A Pooled Analysis on Crizotinib in Treating Chinese Patients with EML4-ALK Positive Non-small-cell Lung Cancer

  • Background: This analysis was conducted to evaluate the efficacy and safety of crizotinib based regimens in treating Chinese patients with EML4-ALK positive non-small-cell lung cancer. Materials and Methods: Clinical studies evaluating the efficacy and safety of crizotinib based regimens on response and safety for Chinese patients with EML4-ALK positive non-small-cell lung cancer were identified by using a predefined search strategy. Pooled response rate (RR) of treatment were calculated. Results: In crizotinib based regimens, 3 clinical studies which including 128 Chinese patients with EML4-ALK positive non-small-cell lung cancer and treated with crizotinib based regimen were considered eligible for inclusion. Pooled analysis suggested that, in all patients, the pooled RR was 59.3% (76/128) in crizotinib based regimens. ALT/AST mild visual disturbances, nausea, and vomiting were the main side effects. No treatment related death occurred in these crizotinib based treatments. Conclusions: This pooled analysis suggests that crizotinib based regimens are associated with good response rate and accepted toxicities in treating Chinese patients with EML4-ALK positive non-small-cell lung cancer.

비소세포성 폐암환자의 Crizotinib과 삼칠충초정 병용투여 경과관찰 1례 (A Case of Patient with Non-Small Cell Lung Carcinoma Treated with Samchilchoongcho-Jung in Conjunction with Crizotinib)

  • ALK 변이는 비소세포성 폐암에서 차지하는 비율은 높지 않지만 비소세포성 폐암의 발생률이 높기 때문에 환자 수는 적지 않다. ALK 저해제인 Crizotinib은 환자들의 무진행 생존을 평균 대략 4개월 정도 연장시키고 증상을 완화시키며 항암 치료를 받는 것에 비교하여 삶의 질을 향상시키는 치료성과를 거두었다. 하지만 약물 내성 발현은 주요 한계이며 여전히 ALK 변이 비소세포성 폐암 환자의 예후가 불량하므로 내성을 극복하고 지속적인 치료 반응을 유도하여 치료율을 상승시키기 위한 연구가 필요하다. 현재 폐암치료 한약제제인 삼칠충초정과 2세대 EGFR TKI 제제인 Afatinib의 동시 사용 시 상승효과가 나타난다는 연구결과는 보고되어 있지만 ALK 저해제인 Crizotinib과의 병용 치료와 관련한 연구는 시행된 바가 없었다. 본 증례는 삼칠충초정과 Crizotinib의 병용 투여를 통해 종양 크기의 불변 및 삶의 질 개선과 Crizotinib의 내성 억제 가능성 등을 보인 점에서 의의가 있으나 대조군이 없는 1례에 불과하여 Crizotinib의 단독 효과일 가능성을 배제할 수 없는 한계를 지닌다. 따라서 향후 삼칠충초정과 Crizotinib의 병용투여에 대한 추가적인 연구 및 임상시험을 통해 더욱 객관적인 효과 판정이 필요할 것으로 사료된다.

Predictive Role of Computer Simulation in Assessing Signaling Pathways of Crizotinib-treated A549 Lung Cancer Cells

  • Non-small-cell lung cancer (NSCLC) is a leading cause of cancer deaths worldwide. Crizotinib has been approved by the U.S. Food and Drug Administration for the treatment of patients with advanced NSCLC. However, understanding of mechanisms of action is still limited. In our studies, we confirmed crizotinib-induced apoptosis in A549 lung cancer cells. In order to assess mechanisms, small molecular docking technology was used as a preliminary simulation of signaling pathways. Interesting, our results of experiments were consistent with the results of computer simulation. This indicates that small molecular docking technology should find wide use for its reliability and convenience.

Crizotinib과 한방치료를 병행하여 호전된 ROS1 양성 재발성 폐암 환자 1례 (A Case Report on ROS1-positive Recurrent Non-small-cell Lung Cancer Treated with Crizotinib and Korean Medicine)

  • Objectives: This study examined the case of a patient with ROS1-positive recurrent non-small-cell lung cancer treated with crizotinib and traditional Korean medicine. Methods: The patient was treated with crizotinib from January 20 2021 to May 22 2021, together with Haedogyangpye-tang and Haengso-tang. The tumor size was measured using computed tomography (CT), and adverse events were evaluated according to the National Cancer Institute's Common Terminology Criteria for Adverse Events (NCI-CTCAE) version 5.0. Results: After four months of combined treatment, the sizes of the lymph nodes in the porta hepatis, hepatoduodenal, retrocrural, aortocaval, and para-aortic areas had decreased, and no lymph nodes larger than 1 cm in diameter were observed. The side effects of chemotherapy also improved. Conclusions: This case study suggests that traditional Korean medicine may alleviate the side effects of chemotherapy, improve quality of life, and complement chemotherapy itself.

