  • Background: Although insomnia and constipation are highly prevalent worldwide, studies examining a possible association between them are lacking. We examined the relationship between insomnia and constipation in shift workers who have a high prevalence of insomnia and other diseases. Methods: This study had a multicenter cross-sectional design and conducted using health examination data including self-reported questionnaires. In total, 12,879 and 4,650 shift workers were enrolled in Severance Hospital and Wonju Severance Hospital, respectively, during 2015-2017. Multivariate logistic regression models and subgroup analysis were performed in each center with the same protocol, using a common data model. Results: The mean age of the total population was 44.35 (standard deviation = 8.75); the proportion of males was 56.9%. Female sex, being underweight and non-smoker were strongly associated with an increased risk of constipation symptom (p < 0.001). Pooled odds ratios (ORs) were calculated using ORs of both centers with weights; there was a significant dose-response relationship (sub-threshold 1.76 [95% confidence interval [CI] 1.62-1.91]; moderate 2.28 [95% CI 2.01-2.60]; severe 4.15 [95% CI 3.18-5.41] in the final model, p for trend < 0.001). Subgroup analysis performed by stratifying sex and pooled ORs showed a similar trend to that of the entire group. Conclusion: We observed a strong correlation between insomnia and constipation in this population. Our findings may help in formulating guidelines and policies to improve quality of life in shift workers through the management of sleep quality and proper bowel function. This study is the first to report this relationship among people working in shifts.

  • In the present study, characteristics of steady state laminar flows of a straight duct connected to a 180$^{o}$ curved duct were examined in the entrance region through experimental and numerical analyses. For the analysis, the governing equations of laminar flows in the Cartesian coordinate system were applied. Flow characteristics such as velocity profiles and secondary flows were investigated numerically and experimentally in a square cross-sectional straight duct by the PIV system and a CFD code(STAR CD). For the PIV measurement, smoke particles produced from mosquito coils. The experimental data were obtained at 9 points dividing the test sections by 400 3m. Experimental and numerical results can be summarized as follows. 1) Reynolds number, Re was increased, dimensionless velocity profiles at the outer wall were increased due to the effect of the centrifugal force and secondary flows. 2) The intensity of a secondary flow became stronger at the inner wall rather than the outer wall regardless of Reynolds number. Especially, fluid dynamic phenomenon called conner impact were observed at dimensionless axial position, x/D$_{h}$=50.

  • Purpose: The purpose of this study was to evaluate the current level of sleep quality among Korean middle-school students using the Pittsburg Sleep Quality Index (PSQI), and to analyze the factors influencing sleep quality. Methods: The study used a descriptive cross-sectional design and a self-report questionnaire. The participants were 744 middle-school students. The questionnaire included the PSQI and health-related questions, such as the types of diseases they had been diagnosed with and the frequency of hospital admissions. The data were analyzed using the independent t-test, analysis of variance, Pearson correlation coefficients, and linear multiple regression. Results: The global PSQI score was 4.21, which indicated good sleep quality. The major factors that were associated with the sleep quality of middle-school students were the number of diseases they had been diagnosed with (β=.54, p=.001) and atopic dermatitis (β=.32, p=.001). In addition, asthma and the frequency of hospital admissions were significant factors influencing each component of the PSQI. Conclusion: The findings from this study suggest that sleep quality was associated with several health-related factors. Thus, nurses, school nurses, and nursing researchers may need to assess the health-related factors associated with adolescents' sleep quality as part of efforts to improve their sleep quality.

For the analysis, the governing equations of laminar flows in the Cartesian coordinate system were applied. Flow characteristics such as velocity profiles, and secondary flows were investigated numerically and experimentally in a square cross-sectional straight duct by the PIV system and a CFD code(STAR CD). For the PIV measurement, working fluid produced from mosquito coils smoke. The experimental data were obtained at 9 points dividing the test sections by 400 mm. Experimental and numerical results can be summarized as follows. Critical Reynolds number, Recr which indicates transition from laminar steady flow to transition steady flow was 2,150. As Reynolds number, Re, was increased, dimensionless velocity profiles at the outer wall were increased due to the effect of the centrifugal force and the secondary flows. The intensity of a secondary flow became stronger at the inner wall rather than the outer wall regardless of Reynolds number.

  • Purpose: This study aimed to assess the prevalence of asthma, allergic rhinitis, and atopic dermatitis in elementary school students according to the body mass index. Methods: This study was conducted as cross-sectional descriptive research using a structured questionnaire. Data were collected from 6,398 students at 15 elementary schools located in Y City in Korea. The cross-sectional survey used the Korean version of International Study of Asthma and Allergies in Childhood questionnaire. Logistic regression analysis was performed to estimate the odds ratios of body mass index. Results: The symptom prevalence of asthma, allergic rhinitis, and atopic dermatitis in the last 12 months was 12.4%, 49.5%, and 24.5%, respectively. The symptom prevalence of asthma was significantly higher in the obesity group. The number of those without any asthma symptom in lifetime was significantly smaller in the overweight (OR=0.70, p=.004) and obesity (OR=0.57, p=.005) groups than in the healthy weight group. Conclusion: These results suggest that asthma, allergic rhinitis, and atopic dermatitis prevention programs for elementary school students should include weight control intervention.

  • The purpose of a national health care system is to improve health care outcome among population. The objective of the study was to explore the determinants of health outcome in the 24 OECD countries between two health care financing systems. The study employed the pooled time series and cross-sectional analysis with tax-funded and social insurance-funded countries over the period of 1980 to 1999 using OECD Health Data 2002. The study revealed that health expenditure per capita, physicians per 1,000 of the population and calorie intake were positively significantly associated, smoking rate was negatively associated with health outcome while controlling all variables in the tax-funded countries. But in the insurance-funded countries, health expenditure per capita and the number of physicians were not statistically significant factors explaining health outcome. Only the calorie intake was positively associated with, and smoking rate, alcohol consumption per capita, and total nitrogen oxide emission per capita were negatively significantly associated with health outcome. In conclusion, healthy life style factors were much more important to improve health outcome in the both systems.

  • Purpose: This study aimed to analyze the practice of complementary feeding and its influencing factors in children under 2 years of age in Indonesia. Methods: This cross-sectional study used data obtained from the 2017 National Socioeconomic Survey. The sample totaled 1,751 households with children under 2 years of age, who received complementary feeding in urban and rural communities. Furthermore, the practice of complementary feeding was evaluated on the basis of the variations in provided food grouped into two categories: complete and incomplete. This study applied bivariate and multivariate analytical methods. Multivariate analysis was performed using binary logistic regression. Results: The proportion of children under 2 years of age who received complementary feeding with complete variant food was 15.9%, while that with incomplete variant food was 84.1%. Furthermore, the factor influencing the practice of complementary feeding among the children was the mother's educational level (odds ratio: 1.481, 95% confidence interval: 0.245-0.943). Conclusion: Complete complementary feeding, which involves a variety of food sources, is the best approach to improve the nutritional status of infants. Therefore, the source of food for complementary feeding must be accessible to all communities.
