A Clinical Research Analysis of the Korean Medicine for Cruciate Ligament Injury (십자인대 손상에 대한 국내·외 한의치료 고찰)

  • Objectives The objective of the study is to review the Korean traditional medicine treatment used in cruciate ligament injury. Methods According to patients, treatment methods, research design, evaluation tools, outcome, we searched for studies which performed Korean traditional medical treatment on patient with cruciate ligament injury. Results In 11 studies, total number of patents were 78. Patients were conducted with 11 kinds of treatment, which is acupuncture, herbal medicine, pharmacopuncture, electro-acupuncture, burning acupuncture, moxibustion, physiotherapy, exercise, Chuna and cupping. The most commonly used measurement was visual analog scale. Conclusions Throughout the study, we could organize Korean traditional medicine treatment for patient with cruciate ligament injury. However it is difficult to figure out among the 11 intervention which interventions were effective in improving symptom. Therefore, further researches like randomized clinical trial and systematic review is needed in order to enhance the evidence of the Korean traditional medicine.

Symptomatic Mid-Substance Posterior Cruciate Ligament Calcification of the Knee Joint (증상을 동반한 슬관절의 후방십자인대 실질부에 발생한 석회화)

  • Calcification in the tendon is a common disease especially in the rotator cuff. The condition can be treated conservatively in the chronic stage, but surgically in the acute stage. On the other hand there are few reports of calcification in the cruciate ligament of the knee joint, especially calcification of the posterior cruciate ligament. A 51-year-old patient with symptomatic calcification in the mid-substance of the posterior cruciate ligament was treated conservatively. The symptoms did not improve after eight months of conservative treatment, so arthroscopic surgery was performed. During arthroscopic removal of the calcification, the ligament was damaged beyond preservation. Eventually a posterior cruciate ligament reconstruction was performed. At two years after surgery, radiography confirmed no recurrence of the calcification, and the patient was free. We report this case study with a review of the relevant literature.

Compartment Syndrome and Deep Vein Thrombosis after Repetitive Posterior Cruciate Ligament Reconstruction (반복된 후방 십자 인대 재건술 후 발생한 구획 증후군 및 심부 정맥 혈전증)

  • Posterior cruciate ligament reconstructions are dangerous procedure in terms of neurovascular complication. But deep vein thrombosis and associated compartment syndrome after posterior cruciate ligament reconstruction has not been reported, yet. We have experienced a patient who developed a deep vein thrombosis and compartment syndrome after revision posterior cruciate ligament reconstructive surgery, and report the importance of diagnosis and management with the discussions.

  • Objectives : The objective of this study is to report the effect of Koran medical treatment and CPM exercise on recovery after reconstruction surgery of torn anterior cruciate ligament Methods : A 27-year-old male patient who had anterior cruciate ligament reconstruction surgery was treated by Korean medical treatment and CPM exercise from March 24th 2015 to April 16th 2015. Results : After treatments, VAS were generally decreased, ROM of Lt. knee got better from $95^{\circ}/10^{\circ}$(flexion/extension) to $120^{\circ}/-5^{\circ}$ (flexion/extension), Lysholm Score increased from 26 to 63. Conclusions : This study showed that Korean medical treatments and CPM exercise has meaningful effect on recovery after reconstruction surgery of anterior cruciate ligament. And it is considered that we need to apporoach recovery of these kinds of operation with more active Korean medical treatment, and further researches should be done steadily.

Effects of Gymnasts Shoes on Risk Factors of Anterior Cruciate Ligament Injuries during Drop Landing in Female Gymnasts (여자 체조선수들의 드롭 랜딩 시 체조화 착용유무가 전방십자인대 부상 위험요인에 미치는 영향)

  • The purpose of the study was to investigate the effects of gymnasts shoes on risk factors of anterior cruciate ligament injuries during drop-landing followed by vertical jump in female gymnasts. Thirteen female gymnasts were recruited and performed randomly drop-landing followed by vertical jump in height of her knee with and without shoes. Kinematics and ground reaction data were collected to estimate the anterior cruciate ligament injuries risk factors. Data were analyzed with paired samples t-test with Bonferroni correction. Female gymnasts with shoes showed more reduced thigh maximum adduction angle, and knee maximum extension moment than without shoes. Female gymnasts with shoes showed more increased shank maximum abduction angle than without shoes. In conclusion, Female gymnasts with shoes reduced anterior cruciate ligament injuries risk factors.

CORA Based Leveling Osteotomy with Tibial Tuberosity Transposition for Cranial Cruciate Ligament Rupture with Concurrent Medial Patellar Luxation in Two Small Breed Dogs

  • A 5-year-old 6 kg male mongrel (case 1) and a 7-year-old 4.3 kg male yorkshire terrier (case 2) were presented to Chonbuk animal medical center (CAMC). Both animals had non-weight bearing hind limb lameness. Case 1 had complete rupture of cranial cruciate ligament with grade 3 medial patellar luxation. Case 2 had complete cranial cruciate ligament rupture with grade 4 medial patellar luxation. During surgery, in both cases, trochlear block recession was performed followed by CORA based leveling osteotomy (CBLO) and tibial tuberosity transposition (TTT). General soft tissue reconstructions for medial patellar luxation including medial releasing and lateral imbrication were also performed. Postoperatively, both animals demonstrated excellent recovery and regained normal weight bearing of the affected hind limb without any recognizable complication. CBLO followed by TTT can be a curative surgical option without complications in cases of cranial cruciate ligament rupture with high-grade medial patellar luxation in small breed dogs.

Effectiveness of Oriental Medical Therapy and Bongchuna on Anterior Cruciate Ligament Rupture of Knee, Two Case Reports (슬관절의 전방십자인대파열에 대한 한방치료와 봉추나의 치료효과 증례 2)

  • Anterior cruciate ligament tear is a common disease of knee injury. We report 1 case of anterior cruciate ligament complete tear diagnosed by MRI(magnetic resonance imaging) imaging and 1 case after reconstruction. Outcomes were measured by visual analogue scale(VAS) and Lysholm knee scoring scale. We applied acupuncture treatment, bee venom injection, bongchuna and prescribed herbal medication. Patients who treated by oriental medical treatments had a significant effect on the pain decrease and range of motion and knee function. Oriental medical treatments are very useful on pain reduction and prevention of muscle contraction, leading to satisfied rehabilitation, as wee as diminishing recurrence after operation.

  • The giant cell tumor of tendon sheath is very rarely present inside the knee joint. The authors report a case of intraarticular giant cell tumor of tendon sheath arising from posterior cruciate ligament which was successfully excised arthroscopically using posterior trans-septal portal at the time of arthroscopic reconstruction of anterior cruciate ligament.

  • This is a case of the core reamer breakage during anterior cruciate ligament reconstruction using bone-patellar tendon-bone autograft. A 19-year-old man with acute anterior cruciate ligament rupture had reconstruction surgery. During tunneling in the proximal tibia, the core reamer have been cracked open and broken. Rest of the procedure was performed routinely except the tibial sided fixation of bone block which was performed with larger interference screw than usual. The patient followed the accelerated rehabilitation program. The result was satisfactory at 2 years after surgery.

  • Localized form of tenosynovial giant cell tumor rarely affects the knee joint, only a few cases can be found in the literatures and there is no case report of the giant cell tumor arising from the posterior cruciate ligament in Korea. We experienced two cases of giant cell tumor arising from the posterior cruciate ligament, they were excised arthroscopically and pathologically confirmed. So we report these cases with a review of the literatures.
