significantly much higher than adjacent normal tissue [$p{\leq}0.0001$]. Further the frequency of CDH1 promoter methylation was significantly different with intestinal and diffuse types of gastric cancer [55.7% vs 82.1%; p<0.05]. Moreover females and cases with lymph node invasion had higher frequencies of CDH1 hypermethylation [$P{\leq}0.05$]. Thus the current data indicate a vital role of epigenetic alteration of CDH1 in the causation and development of gastric cancer, particularly of diffuse type, in our population.

There is growing evidence that soy isoflavone play beneficial roles in the prevention of chronic diseases such as hormone dependent cancers, cardiovascular diseases, osteoporosis and also reduced incidence of menopausal symptoms. However current data are not sufficient to determine the effective doses for beneficial as well as harmful effects and to support dietary recommendation for isoflavones. Since soy products containing isoflavone are one of the common food items that Koreans consume daily basis, assessing consumption of soy isoflavone by Koreans will give a valuable information on making dietary recommendations for isoflavones. the present study was designed to assess dietary intake of isoflavone of Korean middle-aged women who might receive the most beneficial effects from isoflavone consumption thereby prevent post menopausal related symptoms and diseases. In this study isoflavone contents of soy products were analyzed and soy products and isoflavone consumption of these subjects were assessed by a self-reported dietary questionnaire and isoflavone intake data base. Subjects were consisted of 178 Korean women (35~60 y) who live in urban and rural area. All subjects provided detailed information on demographics, anthropometry, health history, menopausal symptoms, health history, menopausal symptoms, and dietary intake using food frequency questionnaire (FFQ) which was developed for Koreans. The average age of the subjects was 46.6 years. The soy products which the subjects frequently consumed were soybean paste soup, braised fried soybean, tofu residue stew, tofu, soy milk, natto stew, bean sprouts, and soybean broth which contained 10.68 mg, 3.34 mg, 2.44 mg, 2.42 mg, 2.42 mg, 1.12 mg, 1.02 mg, 0.33 mg of isoflavone per 100 g, respectively. The estimated daily intake (EDI) of isoflavones ranged from 0 to 144.3 mg, and the mean daily isoflavone (sum of daidzein and genisten) intake of the subjects was 24.41 mg.
In this study, the generating of torque regarding the Control Moment Gyro (CMG) is proportional to the angular velocity of gimbal. This is the case because gimbal affects the attitude control of the satellite directly, and it is necessary to reduce the incidence of torque ripple of gimbal. In this paper, the cause of the torque ripple of gimbal is reviewed and mathematically modeled by assuming the friction imbalance of bearing, the magnetic field and the phase current imbalance of the motor. We are able to confidently estimate the modeling parameters of gimbal disturbance using a constant speed test, and then analyze the influence of applying feedforward control to our modeling. Additionally, the simulation results show that the torque ripple and angular velocity fluctuations are reduced when apply this modeling to the identified study parameters. Finally, we present the disturbance reduction technique using our disturbance modeling.

We report an in-depth analysis of the design and fabrication of multilayer dielectric (MLD) diffraction gratings for spectral beam combining at a wavelength of 1055 nm. The design involves a near-Littrow grating and a modal analysis for high diffraction efficiency. A range of wavelengths, grating periods, and angles of incidence were examined for the near-Littrow grating, for the $0^{th}$ and $-1^{st}$ diffraction orders only. A modal method was then used to investigate the effect of the duty cycle on the effective indices of the grating modes, and the depth of the grating was determined for only the $-1^{st}$-order diffraction. The design parameters of the grating and the matching layer thickness between grating and MLD reflector were refined for high diffraction efficiency, using the finite-difference time-domain (FDTD) method. A high reflector was deposited by electron-beam evaporation, and a grating structure was fabricated by photolithography and reactive-ion etching. The diffraction efficiency and laser-induced damage threshold of the fabricated MLD diffraction gratings were measured, and the diffraction efficiency was compared with the design's value.

