Comparing Changes in Knee Muscle Strength after Reconstruction of the Anterior and Posterior Cruciate Ligaments

  • Purpose: The purpose of this study was to identify changes in knee muscle strength after reconstruction of the anterior cruciate ligament (ACL) and the posterior cruciate ligament (PCL). Methods: Thirteen subjects (males) with anterior ligament injury and ten subjects (males) with posterior ligament injury voluntarily participated in this study. Both groups were evaluated at the pre-and post-reconstruction stages using an isokinetic dynamometer. Peak torque, total work, and the hamstrings to quadriceps (H/Q) peak torque ratio were calculated at angular velocities of 60°/sec and 180°/sec. Statistical analysis was conducted on SPSS 18.0 for Windows using t-tests to compare mean differences. Results: At an angular velocity of 60°/sec, both the ACL and PCL groups showed a significant increase in muscle strength in the flexors and extensors. Muscle strength in the extensors was significantly increased in the PCL group compared to the ACL group. At an angular velocity of 180°/sec, the ACL group showed a significant increase in muscle endurance in the flexors and extensors, and the PCL group showed a significant increase in muscle endurance in the flexors. At angular velocities of 60°/sec and 180°/sec, the H/Q peak torque ratio increased in the ACL group but decreased in the PCL group. Consequently, the H/Q peak torque ratio was significantly different for the two groups. Conclusion: The results suggest that the patients with ACL injury should focus on strengthening the knee extensors and that the patients with PCL injury need to strengthen the knee flexors.

  • This paper studied the influences of the main design parameter-the expansion angle and the material properties of the self-expansion anterior cruciate ligament fixation device on the contact condition with the bone and the initial stability of the device. Using finite element analysis, the stress distributions of the ring part of the device and the wall of the bone tunnel were calculated. And the micro-migration of the device by the pull-out force was calculated. From the analysis results, it was found that when designing the self-expansion type anterior cruciate ligament fixation device, it is desirable to use the material having higher Young's modulus and to design the fixation device that all wedges uniformly maintain contact with bone to obtain initial stability after operation.

  • The isometric area of the anterior cruciate ligament was calculated during knee flexion-extension. Flexion-extension motion data of the joint were obtained using Fastrak and a three-dimensional motion measurement system. A total of five subjects were seated on a flat table and the tibia sensor position was measured with the femur fixed on the table. A three-dimensional knee model was constructed using a graphic tool to simulate the knee motion. Twenty seven positions of the tibia region and forty two positions of the femur region were selected and the distances between the determined tibial and femoral points were calculated. Highly isometric areas were found and displayed as three dimensional aspects.

Flexion-extension motion data of the knee joint were obtained by Fastrak, a three-dimensional motion measurement system. A subject was seated on a flat table and the tibia sensor position was measured with the femur fixed at the table. A three-dimensional knee model was constructed using a graphic tool to simulate the knee motion. Three surgical positions of the femoral tunnel were selected and the distances between the determined tibial tunnel and each femoral tunnel were calculated. The maximum elongation position was found to be in the ten thirty direction of clock.

Telos Device를 이용한 전방십자인대 Lachman 검사 시 무릎두께와 활성도에 따른 인가압력에 대한 고찰 (A Study of Applied Pressure According to Knee variable During Lachman Test for Anterior Cruciate Ligament using Telos Device)

  • 본 논문에서는 Telos Device를 이용한 검사 시 인가한 daN 힘의 차이와 무릎의 안정성에 관여하는 근육들의 개인차에 따른 전방십자인대의 동요 정도에 대해 연구하고자 한다. Telos Device를 이용한 Lachman Test로 검사하였으며, 오른쪽과 왼쪽을 각각 0, 15, 30 daN, 운동부하 후 30 daN의 힘으로 변화하며 검사하여 전방십자인대의 변화를 측정하였다. 근육량 측정은 CT(Computed Tomography)를 이용하였으며, 힘줄(tendon)을 피해 근육을 측정하기 위해 무릎뼈(patella) 상방 8 cm의 위치를 검사하였다. 전방십자인대(Anterior Cruciate Ligament)의 변화 정도는 Piview의 Caliper 기능을 이용하여 측정하였으며, 근육량은 CT 단면 영상을 Image J 프로그램을 이용하여 지방을 제외한 뼈와 근육의 면적을 측정하였다. 측정값들은 SPSS Ver. 18을 이용하여 결과 값을 얻었다. Telos Device 로 인가하는 힘의 변화에 따라 전방십자인대의 동요 정도를 측정한 결과 오른쪽과 왼쪽 모두 15 daN과 30 daN의 힘을 인가하였을 때 통계적으로 유의한 무릎 동요의 차가 발생하였으며(p<0.05), 30 daN과 운동부하 후 30 daN에서의 동요 정도는 유의한 차이를 보이지 않았다(p>0.05). 근육면적의 차이와 전방십자인대의 동요 정도와의 상관관계에서 15 daN의 압력에서 오른쪽 r=-0.45, 왼쪽 r=0.44로 음의 상관관계를 보였다(p<0.05). Telos Device에 인가하는 힘의 정도에 따라 전방십자인대의 동요는 커지는 것으로 분석되며, 또한 개인의 근육량의 차이에 따라 전방십자인대의 동요 정도에도 차이를 보이는 것으로 분석된다. 따라서 개인의 근육량의 차이에 따라 Telos Device에 인가되는 힘도 변화되어야 하는 것으로 사료된다.

  • The anterior cruciate ligament (ACL) is liable to a major injury that often results in a functional impairment requiring surgical reconstruction. The success of reconstruction depends on such factors as attachment positions, initial tension of ligament and surgical methods of fixation. The purpose of this study is to find isometric positions of the substitute during flexion/extension. The distance between selected attachments on the femur and tibia was computed from a set of measurements using a 6 degree-of-freedom magnetic sensor system. A three-dimensional knee model was constructed from CT images and was used to simulate length change during knee flexion/extension. This model was scaled for each subject. Twenty seven points on the tibia model and forty two points on the femur model were selected to calculate length change. This study determined the maximum and minimum distances to the tibial attachment during flexion/extension. The results showed that minimum length changes were $1.9{\sim}5.8mm$ (average $3.6{\pm}1.4mm$). The most isometric region was both the posterosuperior and anterior-diagonal areas from the over-the-top. The proposed method can be utilized and applied to an optimal reconstruction of ACL deficient knees.
