One Case Report of Internal Derangement of Knee with Patella Tendon and Posterior Cruciate Ligament Injury - with a Focus on Burning Acupuncture Therapy and Exercise Therapy - (슬개건 및 후방십자인대 손상을 동반한 슬내장증 환자에 대한 치험 1례 - 가열식 화침 치료 및 운동 치료를 중심으로 -)

  • Objectives: The purpose of this study is to evaluate the effect of the burning acupuncture therapy and exercise therapy on the internal derangement of knee with patella tendon and posterior cruciate ligament injury. Methods: Burning acupuncture therapy was administered three times and exercise therapy was administered during 4 weeks. The improvement of clinical symptom was evaluated by VAS(visual analogue scale) and KOOS(knee injury and osteoarthritis outcome score). Results: After treatment, the patient's VAS score was reduced to 0 pant. The KOOS score of pain, symptom, ADL, sport/rec, QOL(quality of life) improved from 28 to 75, from 36 to 71, from 22 to 65, from 0 to 15, from 25 to 38 respectively. Conclusions: Burning acupuncture therapy and exercise therapy fer internal derangement of knee with patella tendon and posterior cruciate ligament injury was effective. Although this case presented valuable result, further research is encouraged to confirm the effectiveness of this treatment with large number of patient.

  • Objective : The objective of this study is to report the effect of Korean Rehabilitation Treatment with CHUNA Manual Therapy and Exercise Therapy on Postoperative Anterior Cruciate Ligament rupture of knee. Methods : Two patients were treated by Korean Rehabilitation with CHUNA Manual Therapy and Exercise Therapy at knee. We evaluated the effectiveness by Visual Analog Scale(VAS) and Western Ontario and McMaster Universities Arthritis Index(WOMAC) and Range of motion(ROM) Results : Both patients had significant improvement on Knee ROM, VAS score and WOMAC index. Also physical examination of Knee was improved. Conclusions : This study showed that Korean Rehabilitation Treatment with CHUNA Manual Therapy and Exercise Therapy has meaningful effect on Postoperative of anterior cruciate ligament rupture of knee. But there is a limit on this study due to sufficient number of cases. Further studies will be needed.

Triple Tibial Osteotomy (TTO) for Treatment of Cranial Cruciate Ligament Rupture in Small Breed Dogs

  • Twelve dogs weighing less than 10 kg underwent unilateral TTO to stabilize the stifle joint with cranial cruciate ligament rupture. Surgical findings, intra-operative and post-operative complications were recorded. Radiographic examinations were performed for 8 weeks following surgery. Postoperative outcome was evaluated using a visual analogue lameness scoring system. Mean preoperative PTA (the angle created by the intersection of the tibial plateau extrapolation line and the patellar tendon) was 103.8 degrees. Mean tibial wedge angle was 16.6 degrees. Mean postoperative PTA was 92.1 degrees. Intraoperatively, fracture through the caudal tibial cortex occurred in all dogs, through the distal tibial crest cortex in 2 dogs, through the lateral tibial cortex in 2 dogs and through the fibula in 1 dog. Four-week postoperative radiographs demonstrated evidence of progressive bone union at osteotomy site and complete unions were identified at 8 week in 10 dogs. All dogs were healed in 11 weeks. Most of dogs revealed weak lameness in 4 weeks and normal ambulation in 8 weeks postoperatively except for only one dog returned in 11 weeks. Despite frequent minor complication, it appears that the TTO is an alternative procedure for management of cranial cruciate ligament rupture in small breed dogs.

The Effect of Silicone Sleeve and Taping on Balance and Strength in Anterior Cruciate Ligament Reconstruction Patients

  • Purpose: This study was to determine the effects of before and after application of silicone sleeve on balance and muscle strength in anterior cruciate ligament (ACL) reconstruction patients. Methods: 13 subjects who had one or more months after ACL surgery were involved. Dynamic balance, timed up and go (TUG), stair step, vertical jump, proprioception and isokinetic knee strength were measured while subjects under taped, untapped and silicone sleeve conditions. Results: For 30 seconds one-leg standing, there was a significant improvement under silicone sleeve on operated side with eyes open and both taping and silicone sleeve revealed similar effects with eyes closed (p<0.01). Application of silicone sleeve showed significant effects in proprioceptive function on the operated side compared to both taping and none (p<0.05). For stair step test, TUG and vertical jump was a tendency to improve after application of silicone sleeve, but no significant different. Muscle strength on operated side of quadriceps and hamstring was significantly improved compared with none or taping(p<0.05). Conclusion: Silicone sleeve application for ACL reconstruction patients was effective immediately on improving strength and balance. Therefore, depending on the intended use and the disease is considered appropriate use of silicone sleeve will be able to help prevention and functional movement.

The Effects of Closed kinetic chain Exercises of Unstable Floor on the Stability of the Knee Joints of Patients with Anterior Cruciate Ligament Reconstruction (불안정한 바닥에서의 닫힌 사슬운동이 전십자인대 재건술 환자의 슬관절 안정성에 미치는 영향)

  • Purpose : This study was to comparison of EMG of an stable exercise group and unstable exercise group on patients who have had anterior cruciate ligament reconstruction(ACL reconstruction). Methods : The subjects of the study were patients who had no less than 4 weeks after ACL reconstruction and could stand on one leg, and they divided into a control group with 9 patients doing closed kinetic chain exercises on the stable floor and an experimental group with 10 patients doing closed kinetic chain exercises on the unstable floor and in order to compare degrees of the muscle activity of the thigh extensor and flexor was tested each before the exercise, in 3 weeks and 6 weeks after doing exercises by using surface electromyography (Surface EMG). The patients made 3 sets of exercises (10 times per set), each of which consisted of exercises using elastic bands and the squat. Results : There was statistic significance about the vastus medialis muscle. Conclusion : It is thought that the closed-chain exercise could be an exercise program through which patients could enhance the muscle activity of the vastus medialis muscle optionally among the quadriceps muscle and the hamstring muscles which should weaken after ACL reconstruction.