BACKGROUND/OBJECTIVES: Due to the rapid increase of global cancer incidence and mortality and a high level of interest in cancer prevention, a systematic review of garlic intake and cancer risk is needed. SUBJECTS/METHODS: We implemented a systematic review to examine the effects of varying levels of garlic intake on cancer. We conducted comprehensive literature searches in three electronic databases (MEDLINE, Embase, and Web of Science) for studies published between database inception and July or September of 2018. Two investigators independently screened abstracts and full-texts, extracted data, and assessed risk of bias (RoB). A total of one medium-quality randomized controlled trial (RCT) and 13 cohort studies graded as high RoB were included. RESULTS: The 1-year follow-up results from a RCT showed that a significant decrease in the number and size of colorectal adenomas among participants with colorectal adenomas who received high-dose aged garlic extract (AGE) compared with those who received low-dose AGE (P < 0.05). The results of prospective observational studies provided inconsistent associations of colorectal cancer risk with garlic supplements and garlic intake as food. CONCLUSIONS: In summary, the AGE was effective in reducing the number and magnitude of colorectal adenomas in one RCT, but there were inconsistent associations between garlic intake and colorectal cancer in cohort studies. Therefore, we could not draw a firm conclusion regarding the effects of garlic on cancer, because the current strength of evidence is inadequate due to a lack of number of high-quality RCTs.
The study aims to examine the current status and differences in the burden of disease in Korea during 2008-2018. We calculated the burden of disease for Koreans from 2008 to 2018 using an incidence-based approach. Disability adjusted life years (DALYs) were expressed in units per 100 000 population by adding years of life lost (YLLs) and years lived with disability (YLDs). DALY calculation results were presented by gender, age group, disease, region, and income level. To explore differences in DALYs by region and income level, we used administrative district and insurance premium information from the National Health Insurance Service claims data. The burden of disease among Koreans showed an increasing trend from 2008 to 2018. By 2017, the burden of disease among men was higher than that among women. Diabetes mellitus, low back pain, and chronic lower respiratory disease were ranked high in the burden of disease; the sum of DALY rates for these diseases accounted for 18.4% of the total burden of disease among Koreans in 2018. The top leading causes associated with a high burden of disease differed slightly according to gender, age group, and income level. In this study, we measured the health status of Koreans and differences in the population health level according to gender, age group, region, and income level. This data can be used as an indicator of health equity, and the results derived from this study can be used to guide community-centered (or customized) health promotion policies and projects, and for setting national health policy goals.
Objectives: Patients with respiratory diseases are increasing as air pollution due to fine dust gets worse. Diseases that occupy a large proportion of respiratory diseases in medical institutions are acute bronchitis, chronic bronchitis and Allergic rhinitis. The number of patients with all three diseases is gradually increasing. This study was to suggest assignment of medicine policy for improving accessibility to Korean medical treatment of respiratory diseases analyzing the current treatment status of acute bronchitis, Chronic bronchitis and Allergic rhinitis patients in Western medical and Korean medical institutes in this situation. Methods: This study used 2017 National patient sample data from the Korean Health Insurance Review and Assessment Service for research. Acute bronchitis, Chronic bronchitis, Allergic rhinitis was according to KCD code. This research contains Socio-demographic analysis classified by sex and age, the number of three disease's patients. Also the number of medical treatment, the expense of recuperation cost, medical practices were analyzed compare with Western and Korean medicine. Results: The incidence of three diseases is higher among female than male. There are many patients under the age of 10 in the case of acute bronchitis and allergic rhinitis, while there are many patients over 50 years of age in chronic bronchitis. Western medical treatment take up a larger proportion than Korean medical treatment in part of the number of three disease's patients, the number of medical treatment, the expense of recuperation cost. Conclusions: Much more patients of these diseases visit in Western medical clinic and hospital than Korean medical clinic and hospital for treatment. There are many parts of Korean medical treatment that are not covered by Health insurance benefits like herbal decoction, pharmacopuncture, etc. Korean medicine need to do the efforts for expanding medical field in variety. Also it is demanded for institutional support for reduction of the cost burden and improving on accessibility of Korean medical treatment in order to treat with the increase in respiratory diseases due to fine dust.

For the spatiotemporally aligned observation of photothrombosis induction and transient variations of in vivo brain stroke, we developed a novel photothrombosis inducing system compatible to a magnetic resonance imaging (MRI) system using nonmagnetic stereotaxic equipment. From the spatial point of view, the system provides a more reliable level of reproducibility of the photothrombosis in each brain. From the temporal point of view, from T1- and T2-weighted in vivo MR (magnetic resonance) images, the transient variations such as incidence, location, and size of the thrombosis are measured quantitatively. In addition, the final variation is observed in the ex vivo brain by TTC (Triphenyltetrazolium chloride) staining based on histological assay and utilized for the verification of the MR images. From the experimental result of the rat brain, the proposed system shows more reliable characteristics for transient variations of brain strokes.