Identification of inhibitors against ROS1 targeting NSCLC by In- Silico approach

  • ROS1 (c-ros oncogene) is one of the gene with mutation in NSCLC (non-small cell lung cancer). The increased expression of ROS1 is leading to the increase proliferation of cell, cell migration and survival. Crizotinib and Entrectinib are the drugs that have been approved by FDA against ROS1 protein, but recently patients started to develop resistance against Crizotinib and there is a need of new drug that could act as an effective drug against ROS1 for NSCLC. In this study, we have performed virtual screening, where compounds are taken from Zinc 15 dataset and molecular docking was performed. The top compounds were taken based upon their binding affinity and their interactions with the residues. The compounds stability and chemical reactivity was also studied through Density Functional theory and their properties. Further study of these compounds could reveal the required information of ROS1-inhibitor complex and in the discovery of potent inhibitors.

비소세포폐암 환자에서 크리조티닙 치료 후 발생한 급속 파괴형 슬관절증 (Rapidly Destructive Arthrosis of Knee Following Treatment of Crizotinib in a Patient with Non-Small-Cell Lung Cancer)

  • Rapidly destructive arthrosis is a rare syndrome characterized by narrowing of the joint space and rapid joint destruction within 6~12 months. A 62-year-old woman with anaplastic lymphoma kinase rearranged non-small-cell lung cancer presented with both knee pain and varus deformity. She was treated with crizotinib 500 mg/day for more than 2 years, with partial tumor response. Initial plain radiography showed Kellgren and Lawrence (K-L) grade 1. After 10 months, varus deformity was worsened and plain radiography aggravated to K-L grade 4 despite conservative treatment including activity modification. Diffuse synovitis with massive joint effusion and destruction of anterior cruciate ligament, medial meniscus, medial collateral ligament, and lateral collateral ligament were shown in magnetic resonance imaging. The patient was diagnosed with rapidly destructive arthrosis of knee and underwent a both total knee arthroplasty.

폐암과 보험의학 (Lung cancer and insurance medicine)

  • Lung cancer such as small cell lung cancer(SCLC) and non small cell lung cancer(NSCLC) have high mortality rate, so, we insurance doctors have little interest in their risk. But nowadays there's a lot of development in targeted therapy of NSCLC. Screening by CT scanning and early resection strategy also shows better prognosis. It is helpful for underwriters and insurance doctors to review the current development of targeted therapy of NSCLC and estimation of extra-risk of early lung cancer. The preferred treatment option for patients whose tumors contain EGFR-activating mutations are one of the EGFR-directed tyrosine kinase inhibitors, such as gefitinib or erlotinib. In patients with NSCLC whose tumors harboured an ALK rearrangement, there was 61% objective response rate to crizotinib in the phase 1 study. The median survival progression-free survival was 10 months. Mortality analysis of early lung cancer who were detected by CT screening, MR of 105% and EDR of 1‰ were calculated.

Gefitinib Inhibits the Growth of Toxoplasma gondii in HeLa Cells

  • Toxoplasma gondii is the causative agent of toxoplasmosis with symptoms of congenital neurological and ocular diseases and acquired lymphadenitis, retinochoroiditis, and meningoencephalitis. Small molecules which block the activity of protein kinases were tested in in vitro culture of T. gondii to find new therapeutic drugs of safer and more effective than the combined administration of pyrimethamine and sulfadoxine that sometimes provoke lethal Stevens-Johnson syndrome. Among them, Gefitinib and Crizotinib inhibited intracellular growth of T. gondii in HeLa cells by counting the number of T. gondii per parasitophorous vacuolar membrane whereas Sunitinib did not. Gefitinib inhibited the growth of T. gondii in a dose-dependent manner over $5{\mu}M$ up to the tolerable concentration of HeLa cells and halted the division of the parasite immediately from the time point of treatment. Gefitinib inhibition suggests that tyrosine kinases of EGFR family or other homologous kinases of the parasite itself may be the target to cause the block of T. gondii growth.