  • The incidence of anterior cruciate ligament tears is increasing as a result of the increasing participation of individuals of all ages in high-risk sports. Endoscopic anterior cruciate ligament reconstruction using autogenous central third bone-patellar tendon-bone graft is the most commonly used method. With regard to BPTB graft as the go]d standard in ACL reconstruction, there are no data that refute this claim to date. Author reviewed the biomechanical properties, donor site morbidity and selection of the bone-patellar tendon-bone graft and described the surgical technique of endoscopic ACL reconstruction using BPTB autograft.

Tibial Plateau Leveling Osteotomy Combined with Tibial Tuberosity Transposition in a Dog with Medial Patellar Luxation and Cranial Cruciate Ligament Rupture

  • A 30 kg, 6-year-old spayed female Samoyed dog was referred with a history of intermittent weight-bearing lameness in her right hindlimb for 3 weeks. The patient hadsurgery to correct a medial patellar luxation in the same limb 3 years prior. Based on the physical examination and radiographic findings, MPL and CCLR of the right hindlimb were diagnosed. Pre-surgical arthroscopy examination was performed, revealing a complete rupture of the cranial cruciate ligament, medial caudal meniscal tears and fibrotic cartilagechanges on the trochlear groove. An arthroscopy-assisted partial meniscectomy was used to repair themedial caudal meniscus. To correct the tibial plateau angle and medial patellar luxation, a tibial plateau leveling osteotomy (TPLO) was performed. A tibial tuberosity transposition (TTT) was performed to realignthe quadriceps mechanism with the trochlear block recession followed by soft tissue reconstruction. The post-surgical recovery was uneventful, and the patient was weight-bearing with normal ambulation on the repaired limb. There were no complications, and the implants were well positioned at the last follow-up. The clinical outcome of the caseindicates that combining TPLO with TTTis a good surgical option for treatingconcurrent CCLR and MPL.

  • Anterior cruciate ligament(ACL) of human body experiences a large deformation. May during everyday when large deformation is repeated by various activities such as outdoor activity, ACL easily get damaged. In order to acknowledge the effect of the cyclic large deformation to ACL, the constitutive equations for ACL are derived from experiment data. The concept of the objective stress rate plays a important role wherever large deformation occurs. In order to obtain the objective stress rates the eigenprojection technique is used. A comparison is made for four different cases: Jaumann rate, Green-Naghdi rate, logarithmic rate and twirl tensor of Eulerian triad rate for an isotropic material subject to cyclic deformation, such as simple shear motion. Four different materials are studied to compare the behavior of the materials for ACL using different objective rates. Finally, more complicated model with fibers for soft tissues is used to calculate the behavior subjected to cyclic large deformation.

  • Congenital deficiency of the anterior cruciate ligament (ACL) is a rare disorder that has been reported in association with other knee dysplasia like as congenital knee dislocation. congenital short femur, congenital absence of menisci, congenital ring meniscus, and thrombocytopenia-absent radius syndrome. There has been no published explanation about the etiology of bilaeral ACL deficiencies without other abnomality. The patient of congenital ACL deficiency must be carefully inspected about combined anomaly. Those efforts may be helful in treatment or ACL deficient patients and evaluation of pathophysiology or ACL deficiency. However there has not been a ruptured congenital deficiency of the ACL without other dysplasia or the knee and other congenital skeletal abnomalities. We reported a case of symptomatic bilateral congenital deficiencies of the ACL which have not been associated with other skeletal abnormalities.

Comparison of the Muscle Contractile Properties on Lower Extremity Between Non-injury and Injury in Subjects with Anterior Cruciate Ligament Reconstruction (전방십자인대 재건술 환자에 대한 비손상측과 손상측 간의 하지 근수축 특성 비교)

  • Background: The purpose of this study was to identify the comparison of the muscle contractile properties on lower extremity between non-injury and injury in subjects with anterior cruciate ligament reconstruction (ACLR). Methods: Twenty-four subjects on the post-operative ACLR participated in this study. Measurement method were using tensiomygraphy to analyze the displacement maximum(Dm) and contraction time(Tc) on the both quadriceps. Results: Compared difference of the Dm between non-injury and injury, the rectus femoris (RF) and vastus lateralis (VL) increased significantly (p<.01). However, vastus medialis (VM) no significantly difference between non-injury and injury. Compared difference of the Tc between non-injury and injury, the RF increased significantly (p<.01) but VL and VM no significantly difference between non-injury and injury. Conclusions: These findings suggest that occurred muscle atrophy of the RF and VL and change properties of muscle fibers on the RF from fast muscle fibers to slow muscle fibers on the injury side of post-operative ACLR. Therefore, when apply to rehabilitation of lower extremity for post-operative ACLR, we should consider the enhance of RF and VL on injury side.
